Pelvic floor health is critical in managing interstitial cystitis for some people, but how do Kegels fit in (or not)? This week, Callie is joined by Darla Cathcart, a pelvic floor physical therapist with over 20 years of experience to talk about kegels and how they fit into pelvic floor health. Dr. Cathcart demystifies Kegels by talking through what they are, why they are used, and who they are appropriate for. Callie and Dr. Cathcart also talk about the intersections of IC, stress, and pelvic floor dysfunction as well as strategies to move stress through the body. Irritants can affect pelvic floor dysfunction as well trigger an IC flare, so callie and Dr. Cathcart talk through how to approach reducing the impact of irritants. Getting in to see a pelvic floor physical therapist can be hard, so Dr. Cathcart also shares some tips on what to do at home to help your pelvic floor. So why might some people need to do kegels and some not? Tune in to find out!

This week I talk with Brittany Modell, Intuitive Eating RD. Brittany helps people break free from yo-yo dieting and heal their relationship with food and body. We chat about food fear created by the IC diet. We discuss the importance of mental health, what food rules or beliefs are, how to unlearn patterns and behaviors with eating disorders and so much more. Again, we can’t stress it enough on these episodes but there is no size fits all, what works for someone may not work for you so it’s important to work with a Registered Dietitian to help you through this journey. If you believe you have an eating disorder and don’t know where to turn, the best thing for you is to reach out to a Psychotherapist and/or a Registered Dietitian who specializes in eating disorders.

As a pelvic floor therapist with almost 20 years of experience in the field, she takes an orthopedic approach to treating the pelvic floor. The most effective treatment for pelvic floor dysfunction is physical therapy, which has been proven time and time again.

We also dive into the misconception that the IC diet is the way to recovery, when in reality most people with interstitial cystitis aren’t diet sensitive or only have a few diet triggers. Nicole shares how you can reach recovery and how to create a healthy environment along your journey.

Women’s stress management and mindset coach, Lindsay Dorka, joins Callie this week to share her expertise on how stress affects the body, ways to deal with perfectionism, and how stress is related to interstitial cystitis. With a background in healthcare as as an occupational therapist, she found that the root of many of the issues patients were coming in with could be traced back to one thing: stress.

While stress is normal to a certain extent and helps us survive, chronic stress is not and can manifest through physiological symptoms like pain, issues with digestion, and trouble sleeping. The internal tension of suppressing emotions is more powerful than we may think and can create real problems. But luckily, there is also a solution. Lindsay and Callie discuss the ways we can retrain our brain during states of stress to reduce the physical symptoms and live a better life.

After seeing the intense struggles her mom went through after being diagnosed with interstitial cystitis over 20 years ago, Andrea was terrified to be diagnosed with IC last year. When going through a stressful time at the beginning of last year, she got a UTI out of nowhere and the symptoms never went away. After seeing a urologist and eventually a specialist, she was diagnosed and immediately began trying to find relief through medication, physical therapy, and bladder instillations. 

Now, just one year later, she is almost completely symptom-free. And when she does have flares, she has the tools to get rid of them in a matter of days. We discuss how she has managed her symptoms, why focusing on her nervous system was the key to her recovery, and how Road to Remission helped her feel hope for the future for the first time. Andrea shares her story of hope for a normal life with IC by managing her symptoms to the point of being able to swim, go for long car rides, and enjoy almost all foods.

In this episode, I’m joined by Lance Frank, a pelvic floor physical therapist based in Atlanta who works mostly with male clients. We dive into a topic that’s so overlooked: pelvic floor dysfunction in men.

Lance breaks down what the male pelvic floor actually is, how it differs from the female anatomy, and why so many men struggle to get answers when dealing with symptoms like bladder urgency, pain, or even sexual dysfunction.

We talk about the stigma around men’s pelvic health, the confusion between pelvic floor issues and prostate problems, and how hard it can be for guys to find the right provider—or even feel comfortable enough to ask for help.

Lance also explains what chronic pelvic pain syndrome really is, why internal exams can be helpful (but aren’t always necessary), and what men should know if they’re dealing with bladder or bowel issues.

Lance is a pelvic floor physical therapist and owner of his own practice in Atlanta. Originally from Missouri, he earned his Doctorate of Physical Therapy from Emory University and has a background in orthopedics, public health, and sports performance.

He’s certified in dry needling, trained in Redcord and mat Pilates, and now primarily focuses on pelvic health for all bodies—including men, trans, and intersex individuals. As an openly gay clinician, Lance is passionate about creating safe, inclusive spaces for LGBTQIA+ clients to get the care they deserve.

Dr. Moldwin believes that every person is so different and there isn’t just one solution for people who are experiencing pelvic pain. His treatment approach is to find the underlying cause of the issue and treat that first instead of assuming all pelvic pain is interstitial cystitis. He shares what people struggling with interstitial cystitis and other forms of pelvic pain can do to find solutions with their clinicians to truly make a difference in their lives.

Dr. Curtis Nickel, a professor of urology and urologist at Queens University in Kingston, Ontario joins Callie to discuss his work studying and educating people about interstitial cystitis. Dr. Nickel discusses his career trajectory from microbiology to urology and his experiences along the way. As one of the pioneers in research on interstitial cystitis and chronic prostatitis, Dr. Nickel shares his findings and the evidence behind an individualized approach to treatments for IC and chronic prostatitis. Dr. Nickel, as a professor, also sees how little discussion of IC happens in medical school and training. He talks about his approach to educating urologists to better treat IC. Being a physician in Canada is much different in practice than in the United States, especially treating IC and Dr. Nickel discusses these differences, from treatment approaches and reimbursement for physicians to guidelines for diagnosis and treatment. Dr. Nickel also breaks down the “phenotype system” approach to treatment he developed. The episode wraps up with Dr. Nickel discussing other aspects of treating IC he utilizes including mental exercises, patient education, and elimination diets.
