A few months ago, there was a HuffPost article entitled "Everything You Know About Obesity is Wrong," by Michael Hobbes. It discussed the struggles of people who have obesity and how the medical system has failed them. It quoted Stephanie Sogg, PhD, a clinical psychologist at the Weight Center at MGH (notice how it isn't called the "weight loss center"). In it, Dr. Sogg describes her approach as "being nicer to her patients than they are to themselves." Given that this article lambasted physicians for not being able to effectively discuss weight issues with their patients, I contacted Dr. Sogg to find out how we can improve at this. She provided me with an article she wrote. This article should be read by EVERY CLINICIAN IN THE COUNTRY. It is that powerful and important. In the episode title, I paraphrased an article she wrote entitled, "Bad words: why language counts in our work with bariatric patients." In it, Dr. Sogg explains how language can influence they way that we think about things. As physicians, we are authority figures and community leaders, so thereby, the language we use can influence how our patients think about themselves and our colleagues think about their patients, for better or for worse. This isn't a discussion on how to lose weight. This is a discussion on how to effectively discuss a topic that is extraordinarily sensitive and full of landmines. This is a discussion on how to avoid those landmines and earn the trust of a patient who is struggling and could use your empathy and understanding. In 2019, Dr. Aysha (i-sha) Khoury (koo-ree) was recruited to become founding faculty at Kaiser Permanente Bernard J Tyson School of Medicine. She developed case-based curricula, implementing flipped classroom and active learning pedagogies. She served as a small group facilitator and Equity, Inclusion, and Diversity Champion and Workgroup member. On August 28, 2020, within hours of engaging her small group in a critical discussion on bias and racism in medicine, she was targeted, suspended, and ultimately terminated without transparency or due process. We begin the interview by discussing her termination, what she felt led to it, although with the lack of transparency, she still hasn’t been informed. We use this as a jumping off point to discuss allyship, or in her situation, lack thereof. We then move on to why there is medical mistrust among minoritized communities, including a discussion on why we now use words like minoritized and enslaved. We finish with a discussion on how to address SARS-Co-V-2 vaccine hesitancy in the setting of justifiable medical mistrust. Her argument is that there is less hesitancy in the older population, so if we address the inequities in vaccination access, the older population will be able to influence the hesitations of the younger population. Dr. Khoury grew up in Atlanta, GA and completed her medical degree there at Morehouse School of Medicine. After completing an internal medicine residency and working as a primary care physician, she returned to Morehouse School of Medicine to complete training in public health and preventive medicine and earned a Master of Public Health. After this, Dr. Khoury returned to the Southeastern Permanenete Medical Group and pioneered a new role there as a Clinical Decision Unit Internist. This encompassed work in quality management, process improvement, and evaluation of treatment outcomes. Committed to education and advocacy early in her career, she served as an adjunct clinical assistant professor and Admissions Committee Member at Morehouse School of Medicine, where she was later inducted in the White Coat Society for demonstrating compassionate care and community service. This was one of my favorite interviews. I was a fledgling podcaster at the time when I interviewed Dr. Stephanie Sogg, a clinical psychologist at the Harvard Weight Center. She had been quoted in an article in the Huffington Post that lambasted physicians for not knowing how to talk to our patients about their weight. Interviews like this were the reason why I started podcasting. I emailed her and asked her if she could teach us the right way to have these discussions and she agreed! Talking to patients about their weight is fraught with landmines created by inordinately complex psychosocial issues. It is very easy to alienate a patient. Dr. Sogg teaches us the power of language and this has come up in other interviews since. Changing racial language from minority to minoritize and slave to enslaved changes the perspective of the listener. It subtly influences how that person is perceived. The same applies to discussing weight; when we change our language about body habitus to be less stigmatizing, it subtly influences the listener, be it our patients, students or colleagues. We also need to change the way we talk about food. According to Dr. Sogg, the only bad food has gone bad, tastes bad or is poisonous. Dr. Stephanie Sogg is a clinical psychologist and an Assistant Professor of Psychology at Harvard Medical School. She has treated patients with obesity at the MGH Weight Center since 2003. Dr. Sogg earned her PhD in clinical psychology from Rutgers University in 1998, and completed a post-doctoral fellowship with Harvard Medical School. In addition to her clinical work, Dr. Sogg conducts research on obesity and bariatric surgery, and the intersection between obesity and addiction, and has published widely on obesity and related topics. She is an author of the Boston Interview for Bariatric Surgery, and of the official ASMBS Recommendations for the Pre-Surgical Psychosocial Evaluation of Bariatric Surgery Patients. This interview is one of my most important. If you are doing to share any of my episodes, this is one that I would implore you to share with your friends, family and colleagues. This is part 1 of 2 of my interviews with BJ Fogg, PhD, author of the book Tiny Habits: The Small Changes That Change Everything. We all struggle to change our behaviors, to develop good habits and stop bad habits. There is a lot of popular wisdom about this and most, if not all, is just wrong. This is where Dr. Fogg steps in. Dr. Fogg discovered the keys to changing behavior through changing habits. For those of you on medical school faculty, this should be a class. This should actually be taught in high school. Until then, as physicians, this information is critical, not just for lifestyle changes that can help patients eat better, move more, and smoke less, but even applies to checking their blood pressure and taking their medication. Popular wisdom is wrong. Guilt and shame are destructive. People don’t start habits by feeling badly, they start habits by feeling successful. And we are more likely to be successful by starting a habit that is small, that we actually want to do, and the third key to this is a prompt that reminds you it is time to perform the behavior. If you are going to learn piano, you start with chopsticks. If you are going to start to exercise, you do one sit-up. The smallest increment that you can fall back on when you motivation is waning so you don’t fall off the wagon completely and you keep your habit. And you do it at a point in your day that you can associate with the new behavior, even if they are completely unrelated. You’ll have a reminder that is baked into your day. Scott Dikkers founded the world’s first humor website, TheOnion.com, in 1996. A few years earlier he helped found the original Onion newspaper. He’s served as The Onion’s owner and editor-in-chief, on and off, for much of the last quarter century. He led The Onion’s rise from small, unknown college humor publication to internationally respected comedy brand. He is also a New York Times best seller, and Peabody Award winner. He documented his process for creating humor in his book, How to Write Funny, and the second in the series, How to Write Funnier, and next on the way, How to Write Funniest, which are the basis of the Writing with The Onion program he created and teaches at The Second City Training Center in Chicago. Scott offers other courses and free resources for comedy writers on the How to Write Funny website. He first dispels the myth that funny is innate an then we dive into how to be funny, starting with how to just dip our toes in the water. He teaches us how to recover from a failed joke, how to joke about subjects that might seem taboo, how to work humor into our office visits and lectures. Apparently, stand-up comics are jealous of our mundane topics. He has a system of 11 funny filters, or types of humor of which all jokes are made, and which are the most and least appropriate for physicians. Deena Kishawi, MD, is a Chicago-based OBGyn resident physician who is particularly interested in health outcomes that are unique to Middle Easterners and North Africans residing in the United States. Dr. Laurence Orbuch completed his training at the New York Hospital Weill Cornell Medical College and SUNY Upstate Medical Center. He served as the Director of Minimally Invasive and Pelvic Reconstructive Surgery at The Mount Sinai Hospital and Director of Gynecologic Robotic Surgery at The Mount Sinai Beth Israel Hospital. Dr. Orbuch is currently Clinical Professor of Obstetrics and Gynecology at both Cedars- Sinai Marina Del Rey Hospital and Providence Saint John’s Hospital in Los Angeles and Lenox Hill Hospitals in New York. He is also the Medical Director of GYN Laparoscopic Associates LA in Beverly Hills, California. In my quest to be taught how best to have a fruitful discussion with those hesitant about the vaccine, I interviewed two science communicators, a social engineer, a lawyer, an expert in cognitive biases, and a motivational interview and of all of those, the last one was the most powerful tool for moving the needle on those hesitant to get the vaccine. Motivational interviewing comes from addiction medicine, but using it for vaccine hesitancy actually precedes the COVID pandemic. It is used for new parents who are considering forgoing their newborns' vaccines. Mandates are going to get some to get the vaccine, but for others, a conversation with their trusted physician can be a powerful thing. Dr. Joseph Weiner teaches us how to use it to help our patients with vaccination and other decisions. This interview is one of my most important. If you are doing to share any of my episodes, this is one that I would implore you to share with your friends, family and colleagues. This is part 1 of 2 of my interviews with BJ Fogg, PhD, author of the book Tiny Habits: The Small Changes That Change Everything. We all struggle to change our behaviors, to develop good habits and stop bad habits. There is a lot of popular wisdom about this and most, if not all, is just wrong. This is where Dr. Fogg steps in. Dr. Fogg discovered the keys to changing behavior through changing habits. For those of you on medical school faculty, this should be a class. This should actually be taught in high school. Until then, as physicians, this information is critical, not just for lifestyle changes that can help patients eat better, move more, and smoke less, but even applies to checking their blood pressure and taking their medication. Popular wisdom is wrong. Guilt and shame are destructive. People don’t start habits by feeling badly, they start habits by feeling successful. And we are more likely to be successful by starting a habit that is small, that we actually want to do, and the third key to this is a prompt that reminds you it is time to perform the behavior. If you are going to learn piano, you start with chopsticks. If you are going to start to exercise, you do one sit-up. The smallest increment that you can fall back on when you motivation is waning so you don’t fall off the wagon completely and you keep your habit. And you do it at a point in your day that you can associate with the new behavior, even if they are completely unrelated. You’ll have a reminder that is baked into your day. Dr. Fogg founded the Behavior Design Lab at Stanford University. In addition to his research, Dr. Fogg teaches industry innovators how human behavior really works. He created the Tiny Habits Academy to help people around the world and interestingly, the Tiny Habits Academy long preceded the Tiny Habits book. He lives in Northern California and Maui. In my quest to be taught how best to have a fruitful discussion with those hesitant about the vaccine, I interviewed two science communicators, a social engineer, a lawyer, an expert in cognitive biases, and a motivational interview and of all of those, the last one was the most powerful tool for moving the needle on those hesitant to get the vaccine. Motivational interviewing comes from addiction medicine, but using it for vaccine hesitancy actually precedes the COVID pandemic. It is used for new parents who are considering forgoing their newborns' vaccines. Mandates are going to get some to get the vaccine, but for others, a conversation with their trusted physician can be a powerful thing. Dr. Joseph Weiner teaches us how to use it to help our patients with vaccination and other decisions. This is a flashpoint in our nation’s history and our positions as physicians endow us with a certain amount of respect and authority and with that authority comes responsibility. Responsibility to have difficult conversations. It is not uncommon for us to have difficult conversations, like breaking bad news, discussing an adverse health outcome, but these conversations are about others. We need to have difficult conversations about ourselves. We need to reflect on our own biases and how these can affect the health outcomes of our minority patients and career trajectories of our minority colleagues. In light of current events, I have chosen to rerelease episodes from earlier this year that should be thought provoking and allow us to reflect on the ivory tower, conservative institution that is medicine and how we can help to move the needle to improve health outcomes for our minority patients and the careers of our minority colleagues. One way to improve the former, is to improve the latter. Max Jordan Nguemeni Tiako is a 5th year med student (out of 5) at the Yale School of Medicine and hosts his own podcast, Flip the Script, about healthcare disparities. He starts out discussing how some of what we learn about race being a risk factor for some diseases are a product of social constructs, not genetic predisposition. We then discuss racial disparities in substance-use disorder treatment. We then pivot from health disparities to physician training disparities. We learn about the hardships faced by minority medical students from microaggressions from students and faculty to disparities in grading and how this can affect career trajectory. He ends by discussing some pearls he has learned from his own podcast. Mr. Tiako grew up in Yaounde, Cameroon, and moved to the US to attend Howard University, got a BS in in civil and environmental engineering and then a Masters in Bioengineering from the Georgia Institute of Technology. He is currently doing research connecting his civil/environmental engineering background to identify elements of urban design that can be leveraged to improve health outcomes and move the needle towards health equity. His research fellowship is at the Center for Emergency Care & Policy research at the University of Pennsylvania. He spends his free time writing about racism and medical education in the medical student magazine InTraining. Germ theory became popularized during that era, so this is when people realized that we were the vectors of our own illnesses. One would think that this would cause us to isolate ourselves from one another, like we are doing now, but the literature of the time, which reflects the thinking of the time, shows us that it brought us closer together. A message of hope in these dire times. Dr. Kari Nixon is an assistant professor of English at Whitworth University. She teaches medical humanities, Victorian literature, and is forever interested in death, disease, risk, and why we fear them. Dr. Nixon’s work has been shared on Huffington Post, March for Science, and more. Dr. Sacharitha Bowers went to medical school and completed her dermatology residency at Southern Illinois University. Dr. Joseph Weiner is an Associate Professor of Clinical Psychiatry, Medicine and Science Education at the Zucker School of Medicine at Hofstra/Northwell, where he co-directs the four-year curriculum in Physician-Patient Communication and Interpersonal Skills. Thinking and writing about how patients and clinicians communicate with each other has been a major interest in his career. He teaches us today about something I’ve been wanting to learn about for a long time. The tagline of this show is “everything we should have been learning while we were memorizing Kreb’s cycle.” This doesn’t hold true for this episode because Dr. Weiner teaches motivational interviewing to medical students; it just wasn’t being taught when I was there. We go over the basics of motivational interviewing, its origins, the spirit of it, how to go about it and then we discuss how it can be used to have a productive conversation about the SARS-CoV-2 vaccine. I haven’t had training in it, but I’ve used it already and it WORKS! You can’t use it to get someone to do something they don’t already want to do, but it helps them clarify their thoughts and hesitations and, using BJ Fogg’s language, either get closer to or over the action line. Dr. Weiner received his M.D. and his Ph.D. in Physiology and Biophysics from New York University. He did residency training in psychiatry at NYU/Bellevue and a fellowship in public psychiatry at Columbia College of Physicians and Surgeons. In addition, he pursued advanced psychotherapy training at the American Institute of Psychoanalysis. Dr. Weiner has received national and regional awards for his work as a clinician and educator. One he is particularly proud of was the 2015 Teacher of the Year Award from the Zucker School of Medicine at Hofstra/Northwell for the first 100 weeks of medical school. Dr. Weiner’s career interest in communication has expanded to the written medical narrative. He is writing a book about lessons learned from his late wife’s battle with cancer and is currently an MFA student in Creative Writing and Literature at Stony Brook Southampton. That being said, we need to be prepared, so we talk about what she should expect, like more mosquito borne illnesses, climate refugees, more issues with supply chains causing us to practice with scarcity. After training near me at SUNY Downstate in Brooklyn in neurology and neurophysiology, Dr. Scott Abramson moved cross country and joined Northern California Kaiser Permanente in July 1979 and retired from the Neurology department in San Leandro in 2020. For over 25 years, he had been involved in the communication mission at Kaiser Permanente, which all started at Toastmasters. He has been a physician coach and communications instructor. His two favorite courses were: “The Secret of Happiness” and “All the Wisdom We Need To Be Joyous Physicians Can Be Found in Country Music.” For over 20 years, he wrote a monthly column in our Kaiser newsletter on communication issues such as, “The Three Most Intelligent words a Physician Can Utter,” “Why I failed to cure my wife’s headache” and The Most Important Words One Can Say to the Bereaved.” Several of these stories have been published in medical journals. On today’s show we speak to allergist, Dr. Payel Gupta about penicillin allergy. Dr. Gupta is triple board certified in Allergy & Immunology, Pediatrics and Internal Medicine and currently has a practice at Ease Allergy in Brooklyn. We discuss how common penicillin allergy is and how commonly the diagnosis is incorrect. She goes through the four types of hypersensitivity reaction, and then focus in on type I, the IgE-mediated reaction. We go through presentation, treatment, and some commonly confused conditions. She teaches us how penicillin testing is done, why we can trust it and dispels some misconceptions about penicillin allergy. Dr. Gupta earned her medical degree from Michigan State University; and then pursued a residency in both Internal Medicine and Pediatrics at Rush University Medical Center in Chicago. She then moved to New York City where she completed a fellowship in Allergy and Immunology at the State University of New York, Downstate Medical Center. She is currently on the board of the New York Allergy and Asthma Society and serves as the treasurer/ secretary. She is also a National Spokesperson for the American Lung Association. It is a narrative-style series about the psychological and practical preparation for malpractice litigation, and she speaks nationally on the topic of malpractice litigation. We talk about whether we’d be willing to be friends with a personal injury attorney and her answer was surprising. However, expert witnesses are a different story. Unfounded lawsuits couldn’t happen without them, and they are supposedly one of us. If you are named in a lawsuit, we discuss all the players and how to comport yourself to improve your outcome. Theatrics often trump the medicine in this situation.
