Our brains have been conditioned to grasp and cling. This conditioning, which we believe protects and helps us, actually limits our ability to live life fully. Now apply that metaphor to “our brain on infertility.” It’s the idea of a baby that we are grasping. And it’s understandable – having a baby is a primal urge. We clench with all our might around the idea that we want a baby and that we cannot be happy until we have one. 
 Meditation is a powerful technique for learning how to “unclench” the mind and find happiness that is not dependent on anything outside ourself. When I’m coaching people, it’s usually a 90 day process. I want them to be patient.” -Laura Today’s guest is Laura Ortner, a certified fertility coach and health and wellness coach from the US. After being told she has a less than 1% chance of having children, her FSH was elevated and was in the “pre-menopausal” range. She was told that adoption or egg donors were the only other option. Laura didn’t take no for an answer. She sought the opinion of multiple REs, she researched, worked with a naturopath and an acupuncturist. She cleaned up her home environment and her diet. She was able to decrease her FSH naturally. After 4 rounds of IVF, her daughter was born without the use of an egg donor. As a companion to Amanda’s story, Mary is diving into hydrosalpinx. Hydrosalpinx refers to a fallopian tube that’s blocked with a watery fluid. To break down the term, “hydro” means water and “salpinx” means fallopian tube. This condition is typically caused by a previous pelvic or sexually transmitted infection, a condition like endometriosis, or previous surgery. Although some women don’t experience any symptoms, others may experience constant or frequent pain in the lower abdomen or unusual vaginal discharge. An academic review examined 5,592 women undergoing IVF. Of these women, 1,004 had untreated hydrosalpinx, and 4,588 had another type of untreated tubal blockage. Between these two groups, doctors performed 8,703 total embryo transfers. The following success rates were observed between the two groups: * Women with untreated hydrosalpinx had a pregnancy rate of 19.67 percent versus 31.2 percent for women with other types of blockages (the control group). * Implantation rates were 8.53 percent for women with untreated hydrosalpinx compared to 13.68 percent in the control. * Delivery rates for women with untreated hydrosalpinx were 13.4 percent versus 23.4 percent for women with other types of blockage. * Women with untreated hydrosalpinx saw higher rates of early pregnancy loss — 43.65 percent — versus 31.11 percent for the control group. Your ability to get pregnant with a hydrosalpinx will vary depending on the severity of your blockage and your treatment choice. Without treatment, pregnancy is possible, but chances of complications, like miscarriage, are higher. With treatment, the outlook is much better, especially with IVF. Your doctor is your best resource for information about your specific case and can help guide you to the treatment option that may work best for you. “Be your own advocate and be more involved with what’s going on with you!” - Amanda Today’s podcast guest is Amanda from West Texas. She and her husband have been trying to conceive since 2018. Amanda tells Mary about her fertility journey with 8 rounds of Clomid. She was diagnosed with bilateral tubal blockage with hydrosalpinx. She later had surgery in 2019 to repair one fallopian tube and was diagnosed with endometriosis. Amanda did 2 IUIs in January and February 2020. Her fertility timeline was disrupted due to COVID and she is taking it day by day. She eventually plans on doing an HSG (dye test) and IVF later in the year. Amanda has improved her diet and has seen improvements in the number of follicles developing. She focused on better portion sizes, lean meats, and increased varieties of vegetables. As part of my Mindfulness series, in today’s short solo episode I talk about how Mindfulness training is a safe and effective treatment with many benefits for women and men undergoing infertility, particularly IVF. Anxiety and stress may actually make it more difficult to conceive. In fact, recent studies found that higher rates of infertility treatment success in people who were undergoing a mind–body interventions. So, reducing stress and anxiety may be beneficial for conception. Today’s guest is fellow fertility podcaster, Monique Farook, host of the Infertility and Me podcast. She is an infertility survivor, former NICU Mom, wife, and serial entrepreneur. She and her husband began their path to parenthood in the Spring of 2012 when she was 29 years old. After completing the HSG test to determine if her reproductive organs were in good health, the radiologist concluded that she had a right tubal blockage. After one unsuccessful IUI (Intrauterine Insemination) she and her husband waited four years before seeking the expertise of a reproductive endocrinologist for a second time. It was then when she was diagnosed with hypothyroidism and uterine polyps. Although the right tubal blockage had "cleared" itself, she had two new unforeseeable issues to deal with before she could begin any form of IVF treatment. In August of 2016, Monique became pregnant with her baby boy after one failed cycle. In January 2017, Monique went into preterm labor at 23 weeks five days and remained in the hospital on bed-rest for an entire week. At 24 weeks four days pregnant(6 months), her baby boy was born and spent 129 arduous days in the NICU. He is the inspiration behind Monique’s passion for infertility advocacy. POI occurs when a person’s ovaries produce fewer eggs. Often, doctors do not know what causes it, but it is often the result of an underlying condition. A doctor can diagnose POI based on a person’s medical history and blood tests. The treatment they prescribe will depend on the individual and whether they want to try to conceive. If they do, a doctor may prescribe hormones or medication to increase the chances of pregnancy, but many people will require donor eggs. Support groups or counseling may help someone cope with the emotional impact of this condition and its affect on fertility. Listen in to learn more about this condition. As a companion to Lauren’s episode, Mary talks about ICSI and what this procedure is. There are two ways that an egg may be fertilized by IVF: traditional and ICSI. In traditional IVF, 50,000 or more swimming sperm are placed next to the egg in a laboratory dish. Fertilization occurs when one of the sperm enters into the cytoplasm of the egg. In the ICSI process, a tiny needle, called a micropipette, is used to inject a single sperm into the center of the egg. With either traditional IVF or ICSI, once fertilization occurs, the fertilized egg (now called an embryo) grows in a laboratory for 1 to 5 days before it is transferred to the woman’s uterus (womb). ICSI helps to overcome fertility problems, such as: * The male partner produces too few sperm to do artificial insemination (intrauterine insemination [IUI]) or IVF. * The sperm may not move in a normal fashion. * The sperm may have trouble attaching to the egg. * A blockage in the male reproductive tract may keep sperm from getting out. * Eggs have not fertilized by traditional IVF, regardless of the condition of the sperm. * In vitro matured eggs are being used. * Previously frozen eggs are being used. Coping with Pregnancy Loss 1. Understand that it's not your fault. Pregnancy loss or complications can strike anyone. Talk openly and honestly with your partner about what's happened and how it's affecting you. Remember, there's no right or wrong way to deal with grief. Accept your feelings as they are and don't judge yourself or your partner for how you respond. 2. Give yourself time to heal. Don't pressure yourself to get past the sadness quickly. Your healing will be more complete if you deal with your grief as it comes. You may find yourself reliving the pain, especially around your due date or other milestones. Over time, things will change and you'll feel better. 3. Take time off from work. Even if you feel physically fine, taking some time away from your job may be helpful. You need a chance to process what's happened, and taking a break from your regular routine will help you acknowledge and accept all that you're going through. 4. Don't expect your partner to grieve in the same way. I 5. Don't close yourself off from others. 6. Seek out professional help through a counsellor or join a support group near you. As a companion episode to Natalie’s story (Episode 3), Mary dives in and educates us on what MRKH is and how it affects fertility. MRKH syndrome is the common name for Mayer-Rokitansky-Küster-Hauser syndrome. MRKH syndrome is a condition where young women are born either without a vagina or uterus or with an underdeveloped vagina and uterus. MRKH affects at least one in 4500 women. There are two types and this condition can be diagnosed with imaging such as MRI or ultrasound or through laparoscopic surgery. As a companion to Kennie’s episode, Mary dives into how anemia can affect fertility. Researchers from India have found that “women who do not get sufficient amounts of iron may suffer anovulation (lack of ovulation) and possibly poor egg health, which can inhibit fertility at a rate 60% higher than those with sufficient iron stores in their blood.” This is from a 2014 report in the International Journal of Current Research and Academic Review . When the blood does not get enough iron, anemia, or an insufficient number of red blood cells, may develop. Since it is these red blood cells that deliver oxygen to all of the body’s tissue and organs including the ovaries and uterus, this deficiency may cause the eggs stored in the ovaries to weaken over time and become unviable. Worse yet, should conception occur, anemia makes it impossible for the growing fetus’ cells to divide and grow properly. This may result in a miscarriage in some cases. The symptoms of anemia are: * mild to severe fatigue * chronic headaches * dizziness * brittle or weak nails * decreased appetite * low blood pressure Speak to your doctor in order to get a blood test if you have any of these symptoms. Always have your iron levels checked by a health care professional before beginning supplementation. If it turns out your iron levels are adequate, supplementing can actually be dangerous. Supplementation is not always necessary if you’re not in an “unhealthy” state. Incorporating iron-rich foods into your diet can also provide major fertility benefits! Again, be sure to pair iron-rich foods with foods containing vitamin C! Foods Rich in Non-Heme Iron: * Legumes such as peas, beans, and lentils * Nuts and seeds including peanuts and hazelnuts * Dried fruits including prunes, raisins, and figs * Leafy green vegetables (kale, broccoli, spinach) * Whole grains such as oatmeal Foods Rich in Heme Iron: * Lean chicken, turkey, salmon, tuna, and eggs! * Avoid red meats as they are inflammatory and can be detrimental to fertility! “Don’t give up and know your body. It helped me a lot. I started learning more about my body and I started asking questions and doing research about what works best for me and my husband. If it doesn’t work for you, stop, do your research and get back at it. Don’t give up on faith. You’d be surprised how far faith will get you and believe in and standing on your word, not listening to everybody. Not everybody knows what’s going on. They may not have bad intentions and they may not know that (what they’re saying) is hurting you. And even for doctors, maybe sometimes you need a second opinion.” -Amy Amy tells Mary about her TTC journey since 2014. She first got pregnant in 2016 after years of trying to conceive naturally. However, she experienced spotting and unfortunately miscarried. After testing, Amy was diagnosed with fibroids, which the doctors believed was the reason behind the miscarriage. Amy felt hurt, emotional, sad. In 2017, she underwent a myomectomy to remove the fibroids. She recovered well and got pregnant again in 2019. Amy felt nervous and tried to stay on top of her health. However, she later lost her son prematurely due to an incompetent cervix. She was able to cope with this incredible loss through her faith in God and the support of her husband and her family. Amy felt that really getting to know her body and knowing what worked for her body helped her conceive. She took Vitamins A, C, E, Blood Builder, beet powder, probiotics, seed cycling, and changed her diet. She ate more fresh food. She cut out soda and sugar and increased hydration. Then let it go. Do art therapy. Watch movies, watch something funny. Do not forget about your partner, that there is a relationship there. Miscarriage or early pregnancy loss is more common than most people imagine, with as many as one in every four pregnancies ending this way. A miscarriage is the term used to explain the loss of a pregnancy in the first 23 weeks. Later loss is known as a stillbirth. Abdominal cramps (ranging from a dull ache to severe pain) and bleeding are the first signs of miscarriage. On the other hand, some women have no symptoms at all and may only learn that their pregnancy has come to an end when it is detected during a routine scan.There are three options for women who would prefer to actively manage their miscarriage. These are medication, surgical procedure (D & C), or induced labour. If you’ve been affected by miscarriage, speaking to a counsellor can give you an opportunity to express and accept your feelings about what has happened, in your own time. Although it’s easier said than done, it’s important to remind yourself that your miscarriage is highly unlikely to have occurred as a direct result of anything that you did or did not do. As a companion to Sadie’s episode, Mary dives into PCOS. Polycystic ovarian syndrome (PCOS) affects 5%-10% of all women of reproductive age making it the most common endocrine disorder among women. While the exact cause of PCOS is still not clearly identified, the majority of research implicates having excess levels of insulin or insulin resistance as its main cause. In fact, 8 out of every 10 women with PCOS could have insulin resistance, resulting in higher than normal insulin levels. This increase in insulin decreases sex hormone binding globulin (SHBG), which increases androgens (testosterone). This has a direct effect on ovarian cyst formation. The end result is that a PCOS sufferer may stop ovulating, gain weight, develop skin conditions like acne and skin tags, and grow abnormal facial and body hair. The clinical picture of PCOS varies widely from woman to woman, and because the symptoms vary so widely and not all women display all the symptoms, doctors often misdiagnose PCOS. Today, Natalie tells Mary about her long and difficult journey to motherhood. Natalie went through a difficult childhood and experienced a delayed period. She thought that “something is terribly wrong with me”. As a Christian, her faith was a comfort to her, particularly reading Psalm 91. Natalie was diagnosed with MRKH, after a whirlwind of tests were done. Connecting with friends, crafting, and reading helped her go through and process what she was told. Natalie has fostered children, had a uterine transplant, has gone through IVF, and is waiting for COVID 19 to pass to undergo surrogacy. For her health, she consistently takes a multivitamin, probiotic, magnesium, and a metabolism protein drink daily. Fibroids are benign, or noncancerous, muscular tumors that grow inside the uterine cavity—on the uterine wall or outer surfaces—or on organs inside the pelvic cavity such as the bladder or bowel. Fibroids usually occur during a woman’s reproductive years, most commonly from the ages of 30 to 40. Fibroids usually don’t cause fertility problems, but here is how they can. * They can change the shape of the cervix or the uterus, both of which can affect the ability of the sperm or fertilized egg to enter the uterus or to move properly. * Fibroids can block fallopian tubes. * They may cause the lining of the uterine cavity to change. * Fibroids can also affect the blood flow to the uterine cavity which can decrease the embryo’s ability to implant, or adhere to the uterine wall. Oftentimes your fertility doctor will discuss the size and location of your fibroids with you, as well as any problems they may be causing and possible treatment options, if necessary. As a companion to Monique’s episode, Mary dives into Hypothyroidism and how it affects fertility. Hypothyroidism is a leading cause of difficulty in achieving and maintaining pregnancy. Low thyroid function has a negative impact on reproductive health and is more common than most women realize. When your thyroid gland isn’t making enough thyroid hormone, it can have a serious effect on every organ in your body — including your reproductive system. This means hypothyroidism may make it difficult to conceive if you’re planning on having children. A study published in August 2015 in the Journal of Pregnancy found that women with hypothyroidism were less likely to become pregnant — and more likely to take longer to become pregnant — than women without the condition. That’s because women with hypothyroidism may not ovulate or ovulate with any regularity, and you have to ovulate to get pregnant. Hypothyroidism can affect fertility in men as well, according to the Thyroid Foundation of Canada. Although hypothyroidism is less common in men, those who do have an underactive thyroid may have low libido and low sperm count, according to a review of research published in November 2013 in Frontiers in Endocrinology in November 2013. If you’re experiencing symptoms of hypothyroidism — fatigue, increased sensitivity to cold, constipation, dry skin, weight gain, muscle weakness, and heavier than normal or irregular menstrual periods in women, among others — you should get tested for the condition. A simple blood test can reveal whether your thyroid gland is functioning normally. The test measures the amount of thyroid stimulating hormone (TSH) in your bloodstream, and high levels suggest hypothyroidism, according to the American Thyroid Association.  Hypothyroidism is one of the many common conditions that your doctor can test for as part of a pre-pregnancy health check, according to the American Pregnancy Association, noting that it’s better to identify and treat health issues before conception. A conversation about pre-pregnancy testing is especially warranted if thyroid health problems run in your family, the British Thyroid Foundation says. Women who have difficulty getting pregnant or who have had miscarriages previously should be tested When hypothyroidism is the reason for infertility, taking thyroid medication will enable most women to conceive, from as soon as six weeks after treatment, according to a study published in February 2015 in the IOSR Journal of Dental and Medical Sciences. The study also pointed out that many women who have a problem conceiving may have no apparent symptoms of hypothyroidism and only slightly elevated TSH levels, making it all the more important to have a TSH blood test if you’re having a hard time getting pregnant and don’t know why. Another study found that treating hypothyroidism with medication not only improved conception rates, but also reduced miscarriages early in pregnancy, which can happen as a result of untreated severe hypothyroidism. Those results were published in January 2015 in The Obstetrician & Gynaecologist. As a companion to Traci’s episode, Mary dives into balanced translocations and how they can affect fertility. A balanced or chromosomal translocation is a condition in which part of a chromosome has broken off and reattached in another location. In other words, it means that sections of two chromosomes have switched places. It can increase the risk of recurrent miscarriage. In about 4.5% of all couples with recurrent miscarriages, one or both parents has a balanced translocation. Research has shown that couples with balanced translocations are more likely to have miscarriages than couples without balanced translocations. Testing is done through karyotyping, which is a blood test done on both partners. Some research suggests balanced translocation in the mother is most likely to be associated with recurrent miscarriages, but fathers can be carriers as well. There is no cure for balanced translocation.In some cases, couples with balanced translocation conceive naturally or they can opt for a treatment called preimplantation genetic diagnosis or PGD. In PGD, the couple conceives via in-vitro fertilization along with gene testing of the embryos to be sure that they do not have an unbalanced translocation. Ovarian hyperstimulation syndrome (OHSS) is an excessive response to taking the medicines (especially injectable gonadotropins) used to make eggs grow. Rarely, OHSS can result from taking other medications, such as clomiphene citrate or gonadotropin-releasing hormone. Women with OHSS have a large number of growing follicles along with high estradiol levels. This leads to fluid leaking into the abdomen (belly), which can cause bloating, nausea, and swelling of the abdomen. When OHSS is severe, blood clots, shortness of breath, abdominal pain, dehydration, and vomiting are possible. Listen in to learn more about this condition. Sperm that have been washed and concentrated are placed directly in your uterus around the time your ovary releases one or more eggs to be fertilized. The hoped-for outcome of intrauterine insemination is for the sperm to swim into the fallopian tube and fertilize a waiting egg, resulting in a normal pregnancy. She had 2 egg retrievals back to back. But they decided to postpone the fresh transfer due to her husband’s diagnosis of COVID. As a companion to QuaShawnda’s episode, Mary dives into Hashimoto’s Disease and how it affects fertility. It is very important to solve thyroid problems before conceiving a baby. If Hashimoto’s Thyroiditis is left untreated, symptoms of an underactive thyroid can worsen and cause fertility problems including: menstrual cycle issues such as menorrhagia (heavy menstruation) or irregular periods, low basal body temperature (BBT), low libido, muscle cramps/tenderness, hair loss, severe PMS, sleep disturbances and weight gain, problems getting pregnant and infertility, miscarriage, preterm birth and even neurodevelopmental issues for your baby. Hashimoto’s Thyroiditis, also known as Hashimoto’s Disease (and frequently just referred to as Hashimoto’s) is an inflammatory autoimmune disease. Autoimmune diseases are accompanied by an elevated inflammatory response due to the body identifying various organs and systems of the body as a foreign invader. This immune system attack may lead to the loss of, and degeneration of tissue and/or organ structure and function. What to Consider Eliminating from Your Diet… * Gluten – “The gluten molecule is very similar to thyroid tissue, and it is thought that the immune system identifies the thyroid gland as gluten and attacks it.” * Non-organic dairy – non-organic dairy contains the synthetic growth hormone rGBH which has been found to increase inflammation in humans (even those without autoimmune diseases). * Aspartame – research is showing Aspartame to be problematic for the thyroid gland. * Unsaturated oils – doctors speculate that these oils may contribute to hypothyroidism possibly in part because they contain high amounts of omega-6 fatty acids which promote inflammation and are often rancid even before they make it to the grocery store shelf. * Soy – soy isoflavones are known to disrupt endocrine system function and may lower concentrations of the thyroid hormone T3. It would be best to consult your doctor or naturopathic doctor to run a thyroid panel to establish a baseline and a diagnosis before starting any supplementation. In this companion episode to Marguerite’s episode, I talk about ERA or Endometrial Receptivity Analysis. The ERA (Endometrial Receptivity Analysis) is a genetic test that evaluates the expression of genes to evaluate whether the endometrial lining is properly developed to accept an embryo.  This technology uses the latest scientific technology available to analyze the expression levels of the 236 genes to assess the optimal time to place an embryo into the uterus to promote a successful implantation and pregnancy.
