04 Nov 2024 | HOT TOPICS: The ORANGE Study With Dr. Giuseppe Citerio | 00:38:28
In this episode of Hot Topics, Dr. Nicholas Morris interviews Dr. Giuseppe Citerio, professor of anesthesia and intensive care at Milano-Bicocca University, about the ORANGE study, which investigated the neurological pupil index (NPI) for prognosticating outcomes in patients with acute brain injury. The study, involving 13 centers in Europe and North America, found that repeat abnormal NPI values, including zero, were predictors of poor outcomes. The study collected data every four hours for seven days and used a sophisticated statistical model to account for longitudinal changes. Despite initial expectations, the study did not find a strong correlation between the NPI and ICP, suggesting NPI should not be used as a surrogate for ICP. Future research may focus on developing a standardized response to low NPI values. 10 Jul 2024 | MASTERCLASS: Critical Care EEG Monitoring | 00:34:55
Electroencephalography (EEG) is an important and relatively inexpensive tool that allows intensivists to monitor cerebral activity of critically ill patients in real time. Seizure detection in patients with and without acute brain injury is the primary reason to obtain an EEG in the Intensive Care Unit (ICU); and in response to the increased demand for EEG, advances in quantitative EEG (qEEG) created an approach to review large amounts of data instantly. Today, rapid response EEG is now available to reduce the time to detect electrographic seizures in limited-resource settings. In this week's MasterClass, our hosts Dr Jon Rosenberg and Dr Stephan Mayer are joined by Dr Lawerence Hirsch, Co-Director of the Critical Care EEG Monitoring Program at Yale School of Medicine to discuss the evolution, value, and impact of brain monitoring with EEG in the critically ill.

16 Aug 2023 | Live at Annual Episode 3: Clinical Trials - Early Drain with Dr Stefan Wolf | 00:14:38
Aneurysmal subarachnoid hemorrhage (SAH) may be complicated by delayed cerebral ischemia, which is a major cause of unfavorable clinical outcome and death in SAH-patients. Delayed cerebral ischemia is presumably related to the development of vasospasm triggered by the presence of blood in the basal cisterns. To date, oral application of the calcium antagonist nimodipine is the only prophylactic treatment for vasospasm recognized under international guidelines. In retrospective trials lumbar drainage of cerebrospinal fluid has been shown to be a safe and feasible measure to remove the blood from the basal cisterns and decrease the incidence of delayed cerebral ischemia and vasospasm in the respective study populations. However, the efficacy of lumbar drainage had not been evaluated prospectively in a randomized controlled trial until Dr Stefan Wolf and his colleagues at Charité – Universitätsmedizin in Berlin conducted their study. Dr Wolf joins Dr Nick Morris to discuss the study's results. .

16 Aug 2023 | Live at Annual Episode 1: Defining Blood Pressure Control in Acute Stroke with Dr Ali Seifi | 00:09:39
Several measures of blood pressure (BP) control have been used including time to reach target BP, maintaining BP within target range, reducing BP fluctuation, achieving goals with monotherapy and avoiding multiple anti-hypertensive agents, achieving goals with minimum dose adjustments, avoiding discontinuation due to side effects, and preventing hypotension. There are multiple methods of quantifying these measures. The clinical relevance of various measures particularly relationship with acute kidney injury, and death or disability in patients with acute stroke or traumatic brain injury is not well understood. A better understanding of these measures is essential to identify the ideal therapeutic agent and for comparison of effectiveness between various antihypertensive agents. Dr Nick Morris is joined by Dr Ali Seifi, Director of Neuro ICU at University of Texas Health whose session at the Meeting focused on the need to review existing data to evaluate the relationship between various measures of BP control and acute kidney injury and death or disability in stroke patients.

03 Jan 2024 | CURRENTS: Dr Brian Gilbert & Dr Katie Qualls | 00:18:59
The transition from alteplase (ALT) to tenecteplase (TNK) in the management of acute ischemic stroke (AIS) has been an exciting and thoroughly discussed topic within the neurocritical care community. Although debates on the clinical efficacy and safety of TNK for AIS have subsided somewhat in light of recently published data, what remains lacking is a clear “road map” for institutions on how to properly implement the transition in thrombolytics. Brian Gilbert, PharmD, and Katie Qualls, PharmD, join Dr Lauren Koffman to discuss the transition from ALT to TNK for AIS. You can read Brian and Katie's February 2023 article in Currents at https://currents.neurocriticalcare.org/Leading-Insights/Article/transitioning-alteplase-to-tenecteplase-for-acute.

27 Mar 2024 | CURRENTS: POCUS - A Call to Action | 00:18:14
Point-of-care ultrasound (POCUS) is increasingly recognized as an essential skill for critical care providers, intended to enhance clinical assessments to improve patient care. Beyond its adaptation for procedural guidance, bedside ultrasound assessments can provide critical diagnostic information in a timely manner to guide real-time decision making for the acutely ill patient. An accumulating body of evidence has shown that POCUS can improve clinical outcomes and lower medical costs. Over the past decade, there has been widespread adaptation of bedside ultrasonography by various critical care specialties, with respective governing societies issuing guidelines and position statements encouraging the use of POCUS. However, the field of neurocritical care has yet to fully embrace this innovative field, and neurocritical care training programs vary in their efforts to provide accessible, well-structured educational programs for trainees. This week, Dr Lauren Koffman is joined by Dr Thanujaa Subrahmaniam and Dr Judy Ch'ang to discuss the use of POCUS in neurocritical care and the opportunities to extend its application across NCC programs. You can read Dr Ch'ang and Dr Subrahmaniam's Currents article on POCUS in NCC at https://currents.neurocriticalcare.org/Leading-Insights/Article/point-of-care-ultrasound-education-in-neurocritical-care-a-call-to-action.

07 Feb 2024 | CURRENTS: Dr Jacob Yakoby | 00:18:51
The pandemic has finally been declared ‘over,’ though COVID-19 continues to linger. The phenomenon of burnout has been well documented pre-pandemic, in many cases due to understaffing. This was exacerbated by the severe strain of COVID-19 placed on the healthcare system, with nurses leaving in droves post-pandemic. While nursing professional organizations and unions have long advocated for formalized standards around nurse staffing in a variety of care settings, there has traditionally been stiff opposition, particularly from hospitals, nursing homes, and certain healthcare associations. Dr Jordan Yakoby joins Dr Lauren Koffman to discuss nurse staffing - a public health crisis in the making. 05 Jun 2024 | CURRENTS: Innovation for Health Disparities Research in Hemorrhagic Stroke | 00:33:31
As clinicians working with patients who have sustained an acute brain injury, we are naturally focused on caring for the injury itself. Until recent years, the clinical severity of brain injury was regarded as the main driver of prognosis and outcomes. However, we are beginning to understand how non-clinical factors, including the social determinants of health (SDOH), contribute to differential risk for all brain injuries including hemorrhagic stroke, and continue to impact our patients’ recovery even after injury. In this week's episode Dr Lauren Koffman is joined by Dr Nirupama Yechoor and Dr Rachel Forman to discuss how understanding the complex interplay between clinical and social determinants of health is crucial for clinicians caring for hemorrhagic stroke survivors, not only for recovery and secondary prevention, but also to effectively counsel families on primary prevention. Lauren is also joined by Mike Foster, a member of Yale's Stroke Patient Advisory Board who talks about his experience as a stroke survivor. You can read the Currents article at https://currents.neurocriticalcare.org/Leading-Insights/Article/innovation-for-health-disparities-research-in-hemorrhagic-stroke. 03 Jul 2024 | CURRENTS: Consent for Brain Death Determination - the ends justify the means... or it does not. | 00:21:57
The purpose of informed consent, as it exists now, is primarily to maintain patient autonomy, but additional benefits include the protection of patients, avoidance of fraud, and promotion of rational decisions, amongst others. It is now considered both an ethical and legal obligation which should, at a minimum, always include: 1) the nature of the procedure, 2) the risks and benefits of the procedure, 3) reasonable alternatives, and 4) risks and benefits of alternatives, with an assessment of comprehension of these elements (1).
