In Part 1 of this mini series, Ellisa introduces what “authenticity in music” means and builds a case for the importance of musical authenticity in music therapy.

For those that don’t know Ellisa, she is a board certified music therapist with her bachelors in music therapy from Drury University and has further training in NICU-MT. To learn more about Ellisa listen to her interview over at The Music Therapy Podcast: Episode #14.

Show Notes:

The questions to ask yourself when preparing music are:  

What are the essential parts of this song that make it what it is? 

What components of the music can I replicate? Rhythm, pitch, melody, chord structure, runs/licks. 

What can I leave out and still have a recognizable version of the song? (ex: sweet home Alabama) 

Am I doing my best? The reason I ask this question is to simply remind myself that I am a music therapist. I need to do music well. I don’t need to try to compete with Ariana Grande but I do need to compete with Ellisa- me. Just like you are only competing with you. 

The first step to authenticity is education: “Who, what, when, where, why, and how!” 

Who wrote the song? 

What is their biographical background? 

When did they write the song? 

Why did they write the song? 

What was the intended purpose of the song? 

What style of music is the song in? 

What is the history of the genre? 

What was going on in the world at the time the song was released? 

How does the song/genre conflict or relate with your background/beliefs?

Maggie Ford is originally from Kentucky and grew up with two music loving parents. Maggie graduated with her undergraduate degree in music therapy from the University of Dayton and completed her internship at University Hospitals Connor Integrative Health Network in Cleveland, OH. Maggie is a board certified music therapist.

Show Notes

Wellness, as defined by The Global Wellness Institute is “the active pursuit of activities, choices and lifestyles that lead to a state of holistic health.”

6 dimensions of wellness:

Physical: A healthy body through exercise, nutrition, sleep, etc.

Mental: Engagement with the world through learning, problem-solving, creativity, etc.

Emotional: Being in touch with, aware of, accepting of, and able to express one’s feelings (and those of others).

Spiritual: Our search for meaning and purpose in human existence.

Social: Connecting with, interacting with, and contributing to other people and our communities.

Environmental: A healthy physical environment free of hazards; awareness of the role we play in bettering rather than denigrating the natural environment.

Holistic, as defined by Oxford Languages, is “characterized by comprehension of the parts of something as intimately interconnected and explicable only by reference to the whole.”

Holistic Approach to Medicine, as defined by Oxford Languages, is “the treatment of the whole person, taking into account mental and social factors, rather than just the symptoms of a disease".

Well-being is defined by Oxford Languages as “the state of being comfortable, healthy or happy.

“When you think about wellness, think prevention and health. When you think about well-being, think happiness.” - Sue Ellis (Chairman and CEO of The Global Wellness Institute)

Self-Care, according to The World Health Organization, is what people do for themselves to establish and maintain health, and to prevent and deal with illness. It is a broad concept encompassing hygiene (general and personal), nutrition (type and quality of food eaten), lifestyle (sporting activities, leisure etc), environmental factors (living conditions, social habits, etc.) socio-economic factors (income level, cultural beliefs, etc.) and self-medication.”

Wellness and Music Therapy

“Music therapy in wellness is used to meet the needs of individuals looking to "enhance quality of life, maximize well-being and potential, and increase self-awareness. Although music itself is used extensively to encourage preventive practices that help one maintain wellness, music therapy, which involves evidence-based practices designed and implemented by a music therapist, can provide interventions to help one regain wellness” - Lorna Segall, Phd, MT-BC

08 Feb 2021Clinical Ideas for Trauma-Informed Care | #600:40:07

Important points to consider when speaking about trauma-informed care:

We can (and should) assume that everyone we encounter has some level of trauma.

Due to the neurologic reactions from prolonged and persistent trauma the brain is negatively altered which impairs an individual’s ability to function.

This unbalanced system can affect numerous functions such as the ability to feel empathy, academic performance, inability to regulate self, etc.

The initial episode in this trauma-informed series was Episode #1: Trauma-Informed Care - A Review and Call to Action. For a firm foundation in which to build clinician strategies on, please head over and listen to that episode.

This episode encourages the listener to begin their trauma-informed journey through 3 steps:

Awareness (becoming aware of the prevalence and effects of trauma)

Learning (gaining education and strategies to consider)

Intention (putting these strategies into action, as is appropriate, in their own setting)

We’ve narrowed down three factors of change in the process of mitigating and healing trauma:

Consistency and predictability

Connection (building positive relationships)

Learning social and emotional skills

Strategies to consider within these 3 factors:

Consistency and predictability

consistent routines and schedules

consistent and positive routines for entering and exiting a session (classroom, etc.)

consistent and predictable transitions (the space between interventions, routines, etc.)

MACRO (typically those spaces where an individual is physically involved in movement - bigger shifts during the day)

MICRO (typically within an activity, session, or routine)

consistent and predictable physical environment

predictable environment

clear and consistent boundaries

clear and consistent expectations

predictable consequences (highlighting natural consequences for positive and negative actions - not in a punitive punishment reaction)

consistent and predictable caregiver/adult emotional responses

even temper and tone

clear directives

assertive tone (in comparison to a passive or aggressive tone)

Connection (positive relationships)

Connections to self

Much of this foundational work happens when we address emotional and self awareness strategies

Connections to a caring adult or other individual

building relationships based on safety through predictable and consistent interaction

overarching relationship built on unconditional positive regard

Connections to our wider community

building relationally-rich environments in our families, schools and clinical settings is important to build strong and supportive systems of safety to help an individual build resilience!

This may look like building relationships with parents, families, siblings, teachers, staff, and extend that to larger settings such as schools and other clinical facilities.

Consulting

Modeling and sharing resources to use at home/school/etc.

Hosting collaborative sessions with families and siblings (when appropriate and safe to do so)

and others….

Learning Social and Emotional Skills

Awareness of

self within a group environment

one’s own emotions

Learning strategies of regulation:

sensory integration including music, movement and other sensory activities that involve the 5 senses (touch, sight, taste, smell and sound).

Implementation and practice strategies in moving from state of being unregulated to a state of regulation.

*Note on regulation: in order to foster healthy relationships all parties must be regulated. The 3 step process to assisting an individual to move from a state of stress to a state of critical thinking is:

Regulate

Relate (connect with an individual through an attuned and sensitive relationship

Reason (supporting the individual to reflect, learn, remember articulate and become self-assured after an incidence of overwhelm.

Today Ellisa, Maggie and Kristin sit down to talk about their journey towards wellness, current wellness strategies and share high-impact tips and tricks. Wellness is a journey and looks different in different seasons of life. Today, we talk about what wellness looks like for us in the hopes it will spark an interest in listeners to think about their own wellness journey and find encouragement in the process.

Episode #3: What is Wellness?

Wellness Tree Analogy:

trunk = “holistic”

branches = “wellness”

leaves = “well-being”

nutrients (rain & sunshine) = “self-care”

“Wellness” is an active pursuit and is always growing when served the right nutrients.

Maggie’s Wellness Strategies:

Yoga

mindfulness practices such as meditation

eating healthy (with the occasional treat)

surrounding yourself with healthy and positive people

going for a walk

doing a puzzle (an actual physical puzzle) - while listening to music or just in quiet

taking a bath

sensory breaks of quiet

cleaning tasks around the house add energy and productivity and sometimes can be exactly what you need.

playing music (but also knowing when you need quiet and a break from sensory)

ask for help!

go to therapy, read books, watch youtube videos, etc.

seek your community and what they are doing…they might have some helpful tips for you!

Riding horses and working at an equestrian center

Therapy/Counseling

has used BetterHelp in the past, but also recommends in-person.

Maintaining a schedule on the wall in the house for tasks and to-dos!


Ellisa’s Wellness Strategies:

choosing a word of focus for your year

giving yourself grace

creating ‘spaces’ in your home for wellness (especially during the current pandemic)

tending to your physical body everyday (working out, stretching, resting)

Should shaped heating pad to ease muscle tension

exercise creativity at least once a week such as

playing music

exploring a new music technique

songwriting

writing poetry

working on music production techniques

essential oils

ASMR

listening to nature sounds when sleeping

Get Sleepy Podcast - sleep stories

pamper yourself once a week with an at-home spa night

Candles! This counts as self-care!

set up routines

morning routine with quiet time and prayer

evening routine of winding down, heating pad, sleep stories, ASMR, peppermint or Sleepy Time tea, etc.

schedule a consistent bedtime including quieting technology and unplugging before bed

drinking more water and sometimes using Liquid IV - for extra nourishment

Full Spectrum UV light to bring sunshine inside during winter months

standing and stretching while at work (in between clients/tasks)

schedule regular massages

probiotic and supplement regimen to fuel your body

write down tasks and keep a to-do list

enjoying time to decompress with a snack and an episode of a favorite TV show after work

keeping a calendar for all aspects of your professional and personal life.


Kristin’s Wellness Strategies:

move your body in some way every day!

running, walking, yoga, strength training, stretching, etc.

set boundaries around your personal and professional life

schedule in ‘white’ space in your schedule where you don’t have anything scheduled.

schedule in personal care nights

calendar or a command center in your home

Don’t be afraid to ask for help! Seek outside therapy/counseling and support.

Identify things that you need. What feel restorative for you? What are you missing?

Have items or reminders where you can see them! Have them available quickly so you can take advantage of even those small bits of time during the day to do something that feeds your wellness journey.

Main points of learning in today’s episode:

the interventions used in trauma-informed care depend greatly on

the age of the client

the type of trauma (past or present)

the duration and type of trauma

the type of therapy (individual/group/family)

stage of the trauma journey the client is on

The theoretical approach (ex. Neurosequential Model of Therapeutics) directly informs the music interventions being utilized.

For example: Neurosequential Model of Therapeutics

focus on stabilization and regulation

6Rs to inform clinical practice for stabilization/regulation as a goal:

Relational (safe)

Relevant (developmentally-matched)

Repetitive (patterned)

Rewarding (pleasurable)

Rhythmic (resonant with neural patterns)

Respectful (with the child, family and culture)

4 trauma-informed goals/purposes:

stabilization

entrainment

exploratory

performative

Interventions based on goal/purpose:

Stabilization: body-based interventions: crossing midline, deep breathing with coordinated movement, opportunities for impulse control, drumming, spinning, music and movement, fine motor tasks, deep pressures, body percussion, and attention tasks….

Entrainment: opportunities to entrain to the music and with others - Ex.: walking around the room in beat with peers and the therapist.

Exploratory: used when a client is regulated/calm and able to a access their higher order cognitive processes may benefit from songwriting, lyric analysis and discussion, art creation, deeper mindfulness moments/strategies, song learning and expression of emotions.

Performative: used for greater self awareness, self-regulation, building resilience and confidence - learning an instrument or song to share with others.

Note on explicit music - the use of explicit music may be important for relating and creating a safe environment for your clients - please listen to this podcast ( ) for a great discussion between two licensed music therapists.

Caution:

Potential harm. Music is a powerful tool and can be a trigger from traumatic events. Please do your education and know your scope of practice. Please be cautious when using music and continually be assessing and attuned to your client’s reactions.

Use your clinical judgment. We are offering ideas to consider and do not know the needs of the individual(s) you work with. Please utilize an evidence-based practice model (current research, clinician experiences and expertise, and client characteristics) to inform your clinical and musical decisions.

Procedure:

For each "verse" there is a face for each emotion "Sometimes I'm ______" (TEACHING)

Then show all the emotions for the first chorus "We have lots of feelings"

Then you repeat how each emotion feels and how it presents- "If I'm______ I might _________" (TEACHING)

Repeating part one of the song ("Sometimes I'm..."), use a musical cue for the client to point to the cued emotion when given 2 options- (TESTING/IDENTIFICATION)

Repeat for the second group of feelings. Feel free to add your own or pair down the information as appropriate! 

***Note that for each emotion, I mimic the sound of the feeling with my instrument to further teach the contrast between emotions.***


Adaptations: 


For appropriate clients you may sing/ask: If I’m smiling, I might feel…” or show the picture and sing “If I’m ____, I might smile!” for lyric fill in. Lastly, for any clients using an AAC device you could have them name the emotion word or match the image. 

You can also cue them to simply imitate the emotion, for expression and nonverbal clients. 

And of course, for pre-verbal or nonverbal clients I would encourage you to use sign language and perhaps this is simply vocabulary/language teaching song.   

For simple emotion identification when singing about each emotion you can sing the cue: "Do you feel_____?"  for the client so simply answer "yes" or "no".

3 Components of Burnout (coined by Herbert Freudenerger in 1975)

Emotional exhaustion

Depersonalization

Decreased send of accomplishment

Stress vs. Stressor

Stress: neurological and physiological response when you encounter a stressor (often includes heightened awareness, increased heart rate and blood pressure, etc.).

Stressor: what activates the stress response in the body (anything you see, hear, smell, touch, taste or even imagine that could harm you)

External stressors may be: work, family, cultural norms and expectations, experiences of discrimination, etc..

Internal stressors may be: self-criticism, body image, identity, etc…

How to complete the stress cycle:

Physical Movement (~ 20-60 minutes most days of the week)

Deep Breathing (deep, slow breathing being sure to breath all the way out until your belly contracts) - this can down regulate the stress response

Positive Social Interaction

Laughter - full belly, can’t breath laughter

Affection - deep connection with a loving presence (20 second hug; 6 second kiss, etc.)

A Big Ol’ Cry

Trauma Stewardship is being fully present with others in their pain, trauma, and suffering without taking it on as our own. It is a long-term approach to tending to our own wholeness so we can be helpful to others in our full integrity. 

term founded by Laura van Dernoot Lipsky, the founder and director of The Trauma Stewardship Institute.

Primary Trauma: when you yourself survive something that fundamentally changes your worldview. (E.g., losing a parent to cancer, natural disasters, chronic illness

Vicarious Trauma: exposure to someone else’s trauma over time also known as: compassion fatigue, empathic strain, and secondary trauma.

16 warning signs of the Trauma Exposure Response:

Sense of doom/hopelessness

Feeling the weight of systemic oppression (“I can never do enough”)

Remaining in a state of hypervigilance/hyperarousal

Decreased creativity/increased need for structure

Inability to embrace complexity

Minimizing your own suffering

Chronic exhaustion/new onset of physical ailments

Inability to listen/deliberate avoidance

New onset of dissociative moments

Feeling a sense of persecution

Guilt

Fear

Anger and Cynicism

Inability to empathize/numbing/feeling overwhelmed and overstimulated

New onset of addiction/relapse of former addiction

Grandiosity/inflated sense of one’s work (work = identity)


Steps towards Trauma Stewardship 

Step 1: Explore our own values and purpose, feelings and emotions, and past experiences and the meaning we continue to make of them.

Regularly check in with yourself.

Notice where you are holding pain and suffering in your body

Step 2: Seek support from others

Seek peer supervision from clinicians who are doing similar work.

If your work environment is unhealthy, you may need to do some serious reflection about whether or not you can continue in that environment long term.

Step 3: Create healthy boundaries and practice self-care.

Create a mental compartment for your work.

Create a routine that helps you put that work away each night.

Find practical ways to care for yourself throughout the day.

A few tips and tricks for creating a work/life balance:

-Play to your strengths

-Prioritize your time

-Know what time of day you are most creative and productive

-Do what you love! This is especially true for musicians. Finding time to make music for yourself.

-Be realistic about your workload and capacity. 

-Step out: Whether this means simply going for a walk, getting out of the house/office, or just closing your laptop for just a few minutes to give yourself a break. Find what works best to help your brain turn off for just a few minutes. 

-Manage your mind: I have talked in previous episodes about the fact that I find going to see a therapist as a therapist is very important. But, this can also mean picking up meditation or yoga as a regular practice. Whatever works for you!

-Take a break: Are you using your PTO effectively? Do you know the warning signs that your body gives you when you are feeling overwhelmed? Maybe take some time to reflect on these as well and notice the signs your body tells you when it needs a break. 

-Have that holiday: Take that vacation! You deserve it!

A few ideas for using music more intentionally in your day:

Bring your attention to the music! - this is both simple and hard. Over the next week intentionally bring your focus to the music around you. At home, at the office, in the car, at the grocery store….simply bring your attention to it.

Listen for the intricacies of the music. Here is where you can start to look deeper at the music you are exposed to everyday. Answer the following questions:

WHEN do you tend to gravitate towards music?

HOW are you using the music?

WHAT music are you listening to? Also, what parts of the music do you like, find pleasing, or dislike?

What LEVEL OF INFLUENCE do you have over the music you listen to?

Intentionally utilize music!

LISTEN! Intentionally set aside time each day to listen fully to music. This can be a short period of time where you set down everything else and must focus on listening.

Use music for a desired action. Select a playlist or songs for a desired action such as a transition from work to home, for motivation to exercise, to support a good night sleep….to name a few ideas.

Iso-Principle: choose music that matches your current mood and gently alter the music/songs towards the desired mood.

Use music as a cue. Use music to support positive associations like exercising, calming before bed, and/or to cue a specific routine or habit.

Learn an instrument.

*Identify where you land on the spectrum of auditory stimulation. Do you tend to avoid sensory stimuli or seek it?

Bio: Claire Ghetti is Professor of Music Therapy at the Grieg Academy Music Therapy Research Centre (GAMUT), The Grieg Academy - Department of Music, University of Bergen, Norway. Claire’s research centers on how music and the relationships that are enabled through musicking serve as resources for health in intensive medical contexts. Claire has published research and theoretical work in the area of music therapy as procedural support for invasive medical procedures, music therapy for hospitalized children at risk for traumatization and resource-oriented approaches to music therapy for parents of premature infants. Claire is co-editor-in-chief of the open access, social justice-oriented online journal, Voices: A World Forum for Music Therapy.
