Physician burnout is a growing problem in the medical community. It's characterized by emotional exhaustion, depersonalization, and a decreased sense of accomplishment. These symptoms can lead to decreased job satisfaction and an increase in errors.
Several factors contribute to physician burnout. The job demands, long hours, and unrealistic expectations can all take their toll. In addition, many physicians feel like they cannot provide the best care for their patients due to time constraints and other factors beyond their control.
In this episode of the Wellness MD Podcast, Dr. Daniela Steyn and Dr. Richard Sztramko discuss physician burnout. Dr. Richard Sztramko practices Geriatric and Internal Medicine at St. Peter's and Juravinski Hospitals in Hamilton, Ontario. He is an Assistant Professor at McMaster University. He is also the co-founder of Arya Electronic Health Records and Handover (https://www.aryaehr.com/), a cutting-edge health tech company rapidly expanding in hospitals and outpatient clinics throughout Canada.

Health and wellness coaches are the backbone of functional medicine!
We all know that just having a prescription or treatment plan does not necessarily mean you will consider the cause of your symptoms. It will also not force you to make any changes necessary to improve your overall wellness.
In this podcast, Dr. Daniela Steyn talks with Cecilia Lanier, A professional Teacher and Health Coach with the Functional Medicine Coaching Academy. Cecilia explains what a health coach does and why we all need one.
Health coaches are specialists in behaviour change. They help people change when change is hard. Coaching is a partnership between experts. Coaches’ expertise is in behaviour change, and they believe the clients are experts in themselves and their own experience. The relationship is formed within an atmosphere of compassion, unconditional acceptance, and empowerment. The sessions are client-led and coach-supported.
Health coaching is, however, not for everyone as:
Sometimes people aren’t ready to change in one area, so coaches support them wherever they are on this readiness spectrum.
Some people are just thinking about it, sitting on the fence.
Others might resist change and may only seek a coach because it’s been suggested by a doctor or a friend or loved one.
Others know they need to change, are sure they want to change, and yet are discouraged because they have tried so many times and cannot succeed or sustain the change. So, they remain stuck.
Or maybe they don’t know how to start the change or how to maintain it. It’s one thing to get a food plan or a directive for eliminating stress, but those plans, or directions don’t work if we don’t work them.
People seek coaches for many reasons, but burnout is one of the main reasons. Health and wellness coaches primarily meet people whose challenges are in one or more of these five areas:
Food/nutrition
Movement/exercise
Sleep/rest,
Stress management
Relationships
However, coaches will also address a person’s emotional, mental, physical, and spiritual realities as it’s hard to separate the person into parts. A holistic view of the person is the best approach.

Physician burnout is a growing concern in the medical community, affecting both doctors and healthcare professionals alike. Teenage sleep with Dr. Cara Ooi

Sleep is one of the most essential functions of the human body, and yet it is often overlooked or given short shrift. Sleep Is critical for our physical and mental health; It helps protect our immune system, repair cell damage, and support brain function. Sleep also plays a vital role in emotional regulation and has been linked to improved mood, decreased anxiety, and reduced stress levels.
In this podcast, Dr. Daniela Steyn talks to Dr. Cara Ooi. Dr. Ooi is a sleep physician and psychiatrist working in Toronto, Canada. She obtained her medical degree from Western University, followed by her residency and subspecialty training in General Psychiatry and Child & Adolescent Psychiatry at the University of Toronto. Dr. Ooi subsequently completed a fellowship in Sleep Medicine, during which she received training in cognitive-behavioural therapy for insomnia.
Dr. Cara Ooi helps teenagers and young adults get their sleep on track to have more time, energy, and motivation for what matters to them. She is the creator of DECODE ONLINE, an online course for youth and parents.
Dr. Ooi explains why teenagers suffer more from insomnia and sleep difficulties. She also explains why traditional sleep advice does not work for teenagers and what mistakes parents make while guiding their teens through this challenging phase. Dr. Ooi shares some valuable tips on how to get optimal sleep.

Dr. Amanda Marrone has been a family physician in the Niagara region in Ontario for almost 18 years, after completing both her medical and residency training at McMaster. She practiced rural family medicine at the Smithville Family Health team from 2004 – 2020 where she included hospital in-patient care, low risk obstetrics, surgical assisting and palliative care in her scope of practice. She is an Associate Clinical Professor at the Michael G. DeGroote School of Medicine, and enjoys teaching both undergraduate and post-graduate trainees.
In July 2020 Dr. Marrone made the decision to transition away from her busy full-time family practice and currently fills her professional time with surgical assisting, teaching and pursuing her dream of practicing lifestyle medicine full time. In December 2021, Amanda completed the rigorous process of becoming officially Board Certified in Lifestyle Medicine through the American College of Lifestyle Medicine. Dr. Marrone believes that lifestyle-based patient education is medicine's "missing link", and it has the potential to change the face of chronic disease in Canada.
She is the creator of the "Welcome Home! Lifestyle Medicine Program", a 12 week group-based patient education program which focuses on the six pillars of lifestyle medicine: nutrition, physical activity, stress management, substance avoidance, sleep hygiene and social interconnectedness. She is excited to launch this program virtually by early 2023.

Let's Talk About Weight Loss With Dr. Doriana Parkin

This week, I had an eye-opening conversation with the incredible Dr. Doriana Parkin. We discussed the essence of weight loss and its connection to self-care, debunking the myth that it's all about spa days and pampering (though those are lovely too).
Dr. Doriana Parkin is a working mom of 3 and knows what it feels like to be overwhelmed & exhausted. To have no time for yourself. To be too busy to fit exercise into her day when juggling work, extracurricular activities, cooking and taking care of family members.
She struggled with weight and body issues as a teen, dieting and hating how she looked until she realized that she needed to eat in a way that wasn't overly restrictive, and that her self worth wasn't related to her weight.
We talked about two common misconceptions when it comes to weight loss. One is that weight loss starts in the gym. It doesn't, weight loss starts in the kitchen! The second is that in order to lose weight, you have to starve yourself. You don't! what you need to learn is how to eat more food volume for fewer calories.
For 15 years in the medical field, Dr. Doriana counselled patients on how to make healthy decisions, and now she's focused on helping as many busy women as possible take back control of their lives and health.

Being Athletic in Perimenopause and Beyond

Join Dr. Daniela Steyn and Tenille Hoogland for this Episode of the Health and Wellness Podcast. Tenille Hoogland's mission is to transform how female athletes experience sports and reach lifelong health and athletic excellence. Her athletic journey began as an Olympic hopeful in synchronized swimming. In her late 20s, she became a professional triathlete. On the surface, her athletic career looks stellar. She represented Canada at International Triathlon Union (ITU) Pan-American Championships, long course events and at IRONMAN World Championships several times, winning numerous titles along the way. However, the method to achieve success had a tremendous cost. She trained relentlessly, unknowingly developed disordered eating and lost her period for 5 years. This led to several injuries and significant hormone challenges. Mentally, she lost joy in sport and retired from triathlon in 2013. Her struggle to find health and her athletic self spanned several years; first, she overcame 2 years of fertility challenges, learnt how to move again without pain and then to move consistently with joy. It came through deep curiosity, education, and trial and error.
This journey led her to create the Mindset, Nutrition, Training, and Recovery Integrated Training System (MNTR-ITS), which she uses alongside her coaching practice. This accessible, step-by-step, evidence-based system empowers female athletes to establish a foundation of health for excellence, performance, and resilience. It works through the physiological life stages of the female athlete, from pre- to post-menopause.
Professionally, Tenille was a Senior Planner at the 2010 Olympic and Paralympic Winter Games Federal Secretariat, managed numerous IRONMAN events, held a Management position at Triathlon Canada and was recognized by her national federation and Canadian Olympic Committee as a Future Sports Leader. Now, through her company, Tenille Hoogland Applied Athletics, Tenille coaches female athletes in perimenopause and beyond. She owns the Element Race Team, an amateur women’s cycling team whose mission is to get more women on the start lines at cycling events. Having given birth to two beautiful and healthy sons in her 40s, her purpose in sport now is to be fit for a big, long life full of play.

Our energy levels naturally wax and wane with our bio-rhythms. Depending on some genetic factors, lifestyle habits and hormones levels many people experience decreased energy levels at certain times of the day, for women especially their energy level fluctuates during their cycle.
With the added chronic stress of this pandemic, many people are experiencing mental fatigue worsening physical fatigue.
Dr. Daniela Steyn discusses the way our bodies manage stress. This is called our pituitary-adrenal axis (or HPA axis). When we experience chronic stress, our bodies need to deal with a high amount of stress hormones, such as cortisol. This causes us to feel tired and wired and generally exhausted even before we have begun our day.
The good news is: there are many simple ways you can prevent energy dips and avoid mental fatigue.

1: Drink water JB and Dr. Daniela discuss how mild dehydration can cause physical fatigue, mental fatigue, brain fog and headaches. Dehydration can even lower your mood.

Meditation has been practiced for many years in many religions. However, as more scientific evidence emerges praising the health and wellness benefits of meditation, it is becoming increasingly more popular.
JB and Dr. Daniela Steyn discuss the research behind meditation.
Dr. Daniela also discusses the scientific health benefits of incorporating meditation into your daily practice.
1: Meditation promotes better sleep
2: Reduce stress, anxiety, and depression
3: Reduce pain, especially in chronic pain syndromes, chronic illness, migraines, fibromyalgia and irritable bowel syndrome.
4: Meditation boosts your memory, protects against cognitive decline (ageing of the brain), it sharpens focus and concentration, it improves creativity.
5: Improved cardiovascular health. One study showed that after meditating for only 5min a day, after only 10 days the participants showed an improvement in heart rate variability. Other studies have shown meditation decreases blood pressure and improves blood cholesterol levels.
Some of the science discussed in this podcast:
Our brains consist of billions of nerve cells called neurons. These neurons talk to each other. This communication is called electricity. When millions of neurons are communicating at the same time this generates a significant amount of electrical activity. In the hospital, we can measure this electricity with an EEG machine (electroencephalograph). This combined electrical activity in the brain is known as a brainwave pattern. It is called a brainwave because when we record it, it shows a wave-like pattern. Brain waves can be sorted into four different categories based on wave frequency that gets measured in cycles per second (Hz).
Beta waves: 15-40Hz
This is when you are superconscious, awake, alert, active, engaged, talking with your boss, teaching your children Math.
Alpha waves: 9-14Hz
This is the frequency when you are awake and conscious, but calm, lucid, reflective. For example when you are staring into a fire, looking at the rolling waves of the ocean, listening to soothing music.
Theta waves: 5-8Hz
This is the state we enter when we are deeply relaxed, in a deeper meditative state, almost semiconscious. These are the frequency during daydreaming and REM sleep.
Delta waves 1.5-4Hz
This is when we are in dreamless sleep, minimally aware, unconscious of our surroundings.

Skin disorders can be very frustrating and lead to decreased self-esteem. There is a wide range of skin disorders and it is best to have your specific skin disorder diagnosed by your Physician. The most common of these conditions include Acne, Eczema (atopic dermatitis), Rosacea, Seborrheic Dermatitis.
The good news is that there are many things within your control that you can do to improve your skin.
Dr. Daniela will discuss different causes of acne including:
1: Hormonal Acne: She explains what an excess of androgens are, how inflammation causes hormonal acne, and how some foods can trigger hormonal acne.
2: Food Sensitivities are very commonly a cause of acne.
3: They explain why stress causes acne and how to manage your stress.
4: You will learn which fats in your diet can improve your skin
5: Foods to avoid.
6: Check your beauty products
Your current beauty routine might be causing/exacerbating your acne. More than 50% of acne cases are triggered by cosmetic products. The Environmental Working Group’s database Skin Deep helps to compare safety ratings for various personal care products on the market. Sticking to a high-quality, natural cleanser, moisturizer, and toner that you use once daily. Less is always better.
7: Drink more water!

Vitamin D is often called the sunshine vitamin due to our skin’s incredible ability to absorb ultraviolet light from the sun to form Vitamin D (cholecalciferol) in the skin from 7-dehydrocholesterol. We can also get vitamin D from our diet. Unfortunately, Vitamin D naturally occurs only in very few foods.
We need Vitamin D for:
· normal calcium homeostasis.
· support the immune system and fight inflammation.
· Oral health
· Vitamin D can help treat hypertension: According to a 2019 review published in the journal Current Protein & Peptide Science suggests that vitamin D may play a role in the treatment of high blood pressure where they have found even short-term vitamin D deficiency may directly raise blood pressure which in turn may lead to organ damage.
· Weight management. Obesity is a risk factor for Vitamin D deficiency. On the flip side, one 2009 study in the British Journal of Nutrition found that, in overweight or obese women with low calcium levels, those who took a daily dose of calcium paired with vitamin D were more successful shedding pounds than those who took a placebo supplement
· Effect on mood: The sun can brighten up your mood, and so can vitamin D. According to a 2017 review article in the journal Neuropsychology, researchers found "a significant relationship between depression and vitamin D deficiency.”
· cancer-fighting powers.
They discuss the causes of Vitamin D deficiency.
Canadian recommendation is to take a vitamin D supplement year-round and not only in the winter months. Healthy adults require 1000iu Vit D3 (cholecalciferol) daily.
Here are some natural food sources of vitamin D:
· Swordfish
· Rainbow trout
· Salmon
· Cod liver oil
· Snapper
· Egg yolk
· Tuna
· Mushrooms
Vitamin D fortified products:
· Cow’s milk
· Plant-based milk substitutes
· Margarine
· Infant formula
· Formulated liquid diets/foods for use on a very low energy diet/meal replacements
· (Fortification of these products are mandated in Canada, fortification is voluntary for butter substitutes, condensed milk and goat’s milk)

Sleep is the ultimate key to optimal health. Sleep influences every function in your body. It affects how your body processes food, how it regulates blood sugar, processes cognitive information and reduces inflammation. Optimizing your sleep is critical for optimal health. Getting enough good quality sleep at the right time can help you feel your best and be high functioning during the day.
A quick summary of the tips they discussed during this episode:
Aim to get 7.5 hours of sleep, of which 10pm -2am is the most critical, so for many of us, it means going to bed earlier.
Avoid screens, especially your phone, the hour or two before you go to bed. Ideally remove your phone from your nightstand so that is not the last thing you look at, at night. Try reading something positive or uplifting just before drifting off.
Go outside for a walk or run first thing in the morning - or use a sunlight light if you live where there is no sun in the morning.
Avoid caffeine in the afternoon.
Meditate. even if it is 5 minutes a day.
Create a bedtime routine, it can include lavender essential oil.
Take 200mg magnesium biglycenate before bedtime.

Nutrition is one of the most critical components of healing and living an optimal life.
Poor nutrition is common. Our bodies need food to survive. We need good nutrition to thrive. Many of our patients often don’t even know that they are malnourished, because you might be overweight, and still malnourished because your body lacks certain important nutrients. This can often be a cause of being overweight as well. When your body needs certain nutrients, for example, some healthy fats, you might stay hungry, and if you are on a low-fat diet, you might stay hungry and eat heaps of low-fat food which will cause weight gain, whereas if you had a hand full of macadamia or walnuts, you would have given your body what it needed, consumed less empty calories, and not gained any weight. Stress often impacts my weight as well.

New Years Resolutions: How to guarantee success!

Some people call them resolutions. Some call them intentions. Some love them and some avoid them. In this podcast JB and Dr Daniela discuss how you can capitalize on the beauty of an empty first page of the new yearbook we all get to narrate our own story with intention. The most effective way to set resolutions that you will want to prioritize, is to align them to your life goals. If your life goals include being healthy and happy - there are three New Year’s Resolutions that we recommend making for 2022.
i. Improve your Health
ii. Manage technology
iii. #SayItNow.
1. Improve your health
Anyone suffering from a health ailment will know that there is freedom in living healthy and pain free. We at WellnessMDHealth believe that what we eat, how we sleep and how much we move directly impact our short-term and long term physical and mental health. This podcast will give you ideas on which health goals you could set.
i. Nutrition: Add wholefoods and greens
ii. Move more: Exercise is key
iii. Sleep: Sleep well , sleep enough
2. Manage your use of technology
We discuss practical advice on how to manage your technology usage, instead of it managing you. Some practical tips you could try:
i)No technology in the bedroom
ii) Fast from Social Media
iii) Implement Technology free times
3. #Sayitnow
Expressing appreciation, gratitude and kindness to others.
#Sayitnow is a challenge - to identify people that you want to thank and share gratitude with, now.
The idea is to identify people, in three buckets.
i) People not close to you. Just flowers on the path that you’ve been journeying on in your life, such as a colleague you speak with once a year, or a cashier at your favorite grocery store.
ii) People that were close at some point. But you rarely speak. Maybe it’s a college friend. A neighbor the in city where you lived ten years ago. Your nanny growing up.
iii) Those close to you.
The goal is to make a list of these people and express gratitude to them in a special way - it can be with a hand-written note or a video you record and send. Our challenge to you - is to do one every single week. 52 people who you are going to make happy and enrich their 2022. You will be happier as a result. And then, if you get the call - or they get the call – there will be nothing to regret.
