08 Sep 2020Fix the Magnesium Deficiency Before the Potassium Deficiency!00:11:28

When patients have hypokalemia, you want to fix it. So you give potassium to make the numbers pretty.  You go ahead and give some potassium and do what the protocol says to do: recheck. Labs come back, poor patient got stuck for these labs. Potassium burned a bit on the way in because the IV was acting funny. Low and behold, the potassium did not go up. What just happened? Where did it go? Did you check the magnesium level? Probably not.

In this episode, we discuss the importance of correcting magnesium deficiencies in critically ill patients before correcting potassium deficiencies. Magnesium plays a crucial role in regulating potassium levels in the body. 22 Oct 2021End-Expiratory Occlusion to Determine Fluid/Volume Responsiveness00:12:29

There are various means that we can use to determine fluid/volume responsiveness in our critically ill patients. These include PLR with SV changes, SVV, PPV, VTI, etc. End-expiratory occlusion testing can be performed on patients who are on mechanical ventilation. In this podcast episode, we discuss the use of end-expiratory occlusion (EEO) as a method to determine fluid and volume responsiveness in critically ill patients. We explore the physiology behind EEO and how it can be used as a non-invasive, bedside tool to guide fluid management in patients with varying degrees of fluid responsiveness. We review the current evidence supporting the use of EEO and its limitations, as well as potential future developments in this area. The ROX index is defined as the ratio of oxygen saturation as measured by pulse oximetry (SpO2)/FIO2 to respiratory rate (RR). It stands for Respiratory rate - OXygenation. ROX ≥ 4.88 at 2, 6, and 12 hours means they're likely in the safe zone. ROX < 2.85 at 2 hours, ROX < 3.47 at 6 hours, ROX < 3.85 at 12 hours means the patient likely needs to be intubated.

In this episode of the podcast, I discuss a new study that found that hydrocortisone can save lives in patients with severe community-acquired pneumonia (CAP). I discuss the findings and what they mean for the treatment of CAP. In addition I also discuss the risks and benefits of using hydrocortisone, and who should be considered for treatment.

CAP is a common and potentially life-threatening infection of the lungs. It is caused by bacteria that are inhaled into the lungs, and it can affect people of all ages. The most common symptoms of CAP are fever, cough, shortness of breath, and chest pain. In severe cases, CAP can lead to pneumonia, sepsis, and death.

The study that we discuss in this episode was a randomized controlled trial that looked at the effect of hydrocortisone on mortality in patients with severe CAP. The study found that patients who received hydrocortisone were less likely to die than those who did not receive hydrocortisone. This finding is significant because it suggests that hydrocortisone may be a life-saving treatment for patients with severe CAP.

Cardiac power output is a calculation one should do when caring for patients with cardiogenic shock. Data has shown it to be the strongest hemodynamic correlate of mortality in cardiogenic shock. We should be using it in our practice to determine whether a patient will benefit from mechanical circulatory support (MCS). In this episode, we explore the concept of cardiac power output (CPO) as a potential tool to help guide management in patients with cardiogenic shock. We review the underlying physiology of CPO and its potential advantages over more traditional hemodynamic parameters, such as cardiac output and mean arterial pressure. We also discuss the practical aspects of measuring CPO and the potential limitations and challenges of its use in the clinical setting.
