Want to know how to get your physician to listen to you during an appointment?

Tired of leaving an office visit feeling depleted and ignored?

Listen up because in today's episode we are discussing ways to help you feel seen and heard. 

 

It is important to prepare for your appointment by setting a goal for your intended physician. This means that each appointment needs to have a small and specific goal that you want to achieve. Once this is set, you can collect the necessary information so that you can give a detail, but concise history to your doctor. Finally, give yourself space to think about the appointment. Get clear on how you feel and what steps you will take until you see that physician again. Be clear on your boundaries and what your outcomes will be. If you can bring someone with you to help with advocacy. At the end of the day, remember everyone has different perspectives. It is your job to have empathy and use this information to help you get where you ultimately want to go.

When my Son was diagnosed with Club foot, it was a lot of processes. I was scared and miserable at the thought of losing out on the post-partum period I craved. I was most worried I could not handle it. Through help via counseling, friends, and medications, I was able to manage the depression and lead my son into a successful treatment even though I was doubtful of my ability to manage all of it. 

This podcast is about how I accepted his diagnosis without having to feel the pressure to "overcome it." I did not have to be the mom that loved the treatment options available to me. I did not have to be vital for anyone else but me and my son. Acceptance was not an easy battle, and it was a real struggle for me. I hope this podcast gives you a little experience about how to look at your diagnosis.

Success does not happen overnight but it compounds massively as you continue to learn how to show up for yourself daily.

When dealing with depression it is important to have an action plan. The best time to create an action plan is when you are not in a depressive state and that is the goal of today. If you are currently struggling with depression, this exercise should be done under the supervision of a licensed health provider. 

It is important to know what are your warning signs for a depressive episode or for your other mental health disorders. These are things that usually cue you into the know that you need to support yourself today. Second, you should have a list of coping strategies that help you feel better. You should have a list of reasons you need to live. Reasons could be people in your life or events that are coming up or work that needs to be done. You are here for a reason. You are worthy of this life. Next you need to have a list of people you can contact when you are feeling down. Finally, create a list of places you can go to distract yourself~ this can be a park, a library or mall.

In my podcast, I, Dr. Kailyn Galloway, open up about my personal journey as a mother and share my reflections on the challenges and transitions that come with pregnancy, postpartum, and becoming a mom. I've realized that many of my life transitions revolve around pregnancy and motherhood. I want to use this platform to support and connect with fellow moms who may be going through similar experiences.

During moments of contemplation, I realized that the stories I want to share are centered around motherhood's mental and emotional aspects. It's not just about physical parenting techniques or guidelines but about the mental health transition that occurs when you become responsible for another human being. There are countless decisions to make as a mom, and I want to create a space where moms can exercise their autonomy and make choices that align with their values.

Despite having medical training, I found myself overwhelmed and constantly seeking guidance during the early stages of motherhood. I believe that having a mentor or support system would have been invaluable during that time. It's essential for moms to know it's okay to feel afraid and to be uncertain, and to disagree with others. Each mom has her own path to follow, and I want to provide advice and encouragement on embracing that journey and stepping into one's own power.

 

In the podcast, I touch on various topics, such as the physical changes of pregnancy, postpartum emotions, and the challenges of making decisions that may cause discomfort for our children but are ultimately for their well-being. I share my experience with my child's clubfoot treatment, a difficult decision with long-term benefits. Being a mom involves constantly navigating our children's emotions, communication, and development, which can be challenging and rewarding.

Through my podcast, I aim to create a sense of community and reassure mothers that they are not alone in their struggles. Whether it's dealing with postpartum depression, finding empowerment as a mom, or embracing the changes in our bodies, I want to provide support and resources. Motherhood is a transformative journey, and I hope to be a source of guidance and understanding.

    • A serious mood disorder after childbirth, more intense and longer-lasting than baby blues.

    • Can start anytime in the first year after childbirth, lasts longer than baby blues.

    • Hormonal changes, history of depression or mental illness, stressful life events, lack of support.
  • Management Tips:
    • Talk to a healthcare provider immediately, counseling or therapy, medications if prescribed, support from family and friends, support groups.

Sometimes getting sleep postpartum means sleep training and using available resources. Still, it usually involves setting boundaries, asking for help, creating routines, and readjusting your expectations daily.

4️⃣Unconscious Competence: You can manage their postpartum emotions and challenges with consistent practice and coaching. It becomes an ingrained part of their nature.

In this episode, I discussed how I planned my meals postpartum. It is ideal to plan some freezer meals during the end of the 3rd trimester. The meals last about three months in the freezer and can be used through that 4th trimester to help keep you from eating out when you are overwhelmed and exhausted. Nothing is more comforting than a warm meal after a day of crying (I don’t just mean the baby).  

I used three different resources to help me get meals completed: 

This resource gave me recipes to try for my postpartum. They are easy to make and delicious. My husband and I love them. And then you don’t have to worry about meal planning 

Send this to your friends and family so they can provide you with meals. This app lets your friends tell you when they can drop off food. And for those with family out of town, it can send you a gift card. If you feel awkward asking friends for help (which you shouldn’t!), have a close friend send it out. 

This meal service is excellent. It takes less than 30 minutes to cook the meals and minimal groceries. I felt great; The meals were healthy and delicious. I was very thankful my husband got it for me. 

Eating during postpartum is hard. You are so focused on the baby and their needs (and catching up on sleep whenever possible) that you forget to eat. And some of you might be thinking, there is no way I could forget about the food! But from one foodie to another, yes, Yes, you can.

How to ask for help as a new mom? The answer is to prepare yourself and be ready with these three things before entering the postpartum experience! 

Lesson 1-  Plan your meals 

  • Create snacks, freezer meals, and meal train. 
  • Have groceries delivered if possible, or schedule you to go out one day a week. 
  • Have snacks by the rocker, bed, and in the diaper changing station to remind you to eat 
    • Make them healthy; make them protein heavy.y

 

Lesson 2-  Plan your visits 

  • Schedule visits 
  • Have a preset list of chores you will need help with 
  • One chore per visit
  • If you are uncomfortable with holding a baby - babywear!

Lesson 3-  Plan for alone time. 

Part of asking for help is also essential to ask for time to be alone. You have to build that relationship with them.

In today's podcast, I tell you about the struggles of nursing during postpartum. Breastfeeding can be challenging, even though it should be second nature. I shared how I managed a difficult start to my breastfeeding journey. I discuss how I handle tongue ties with my son and ways I would do postpartum differently in terms of breastfeeding. I give some breastfeeding tips, whether finding the right breastfeeding positions or having compassion for yourself.  

As always, I tell you who you should contact, even if you are at your wit's end. The main point is if you want to breastfeed, you can breastfeed. Period. But also, you should give yourself grace and know you can still connect with your child even if you do not choose to nurse.

You got this, mama! Nursing is not easy. Postpartum is difficult, but I am here as you go through each step in this beautiful journey.

In this episode, I address how to make a tough decision as a new mom. From choosing circumcision or not to seat belts and rear-facing car seats to give up coffee because your toddler wants to join you every morning, the choices can be easy and difficult.

I understand the challenges of being a new mom, as I experienced it firsthand. Two weeks into my son's life, he couldn't latch, and I was a crying mess. I share this personal story to emphasize the problem and make you feel heard.

But I want to offer you a message of hope. There is a solution to this decision-making struggle. I break it down into three steps to allow you to follow along.

Step #1 is to assess your feelings, both bodily and intellectual. Take a moment to ask yourself what you're feeling, where you've felt this way before, and what specific conflict you're worried about.

Step #2 involves journaling or discussing feelings to create an action plan. Open your mind to different possibilities and make decisions with open and closed-fisted approaches. Consider what you'll do when certain situations arise.

Step #3 is all about giving yourself grace. Remember that the emotions you feel are valid. Find ways to support yourself emotionally while making decisions for your child. It's essential to distance yourself from mom groups that disagree with your boundaries and seek support from your family, especially your partner. Take a deep breath and believe in yourself—you've got this!

As a mom with a child going through clubfoot treatment, judgment was assumed everywhere I went. My son was wearing casts from toe to thigh as an infant. People automatically assumed something happened. My gut reaction was a total shame, as people assumed I did something to my child. I harmed him or neglected him in some way. But in reality, I was making a huge sacrifice of my son's postpartum snuggles so that he could walk. 

 

In this episode, I walk you through the steps I took to help myself deal with judgment from other mothers clearly so that you do not have to take on the judgment of others. 

Ignore [8:12]

The easiest thing to do is to let people think whatever they want about you as a mom. You cannot control others' opinions and know you are doing an excellent job. But this can be super challenging because, as a new mom, you are filled with doubt. But I am here to tell you, you can ignore others' judgments. You do not owe anyone a reason for making your choices as a new parent. Period.

Educate [9:19] 

Sometimes I wanted to take the time to tell people about my son’s condition, and this is an excellent time to let people know that sometimes their questions are not as benign as they think. It is never ok to assume I harmed my child, even if they are undergoing medical care.  Remember, you do not owe anyone an explanation for how

you choose to parent, but sometimes there are windows of opportunity to help people grow. 

Look in the MIrror [10:29] 

Ugh, I hate this one.  Yes, sometimes moms would come up to me and say, " My son has a clubfoot. You are doing a good job, Mom. And they had been staring at me for 20 minutes. That whole time, I had been judging them, thinking, how dare you judge me and my son’s treatment? And then I realized they were empathizing with me!  How are you letting yourself feel others? How are you assuming someone is judging you?  I learned this from the Judgement Detox from Gabby Bernstein. I encourage you to check that book out for more details on clearing judgment. It is not easy, but it is so worth it.

Hey, there, Mama!  Welcome back to the New Mom Naturopath Podcast. This is your host, Dr. Kailyn Galloway, and today, we're unpacking something profoundly personal and incredibly important in Episode 70: "What Are Intrusive Thoughts and What Do They Mean About You as a Mom?"

 

Before we dive in, a gentle heads-up: today's episode touches on intense content that is not intended for little ears. We're going to talk about intrusive thoughts, mainly those violent, that can occur in the postpartum period.

 

Have you ever found yourself in a standard, everyday situation, like grabbing a cup of water in the kitchen, when suddenly, a terrifying thought flashes through your mind? Thoughts of harming your baby in unimaginable ways? If you've experienced this, you're not alone, and you're not a bad mom. These thoughts are called intrusive thoughts, and they can be terrifying.

 

Let me share a story I heard recently. With her three-week-old baby, a mom went to the kitchen to get some water. As she moved across the room, her mind was bombarded with horrifying images—of the blender, the knives, and other ways she could harm her baby. She was distraught, telling me, "I thought about putting my baby in the blender."

 

These moments are heart-stopping. But it's crucial to understand, as we did in our conversation, that "the clinical significance is the level of suffering, rather than the content of the thought itself." Your distress over these thoughts is a sign of your deep care and concern for your baby's well-being. It reassures us that these thoughts are anxiety-driven, not psychotic. Your worry about these thoughts is a very good sign.

 

Why Do These Thoughts Happen in the Postpartum Period?

 

Transitioning from being responsible only for yourself to caring for an entirely dependent little human is monumental. It's okay to miss your old life and feel overwhelmed. Add in the lack of sleep and the immense pressure to be a "perfect" mom, and it's a recipe for anxiety that can lead to these intrusive thoughts.

 

Interestingly, these experiences can differ across cultures, with white women and minorities experiencing and expressing these thoughts in varied ways, influenced by their unique pressures and societal expectations.

 

Treatment Ideas

 

- Get Solid Sleep: It sounds simple, but rest can significantly reduce the intensity of these thoughts.

- Self-Care: Time alone, showering, and eating nutritious food are essential. Support, stability, and basic needs can help, but even with these, feeling overwhelmed is a common trigger for intrusive thoughts.

- Self-Forgiveness:  Be gentle with yourself. You're doing an incredible job in a challenging role.

- Mindfulness: Techniques like the Think/No-Think test can help train your brain to focus intentionally. When disturbing thoughts arise, get curious about them. What's your mind really trying to process?

 

I follow Corrine Crabtree’s 4N’s when I have a thought I want to process and work through:

- Notice: Acknowledge the thought.

- Normalize:  Remind yourself that many new moms have these thoughts, which doesn't make you a wrong person.

- Neutralize: Understand the context—tiredness, stress, baby's crying—it's all playing a role.

- Next Best Thought:  Shift your focus to self-care and understanding why these thoughts are popping up.

 

Remember, Mama, having intrusive thoughts doesn't mean you'll act on them. It's a bizarre, distressing part of the postpartum experience for many, but there's help, and there's hope. You're not alone, and you're not a bad mom. You're human and navigating one of life's most complex transitions.

I'm also trying to manage a life where I have a two-year-old. I have a husband who works all the time, and I am also trying to get credentialed with insurance to make more income as a naturopath.

 

In this episode, I gave a little peek into the window of what it's like to manage 1,000,000 and 1/2 things, which most moms are dealing with daily, and postpartum is no exception.

 

I wanted to be clear that this is not me to give you a pity party, but this is more of a way for me to say you're not alone and I'm here to help even though your thoughts are hard and your thoughts are complex.

But know that being down or feeling crazy, you're feeling mentally unstable, is also temporary.  You won't feel like this forever and are not alone, Mama. 

Also, it’s ok to cry. Postpartum isn’t easy, but you can still love it.

I had a scheduled C-section so some of this advice may be slightly different from what you have seen from other mothers.  I made sure that everything I needed during postpartum was within reach so that I could be present with my baby while healing. In this episode, I talk about what I had in my postpartum cart and how I managed to make sure I ate during the postpartum period. This is a time a healing, so it is vital that as a new mother you allow yourself to have everything you need to take care of your baby so that you can be the best mother to your new baby. Whether that means food or extra clothes, I share my experience in this episode.

Postpartum is a time with a lot of demands on you, mama. There is so much to think about, learn about, and manage. You will often realize that prioritizing your mental and physical health will be complex. 

In this episode, I dive into the three most common reasons you aren't going to prioritize your health needs as a new mom. 

1 - TIme 

There are so many demands put on you as a new mom. You will only have so much time and won't be able to take your needs first. 

2 - Your biological need to take care of the baby

You will tend to forget you need to take care of your baby. This is normal and natural to do.  

3 - Lack of routine 

You don't have a clear set of patterns to make your day predictable and form the habits you need to make yourself a priority. Make habits small and manageable, even more so than you need. 

 

And then I give you tangible advice for how to combat this postpartum!

This podcast discusses water consumption during postpartum and how much water you should drink daily. Remember, water is not something you just obtain from the water you drink, but foods you consume. Also might spill some beans about that coffee and how it affects your hydration status! (and the research is pretty funny on this).

Make sure you are drinking adequate amounts of water, but here is the punch... there is no universally accepted amount of water you must consume daily.  Moral of the story: Drink when you are thirsty; drink the amount of water that makes you feel better!!

Introduction to Oversupply:

It’s common to stress over every drop of milk you produce. Watching those pumping sessions closely and seeing only a few ounces can be disheartening, especially when it feels like everyone else has a freezer full of milk. You might think having an oversupply would make life easier, but let's unpack why it can actually be quite the opposite.

I personally had a significant oversupply, and trust me, it was nothing short of chaotic. It was messy—milk leaked on everything all the time, leading to constant discomfort and a never-ending laundry pile.

The Downsides of Having Too Much Milk:

  1. The Mess Factor: 
  2. Increased Risk of Clogged Ducts: 
  3. Greater Susceptibility to Mastitis: 

Reducing milk supply intentionally can be a delicate and frustrating process. I'll discuss some strategies that might help, but remember, it's often a trial-and-error journey.

While it might seem like having an excess of milk would ease all your breastfeeding woes, the reality can be much more complicated. I've heard countless tips and tricks over the years about boosting milk supply, and it can be overwhelming to sift through all the advice. Today, we’re going to cut through the noise and focus on the most effective strategies backed by physiology and expert advice.

First, we'll dive into the physiology of milk production and the let-down reflex. Understanding these biological processes can help you optimize your body's natural ability to produce milk.

Next, we'll explore the infant sucking reflex and its crucial role in stimulating milk production. This will give you insight into how your baby's feeding habits influence your milk supply.

The primary concept we'll discuss is the idea of 'empty to increase supply.' This means increasing the frequency of breast stimulation—whether by nursing or pumping—rather than extending the length of each session. It’s about how often, not how long.

Additional Ways to Boost Supply:

  • Adequate Hydration: While staying hydrated is essential, you don't need to overdo it. We'll debunk some myths and set realistic expectations for fluid intake.
  • Adequate Nutrition: We'll discuss the importance of maintaining good nutrition, continuing your prenatal vitamins, and ensuring you're getting enough protein and overall caloric intake.

Lastly, I highly recommend talking to an International Board Certified Lactation Consultant (IBCLC). They can provide personalized advice and support to help you understand and increase your milk supply effectively, without adding to your stress.

In This Episode:

 

I talk about why anxiety in postpartum is so common and that if you experience a heightened amount of stress, that can be normal. I want you to understand why your body is experiencing anxiety for a multitude of reasons

  1. Changes in hormones in your body (rapidly!)
  2. Learning something new under a lot of pressure
  3. You are sleep-deprived like you have never been sleep-deprived before.
  4. Most commands in your life take the form of WaHH and Ahh.
  5. You have no self-autonomy as your life is run by a dictator that has left function than a sack of potatoes (because at least you can eat those).

 

You have every reason to be anxious as a new mom. You are learning at a rapid pace, and the steaks are high!

 

So when do you start to reach out for help?

 

Before you think you need it.

 

When you are unable to manage your life around it.

Treatment options: Talk therapy or cognitive behavior therapy.

Herbal treatments for postpartum stress

Exercise and diet.

And yes, even pharmaceuticals.

 

You are going through a lot as a new mom. You have a lot of things going on around you. It is ok to take action to help you build the foundation of mental health for your family.

Mindfulness needs to be a core fundamental need for new moms as they enter postpartum. In this episode, I will give you the real reasons to prioritize mental health during postpartum. 

 

  • Daily challenges

As a new Mom, You will face new challenges every day. Your child will be growing, and so will you every single day. I swear you will wake up each morning and go, “Holy crap, you grew,” and your child will have grown right before you. It is insane. How do you navigate through all those changes? You have mindfulness practice to help you sort out your emotions and your fundamental needs. 

  • Physiological Changes are happening: 

So you have to keep in mind that your body is changing very quickly in many different ways. Most importantly, your body is rewiring what is happening in your HPA axis, which is your stress response pathway. 

  • You’re Not Going Backward 

Goodness, you will grow emotionally, mentally, and physically as a new mom. Undoubtedly, you are either creating better habits for your mental health, or it will be neglected as your child gets old. Building these habits now is super important as you grow into your role as a new parent.

On today's podcast, I got personal about how I feel about being a mom I am. I had a tough week this week.

 

I've been trying to get my business up and running. I'm trying to get this podcast to have more views so that I can help more women like you.

I'm also trying to manage a life where I have a two-year-old. I have a husband who works all the time, and I am also trying to get credentialed with insurance to make more income as a naturopath.

 

In this episode, I gave a little peek into the window of what it's like to manage 1,000,000 and 1/2 things, which most moms are dealing with daily, and postpartum is no exception.

 

I wanted to be clear that this is not me to give you a pity party, but this is more of a way for me to say you're not alone and I'm here to help even though your thoughts are hard and your thoughts are complex.

But know that being down or feeling crazy, you're feeling mentally unstable, is also temporary.  You won't feel like this forever and are not alone, Mama. 

Also, it’s ok to cry. Postpartum isn’t easy, but you can still love it.

Remember, your well-being matters, and we're here to support you every step of the way.

  • EPISODE TITLE AND NUMBER: The Baby Won't Stop Crying:3 ways to center yourself now 

 

In this episode, I discuss 3 tactical tips to help you regulate as your child won’t stop crying. During the fourth trimester, you and your child are learning how to communicate with each other. And to make it more interesting, they can only cry. As a new mom, this can be super triggering when you are only trying to soothe your child and get them back to sleep. There are many ways to help calm your baby down, but they won’t work if you are in a state of panic. Trust me; they know exactly how you feel. They can sense fear from a mile away. During my postpartum, I used these strategies to get you in the right mindset so that you can care for your baby today. 

  • Box Breath: 

This is a straightforward strategy to get you to regulate your vagus nerve to reduce your heart rate and stop you from feeling overwhelmed at the moment. Box breath is 4 counts; hold for 4 counts, breath out for 4, and hold for 4. You should repeat this around 6 times, which guesses about a minute. No matter how many tears are flowing, you can take 1 minute to ground yourself! I promise you it’s worth it! 

  • Mantras or affirmations: 

Yes, this is gonna sound very corny. As I would say to my husband when we first started dating, “Ugh, that's cheesier than cheese!” But I promise you these little phrases can help you when you are buried in the messiest of diaper changes with dinner burning on the stove. So in this section, I give you mantras to help you in the times of chaos you will surely face as a new mom. 

  • Get some alone time:

You do not need to feel ashamed if you need a minute to recoup. This means ensuring your child is in a safe space whether that is a carrier, a jumper, a crib, or a pack-n-play, and then leave the room for a few minutes. Sometimes the only way to reset is to get away from the stimulus.  Hint: this is a great time to do some of that box breathing!

Go for walks! This is super easy and will not only help you feel better physically and emotionally, but it will also help your baby feel better too. The moments I really needed a walk were when my son was crying like crazy. I made sure to keep this realistic  

  • Topic #2 -  Get Sunshine!

You have just done a number of you pelvic floor, your abdominal muscles and your lower back. Not only did you go through labor and delivery, but you completely altered your anatomy to fit that adorable snuggle monster inside you. It is so important to focus on core recovery. Of course you need to get clear from your doctor first!!

Regular exercise also boosts serotonin and dopamine levels, neurotransmitters critical for maintaining a positive mood. Engaging in various forms of exercise, such as aerobic activities, strength training, yoga, and outdoor pursuits, not only helps manage anxiety but also provides a sense of accomplishment and mental resilience. In this episode of "The New Mom Doc" podcast, I, Dr. Kailyn Galloway, discuss birth plans and the healthy and not-so-healthy ways of approaching them. I share my personal experience and shed light on the anxiety that can arise from striving for a perfect birth plan.

Initially, I hoped for an unmedicated, natural birth at an outpatient birth center. However, my pregnancy had complications, forcing me to change my plan for my safety and mental health. Each adjustment meant mourning the loss of the pregnancy I had envisioned.

It's important to address more than just personal desires in birth plans. While we would love to get everything we want, the reality often differs. I experienced challenges like painful contractions without progress and exhaustion due to medical school commitments.

Setting boundaries when seeking advice and support is crucial. I made it clear in my birth plan who I would accept advice from and engaged in open discussions with my healthcare team to ensure my needs were met.

I believe organizing plans is similar to the birthing process. Birth plans act as guideposts amid the chaos and pain of labor, helping us stay focused on our heart's desires and providing direction in difficult moments.

Preparing for worst-case scenarios and having action steps in place is essential. It's also crucial to have plans for the postpartum period, including emotional support and seeking help if needed.

Acknowledging and processing emotions is vital when birth experiences are not as desired. I personally struggled with my child's birth and postpartum period, realizing the importance of allowing myself to feel and express my emotions, even if they deviated from my expectations.

Hey there, beautiful Mama!  It's Kailyn Galloway from the New Mom Naturopath podcast, and today, we're diving into the oh-so-complex world of infant sleep. You know, that elusive state we all wish our little ones would blissfully drift into more often. It can be really hard to have open and positive conversation with your partner during postpartum. You do not have the most well-rested mind and while you have the best intentions of being kind and courteous, lack of sleep can make you say some crazy things.

In this episode, I give you tangible tips for creating open communication during postpartum so that you can avoid some of those postpartum fights. I will talk about when to have a discussion with your husband about postpartum, how to create code words for getting help from each other and how to create a plan for repair after a fight. 

The postpartum period is filled with a lot of love for your new born. While it is a time of love and happiness, it is also a time of a lot of stress and sleep deprivation. Peripartum depression is challenging for mom, without a doubt. But it was hard for my husband to help me through my 3rd trimester. One of the best ways he helped me through this mental health crisis was by telling me it was out of his comfort zone to give me support. He gave me the push I needed to get help from mental health professionals and reach out to others. I had to realize that I was in charge of my mental health and no one else could take the actions step for me.  

  • [6:10] - Your mental health is NOT your partner's responsibility 

Postpartum depression is hard on your partner and you, and recognizing that it is not their responsibility to fix your mental health problems. Working with your partner to create boundaries on what they can handle and cannot do for you is really important. The next step is to respect these boundaries and determine what steps to take to improve.

While depression is so dark and scary, it is essential to remember that your mental health is not dependent on anyone. It is necessary and motivating to realize that while you are in this moment right now, you can get yourself out of this trial despite how bleak it may seem.

I struggled with accepting help from my healthcare team. They wanted me to increase my antidepressant dose primarily because of the increased blood volume during pregnancy. Intellectually I knew I needed to increase my amount, which would be helpful. However, I resisted increasing my medication because I thought it made me a failure. I could not handle my mental state without the help of my medication. In retrospect, I wish I had increased my dose sooner to feel better. In what ways are you rejecting the service that is being offered to you? What is the roadblock? Why are you stuck? If your future self was better than you are today, what would she say to you?

    📍 I'm here. I'm kind of sick, so I'm gonna have to play a lot of pause, restart, pause, restart on this to avoid editing. But today I really wanted to talk about the common things that happen which make breastfeeding or nursing very difficult for new moms. And At first, I want to talk about what's normal. So, what's typically normal is that the first 24 hours, your child's going to have between 2 and 10 milliliters of breast milk intake.

And technically speaking, this is not breast milk, this is what is called colostrum.  And it's the very sticky first part of the  production of milk in the breast tissue. And it's got a very high density, high caloric density and high amounts of nutrients for your child to really wake up their, their bowels and to wake up their body and to give them the nutrients they need to maintain their glucose levels, to maintain  all the changes that are rapidly happening in the body.

It's actually really. Like, the physiology of childbirth from the baby's perspective, what they have to go through in terms of changes of their physiology is fascinating and I'm probably going to do a, an episode on that in the future,  but  during this day they're going to have two to ten milliliters of feeding.

So for all you Americans, two to ten milliliters of feeding is only    ten milliliters is two teaspoons.   Which is not very much. So, we're talking about half a teaspoon to two teaspoons  in the first day. Now, they're going to get that over numerous feedings.  Which seems kind of crazy, and you kind of will feel during this time that nothing is coming out or they're not getting enough.

In reality, they don't need very much to be able to be satisfied in that first 24 hour period.  What's really important during this time frame is that they actually do have their first bowel movement, which is called  meconium.  It's the first bit of  of poop that your body will, that your baby will ever secrete.

And it's really important that it passes within the first 24 to 48 hours.  And so your  Care team is going to be looking for that and making sure it occurs. And then between 24 and 48 hours, you'll have the infant take anywhere from 5 to 15 milliliters, which is 1 to 3 teaspoons of colostrum. And they'll probably have around 2 bowel movements in the second day.

And it should be this dark green to black, and it's sticky.   And then after that, we have between 48 and 72 hours, they're going to have between 15 and 30 milliliters.  And this is when you'll have a huge spike  of 6 to 8 stools per day.  And that stays fairly consistent throughout the newborn phase.  The poop in this part, the color of the stool, is going to start changing from green and moving to a light mustard.

seed.  It's totally normal for it then to have green colored poop until  96 hours or 4 days after birth.  After 5 days,  the breast milk should definitely be in. So the patient, the    infant will be taking in around 60 to 120 milliliters of breast milk per day.  Or per, not per day, per feeding, and that is  2 to 4 ounces.

Most newborns are going to be around that 2 to 3 ounces per  meal.  But the crazy thing about this, you're sitting there going,  but how do I measure that?  How do I measure, how do I know if they're going to be getting enough? Because it's coming out of me, I'm not like measuring it before it goes into their mouth. 

So there's a lot of milestones  that happen in the first five days that people look at to make sure that you're actually meeting targets for feeding your baby enough.  It's super difficult  to   measure without some sort of external help. So for a totally healthy baby,  newborns are going to feed between 10 and 12 times per day.

They're going to have  about 6 milliliters of breast milk per feeding after that first 24 hours, and eventually it'll increase, like I said, to go. Greater than. After the first couple of weeks, your baby is going to gain  about 105 to 210 grams per week, which is  around 15 to 30 grams per day.  Milestones that some physicians will look at is going to be is the patient or is the infant  Losing more than 7 percent of their birth weight, that's kind of a red flag that a clinician is going to look for.

And then, is the  infant losing weight after 3 days of life?  When they latch and they are nursing, the baby isn't actually swallowing, there's no sound of the swallowing.  Which, it is a very nuanced sound. So,  at first you probably as a new mom are not going to know about what it is.  So make sure when the lactation consultant comes to your room, you ask them to like kind of walk you through when you hear it and to help you get the pattern of what is actually going on during a feeding. 

If they have fewer than six wet diapers a day after four days of life, if they have fewer than three stools per day after four days of life, that's kind of a red flag. And then minimally.  Or the minimal breast changes after five days. So if mom starts to not have her milk come in, which is when you have  a significant change in the structure of your breasts, and the  all the milk comes, it's, I don't know how else to describe it.

It's  quite crazy. You'll wake up one day and you will have boobs that look like they've had   some real good work done and they'll be really painful. So if that doesn't actually occur after five days, usually it happens between two and three days.  And then if you actually end up having nipple pain as a result of latching,  there are many reasons why nipple pain would be associated with a latch issue. 

But if that is occurring, there's reason to have that explored, either by a lactation consultant immediately, or going to your pediatrician for some extra support and guidance, and seeing what they are saying.  So in terms of a good latch, what does a good latch look like? How do you know if you're doing it right?

You know, the one thing I will say before I do any of this, you are learning. And baby is learning in this moment in time. And when you're learning something for the first time and you don't know what you're doing, you feel really dumb. So it is completely normal in this instant, especially during nursing, to feel stupid  and to be really frustrated that things are not going the way you want.

Give your brain the rest it needs. Go, hey   I knew I was going to have this issue.  I'm going to learn how to overcome it, rather than think you're a failure for not being able to have the immediate latch that you desire.  So signs of a good positioning for latch. The infant's nose is free from the breast.

How a baby actually ends up nursing is they have their nose free so they can breathe through their nose while they're swallowing, not while they're swallowing, but while they're taking in milk.  The infant's chin should be on the lower end of their breast, the breast tissue.   And it should be touching.

The infant's cheeks should be  rounded. They shouldn't be sucked in. Like, it shouldn't be as if the baby is drinking out of a straw. It should be, they should be flat.  And then their mouth needs to be open like a yawn.  There shouldn't really be a lot of areola seen, so your nipples should be like all the way in their mouth.  

And then, the infant's lower lip needs to be flanged outward. So, no curling the lips inward. That will cause, that will show that you don't really have a great latch. And they won't be able to get that sucking   feeling.  They need to get   The infant's body should be in line with the head, and facing towards mother.

Now, this is kind of a caveat. This is only if you're talking about cross playing position. There are many, many, many different types of nursing positions. I encourage you to Google it and to go on YouTube to find different ways that you can get comfortable. But the most common position, especially the one that most lactation consultants start out with, is cross lying position, where you lay the baby across your chest and you have  The side that their head is on that arm is underneath their head supporting them and you have one arm over the baby to try to guide them  Babies are strong  So you'll be really surprised at how much they'll fight you  But that extra arm can help you to guide them in the way that they need to go lastly  the mother's Should not feel pain after the initial 30 seconds to a minute.

So, if you're just, you've never had someone nurse on your nipple, it's going to hurt a little bit. And I don't  mean, like, agonizing pain, but it is an uncomfortable sensation.  Especially if it is an improper latch.  It will last past that minute long  feeling. And so if that's happening  You definitely want to go talk to a lactation consultant, definitely want to get someone to assist you in how to go about  fixing the latch, whether that be something in your positioning or something in your  the development of your child's mouth, something of that nature. 

And then the last thing that is a really good sign of a good latch is a rhythmic suck and swallow pattern.  And I think that this is something that I didn't have real knowledge of, but there is a very definitive, like.  pattern that you will notice your infant  undergoing, I don't know how else to say  during a nursing session, and it really is helpful to know that they've got a great latch if you know what you're trying to listen for.

So asking your lactation consultant to point out the rhythmic suck and swallow pattern that is present. when an appropriate latch is formed. 

So how do you know if your child is getting enough milk from you? Well, you're meeting the milestones of regaining,  they're regaining weight  consistently every day, and then at the two week mark they've actually started to increase  in  back to their birth weight.  Those are really good signs.  being able to nurse consistently without fussiness and making sure that your latch is really good can be really solid signs that you're getting enough milk.

Now, if you're concerned and you feel like something's wrong, there's no reason why you shouldn't call a lactation consultant who can come either. You go to their office and they work with you in their office about how to get the appropriate latch, how to get comfortable,  and then do a  a weighed milk session.

So they weigh the baby before without a diaper, and then you nurse, and then they weigh the baby again, and then they kind of establish how much they're actually getting each nursing session, which can be really helpful for letting you know that they're actually getting the two to four. ounces each time from me, because it's really kind of hard, especially when you stop having the engorgement issues, like, because when,  you can kind of feel your breast tissue emptying, but it's sometimes really difficult to know how much was there to begin with, and as you regulate later on,  your body will regulate to your child's needs, and whenever you're actually If you're going to start nursing, you'll notice that you'll get more engorged or have a larger amount of milk supply in your, in your breast tissue versus times when your baby doesn't typically nurse. 

And so if you feed on demand, it will stimulate to have your body to have the exact amount of milk that you really do need for your one infant. Or if you're having multiples, your body can adapt and learn to produce more based on how many you've actually had.  But, there are some really common problems that can happen with moms that are new to nursing  and one of them would be an infection.  

A lot of times if you're having pain either outside of nursing and or during nursing, a lot of times the, there is sometimes where you'll get lesions of sores that end up being infected and because it's a tissue that's being disrupted. It's very sensitive, you've got a lot of hormonal changes, and then you've got a child that's learning how to eat.

And even if they don't have teeth yet, it's, they're, they've got very strong jaws.  And there's always the chance that you're going to have some form of infection, either bacterial or fungal. So, if your skin starts to look really red, you start to have salmon colored nipples  You have some sort of discharge, anything like that that doesn't look like milk, obviously.

Or if they start to burn or itch, those are all real, real positive signs that you probably do have some sort of infection. But it can also be that you have some sort of dermatitis because you've inflamed that area and your body for some reason is creating more of an inflammatory response.  And so all those are signs that you should probably talk to your physician about a  medication.   

You can have them do a compounded cream called triple cream and it can be applied to your nipple to prevent an infection and or to treat one. So that's something that you can ask for.  Other issues that you can have are engorgement. And that sounds so fun, doesn't it?  It's actually really hard for a baby to nurse when your breasts are full.

And so those first couple of days where you have your milk come in and you are engorged, it's very difficult for you to get the appropriate latch.  So in those days, it's really important that you watch for engorgement. If you're having a difficulty latching, if your baby is having a difficulty to attach, and or your flow is really strong, two things that you can do is self express into a  A towel, or a pad, or   like a nipple shield of some sort, or collect that into a    a haka cup, which is kind of like a manual.

It's like a, a leak catcher, if you will say.  Breast milk is very leaky. It gets everywhere. Especially if you overproduce, like I did.  It gets everywhere.  It's everywhere.  But a haka can be placed and you can do a couple of expressions, a little bit of expression of the milk. That can help reduce a lot of the overwhelm some infants get from you having a really heavy flow and also change the shape of your breast tissue so that they can actually get an appropriate latch.  

You also can use a hand pump because that is really helpful in reducing the size of your, of your breasts so that they can actually do really well.  The other thing is if you are trying to pump in those first six weeks, there's no problem with that,  but  the more you stimulate your tissues to secrete milk, the more milk it's going to secrete and it's not going to necessarily be regulated to your child or infant's needs and it can produce an oversupply. 

And an oversupply sounds like this amazing thing until you experience it  because it can lead you to having Milk just sitting in your ducts of your breast tissue, and when that happens, it can result in a clogged, clogged duct, which is painful, but also leads you to be more susceptible to things like mastitis. 

Which is when you have an infection within the breast tissue itself, and that has to be treated with antibiotics. And it's actually really easy for that to become a very serious infection. And so, you can get a fever, nausea, and it actually can lead to sepsis if it's not treated appropriately.  It's pretty scary and a reason for hospitalization.

So oversupply is not necessarily the goal. Even though we watch on Instagram, there's these people with freezers of breast milk. It's not, one, it's not necessary and two, it's not necessarily a good thing. They're having to continue  to produce that much because their body is, if they stop, they can actually  have engorgement and lead to, be more prone to having  clogged ducts and having infections, which is no fun.

And so, those are common reasons why you'd have pain. The last kind is going to be called a vasospasm.  And this kind of happens for a variety of reasons, most of them we don't actually know why.  But there is a lot of vasodilation and vasoconstriction that happens in order to get the blood, the  the blood and the milk, because it all is from the same fluid.  

In order to get the milk out of the tissue sometimes, there ends up being an extra   constriction of the blood vessels in some women and so there actually can be a discoloration of the nipple to turn kind of blue and Because of the lack of blood supply and the lack of oxygen it can result in a lot of pain  and so  It usually if it is a vasospasm the pain occurs after you have  After  You're done nursing, and after nursing is over, and it also happens in those that are in colder environments.

So if you're colder,  if you're not in a warm environment, it can happen more often. So,  the way that they treat this is usually to help you get a warm pad on your nipple after you are done nursing. Make sure you're in a warmer environment. And then also if there are medications you can take to help reduce the pain afterwards. 

The other concern that some women have, and this is still a very rare disorder, but sometimes  the changes in hormones that are happening as a result of you nursing.  prolactin and oxytocin, some women have a very negative response to that, whether they get a sense of vertigo, which is the room spinning around you, or intense nausea, or just a feeling of a very hot feeling in their head. 

And  there are also some women that get intense amounts of depression. And  sadness and doomsday feelings as soon as they start to nurse. All of that can be something that you should probably discuss with your physician. But it is, it is something that can happen. It's very rare.  And so,  I just say all of these things because I want you to be aware that there are going to be issues that come in between you and your desire to nurse.

If you want to have a relationship with your child that involves nursing, involves breastfeeding,  then by all means, there's no reason why you shouldn't be able to.  But,  some people have to try exceptionally hard to get that to work. And so sometimes you're going to have to decide where your commitment levels lie and whether or not that's a priority for you.

There is nothing wrong with not nursing your child.  As much as people on the internet will try to shame you for that, there's no reason why in the 21st century we should not have formula for you available. And while there have been shortages, there has been formula available and  it's  There's nothing wrong with using that as a way to feed your baby.

It's not a cop out. It's not easier.  And honestly, comparing what I spent on pumps and cleaning supplies and bottles and, you know. Pillows for nursing, and pillows for me, and pillows for baby, and like all kinds of gadgets, like nipple shields, and  kind of like massagers, and all kinds of things that were superfluous, like they were not necessary for my nursing journey.

But I was so, so insistent that I needed to nurse, that I had every single thing that I purchased in order to  just if I keep moving and keep trying, and not actually giving my child.  formula. And I didn't get a trophy. Like there was no trophy at the end of the day for me choosing to nurse.  None.  So,  making the decision to nurse is a commitment.

It's a decision to stick with it and to learn  and to suck at it at first. Pun intended. 

But there is no shame in choosing a different plan, a different game.  And I wish someone had told me that.  Because I really struggle with that in postpartum. There was a lot of emotions going on in my brain, and I really struggle with that.  Lastly, I will say that if you're having difficulties with latch, even if your pediatrician doesn't recommend it, it's always a good idea to have your child assessed for a tongue tie, or  lip ties, if you  really don't feel like they're getting the appropriate latch.

And the appropriate people to Assess for this, it would be a international board certified lactation consultant  or  a dentist that is certified to look at   tongue ties in general.  And I will also say this, that I had the pediatrician look at his tongue tie  and. One, a couple of them in the hospital just dismissed it and moved on.

One said, hmm, we probably need to clip that. And we actually even had our pediatrician clip his tongue tie in office.  And it was ineffective. It still didn't change our  latch issues. And so going to see a lactation consultant, they actually assessed that it wasn't actually cut enough during that first initial   Phrenectomy is what it's called. 

And so we had to go in and get it done via a laser at a specialty clinic. So  I will say that if you have had issues with LATCH, it's always a really good idea to go to someone that actually specializes in that specific treatment option instead of just going to a pediatrician who is a  very knowledgeable, amazing resource for so many things. 

If you're having a very, very tailored or specific issue, sometimes they're not going to know how to help you. And breastfeeding and nursing is something that, if it's a common issue, they're going to know how to troubleshoot. But if you're having something that's abnormal, they're probably just going to give you some samples for formula and tell you to be happy.

No, they're not going to be like, my, my pediatrician was super supportive. But,  it's not going, they're not going to get the solution to your answer if you don't go to the right source.  All this information, I'm going to say it very clearly, is all,  it's all going to be educational and it's not recommendation by your doctor.

Of course, follow the guidelines of your pediatrician, wherever you're at in the world, and make sure that you're following their guidelines as to how to go about your nursing. journey and your baby's feeding schedule and overall health. I hope this was a really informative episode and I know it was longer than my usual, but I think it was really fun to dive into why you should really know ahead of time that the journey of nursing is not a simple one and it's not always going to be straightforward.

Well, today we’re breaking down one of those supposedly "simple" tasks that turns out to be anything but: diapers. That’s right, we're answering all those diaper-related questions you might be too embarrassed to ask because, honestly, no one expects you to know all this stuff right off the bat.

Introduction to Diaper Basics: Diapering can seem straightforward until you're a few days in with a newborn. Suddenly, there are so many questions: What type of diapers should I buy? How do I ensure a good fit? And yes, the big one—what do I do about nighttime changes?

Personal Story: When I was a new mom, I thought diapers would be the least of my worries amid nursing, sleep deprivation, and decoding baby milestones. But as it turns out, even "simple" things can get complicated. Today, we'll unpack these everyday challenges and share some practical tips.

Key Diapering Tips:

  1. Choosing the Right Diaper: We'll discuss what to consider when picking out diapers for your little one, from absorbency to material.
  2. Ensuring a Good Fit: A well-fitting diaper can prevent a lot of common issues. I'll tell you what signs to look for to make sure the diaper fits just right.
  3. Handling Nighttime Diaper Changes: To change or not to change in the middle of the night? That's a big question. I'll share insights on how to decide what's best for your baby and your sleep.
  4. Recognizing When It’s Time for a Change: Learn to identify tell-tale signs that it's time for a fresh diaper before discomfort or a rash sets in.
  5. Preventing and Treating Diaper Rash: I’ll offer some tried and tested tips to keep your baby's bottom happy and rash-free.

Hindsight is 20/20. And nothing could be more accurate about the postpartum period. I was worried about postpartum because of my son’s diagnosis. So I feel like I planned more than the average woman. Even still, I found that there were still things I would do differently. First, I wish I had made more of a routine out of going on walks and getting out of the house. I got into a routine of sitting. Mainly because I was healing, but I wish I had made more of an effort to get out to see the sun. Second, I wish I had done more core work after my c-section earlier. I wish I had prioritized scheduling friends to come over before my baby got here, as it was too much for me to handle after my son got here. I would love to get more time before my son goes into treatment for clubfoot. I wish I had created a plan to get back into a normal eating pattern after battling gestational diabetes. 

 

  • Topic #1  [2:48] - Walk more

I live on the 5th floor of my apartment complex. So this was so hard for me to commit to doing during postpartum. But I wish I would take the time to get outside and work out more. 

Topic #2 [3:46] - Delay clubfoot treatment 

This is one I have mixed feelings about. I felt so much pressure to get his clubfoot treatment started. And I am not sure it would have been better for me or worse to delay his treatment. But I wish I had more time with those cute feet and his snuggly self. I wish I had more time, and looking back, I think I would have delayed his treatment until he was four weeks, even though people were advising me otherwise. 

  • Topic #3 [5:15] - Focus on core health 

I still have a postpartum belly. My core is still weak. And as a result, I have back pain, and the scar tissues from my C-section are still sore. I wish I had taken more time to work on my core sooner. It’s not fun. It is not sexy. But it is so healthy. 

 Topic #4 [6:10] - Focus more on my diet. 

I had gestational diabetes. And it was so hard to stop myself from eating all the carbs as soon as my baby was here and safe. I had to restrict my diet so much during pregnancy. But I wish I had started creating habits early in my child life to have those already established. 

  • Topic #5 [8:00] -  Schedule more time with people 

I had a lot of doctor's office visits with my son's treatment, and my struggle with nursing those first two weeks. Because of that, I did not make plans to have people over, and I wish I could have more human contact in this isolating time.
