10 Mar 2025E158: Meera Shah - Living with Multiple Myeloma00:46:42

Trigger Warning: This episode discusses cancer, cancer treatment, cancer relapse, mental health, cancer during pregnancy and living with late effects of treatment.

Description: In this powerful episode, Naman Julka-Anderson and Jo McNamara sit down with Meera Shah. Meera trained as a dentist and following a range of roles as a junior, focused on the treatment of patients with special needs.

Whilst expecting her first child she noticed increasing pain in her chest, back and shoulders, with weakness of the upper body over time. When her son was 6 months old, she was diagnosed with a solitary bone plasmacytoma of the sternum. This was treated with radiotherapy. Due to the large tumour size, recovery was slow and Meera took many years to regain some upper body strength. She had to leave her role in clinical dentistry and go back to the drawing board. In 2018, Meera progressed to multiple myeloma, with multiple tumours in both femori and tibia. This was treated in 2019 with chemotherapy and an autologous stem cell transplant. She has since been on maintenance chemotherapy. There was a discussion of potential radiotherapy to both knees, but fortunately wasn’t required in the end.

Unable to return to clinical practice, Meera completed her Diploma in Healthcare Professions Education. She now works part time in postgraduate dental education at NHSE and also delivers freelance education in the cancer space. Her primary focus is to educate healthcare professionals on the impact of living with a chronic condition/incurable cancer, not just on patients, but to the whole family, especially children. Her main message is to ensure patients are given personalised care based on their own personal needs. She also volunteers for Shine Cancer Support, supporting their network for young adults from minoritised ethnic backgrounds, and is a trustee for National Voices.

This episode is a deep dive into Meera’s cancer diagnosis and subsequent treatments, her family life and how things could have been improved for her care.

Episode 158: part of the Living with and beyond cancer series(part 47).

10 Oct 2022E63: Nick Bennet - FIKA a Mental Fitness Platform00:53:44

**Trigger warning**
Naman Julka-Anderson andJo McNamara catch up with Nick Bennett as part of the Education and Workforce Development Series (Part 18)

Discussion about mental fitness and the FIKA mental fitness platform.

To use this podcast as CPD, look at these reflection points:

1) Mindset. Reflect on the notion of Mental Fitness. Consider that much like physical exercise and fitness you can build and improve your mental health through regular mental activities as an individual and even better, collectively as part of a team. It’s a common misconception that mental skills like confidence and positivity are simply character traits, you either have it or you don’t. This is not true. These mental skills that can be learnt and improved with practice of brief psychological informed activities. Reflecting on this potential you have to improve your mental strength is a growth mindset.

2) Behaviour change. While considering this growth mindset. Reflect on the behaviours you and your teams have when working together. Consider yourself as a role model. Your behaviours are contagious and by role modeling you have the potential to encourage a growth mindset within your team.

3) Commit to act. Like any other form of exercise, you need to act to build your mental fitness. As a role model in your team commit to starting a gratitude wave in an upcoming meeting with your team. How does this work? Simply request a few minutes at the start of the meeting and explain to the team you are about to start a gratitude wave. Choose a team member and publicly declare some gratitude for them, recognise them and share gratitude for what they contribute to the team. Then ask that person to choose another colleague and share gratitude for them and so on. Once everyone has spoken, ask how everyone feels.

To use this podcast as CPD, look at these reflection points:
1) reflect upon your practice in preparing paediatric brain tumour patients, how you can you familiarise them with the space / machines / processes to reduce anxiety and give them greater control over their treatment?
2) reflect upon your communication and use of language with paediatric brain tumour patients, how could this be adapted to help them to understand the process / feel more at ease / be inclusive of communication challenges they may now face? Can you identify what percentage of information is delivered direct to parents to explain, compared to the percentage of information you deliver direct to the child - does this require rebalancing?
3) reflect upon your / your centre’s practice around younger children (< 8) and general anaesthetic; consider the pros cons of GA and the implications for children with brain tumours, and weigh up the implications to treatment against prognosis and the ‘whole’ child.

Government figures show that uptake of bowel, cervical and, in some cases, breast screening is lower among ethnic minorities than among their white counterparts. In ethnically diverse areas, the attendance of bowel screening is 38% compared to up to 58% in other areas, and women from ethnic minority groups are more than 50% less likely than white women to attend a cervical screening. Triple negative breast cancer, an aggressive form of the disease, disproportionately affects black women [2]. Other cancers which, according to Cancer Research, are more prevalent in the black community are myeloma (a type of blood cancer), womb and stomach cancers, while both black and Asian people are at higher risk than white people of contracting liver cancer. Black men are twice as likely as white men to get prostate cancer in their lifetime [3], but, said Oladele: “Men from the BAME community struggle to self-refer to doctors when they first notice signs and symptoms, despite being in pain.”
