Lynette Sheppard is an OG in the menopause space. This former nurse/manager, artist, speaker, advocate has been, for 17 years now, the author of The Menopause Goddess Blog. As she says on the blog, The Change really whooped her upside the head.

Lynette straddles the space between allopathic and naturopathic worlds, and so she's right there in the moderate middle and has some really great insight about how to pull from both of them.  She's also on the other side of this transition and has some really key words for those of us wondering if we will ever make it and if we do, whether it will be better than this.

MK Czerwiec is a nurse and graphic artist (hence her handle, Comic Nurse) who edited 2020’s Menopause: A Comic Treatment. The book went on to be named on the New York Times’ year-end list of the best graphic novels and won two Will Eisner Comic Industry Awards, including one for best anthology. MK talks all about Graphic Medicine, an emerging field that uses comics to enhance medical education and patient care and help people process trauma and painful experiences. MK is also going through menopause, and she's very interested in finding ways to deal with hot flashes and some of the other symptoms we experience. And stay tuned, because near the end of the podcast she offers one of the best tips I’ve heard for creatively making a plan that we can turn to in our hardest, darkest times.

We hear this message all the time: compounding pharmacies and the bioidentical hormones they produce are dangerous and unregulated. But, are they? To find out Ann Marie turns to two people who are inside the industry, which accounts for about 1 to 2 per cent of drugs sold, and gets a very different story. They are Scott Brunner, CEO of the Alliance for Pharmacy Compounding, which represents compounding pharmacists and technicians as well as prescribers, educators, researchers and suppliers in the United States, and Jennifer Burch, a doctor of pharmacy and owner of the independent pharmacy her father started in Durham, North Carolina. Although this episode focuses on the compounding pharmacies industry in the US, there are takeaways that can be applied all over the world.

Jill Angelo is the co-founder and CEO of Gennev, a virtual care platform for menopause in the US. Jennifer Harrington is a naturopathic doctor based in Sydney, Australia. Amy Fleming is way too young to be going through menopause, but her attitude about it would outshine many of her much older peers. 

She was just 27 when a series of painful issues landed her in the hospital multiple times, and ultimately found out that she was going through primary ovarian insufficiency (POI, which I accidentally call "premature" in the intro) and ultimately, premature menopause. Now, at 29, she's sharing her experiences, raising awareness for the #makemenopausematter campaign, changing the face of menopause and smashing the taboo for old or younger people to educate themselves on this inevitable life transition.

Brooke Faught is a nurse practitioner and director of the Women's Institute for Sexual Health, a division of Urology Associates in Nashville, Tennessee. She knows a lot about testosterone and women’s sexual health because she uses it in her practice every day. I first came across Brooke when she presented her research and experience during the North American Menopause Society 2020 Virtual Annual Meeting, and then spoke to her again in 2022 for an update on advances in treatment and guidance.

Lynette Sheppard is such an important voice in the menopause space because she has such depth of experience and context for this conversation. The former nurse/manager, artist, speaker, advocate and more has, for 17 years now, been the author of The Menopause Goddess Blog. She started it, and gathered a group of 15 women around her, when "the change" started and  "really whooped her upside the head".

Kate Deering is a San Diego-based personal trainer, holistic nutritionist and author of the ground-breaking book How to Heal Your Metabolism

Kate has some no-nonsense guidance for healing broken metabolisms and how to make the transition through menopause as easy as possible by drawing on the pro-metabolic approach to nutrition. 

That involves learning to understand your body and giving it the fuel it needs to function properly. She explains why menopause symptoms, and slow metabolism, can both come as a result of over-exercising, eating too little and putting yourself under too much stress in earlier life, and dispels popular myths such as the idea that eating carbs will make you put on weight. I watched a friend of mine transform in her 40s by doing this, and I’ve done it myself and felt the difference. Her take is that throwing hormones and supplements at the symptoms of menopause was never going to be an effective solution. Instead, her take is that we need to learn to look at healing any underlying issues, and and nurture and heal our body so that it can go through the process feeling healthy and strong.

With so much hype surrounding menopause hormone therapy today, it’s very hard to know what or who to believe. 

There is a big push to portray HRT not only as helpful for menopause symptoms, but also as a preventative for cardiovascular disease. But does this bear out in the research we have so far, most of which lumps bioidenticals and synthetic hormones into one basket? That was the question of the latest literature review published in the American Heart Association’ journal Circulation this past Valentine’s Day – Rethinking Menopausal Hormone Therapy: For Whom, What, When, and How Long? – that looked at 96 papers, articles and studies. The authors included gynecologists, women’s health internists, endocrinologists, as well as Stephanie Faubion, medical director for the North American Menopause Society, all members of the American College of Cardiology Cardiovascular Disease in Women Committee. 

Kathryn Lindley, an MD and clinical investigator in the Division of Cardiovascular Medicine at Vanderbilt University Medical Center and one of the authors of the paper is my guest on the podcast this week to break down these findings.

I'm a journalist critical thinking my way through my own perimenopause.

In searching for good information, balance and context for myself, the Substack I publish on the subject, various social media accounts, and the interviews and solo episodes I do on this podcast, sometimes I just get floored at the things I see and hear on social media and in traditional media about menopause.

The reality is, I interview doctors all the time and they are knowledgeable and doing good work and passionate and all over the world, and they are also all over the place  on the basics, and have loads of blind spots.

But you wouldn't know that to look at social media.

The research is scant, considering half of women go through peri/menopause, and I just don't think as a journalist and a researcher there is room for this much certainty.

Hence, this episode.

Day raver, cold swimmer, rabble-rouser, educator, author, podcaster, academic and publisher of scientific papers: Joyce Harper is a professor of reproductive science at the Institute for Women's Health, University College London. 

She’s also author of the 2021 book Your Fertile Years and hosts the podcast Why didn’t anyone tell me this?

Harper is uncompromised and unflappable. She is all about the evidence. She’s not opposed to HRT but isn’t a breathless enthusiast either. It has to be about the evidence, she argues, otherwise it’s just stories and anecdotes that don’t add up to anything. 

NB: We are both members of a balanced-approach group called MenoClarity, and we recorded this podcast before Joyce was quoted in a controversial Daily Mail article alleging excessive and unsafe HRT prescribing practices at a chain of private clinics in the UK. 

When I followed up with her about it, she said: "We need to give women accurate information about the menopause and treatments. In the UK we've created huge negativity – some suggesting menopause is a disorder. It's not fair to women and I really worry for the next generations who are now nearing menopause. Menopause can be difficult for some. But for most it's a transition and life post menopause can be the most exciting and liberating stage of our lives."

Dr Quentin Oury is a leading British GP at a busy NHS service with 16,000 patients. He is focused on men’s health, including the areas of erectile dysfunction and low testosterone. He joins me to talk about a topic that doesn't get much air time, but we are slowly starting to hear more about: andropause.

It's a broad claim, but if I’m more clear on what is going on now that I’ve done it, I can guarantee you will be if you listen.  

Dr Kirstey Holland is a Melbourne, Australia-based doctor of Traditional Chinese Medicine who holds a double degree in acupuncture and herbal medicine. She is a practitioner of environmental and nutritional medicine, functional medicine and naturopathy.

Dr Mikhail Kogan came to medical marijuana via his work as a geriatrician at GW Medical Faculty Associates and as an associate professor of medicine at George Washington University School of Medicine & Health Sciences in the US. 

He co-wrote one of the most comprehensive books on the subject of cannabis in health care, Medical Marijuana, published in 2021. Although he’s not a menopause marijuana specialist (no one really is, yet), he has really helpful insights as to why the research still needs to catch up with what doctors are seeing in patients, coffee is more addictive than cannabis, and what “going low and slow” really means when it comes to experimenting.

Dr Angela DeRosa is a doctor of osteopathy, integrative physician and hormone specialist in Arizona. 16 Sep 202392: Why Paula Rastrick wants you to go low and slow on HRT01:05:32

Paula Rastrick is a British psychotherapist who was prescribed too-high levels of hormone therapy at a private clinic in the UK, which ended up causing more physical and emotional problems than she originally sought treatment for. 

She is also complex trauma survivor and classed as a Highly Sensitive Person and her mission is to raise awareness of both and how they impact menopause transition.

Her episode runs in the same week that the British Menopause Society released a warning about reports of high doses of menopause hormone therapy being given in the UK, and the syndrome known as tachyphlaxis. This is what happens when people taking drugs find the drugs to be less effective over time, and in the case of hormone therapy, can mean women with high estradiol levels may believe they need more estrogen, and also that women with higher levels of estrogen can experience adverse mood-related symptoms. 

Paula has create the brain body method of working with women on post-traumatic growth, resilience and healing.. She works with clients one-on-one and has just begun offering affordable monthly workshops that educate women about their body’s stress and trauma responses and gives them tools to handle and process it which you can access through her website.

We hear this message all the time: compounding pharmacies and the bioidentical hormones they produce are dangerous and unregulated.

But, are they? In an encore episode of Hotflash inc, to find out we turned to two people who are inside the industry, which accounts for about 1 to 2 per cent of drugs sold, and getsa very different story. 

Meet Scott Brunner, CEO of the Alliance for Pharmacy Compounding, which represents compounding pharmacists and technicians as well as prescribers, educators, researchers and suppliers in the United States, and Jennifer Burch, a doctor of pharmacy and owner of the independent pharmacy her father started in Durham, North Carolina. Although this episode focuses on the compounding pharmacies industry in the US, there are takeaways that can be applied all over the world.

Dr Vikram Sinai Talaulikar is a specialist in reproductive medicine at University College London Hospital’s NHS Foundation Trust and an associate professor in Women’s Health at the University College London. Dr June Chin knows about pain and how to treat it on the deepest levels: from her own experiences that started in her teens years and through to that of patients. 

She’s an integrative medical cannabis physician and has spent more than20  years working in holistic medicine. 

She is currently a professor at New York University and Syracuse University teaching Cannabis Health and Science, Vice Chair for the Cannabis Advisory Board for the New York State Office of Cannabis Management and Chair of the Health, Safety and Research Committee for New York State’s Office of Cannabis Management. She is the author of the book Cannabis and CBD for Health and Wellness and founded and operated seven bicoastal integrative cannabis medicine clinics in California, New York, and Connecticut.  

Her training includes studying nutritional biochemistry at Cornell University, osteopathic medicine at Touro University, medical acupuncture at Harvard University, and research at Columbia University, Center for Complementary and Alternative Medicine in Women’s Health. 

Through her work as a clinician, professor, and author, she has created medical protocols for the industry and internationally accepted methods for cannabinoid formulations, research, education, and training programs. She is also the chief medical officer for NNABI.

I started the Hotflash inc podcast two years ago on a wing and a prayer and a glass desk, and quickly learned that is the worst possible surface for audio.

Along the way I have spoken to the top experts in the world, found out an incredible amount of things, and refined and iterated the podcast intensely, getting it to the point that I'm pretty sure it's – and me – are ready for our own slot at SiriusXM, or at least a bunch of other improvements.

I have two power players in this perimenopause space: half of the four-person team that founded Wile, “a plant-powered hormonal and emotional wellness brand for women 40+ that reframes midlife as a time of peak power”. (You will hear the word “tincture”)

Introductions: Gwendolyn Floyd is a venture-backed serial social entrepreneur – five companies and counting – and Wile’s co-founder and CEO. The title of this podcast is “4 women go through perimenopause” because not only is Gwen a perimenopausal woman, but she watched her mother be buffeted by this transition too. Julie Kucinski is Wile’s co-founder and chief marketing officer, coming from the worlds of beauty and wellness and some of the biggest brands of our time. American actress Judy Greer and former journalist-turned-entrepreneur Corey Schlolibo round out the foursome, and they’ve also got some pretty influential investors along for the ride, including US tennis star Serena Williams. 

I did this interview back in the summer, but the conversation has legs. We talked about the current reality of this space, what’s happening with women, how the options facing us are complementary, what we face on the job and in our home lives and what it means to square up with the heroine’s journey of menopause all still stand. Dr Vonda Wright is an American double board-certified orthopedic sports medicine surgeon, who has spent a career working with high-level atheletes and immersed in clinical research.

Sjanie Hugo Wurlitzer and Alexandra Pope are pioneers in the emerging field of menstruality, which my spell check says isn’t even a word! Basically they have created a new lexicon, along with a new approach not only to our health and wellbeing, but our creativity and leadership, and our spiritual life – all based on becoming conscious of the power of monthly cycles. They are the co-founders of Red School, and co-authors of Wild Power: discover the magic of the menstrual cycle and awaken the feminine path to power . Their menopause book Wise Power: Discover the liberating power of menopause to awaken authority, purpose and belonging came out in 2022. I tried to interview them then, sent a few emails, didn’t hear anything back. And then, they wrote to me – and as you will hear, they did it right at the perfect time. 

Between Sjanjie and Alexandra they have more than 45 years of experience doing this kind of work, and today they teach around the world, helping women understand that from puberty to perimenopause and beyond, we are engaged in a bio-psycho-social-spiritual process of maturing… A message that is very, very different to what most of us are used to getting. 

The discussion covers the practical and cyclical nature of menopause, the significance of menstrual cycle awareness, and the power of self-care and setting boundaries. Dr Bibi Ayesha Lockhat is a Traditional Arabic and Islamic Medicine (TAIM) Specialist practicing in the Middle East at Zulal Wellness Resort at China Som – Qatar, the only place in the  in the world to be reviving this traditional holistic healing system.

She holds two degrees from The University of The Western Cape (UWC), a Bachelor of Complementary Health Sciences and a Bachelor of Complementary Medicine with a Major in Unani Tibb Medicine.

TAIM dates back to 1025, the Greek philosophers, Muslim physician Ibn Sina and his famous The Canon of Medicine. The crucial difference is the focus on balance over the Western tendency to suppress symptoms. 

She tells us what TAIM is all about, how it works with prophetic foods mentioned in Quranic scripture, why it’s crucial in diagnosis and subsequent therapy, and we all might want to learn about how it works based on four humors and six lifestyle factors to fuel physis – our body’s own innate ability to heal itself. 

She talks about how TAIM relates to, incorporates and differs from other practices, such as Traditional Chinese Medicine, Auyrveda and Western medicine. And then there is her TAIM-based approach to midlife imbalances of the body and acceleration of inflammatory process – for men in andropause and women in perimenopause – using lifestyle interventions, which is dramatically different that what we hear in North America. (She might have used the phrase “tunnel vision”.)

She believes TAIM as crucially important in both diagnosis and subsequent therapy and is well-versed and knowledgeable on several ‘hands-on’ therapies, such as massage, acupressure, aromatherapy, and therapeutic cupping. She enjoys offering and treating patients/guests with an extensive range of natural herbal medicines registered for specific ailments.

You may have been hearing a lot about testosterone lately – what it is, why we need it, why we can’t get it… I’ve gone to Melbourne, Australia to ask an expert in women’s hormones across our lifespan, and one of the few people who has studied testosterone in-depth. 

Professor Susan Davis is a clinician researcher, consulting endocrinologist and head of the Monash University Women’s Health Research Program in Melbourne.

She is going to help us to tackle the current confusion – and hype – over testosterone, armed with what she knows best: the evidence. This week Florida orthopedic sports medicine surgeon Dr Vonda Wright published a review paper Climacteric, the journal of the International Menopause Society, introducing Musculoskeletal Syndrome of Menopause. The paper has been shared widely across social media, and this interview with Dr Wright is an edited version from last year diving into Dr Wright’s efforts to create awareness of this constellation of muscle and joint problems experienced during perimenopause. 

This is an issue close to Dr Wright’s heart: it was the first thing she went though in perimenopause.

Hotflash inc founder and host Ann Marie McQueen sits down with Dr Leigh Erin Connealy, a leading figure in integrative medicine, known for her unique approach in combining conventional and alternative treatments in addressing chronic illnesses and cancer. Dr Connealy is the founder of the Center for New Medicine and the Cancer Center for Healing in Irvine, California and author of two books: The Cancer Revolution and the Be Perfectly Healthy. She is also co-founder of leading hormone brand, Raena Health.

She talks about why so many people are getting cancer, how to make chemotherapy more effective and less lethal, why prevention needs to be the focus. For Dr Connealy, the question we need to be asking: what is going on with your terrain, and why is it making cancer? Everything from the food we eat to the chemicals we surround ourselves with to the emotional work we aren’t doing is causing this. And find out what she thinks our number one risk is – why and what we can do about it. (It’s probably not what you are thinking)

Dr Connealy wants us all to know that in most cases, cancer is 10 years in the making. She has really good guidance for women considering hormone therapy, explaining the differences in synthetic hormones and the importance of the “orchestra”, including progesterone and pregnanalone, and more than one kind of estrogen. She explains why there is no one solution that fits all women – even though that’s what mainstream medicine wants us to believe – and gives guidance on the particular cocktail, with tweaks, that she likes to prescribe. She also gives her take on whether hormone therapy prevents cancer – its worth listening to the end just to hear what she has to say about that – and addresses that ongoing fear women have, which is weighing our fear of breast cancer risk when considering hormone therapy.

Rebekah is the founder of Buff Bones, a medically endorsed exercise method for bone and joint health with on-demand classes and trained instructors in more than 30 countries.
