Nektarios Karanikas is Assoc. Prof in the Health, Safety and Environment discipline of the School of Public Health & Social Work at QUT. He studied MSc Human Factors and Safety Assessment in Aeronautics at Cranfield University (UK), and he was awarded his doctorate in Safety and Quality Management from Middlesex University (UK). Nektarios worked for over 19 years as an officer at the Hellenic Air Force where he served in various positions related to maintenance, safety and quality management, including accident investigations. Besides his academic qualifications, he holds professional credentials in occupational health & safety, human factors, project management and engineering, and has been a member of various national and international associations. Nektarios has published an edited book and several chapters, (co)authored various peer-reviewed journal and conference articles with a focus on safety/risk management, presented his work in over 80 events, and he actively volunteers in a wide range of scientific and professional activities.

Sebnem Bulan-Worth has been working with organisations across Australia for over 15 years as a learning and development professional. Sebnem specialises in workplace psychological health training programs and is responsible for educating all employees on using interactive and informational training programs, including CALM Suicide Intervention, CALM Care Suicide Awareness, Mental Health First Aid, Workplace Sexual Harassment Prevention and Workplace Bullying and Harassment training programs.

In addition to this, Sebnem is a Principal Master Mental Health First Aid Instructor, who has conducted over 100 training sessions. Using her positive attitude and tireless energy, Sebnem promotes inclusivity,

Glen Cook (Cookie), is an experienced and enthusiastic Public Safety Specialist. Cookie is extremely passionate about raising powerline safety awareness and received the Australian Health and Safety Professional of the year at the 2020 at the Australian Work Health and Safety Awards. Working as an electrician for over 30 years within the construction and utilities industries he has been first responder to serious injury/burns and fatalities due to third party accidental contact with powerlines.

Dr Dan Hill, one of my favourite people to sit down and chew the fat with about training talks about how quality training and lack thereof can directly impact on the safety of your people at the workplace. Peter has more than 25 years of Occupational Health and Safety (OHS) on-the-job experience working with government agencies, volunteer organisations and mining equipment manufacturing industries in front line WHS roles. Professionally Peter is a Registered Nurse with postgraduate specialities in OHS, Disaster & Emergency Management and Psychology.

He proudly provides health, safety and wellbeing business partnering for the Service Delivery Area of Urban Utilities in his Enabling Partner role.

Peter is very proud of the work he has done with the Local Government Work Care WHS Support Unit which supports 65 of Queensland Local Governments who are self-insured for workers compensation with LGMS.

Peter is particularly passionate about his “Safety Coach” role in organisations and thrives on supporting organisations who support our communities.

Karina Bourne is an Early Childhood Professional and holds a Bachelor of Education (Teaching-Early Childhood Education) from the University of Auckland and brings a rich background of experience working with children of all ages across a range of roles and settings, both in Australia and New Zealand. Some of these roles have included working as a nanny, Early Childhood Teacher, Family Day Care Coordinator,Family Day Care Educator, Trainer and Assessor and Mental Health Consultant.

Karina is passionate about the Early Years and is particularly fascinated by brain development, attachment theories and relationship based practice and advocates for the development of Early Childhood Environments that foster the wellbeing of all.

Thirty four years ago the world lost Piper Alpha. Piper Alpha pride of Britain's North Sea Oil was the first oil rig lost to fire.

Of the 226 men on board only 61 would survive the night of July 6 1988. Today's guest is one of the 61.

Piper Alpha was originally designed for oil production but was also connected to three other rigs and modified for gas production.

At the time of loss Piper Alpha was producing 10% of the total North Sea production.

It is estimated that the cost of losing the oil rig was 1.7 billion pounds

The Cullen Inquiry into the incident was damming making 106 recommendations although no criminal charges were ever laid.

Joe Meanen was a scaffolder on Piper Alpha on that fateful night.

He tells his story. The initial explosion, the chaos that ensured and his escape with his life.

Not to be missed.

Joe also gives his prognosis on the causes of the incident and reflects on his life since the disaster which took so many lives.

On March 11, 2011, fifty minutes after a magnitude 9.0 earthquake hit eastern Japan, a tsunami 45-feet high engulfed the nuclear power plant known as Fukushima Daiichi, knocking out electrical power and all the reactors' safety systems. Three reactor cores experienced meltdowns in the first three days, leading to an unimaginable nuclear disaster. The person the Tokyo Electric Power Company called for help was Dr. Chuck Casto.

What an amazing and humble gentleman. A great honour and pleasure speaking with Chuck, I am sure you will love this conversation as much as I did.

Dr. Charles Casto is a nuclear safety and regulatory professional with 38 years of experience consisting of positions of increasing responsibility, including executive, regulatory, technical, and operational areas of expertise. He was a member of the U.S. Government’s Senior Executive Service, and last held the position of NRC, Regional Administrator, Region III, overseeing 23 reactors in eight states as one of the senior administrators for the United States Nuclear Regulatory Commission.

Dr. Casto led the integrated US Government and Nuclear Regulatory Commission (NRC) efforts in Japan during the Fukushima nuclear accident in 2011. Working directly with Ambassador Roos, and coordinating with the NRC’s Emergency Operations Center in the U.S., he represented the United States Government to the Japanese Prime Minister’s Cabinet. His efforts are widely recognized as being a singular contribution to the emergency response during the Fukushima nuclear crisis, through his efforts in coordinating the U.S. government response, providing technical consultation to the Daiichi plant manager and Japan’s nuclear agencies, and serving as a liaison to Japanese Prime Minister Kan Naoto during the Fukushima nuclear disaster. After serving for 11 months in Japan as the U.S. government’s lead nuclear representative during the crisis, Dr. Casto received the Presidential Distinguished Award (2012) from President Obama; for his contributions to the U.S. government, he previously received the Meritorious Rank Award (2009) from President Bush.

Kenneth Bloch is a senior EHS professional who specializes in petrochemical industry incident investigation and failure analysis. He has nearly 40 years of experience in plant maintenance, Process Safety Management, technical, and operations roles.

He speaks regularly at AFPM, API, and AIChE process safety symposiums about experiences that help prevent recurring process safety failures throughout the manufacturing industry. Ken is the author of the book Rethinking Bhopal which uses modern references to chronicle the sequence of events that led to history's worst industrial disaster. He graduated with honors from Lamar University in Beaumont, Texas, with an Environmental Science Degree in 1988.

Brad encourages organizations to react restoratively rather than retributively whenever an unintended deviation from an expected outcome. Brad is currently the Manager of HSSE for an oil and gas company.

Brad has 17 years of HSSE experience in the United States. He holds a Bachelor of Science and a Master of Science in Occupational Safety and Health from Southeastern Oklahoma State University.

Brad is a Certified Safety Professional (CSP), Occupational Hygiene and Safety Technician (OHST), and Construction Health and Safety Technician (CHST).

Brad is well versed in all aspects of HSSE with demonstrated experience in developing, implementing, monitoring, consulting, coordinating, instructing, coaching, and continually improving HSSE and PSM programs and processes.

The marvellous Sara Pazell joins us in this weeks conversation which is all about ergonomics and human factors. One of the least understood areas for most in safety Sara walks us through the maze of misconceptions to teach us about the benefits of ergonomics, defines what it really is, and how it vastly under utilised in workplaces particularly in the design phase.

Sara is the principal work design strategist and managing director for ViVA health at work, leveraging design in all industries (industrial and office work settings). She is here to help when you ask: How to design when humans are part of the system (work, product, technology, or environment) and what 

is the user experience? What if we did things a little differently? What can we dream that can 

become a new reality? She helps build capacity to innovate to generate some good stories to tell! 

Sara consults, teaches, and researches in human factors, human-centred design, and organisational strategy. Sara is affiliated with five Australian universities to advance these ideas for graduate teaching and research. She is also a podcast rabble-rouser with the WhyWork Podcast crew, examining the boundaries of human experiences at work, how this is tested by case law, and the way in which work can be better designed. Sara is part of the international advisory committee for the WELL Movement concept v2 (International Well Building Institute [IWBI]) and their associated project-based committees and an expert faculty member for Australia’s only certified Wellness WiseTM Practitioner training program. 

Sara was the committee chair for the Human Factors and Ergonomics Society of Australia’s Good Work Design position paper and supporting resources. Sara is a series co-editor of the Workplace Insights book series by Taylor & Francis and a book co-editor of the Ergonomic Insights book. Her other passions include instructing yoga, Thai massage, and sports & strength conditioning.

In 2011, he founded Action OHS Consulting, which focuses on providing meaningful and relevant advice to these businesses.

 

Craig's commitment to improving health and safety practices led him to create Safety Champion Software in 2015. This innovative software was designed to meet the unique needs of smaller businesses and make safety compliance more accessible and user-friendly. As of January 2023, Safety Champion has scaled rapidly, supporting over 1,500 clients.

Doublly blessed this week to be joined by not one but two HOP professionals - The Dynamic Duo of Andy Baker and Matt Florio.

Andy started practicing and teaching Human & Organizational Performance (HOP) principles while working for one of the largest multinational industrial companies in the world. As an environmental health and safety leader of manufacturing, assembly and distribution facilities, she worked through the complexity of adopting HOP principles from the ground up.

In 2014, she was named the HOP leader for one of the company sectors. She taught HOP Fundamentals and Learning Teams around the US and helped develop and implement strategies to anchor the concepts in existing operating rhythms. She began extrapolating these strategies to a larger scale, and in 2015 became the HOP Senior Expert for the company’s 300+ multi-sector locations in Europe. In this position, she coached fatality and serious incident investigations while continuing to train and develop culture change strategies to help embed HOP principles into industrial day-to-day activities.

After hearing about HOP for almost a decade from his sister-in-law Andrea (Andy), Matt left his last role as a VP in the software/start-up world to join the HOP movement.

He has a background in education and training (both virtual and in-person) and experience as an executive in the software industry. His immediate focus is on finding ways we can share, teach, mentor, and coach on HOP concepts outside the safety space.

Bob started his career in construction in the UK in 1989. Moving through engineering, site management, health and safety and several executive roles before leaving ‘normal’ work in 2011 in pursuit of making a

difference to other organisations by helping them apply the principles of behavioural science.

Bob founded Sodak limited, a behavioural design agency based in Edinburgh, Scotland. Sodak’s aim is to help people in business make safer, happier, less wasteful workplaces through behavioural design.

Bob is also the author of Behavioural Science for Business Health and Self, Behavioural Incident Analysis and has developed a number of tools to help people change theirs and other’s behaviours. Sodak work

with businesses in Europe, Canada and the US.

This week on the conversation a real treat as Professor Andrew Hopkins joins the conversation. One of my favourite authors and one of those people who I never thought I would have the opportunity to talk to, this conversation is one of the highlights of the show so far. I know you will enjoy it, I know you will learn from it, and I hope you enjoy the conversation as much as I did.

Andrew was an expert witness at the Royal Commission into the 1998 Exxon gas plant explosion near Melbourne. He was a consultant to the US Chemical Safety Board in its investigation of the BP Texas City Refinery disaster of 2005, and also for its investigation into the BP Gulf of Mexico oil spill of 2010. He has written books about these accidents as well as books on the Moura, Gretley, and Grosvenor coal mine disasters.

Over 100,000 copies of his book have been sold.

He has been involved in reviews of Work Health and Safety regulation and regulators and has done consultancy work for major companies in the mining, petroleum, chemical and electrical industries, as well as for Defence. He speaks regularly to audiences around the world about the human and organisational causes of major accidents.

• BSc and MA (Sociology) from Australian National University, PhD (Sociology) from the University of Connecticut.

• Winner of the 2008 European Process Safety Centre safety award, the first in time it was awarded to someone outside Europe.

Murray Ritchie is an occupational health and safety practitioner, researcher, educator and sought after keynote speaker with 40 years of experience working with various industries, governments and NGOs. He obtained a Master of Science degree in Occupational Safety and Health from the University of Greenwich, UK, late in life following a boots-on-the-ground career in the offshore oil and gas industry. Dr Lisette Kanse is a Senior Lecturer in the School of Psychological Science at the University of Western Australia (UWA) and the Course Coordinator of UWA’s Master of and Graduate Certificate in Business Psychology. She has worked as an academic and as a consultant/practitioner in a variety of industrial settings across Europe and Australia, including chemical industry, oil and gas, mining, rail transport, and healthcare settings.

Having entered the field of work health and safety over 25 years ago convinced of the importance of this domain, Lisette’s mission across her entire working life has been to improve work environments, either directly, or through her students, to keep people safe, healthy, engaged, and productive. Occasionally she has been known to extend this mission to keeping people safe outside of the work environment as well.

She uses her dual background in industrial engineering and organisational psychology to identify and facilitate evidence-based improvements, and to warn against ineffective strategies. Her research interests include human factors, work design characteristics, procedures, norms, culture, and leadership, and how these impact on work practice and safety and health.

She has developed significant experience in these areas over the past 20 years, including having worked as an Associate Solicitor practising in personal injury law in Queensland, various WHS management roles in large organisations in the private sector, in the public sector as a national WHS manager for a federal government organisation and in local government WHS, and with an array of businesses as a management consultant and licenced factual investigator.  

 

Andrea's diverse industry experience includes supply chain warehousing and logistics, food services, manufacturing, road surfacing, quarries, construction, arboriculture, retail, not-for-profit, heavy vehicle safety regulatory compliance, legal services and policing.

 

Drawing on her qualifications and experience, Andrea recently established Brisbane-based Organisational Risk Consulting, which provides tailored risk management support and specialises in WHS legal compliance and best practice solutions. The business works collaboratively with clients to achieve successful project outcomes, both locally and further afield throughout Australia and New Zealand.

Rob is the Organisational Performance Manager for Energyworks in Taranaki and has spent the last 30 years working in a number of high-risk industries including oil and gas, electricity, adventure tourism, transportation and waste.

Rob has also held a number of health, safety and environmental roles, however over the last 10 years, Rob has watched the ‘safety industry’ go from non-existent in some sectors to a bureaucratic burden in others with layers of administration that add little value. Despondent with corporate zero harm fallacies, tracking metrics and low trust leadership models, Rob sought out a new way of ‘doing’ safety and the results speak for themselves but in doing so has realised that this has very little to do with safety. Energyworks also won the inaugural Safety II category at the National Health and Safety awards last year and as a result have been instrumental in taking the lead in removing health and safety from the vocabulary.

Rob believes that every person can undertake growth personally and professionally in the right environment. Rob’s approach revolves around the principle that workers are not the problem but the solution so we need to ensure our people are at the core of decision making.

Jonathan experienced being the target of bullying some years ago; a period of his life that impacted on him considerably. He now dedicates much of his time to challenging workplace bullying and is an Ambassador for the Stop Hurt at Work campaign.

In 2017, after nearly 10 years working fifo shutdowns and construction, having already experienced his partners attempt due to the stress of his working away, Lachie lost two colleagues to suicide. He was also later evacuated from site for mental health issues.

Since then he has gone on to become one of the leaders in FIFO mental health, creating a top10 mental health podcast, the FIFO Mental Health Summit and The Association for Mental Health in Mining.

In 2021 he won the 'national mens health champion' award, for his work in FIFO mental health, from the Australian Mens Health Forum, Australia's peak body for mens health.

In 2022 Lachie was named a Linkedin Top Voice for Mental Health and with that, made the decision to transition from FIFO Mental Health Guy, to FIFO Mental Health Group, as the first step towards finding and building a team of people who are deeply passionate about transforming mental health culture within mining.

In this the first of two special episodes of the conversation Nicola MacPhail talks to Associate Professors Drew Rae and Nektarios Karanikas about High Reliability Organisations and their charecteristics.

Dr Drew Rae is an Associate Professor in the Safety Science Innovation Lab at Griffith University, where he teaches courses on research methods and safety engineering, and manages the lab’s research program. Drew’s own research uses a mix of ethnography, field experiments and theory-building to investigate organisational safety practices. He is particularly interested in understanding the myths, rituals and bad habits that surround the work of managers and safety practitioners, and how this work influences front-line operations. Drew co-hosts the Safety of Work podcast and is on the editorial board of the journal Safety Science.

Nektarios Karanikas is Associate Professor in the Health, Safety and Environment discipline of the School of Public Health & Social Work at QUT. He studied MSc Human Factors and Safety Assessment in Aeronautics at Cranfield University (UK), and he was awarded his doctorate in Safety and Quality Management from Middlesex University (UK). Nektarios worked for over 19 years as an officer at the Hellenic Air Force where he served in various positions related to maintenance, safety and quality management, including accident investigations. Besides his academic qualifications, he holds professional credentials in occupational health & safety, human factors, project management and engineering, and has been a member of various national and international associations. Nektarios has published an edited book and several chapters, (co)authored various peer-reviewed journal and conference articles with a focus on safety/risk management, presented his work in over 80 events, and he actively volunteers in a wide range of scientific and professional activities.

This week on the conversation we are joined by the marvellous Sarah Thomas from New Zealand. One of my favourite contributiors to safety conversations on LinkedIn and someone who genuinely thinks before speaking.

Sarah Thomas is an experienced HSE professional who began her career in safety in 2002. However, as a surf lifeguard, professional ski patroller, outdoor instructor, and volunteer St John ambulance officer, she has lived and breathed safety from a young age. Her six years with the Royal New Zealand Naval Volunteer Reserves also gave her advanced command and communication training. 

 

Sarah gained a BA/LLB at the University of Canterbury and a Post Graduate Diploma in Business Administration (HSE) from Massey University. She has worked as a safety coordinator, safety specialist and National HSE/Human Resources Manager. 

 

During her career, Sarah has helped develop nationwide health and safety systems and has experience in a range of industries including transport, oil, manufacturing and the Department of Conservation.  She launched her consulting career in 2011. 

 

In terms of her personality profile, Sarah is outgoing and has an inclusive style of communication developed after years of regularly attending Toastmasters meetings.  Her clients describe her as facilitative; however, she can be direct and hold people accountable when necessary. Along with a sense of humour, she has an innate ability to define a problem and guide her clients to develop a solution that is written in their own language and meets their operational needs.

Alexsandro Siedschlag was born and grow in a small coast city in south Brazil called São Francisco do Sul. There wasn’t much to do there other than fishing, surfing, or working in port activities. So, in 2007 he went to the nearest industrial city and took a technical course in safety. This took me back to my hometown (what an irony) to work in a shipping company. From there, in 2012 he started to work offshore in oil & gas where he spent my next 6 years. After that he experienced the corporate side of the business, taking roles as HSE advisory, supervisor and stepping in as a manager a few times, living in Rio de Janeiro for 3 years.

During this journey, Alexsandro invested in qualification and competences development, from technical skills (environmental degree, process safety, incident investigation, among others) to social skills such as the professional certification in coaching, IOSH coaching for safety and a MBA in human factors in 2023. This helped him to find out I’m passionate about supporting individuals and teams through professional coaching and mentoring. Alexsandro could see selfhimself to make safety differently. In 2022 he left the oil & gas company he was working for the past 9 years and became an associate performance safety coach for a company headquartered in Scotland, working in Australia, in an underground coal mine supporting a safety leadership cultural change program.

So far, in between work, family trips and studying Alexsandro has been around Brazil, Argentina, Peru, Suriname, Costa Rica, United States, South Africa, England, Scotland, Spain, Italy, and Australia. He lives in Brazil but currently working in Australia flying in/out every month, which helps a lot my air miles programs!

This little story gave Alexsandro 14 years’ experience in high-hazard industries among different countries, working in complex social-technical systems, supporting organizations to deliver safe operations.

Family speaking, Alexsandro is married and a dad of two who loves to read and listen to podcasts related to safety science and leadership stuff while exercising. In between working, traveling and all that, he tries to find some time to go surfing (not those big waves – I confess).

Steve's life took an unexpected turn when he was involved in a near-fatal workplace accident. This life-altering experience became a catalyst for change, leading Steve on a mission to promote safety and raise awareness about mental health.

Having personally faced the consequences of workplace hazards, Steven is passionate about encouraging safe behavior. Through his story he aims to highlight the importance of following safety protocols and prioritizing the well-being of ourselves and others in the workplace.

On the last regular episdode for Season 3 we are blessed with James Wood and Michael Weston joining the conversation.

Both popular and professional safety real life experience speakers both have a message you will want to hear (and your people to hear).

Not one to be missed and a good way to end the season and the year.

Michael will live the rest of his life with an acquired brain injury (ABI) as a result of pushing his body to the point that it couldn’t go on.

A career of 20 years in heavy industry saw Michael rewarded with a leadership role, he not only had to ensure production, but he was responsible for other people. A normal 10 to 12 hour workday often dragged out to 14 and 16 hours, days off were spent answering phone calls from the site or ‘Catching up’ on work that just “Had” to be done.

Work/Life balance became Work/Work balance with a bit of life thrown in when it didn’t interfere with the job.

Something had to give, and it was Michael’s body. He walked out his front door not feeling quite right but knowing he had to get to work and he collapsed on his driveway and was found by his neighbor unconscious and not breathing. His head had fallen onto his chest and blocked the supply of oxygen to his brain.

Michael is now affected by; short term memory loss, impaired cognitive function, limited attention span, hypersensitivity to noise and bright light among other health and well-being impacts. Michael was diagnosed with an ABI, Anxiety, Depression and Post-Traumatic Stress Disorder (PTSD).

James Wood offers workplaces something different. He shares the story of his workplace accident! James made some wrong Choices and ended up in a wheelchair for the rest of his life!

James’ Safety Presentation has a long lasting and significant impact on the Choices you make.

James will share his message of ‘choice’, which he hopes will help other people to avoid the same mistakes he has made. James Wood made the wrong safety decision once and now he has to live with it for the rest of his life. As James puts it, ‘It was supposed to be a normal day, I got up, I went to work and went home 9 months later’.
