The World Health Organization (WHO) has designated climate change the top risk to global healthcare. The relationship between climate change and human health is becoming increasingly pronounced as health systems and organisations are responding to public/private sustainability initiatives to mitigate climate risks while advancing the pursuit of health equity. As health systems around the globe grapple with addressing inequity in healthcare, one of the most tangible opportunities for change is the chronic underrepresentation of individuals by race, ethnicity, or sex in clinical trials. Jen & Ryan are joined by Vynamic’s Matt Howard to discuss how life science companies can integrate diversity, equity & inclusion throughout the clinical trials process to facilitate better outcomes. Better representation across patient types is critical to demonstrate safety and efficacy – it’s both good science and good business (01:18). However, truly integrating DEI is going to require more than ‘bolting on’ diverse patient recruitment practices – it will require an operational and mindset shift that necessitates an appreciation of the historic inequities within the clinical trial space (03:22). Opportunities to improve DEI abound throughout the clinical trial lifecycle – from reevaluating exclusion criteria (06:27) and protocol design (08:46), to reallocating resources across sites and focusing on tailored interactions with participant communities (15:00). When we think about HIV, what do we associate with it? Many of us will think about the AIDS epidemic in the US that took off in the 1980s, or perhaps the tangential relationship with the LBGQT community. Six years after the Medicare Access and CHIP Reauthorization Act (MACRA 2015), Vynamic’s Mindy, Ryan, and Jen discuss where we are today in the transition from Fee-For-Service (FFS) reimbursement to Value-Based Care (VBC). The episode opens with an acknowledgment that VBC is at a crossroads – the transition has been bumpy, VBC models have been “consistently inconsistent” (2:10), and the misalignment of payer and provider compensation models continues to be an ongoing challenge (3:18). The conversation then moves to what is needed for this transition to VBC to be successful; it requires operational overhaul (5:10), robust analytics, appropriate engagement of providers, and health system transparency. Innovation takes time and transitioning to VBC requires a shift in cultural mindset and a detailed strategy for change and adoption (8:10). The episode then explores what VBC means for Life Sciences companies, as the industry starts to reckon with the development of more expensive products with higher intrinsic value (14:50), including curative one-time treatments and gene therapies. From a market access perspective, Life Sciences companies should be creative with contracting and how they approach payers and providers about the products they bring to the market (15:18). Ultimately, these stakeholders will need to reach a middle ground to make products accessible to the right patients (20:04). The wave of price transparency is here, with regulations like the No Surprises Act and the Hospital Price Transparency Rules building momentum. The push towards transparency can be traced back over the last decade, but the recently implemented No Surprises Act (01:44) and Hospital Price Transparency Rules (02:13) are directly aimed at helping consumers understand true healthcare costs and make informed care decisions (03:30). Implementation is not without its challenges – with providers citing reporting burdens, administrative costs, and difficulties in accurately reflecting the costs of individual “care episodes” (07:28). With a summer deadline approaching, soon payers (10:00) will be dealing with pace pressures, the disconnect between posted prices vs. true cost, and how to capture quality vs. price (11:13). Despite these challenges, there are opportunities for provider and payer organizations associated with these changes (13:47) – especially for organizations experienced in value-based care and those prepared to leverage price transparency to improve customer satisfaction (18:08).

One of the advantages of serving clients across multiple geographies is that Vynamic can see firsthand how the differences in health systems play out in terms of spend, outcomes, and innovation. Jen & Ryan are joined by Vynamic London’s Oliver White and Vynamic Boston’s Shari Robbins to discuss key differences between the health systems in each country and what they can learn from one another as they move healthcare delivery out of the hospital and into the community setting. In one sense, the US and UK represent two extremes on the delivery system spectrum: the US has the largest private sector system globally while the UK has the largest publicly funded system. They also differ in key metrics around healthcare spend and outcomes (01:19), science and technology (04:21), and wait times for elective procedures and emergency care (06:21). Despite these differences, both systems are focused on improving cost and customer satisfaction – especially when it comes to telehealth and home care (09:05). One model the NHS has put forward in this effort is Community Crisis Response Teams (12:38) – a model that could be attractive as US health systems look to transition care to the home and alleviate emergency department (ED) volumes (15:13). While implementing such a model could present scaling challenges in the US market (16:45), it could also offer opportunities for health plans and providers to improve quality, outcomes, and patient satisfaction (18:29).

As health systems around the globe grapple with addressing inequity in healthcare, one of the most tangible opportunities for change is the chronic underrepresentation of individuals by race, ethnicity, or sex in clinical trials. Jen & Ryan are joined by Vynamic’s Matt Howard to discuss how life science companies can integrate diversity, equity & inclusion throughout the clinical trials process to facilitate better outcomes. Better representation across patient types is critical to demonstrate safety and efficacy – it’s both good science and good business (01:18). However, truly integrating DEI is going to require more than ‘bolting on’ diverse patient recruitment practices – it will require an operational and mindset shift that necessitates an appreciation of the historic inequities within the clinical trial space (03:22). Experts have been ringing the alarm about the impending healthcare delivery professional workforce shortage for over a decade. Mindy & Ryan are joined by Vynamic’s Marisa Deline to explore the broad implications of this shortage and discuss ways healthcare leaders across sectors can think about addressing - or simply learning to live with - this foundational issue. The conversation begins with a detailed overview of the widespread nature of the shortage: from physicians to medical assistants there simply are not enough healthcare workers to meet demand and future projections only anticipate the trend worsening over time (01:54). The COVID-19 pandemic, which contributed to healthcare professional burnout, only served to hasten the trend (0:2:36); as has the ‘Great Resignation,’ where many in direct care delivery positions have left for more lucrative, flexible options in tech, healthcare startups, and other roles where their clinical skills are highly valued (05:11). This shortage has obvious implications for quality of delivery across delivery settings, but also snowballs into other healthcare sectors as well; insurers must focus more keenly on network adequacy and life science organizations must think about different models to provide therapeutic education to their provider partners (11:41). Despite these challenges, there are opportunities for the broader healthcare community to address these issues head-on, and we can take some solace in the knowledge that this is a global issue (16:13). We must collectively ‘ruthlessly prioritize’ to ensure that we are focused on opportunities that will have a real impact on the issue, such as subsidizing clinical training, increasing the number of clinical training opportunities, elevating non-physician/nurse professionals to provide more services, and ensuring that future healthcare delivery professionals feel like they are entering a field that will not just burn them out, but will provide them with a rich and fulfilling career (18:43).
