This is a very low barrier treatment for both the therapist or peer support worker to learn AND for the patient to get access to.
For myself, when I’m working with people who use drugs, it’s a great standalone treatment AND a great gateway therapy to more intensive work.

Wip does ask however what it was like to have had a recent history of drug use and attend a training on chemsex for health care providers, how he can do his job better, and JP gives Wip a few ideas of what to put up into his office to make the environment more inviting.

In this talk, the attendee will
A) learn about the the loose connection between the Indian gods and meditation, and the general origins of mindfulness, and how it connects the various psychotherapies.
B)At the end, the attendee will wonder which type of meditative practice to explore further.
Summary
Wip has always wanted to learn how to meditate better. And here, he explore some ideas that are still in its infancy in his mind with the goal of coming up with a list of concepts to explore. This talk was Inspired by a “short” from @LexClips with @johnvervaeke where Dr. Vervaeke shares the three ways to meditate and a free online course where he covers the different ways to meditate. Keep in mind, Wip uses the word mindfulness and meditation interchangeably and experts regularly try to clarify the difference in those two things.
This talk shares how Wip used to meditate, the difference of 3 waves of psychotherapy, signals in the Geeta on how to meditate, Wip’s foray into mindfulness, Different ways of knowing, Dr. Vervaeke’s 3 key skills for meditation, and wip considering adding yoga nidra and/or hypnosis.
The end conclusion, while not mentioned explicitly:
it’s not enough to learn ingredients, recipe, and read the menu - there comes a point where you want to enjoy the meal.
The above is a metaphor for learning meditation. It’s not enough to know intellectually. You have to practice and experience.
Hopefully Wip will be able to try out some kinds of meditation and bring the information back here.

In a previous talk, they discussed the content and compared and contrasted different kinds of assessments. Today, they discuss the “process”. This includes the context, the questions you ask, how you formulate the person, and how you give recommendations. Here they share different approaches in different situations, pros and cons of different ways of dressing, the importance of authentic acceptance before you can give advice on change, and most interestingly the value of direct vs indirect communication.

Addictions case manager and former addict Jean-Paul (JP) Michael reached out to addiction psychiatrist Dr. Wiplove (Wip) Lamba this week, seeking info on CRAFT (Community Reinforcement, And Family Training). Seeing there was an audiobook, he started reading the version for family members.
For substance use disorders care, we usually focus on the individual. The family is kept separate. Robert Meyers, a psychologist and scientist out of New Mexico developed the CRAFT model so that the family member can help the loved one get sober. There are three main goals to craft.
1. Help reduce your loved ones substance use
2. Help move them towards treatment
3. Improve your life whether or not someone stops using substances.
People are sometimes so stuck waiting until “rock bottom” that they forget we can sometimes create an environment where it is easier for the person to move towards sobriety.

Anton Chekhov is a famous Russian writer and playwright whom some argue is only second to Tolstoy in his acclaim. He grew up in poverty in an abusive household. Medicine was his main professions and he described writing as his mistress. This includes shifting the conversation from how the perspectives of psychiatry model is into how would you actually use it in practice. The concerns surrounding Contingency Management (CM) are many and reflect how poorly understood and rarely utilized this evidence-based treatment model is in practice settings. Despite being identified as the most efficacious intervention for substance use disorders, a significant gap persists between research and practice, at the client's expense. Nancy Petry, an experienced researcher and consultant for organizations such as the National Institutes of Health, has begun to fill this gap by authoring the first clinician-oriented text that focuses on CM protocol development and implementation. In this well-organized and clear book she provides a foundation for understanding CM and details how to design and implement a program that can work for any clinician, whether he or she works for a well-funded program or not. She also addresses realistic concerns such as: (1) How to describe CM to eligible and ineligible patients; (2) How to calculate the costs of CM interventions; (3) How to solicit donations and raise funds to support CM interventions; and (4) How to stock a prize cabinet and keep track of prizes. Over 50 charts, worksheets, and tables are provided to help the clinician pinpoint exactly which behaviors to target, brainstorm how to reinforce change, and develop a treatment plan that incorporates cost, length of treatment, and method for determining patient compliance. Comments by Wip - Wip mentioned that no one wants to be divorced at about the 8 min mark. He meant that he did not want to get divorced and used that language in referring to himself.
Around the 9 min mark, JP asked Wip about “how did you do it”. Thinking about it, Wip realized that it was not that he did “nothing”. He did start rock climbing and other sober activities in the evenings. He just didn’t remember in the moment as he wanted to share a short cut for behaviour change.

The concept of someone "faking" an overdose was viscerally disturbing to me as a former substance user. I needed to address it.)  In this impactful episode of our addiction medicine podcast, we diverge from our regular content to present a sobering and educational perspective. We scrutinize body cam footage documenting the final moments of Jean DesCamps, a 26-year-old man, at an Oregon hospital. This episode is a reminder of the complex nature of situations involving substance abuse. We aim to shed light on issues such as potential overdose, malingering, and the critical need for behavioral awareness in a non-judgmental manner, fostering a learning opportunity and addressing stigma in addiction. Jean DesCamps was discovered in a state of distress on Portland’s TriMet MAX train and was taken to Providence Milwaukie Hospital. Despite exhibiting symptoms of opioid intoxication, the response was limited, focusing more on perceived behavioral problems than on a comprehensive medical evaluation. This approach raises important questions about the assessment and treatment of patients with substance use histories. The body cam footage reveals DesCamps in an obtunded state, clearly struggling to communicate, groaning, and drooling. The decision by a medical to suggest removing him to a bus stop, under the assumption that his condition was feigned, becomes a focal point for discussion. This scenario illustrates the challenges in making accurate assessments and the potential for critical misinterpretation in treating patients with addiction. Law enforcement’s role is also highlighted in the episode. Called to assist with DesCamps’ removal from the hospital, the officers’ reluctance and their recorded conversations demonstrate the ethical challenges they face. Their eventual decision to take him to Unity Behavioral Medical Center, and what followed, underscores the serious nature of DesCamps’ condition. Wip loves this conference. When he started doing addiction medicine, he felt like he was in a bubble. Providing psychiatric care to people who use drugs or addiction medicine care to psychiatric patients may seem like commonplace, but usually those two fields are siloed. CSAM connected Wip to a community of health care providers that were really trying to figure out how to best serve people who use drugs. Anton Chekov is a famous Russian writer and playwright whom some argue is only second to Tolstoy in his acclaim. He grew up in poverty in an abusive household. Medicine was his main professions and he described writing as his mistress. He believed it was his role to share reality as it was and not provide answers and solutions, and that was clear in his stories.
Wip went through the audiobook and commentary for the proposal, the girl with the dog and the beggar. And interestingly, the beggar contained the major controversy in addiction treatment. The role of pushing for personal responsibility vs the value of compassion and serving the other.

Dr. Peter Selby is a Clinician Scientist and Senior Medical Consultant at CAMH. He is the Interim Vice-Chair of Research for the Department of Family and Community Medicine and holds the Dr. Barnett & Beverley Giblon Professorship in Family Medicine Research at the University of Toronto. He is a Clinician Scientist and Director of the Mental Health and Addictions Division in the Department of Family and Community Medicine. He is also a Professor in the Department of Psychiatry and in the Dalla Lana School of Public Health at the University of Toronto. He is also a full member of the School of Graduate Studies through the Dalla Lana School of Public Health and the Institute of Medical Sciences at the University of Toronto.
Areas of Research
Dr. Selby's research focus is innovative methods to understand and treat addictive behaviours and their comorbidities. He also uses technology to combine clinical medicine and public health methods to scale up and test health interventions. His cohort of 280,000+ treated smokers in Ontario is an example. His most recent program of research utilizes a Learning Health Systems approach to investigate how technology enabled collaborative care can enhance the delivery of evidence-based interventions to the patient while providing a more satisfying experience of care for both patient and provider.
Dr. Selby has received grant funding totaling over 100 million dollars from CIHR, NIH, and Ministry of Health, and has published 38 research reports prepared for the government.
Dr. Selby mentors residents and faculty from a variety of disciplines. He is a sought after speaker for various topics including addictive disorders, motivational interviewing, and health behavior change at individual and system levels.

We have always admired strong women. Wip’s Grandmothers and his mother were incredibly inspiring to him, and now he learns great life lessons through his wife and daughter on a regular basis. In over a decade of clinical work in psychiatry, both Wip and JP have worked under many strong women leaders.
This memoir was incredibly inspiring to us for several reasons.
1. This is a person at the peak of their career
2. The childhood experiences this person encountered would have caused PTSD like symptoms in almost anyone.
3. It gives clues as to the character traits and choices that people make that lead to resilience.
Interestingly, Viola dedicates her book to her entire family, names and all. This is her husband, daughter, 4 sisters, her mother and her father.
I have read several memoirs of people I admire, and this one, at this point in my life, has stayed with him the most.

Dr. Wiplove Lamba and Jean-Paul Michael went through Matthew Perry Audiobook, ‘friends, lovers, and the big terrible thing’ on his recovery journey. It was a moving and powerful experience. They learned about early attachment trauma, substance use, and the internal and extremely dangerous life threatening struggles he has faced.
They also learned about his experience in various treatment programs he attended and his near death experience. 
While both Wip and JP do not work in private treatment centers, some of the items brought up were extremely interesting and relevant to addiction practice in general. We are hoping to promote discussion and bring awareness to all things relating to addiction medicine and recovery, both from the practitioner and patient perspective. While we both believe in and practice harm reduction approaches when the patient prefers or if it is indicated, it did not come up in the book or in our dialogue around these quotes.

At John Hopkins, McHugh and Slavney developed the perspectives approach to psychiatry. Addiction psychiatrist Dr. Wiplove (Wip) Lamba regularly uses it in his clinical practice. In this episode, Wip and addictions case manager and former addict Jean-Paul (JP) Michael discuss the model, the different quadrants and how they help us understand the person and match to the appropriate treatment. Wip believes this allows for a broader understanding of the person than a DSM diagnosis alone, and it is a useful complement to whichever clinical approach any of you use. It divides compliments of the person into 4 areas: disease, dimensional, behavior and life story. Disease is what a patient has, dimensional is what the person is, behavior is what the person does and life story is what the person encounters.

Addiction psychiatrist Dr. Wiplove (Wip) Lamba and addictions case manager and former addict Jean-Paul (JP) Michael discuss the basic outpatient psychiatric assessment. Wiplove covers the key headings including identifying data, reason for referral, chief complaint, history of presenting illness, past psychiatric history, review of symptoms and personal history. JP has questions about how the assessment would change in different contexts and also asks “what is an addiction psychiatrist”. For interviews, Wiplove discusses how the content and process are both important. It’s not just what you ask but how you ask it.

Addictions case manager and former addict Jean-Paul (JP) Michael reached out to addiction psychiatrist Dr. Wiplove (Wip) Lamba this week, seeking info on CRAFT (Community Reinforcement, And Family Training). Seeing there was an audiobook, he started reading the version for family members. (Wip and JP currently prefer audiobooks to paper).
For substance use disorders care, we usually focus on the individual. The family is kept separate. Robert Meyers, a psychologist and scientist out of New Mexico developed the CRAFT model so that the family member can help the loved one get sober. There are three main goals to craft.
1. Help reduce your loved ones substance use
2. Help move them towards treatment
3. Improve your life whether or not someone stops using substances.
People are sometimes so stuck waiting until “rock bottom” that they forget we can sometimes create an environment where it is easier for the person to move towards sobriety.

Wiplove gives a brief overview of the didactic content of motivational interviewing. This covers the spirit which include partnership, acceptance, compassion and evocation. He talks about the skills the therapist needs, like the use of open ended questions, affirmations, reflections and summaries. He also discusses the goal, change talk. MI has moved away from “unmotivated” language to “change talk” and “sustain” talk. Keep in mind, MI is learned experientially in workshops, and didactic knowledge is insufficient at improving your clinical practice. In 2015, Jean-Paul ended his two-decade long entanglement with substance use. He has attempted to put that lived experience to a useful purpose. He joined St. Michael’s, Hospital Addiction Service, as one of their first Peer Support Specialists. Today, as a Community Case Manager he provides intensive case management for some of the most marginalized patients. When not working directly with patients, Jean-Paul’s other passion is as an harm reduction educator and as a consultant. Primarily providing guidance on how to better support Persons Who Inject Drugs (PWID). Prior to healthcare, Jean-Paul was a lifelong entrepreneur. At 19, he co-founded Northern Cards, which ultimately grew into becoming Canada’s largest greeting card company.
