Dr. Rohan D’Souza is an obstetrician and associate professor with the departments of Obstetrics & Gynaecology and Health Research Methods, Evidence and Impact at McMaster University, Hamilton, Canada. He specializes in the care of pregnant women and gender-diverse people with medical disorders, and his research work is focused on reducing pregnancy-related adverse events. Dr. D’Souza has made it his mission to reduce pregnancy-related adverse events. Dr. D’Souza and Nikki explore the alarming statistics that may be missed and what kind of advocacy is needed to improve outcomes for women during and after pregnancy.

 

Dr. D’Souza Shares:

  1. The CBC news article “Canada significantly undercounts maternal deaths, and doctors are sounding the alarm” - got a lot of attention. In your interview, you cited the importance of better maternal health monitoring systems to prevent avoidable deaths. The article stated that Canada undercounts the maternal mortality rate.   2. Let’s talk about the increasing cesarean rates. We know they vary tremendously from provider to provider and hospital to hospital. What is the current data and trend over time? Drinking can be a coping mechanism for stressed out moms - but what moms really need isn’t wine. What we need is support. And that is unfortunately difficult to ask for. 

 

We are interviewing Michelle Kapler. She is  a Certified Life Coach, specializing in overdrinking and alcohol freedom. She works with high achieving women, who want to explore their relationship with alcohol, without judgement and without labels. As a highly successful entrepreneur, multiple times over and a mother, Michelle understands what it's like to have a big, full, wonderful life on the outside… but to truly feel like you need that nightly glass (or bottle) of wine to make it through life. To feel like sometimes you are drinking against your own will. To feel like you have lost control. Michelle has now been living an alcohol free life for over two years. To achieve this, she used tools based in cognitive psychology, thought work and coaching, to completely lose her desire for alcohol and create a beautiful amazing life, without identifying as an alcoholic or using a 12 step program. She now helps her clients do the same.

Making big health decisions can be overwhelming..  It can challenge our personal values and beliefs and lead to internal conflict and unrest. It can cause rifts in families. We can wonder “what is the right thing to do?” and have uncertainty when there is more than one way to proceed. Dr. Rebecca Greenberg is a  bioethicist who works with families to help reduce the stress and burden of medical decision-making and provides the necessary support  to make a difficult  decision with confidence. A Bioethicist is a healthcare professional trained to guide people who are unsure about what is the best choice or the right thing to do.

 

Dr. Rebecca Greenberg is a nurse and bioethicist who has been working in healthcare for the past two decades with a particular focus on pediatrics and women’s health. She guides clients through making difficult decisions about their own health and  that of their loved ones. She approaches each unique situation with compassion, no judgements and supports people in getting to the right decision for them. 

 

Rebecca’s aim is to help all those struggling with medical decision making so they can reach  “lasting moral peace” in the face of tough choices.

After being diagnosed with Crohn's disease and IBS in 2009, Megan felt extremely isolated and humiliated about having digestive issues. She was always in pain, always running to the bathroom and avoided a lot of situations so that she didn’t have to talk about what she was experiencing, or risk not being able to make it to the bathroom in time (aka bathroom anxiety). 

 

As Megan slowly began opening up about what she was dealing with, she learned that many others dealt with the same issues—they just weren’t talking about them. The stigma around gut issues is what ignited her passion to increase the conversation, push awareness, and create a community. We talk to Megan about her journey being diagnosed with Crohn’s and how she has overcome her anxiety today. She hopes others will join her on the other side of bathroom anxiety.

Semi retired, he continues to teach both clinical and research skills, and mentors family practice physicians for their research projects. He is also the author of “Dissident Doctor”, a riveting book chronicling his life and work.

Trigger Warning: Birth trauma, Mental Health Struggles, Suicide, and Sexual Assault

SOLACE (Severe Obstetric Laceration Awareness and Community Engagement) is a non-profit that was founded by Caroline Stadler in April of 2023. Caroline sustained a missed a 4th degree tear from a forceps delivery when giving birth to her daughter in 2022. She struggled both physically and mentally as she navigated care for her symptoms and recovery due to a lack of structured resources available in the United States. Caroline was then joined on the board of SOLACE by multiple women who had similar journeys and medical professionals who were passionate about making a change in the awareness and after care of severe tearing.

Caroline and Sepideh Share:

  1. While obstetric data reports a 6-7 percent rate of Obstetric Anal Sphincter Injuries (OASI), looking at the ultrasound data shows that up to 40% of patients who have had a prior vaginal delivery have sphincter defects that were never diagnosed: https://www.nejm.org/doi/full/10.1056/NEJM199312233292601 (Also: https://obgyn.onlinelibrary.wiley.com/doi/full/10.1002/uog.18827). In addition to OASIS, 20 to 35% of moms who have had a vaginal delivery will suffer from a LAM (Levator Ani Muscles) injury which predisposes them to prolapses and incontinence. While there are surgical and non-surgical options for both anal sphincter and LAM injuries, the success rates are low and women often suffer from symptoms for a life-time.
  2. Vacuum and Forceps deliveries, especially forceps are major risk factors for an OASI.
  3. Based on a recent survey by the MASIC Foundation in the UK, 45% of women with OASIS experience postpartum depression and 78% have traumatic memories of their birth experience.
  4. Emily Getz lost her second child unexpectedly at six months pregnant, due to an external infection. Emily has since struggled with her fertility. She has undergone medical interventions such as IUI and multiple failed rounds of IVF, an ectopic pregnancy, (which resulted in the removal of her right fallopian tube) and most recently in February had a miscarriage at 8-weeks pregnant, which resulted in her first D&C after getting pregnant through natural conception.  In this episode we talk about the emotional toll of fertility struggles, what fertility looks like for different people, and how to support those around you experiencing fertility struggles.

Forceps assisted birth increases the risk of OASI (obstetric anal sphincter injury) significantly. Dr. Muraca breaks down her doctoral research on instrumental assisted birth to help provide informed consent for everyone giving birth. Being informed can help decrease the risk of injury. 

Dr. Guilia Muraca is a perinatal epidemiologist and an Assistant Professor in the Departments of Obstetrics and Gynecology and Health Research Methods, Evidence & Impact at McMaster University. 

Her research uses epidemiologic approaches to identify optimal intervention rates in birth  (e.g., cesarean delivery rate, induction rate) that minimize the frequency of adverse maternal and neonatal outcomes (e.g., postpartum hemorrhage, obstetric anal sphincter injury, maternal sepsis, neonatal sepsis, birth injury). 

Dr. Muraca’s research program at McMaster University aims to produce evidence to support enhanced safety and satisfaction in the care of pregnant individuals and their children, with emphasis on care quality, accessibility and equity.

Dr. Muraca Shares:

  1. How commonplace are forceps births? 
  2. What is the impact on the pelvic floor and long term issues with prolapse and incontinence after instrumental assisted birth? 
  3. Dr. Muraca’s research has discovered that rates of trauma after Operative Vaginal Deliveries is higher than previously reported - let’s dive into that. 
  4. What are some changes that could be made to make operative vaginal deliveries safer? 
  5. What are your thoughts on our rising cesarean birth rates? Factors at play? Concerns you may have, if any? 
  6. What do you wish more people knew about birth and recovery? 
  7. What are the biggest changes you would like to see made to the current state of obstetrics in Canada? 
  8. Asian-White disparities in obstetric anal sphincter injury: A systematic review and meta-analysis: 39% of women experience fecal incontinence after 3rd or 4th degree tearing (aka Obstetric anal sphincter injury (OASI). Your research showed that there is an increased risk from women who identify as Asian race vs White. Let’s talk about why.

A note from Hannah: Like many of you, I am no stranger to hard times. I was once a gymnast for the University of Nebraska. That is until I dislocated both of my elbows at the same time. This is only after recovering from breaking both ankles at the same time. I lost my sport, my identity, and my self-worth. I spiraled into depression, anxiety, and extreme sudden weight gain. It is no surprise that eating disorders, obsessive thoughts, and low self-worth found its way into my life. I was then sexually abused, which shattered me. I was completely broken. Shortly after I had a car accident that should have killed me. I am no stranger to pain or hardships. I’ve encountered some pretty hard shit that’s brought me to my knees. But I am thankful for those moments that I couldn’t understand at the time because it’s paved the way for the life I have now, the outlook I have now and the empathy I have for other peoples struggles. It allowed me to dive into my career helping woman find their worth. My page, and my life is dedicated to helping women break through toxic diet and fitness culture. To help mothers navigate confident and safe pregnancies and easier postpartum journeys. More than anything I’m also just a regular mom navigating life and doing the best I can.
