Episode 4 of this season of The Podium is actually a rebroadcast of a conversation I had with Ted King on his King of the Ride podcast. I worked with Ted on pro cycling’s World Tour, and it’s been a pleasure to continue the relationship since. Conversations with him are always some of my favorite. Ted describes himself as a “Professional World Tour bike racer turned professional bike rider.” He now races across the US in numerous events each year, serving as an ambassador for cycling in general. He also has his own (delicious!) nutrition company called Untapped. Tom is the Senior Golf Fitness Specialist at one of the country’s premier golf facilities, and he works with numerous PGA Tour players. Adam Kerley, Nic Catterall, and I sat down with tacos and bourbon to talk through all aspects of high performance. No person can sustain a "pedal to the metal" pace of training for 52 weeks per year. An effective training plan allows for times of increased load, as well as for recovery. A given year or season must incorporate encouragement of varying physiologic processes, some of which should not be trained concurrently with others. The concept of "Periodization" is what allows for a training plan which is complete and goal-oriented.

One of the most frequent questions I receive from patients is, "How much exercise is too much?" We have all seen the stories or personally known someone who suffered a cardiac condition despite being extremely fit. Yet, it is well established that exercise is ultimately good for the heart. This raises the question of whether you can get too much of a good thing.
No one is more qualified to address this topic than Dr. Aaron Baggish, and he joins us on this episode of The Podium. 2. Antibiotics and Athletes

This time of year, a large portion of our population ends up on antibiotics. For those of us that are athletes or inclined to maintain a high level of activity, that can be detrimental. Indeed, antibiotics can cause as many problems as they can cure.
In the US, medical practitioners are notorious for prescribing antibiotics when there is clearly no indication to do so. Head colds and bronchitis are almost uniformly caused by viruses. Viruses are not the same as bacteria. Antibiotics treat bacteria, but they do nothing for viruses. In addition to those conditions for which antibiotics are useless, there are other illnesses which are bacterial but self-limiting. Said another way, your body is strong enough to deal with them on its own.
In this episode, we discuss the dangers and utility of antibiotics with a particular emphasis on how athletes and active individuals can be affected.

3. Continuous Glucose Monitor Use

Continuous Glucose Monitoring (or CGM) is a tool that we use with all of our patients at Podium, most of whom are not diabetic. This has proven to be a very powerful means of providing instant feedback into the body's response to certain variables. Food obviously plays a very important role in one's glucose response and metabolism, but the context around that diet is key as well. Exercise, sleep, and stress all impact glucose response.
In this episode we review some general use cases for CGM and talk about how we find value in these measurements for our patients.

Supplement quality and safety is an area of considerable concern in our practice. Due to the structure of laws in the US, there is very little oversight when it comes to supplement production. In fact, it is probably the case that much (if not most) of the products on the market are worthless or even dangerous.
In this episode, we talk about this very problem, but we also discuss how we navigate this market with our patients. There are safe options out there, but it is up to the consumer be educated on the topic.

5. Bicarbonate for Athletic Performance

Sodium bicarbonate ( or "bicarb") is a well-researched and proven supplement when it comes to promoting athletic performance. It is often used by the highest performing athletes, but there are barriers that keep others from trying this supplement. Its safety is well established. (Bicarb is just baking soda!) However, ingesting bicarb can lead to unpleasant gastric side effects.
In this episode, I am joined by Jeff Byers, the CEO of Amp Human. We discuss the history of bicarb in sports and the ways it is used to improve performance. Additionally, we delve into those side effects and a novel application of bicarb that might hold the key to harvesting the benefits while avoiding the standard side effects.

Jay literally wrote the book on running gait assessments: he is author of “Running Rewired” and "Anatomy for Runners", writes columns for numerous magazines, and has published over thirty-five professional journal articles and book chapters. Jay has had an active research career, and consults and teaches for numerous footwear companies, the cycling industry, the US Air Force, USA Track and Field, and USA Triathlon. His research work led him to a patented design to improve foot and lower leg function through his start-up company, MOBO.
Having taught in the Sports Medicine program at UVA and OSU-Cascades, he brings a strong bias towards patient education, and continues to teach nationally to elevate the standard of care for Therapists, Physicians, Students, and Coaches working with endurance athletes. His expertise in injury prevention, rehab, and performance training is sought out by athletes across the world, and his athlete’s results speak for themselves.

I sat down with Robbie Ventura to discuss what happens when you “bonk”, the complicated variables that impact it, and how you can prevent it. This episode was recorded for The Velocity Cycling Podcast, part of the educational component of Robbie’s newest venture, VQ Velocity.
Robbie Ventura is the founder and owner of Vision Quest Coaching. Robbie was a professional cyclist for 12 years. A competitive racer on the dirt, road and track since the age of 7, Ventura amassed over 70 victories during his professional career, was a member of the U.S. World Team and rode the Track World Championships in Hamar, Norway, placing 5th in the elimination.
