I am so excited to welcome you to the first official episode of the Success with Sjogren’s podcast!  Over the coming weeks and months, my guests and I will share how modern medicine and lifestyle work better together.

 

So what exactly does an allergy & immunology physician do?

In its simplest definition, I see children and adults with misbehaving immune systems. I primarily see patients who are struggling with seasonal allergies, chronic sinus problems, asthma, food allergies, allergic skin rashes and conditions that cause hives and swelling. Although less common,  I also care for patients with mast cell and eosinophilic disorders and immune deficiencies.

 

When we think about the immune system misbehaving, allergies, autoimmune diseases, and immune deficiencies all signal that the immune system is dysregulated or out of balance. In the case of allergies and autoimmunity, the immune system is recognizing something as the enemy that should be tolerated or ignored. Unfortunately, both of these types of conditions can occur more often when there is an immune deficiency too.

 

In short. The immune system gets very complicated very quickly!

 

So what exactly is Sjogren’s syndrome?

Sjogren’s syndrome is a chronic autoimmune condition where the body recognizes the exocrine glands. These are the glands that produce moisture in our body. They include the glands that produce tears, saliva, sweat, breast milk, skin oils, mucus, and more.  White blood cells called lymphocytes relocate to these glands and the inflammation produced results in tissue destruction. The end result is are classic symptoms of dryness but this is just the tip of the iceberg. Fatigue, joint and body pain, and digestive problems affect the majority of those with Sjogren’s too.

 

Traditionally, Sjogren's has been classified as primary or secondary meaning that it can be an autoimmune disease diagnosed on its own or the symptoms can be related to another connective tissue disease like lupus.

How and why this happens is still not fully understood but we will take a deep dive into the science in a future episode. In short, there is a combination of both underlying susceptibility and environmental triggers that are implicated.

 

Sjogren's is estimated to affect 4 million Americans with the vast majority of those affected being women. The average age of diagnosis is around 40 but diagnosis can be difficult to get and takes several years on average.

 

So why am I here? Oh goodness, there have been so many reasons over the last few years…

Where do I start?

 

When I was first diagnosed, I was so overwhelmed. I was exhausted and in pain- both the physical pain but also the emotional pain associated with my diagnosis. I also was incredibly overwhelmed.

 

After the initial shock and I dried my tears, I found myself frustrated that Sjogren’s lacked the effective disease-modifying treatments that similar conditions like rheumatoid arthritis had. Subsequently, I found myself searching high and low for any and all other options that might be out there. Diet. Vitamins. Supplements. Exercise. Mindfulness. You name it.

 

Sadly, what I found was a lot of speculation, slick sales pitches and very little science.

 

With my medical background, it made sense to me to approach my health from the perspective of AND rather than OR.

How could modern medicine AND adjusting my habits possibly improve my health?

Some small studies in asthma and thyroid disease had demonstrated some improvement in inflammation with diet changes. I knew stress was a huge factor in immune system health from research performed at my own institution.

 

But I struggled to find resources that had a basis in the science I was able to find or came from the perspective of AND rather than OR. I also struggled to find resources specific to Sjogren’s.

 

So here I am, several years out from my diagnosis, continuing on my own healing journey. I will always be a work in progress. But I am committed to my oath to do no harm which means taking a look at the possible risks and benefits of particular testing or treatments. I am also committed to building the Sjogren’s Sisterhood community- an uplifting space where each of us can say goodbye to the fog and fatigue & rediscover our vibrant, confident selves.

 

The reality is the vast majority of us with Sjogren’s will not have our lives shortened by this disease but we know the symptoms we are faced with can really take their toll on our quality of life. I look forward to sharing what tips, tools, and techniques I have used to forge my own success with Sjogren's so that you too may be able to find your own version!!

 Amber Patterson, MD is a world-renowned allergy/immunology expert and the US ambassador for intralymphatic allergen immunotherapy (ILIT). Before founding Allergy Unlimited, she developed the first US clinical trial for ILIT (first in the world for pediatrics) as faculty at the Ohio State University and Nationwide Children’s hospital where she founded the Center for Immunotherapy. She holds several patents on novel allergy testing devices. Dr. Yu is an integrative rheumatologist who incorporates complementary medicine with traditional rheumatology.

He is triple board-certified in Rheumatology, Internal Medicine and Lifestyle Medicine. He obtained his MD from Chicago Medical School and holds a Masters in Healthcare Administration and Biomedical sciences. He completed his internal medicine residency and rheumatology fellowship at Loma Linda University in Southern California. He has been accepted with a full scholarship to the Andrew Weil Integrative Medicine Fellowship at the University of Arizona. In addition, he has also taken courses in functional medicine through the Institute of Functional Medicine.

Dr. Yu has a very unique perspective on autoimmune disease and arthritis as he is both a patient with arthritis and physician. Dr. Yu was diagnosed with gout at the age of 17 and later diagnosed with spondyloarthritis as well. He is able to understand his patient’s medical problems from a patient perspective. The foundation of his practice is to combine allopathic medicine with complementary medicine. He works with his patients to come up with a treatment plan that not only fights the disease but also is aligned with his patient’s goals.

His primary focus is on autoimmune disease and arthritis.  Rheumatologists treat conditions such as osteoarthritis, rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, lupus, sjogrens, gout, fibromyalgia, myositis, vasculitis, and other inflammatory conditions.  As a fellow, he has published multiple papers on osteoporosis and spearheaded a double-blind randomized control trial on curcumin supplementation in systemic lupus erythematosus.  He has also had multiple presentations at the American College of Rheumatology national conferences.

Not only is Dr. Yu a doctor, but he is also a patient himself.  He has both gout and spondyloarthritis.  He has been featured in multiple articles and webinars including Forbes magazine, Spondyloarthritis Association of America, Lupus LA, and CreakyJoints.

In this podcast episode, we will talk with Jeff Chilton of Nammex, and gain an understanding of mushroom cultivation, the safety, the things to watch out for, what we are looking for in these products, why they are helpful, why should we consider them, and why it's essential to look for certified organic mushroom extracts in your supplements.

Jeff Chilton studied Ethno-mycology at the University of Washington in the late sixties and in 1973 began a 10-year career as a large-scale commercial mushroom grower. Jeff is the co-author of The Mushroom Cultivator, published in 1983.

In 1989 Jeff established Nammex, the first company to supply medicinal mushroom extracts to the Nutritional Supplement industry. In 1997 he organized the first organic certification workshop for mushroom production in China. Jeff is a founding member of the World Society for Mushroom Biology and Mushroom Products in 1994 and a Member of the International Society for Mushroom Science.

Nammex extracts are used by many supplement and food companies and are noted for their high quality based on scientific analysis of the active compounds.

Nitika Chopra is the founder of Chronicon, a media and events company, dedicated to elevating the lives of those living with a chronic illness.

Nitika was diagnosed with psoriasis at the age of 10 and psoriatic arthritis at the age of 19 and lived over 17 years of her life being defined by her conditions.

In 2010 she decided to take all of the lessons her health journey had taught her and use it to help others with the creation of her online magazine Bella Life.

Since the start of her entrepreneurial journey, Nitika has hosted her own TV talk show on Z Living called Naturally Beautiful, hosted over 40 events with hundreds of guests in attendance, and has created dynamic partnerships with over 150 brands in the wellness space.

After 10 years of event creation, Nitika launched Chronicon in the fall of 2019, focused on those living with a chronic illness.

The event sold out, had over 2,000 live stream viewers, and high-level brand sponsors, and has been growing ever since.

Lena Shahbandar is a physician specializing in Physical Medicine and Rehabilitation (physiatry–pronounced: fiz-eye-a-tree).

A graduate of Northwestern University for undergraduate and medical school, she practices outpatient musculoskeletal medicine with a particular area of interest in women’s musculoskeletal health and pain in pregnancy.

In treating these patients and through the experiences she had herself as a mother, she discovered the need for a solution that allows mothers to safely transport their babies without compromising their physical comfort. She spent many years pouring her medical expertise and knowledge into a scientific solution: a baby carrier that lets people easily move their babies from the car seat, or anywhere, to babywearing, and lead a full life without pain. Isabelle Amigues, MD, is a physician scientist with a speciality in rheumatology. She has trained both in Paris, France as well as Columbia University, in New York City. She is based in Denver, CO. She is the author of multiple book chapters and scientific articles in rheumatology.

At age 40, in the midst of an existential crisis, she was diagnosed with stage IV metastatic breast cancer. A timely meeting with a non-traditionally trained practitioner taught her a different approach to disease. She experienced the power of meditation, visualization, energy healing and love. Her journey through cancer inspired her to learn more about these alternative methods, and now that she has studied many of them she has integrated them into her own practice of medicine.

The Crunchy Allergists Podcast is back for its third season, and in this episode, Dr. Kara Wada sets the stage by introducing the immune system.

The immune system is responsible for preventing and eradicating infections, as well as defending against tumors and cancers. However, it can also damage cells and induce harmful inflammation in cases of allergy or autoimmunity.
There are two main types of immunity: innate and adaptive.

Innate immunity is our first line of defense against infections, and is activated immediately when a pathogen is detected.

Adaptive immunity, on the other hand, is slower to respond, but offers more specific and long-lasting protection.

To understand how the immune system functions, it's important to know about the different cells and tissues that make up the system, and how they work together to find and respond to infections.

Dr. Wada will be exploring these topics in greater depth throughout the season, as well as discussing specific conditions and treatments related to allergy and immunology.

ABOUT DR SARAH MERITT
Dr. Sarah Merritt is originally from Huntsville, Alabama. She attended Birmingham-Southern College and medical school at the University of Alabama in Birmingham. After a residency in anesthesiology at the University of North Carolina in Chapel Hill, she elected to pursue extra fellowship training in pain management at the Johns Hopkins Medical Institute. Her training included the full spectrum of pain management techniques, including interventional pain blocks and injections, as well as pain medication management. Dr. Merritt is ABMS Board-Certified in Anesthesiology and Pain Medicine.

Currently, Dr. Merritt is active as a volunteer in the medical community in addition to her medical practice. She was nominated by her peers and appointed by the state as a member of the Technical Access Committee for the Maryland State Prescription Drug Monitoring Program (PDMP). She is also the Co-Chairperson of the Legislative Council for MedChi, the Maryland state medical society.

In her spare time, she enjoys spending time with her family, bicycling, running, and yoga.

ABOUT DR SIRI CHAND KHALSA
Dr. Khalsa has had a life-long interest in mindful living as the basis for long-term vitality of mind, body, and spirit. She completed a residency in Internal Medicine at The Mayo Clinic in 2005 and is board-certified in Internal Medicine, Integrative Medicine, Lifestyle Medicine and Hospice/Palliative Medicine. Dr. Khalsa has deepened her studies on health and healing by becoming a Yoga Instructor, Reiki Master, and participating in a 2-year full-time program on Ayurveda at The Ayurvedic Institute in Albuquerque and India. Serving as an Integrative Medicine PCP, an instructor at the University of Arizona Integrative Medicine fellowship for physicians, and consultant to other medical practices, she has dedicated her career to promoting an increased understanding in clinical medicine of the link between long term vitality and the daily choices we make. She is currently focusing her energy to support physicians who want to expand their personal understanding of new ways of healing through an experiential process utilizing techniques in Ayurveda, yoga, mindfulness, and plant-based nutrition.

Dr. Saimun Singla is a triple board-certified physician that specializes in pediatric rheumatology and integrative medicine. Her story is unique because she was diagnosed with rheumatoid arthritis AFTER becoming an attending rheumatologist, causing her to slightly pivot her career.
Her pain points as both a patient and doctor led her to found Rheum to Grow, a private medical practice where she helps children and adolescents with autoimmune conditions achieve faster disease control, less disease flares, less reliance on pharmaceuticals, and realistic coping strategies to build resilience in the face of life-long illnesses.

Matthew Zirwas, MD is an American Board of Dermatology certified physician. He is a nationally known expert who has been specializing in contact dermatitis, pruritus, atopic dermatitis and seborrheic dermatitis for the last 20 years.

In addition to the practical experience that comes from taking care of challenging patients, he has published over 195 peer reviewed articles, been an investigator on over 80 clinical trials, co-authored the 7th edition of Fisher’s Contact Dermatitis, and has given over 200 national continuing medical education lectures to his peers.

His passion figuring out the real, practical, take-home messages that we can get from the medical literature. Not just reciting ‘the data’ or what the authors said, but synthesizing it and putting it in the context of how it can help patients. Basically figuring out ‘what this study REALLY means’ and ‘how does this connect to what we already know’, then explaining it in a practical, relatable, entertaining and memorable way.

Dr. Dawn Harris Sherling is a board-certified internal medicine physician who is also board-certified in obesity medicine. She began studying journalism before switching to pre-med studies at the University of Florida where she graduated with honors and as a phi beta kappa. She earned her M.D. with honors from the Yale University School of Medicine and then completed her residency at Harvard’s Brigham and Women’s Hospital where she went on to serve as an attending physician and Instructor in Medicine at Harvard University. She moved back to her native Florida in 2008, and currently sees patients at a clinic for underserved populations and is an associate program director for the internal medicine residency at the Charles E. Schmidt College of Medicine at Florida Atlantic University. She is most recently the author of Eat Everything: How to Ditch Additives and Emulsifiers, Heal Your Body, and Reclaim the Joy of Food. Dr. Baumgaertel, an internal medicine physician with 30 years of experience, founded her business myMDadvocate in 2022, transitioning from in-person clinical medicine to the virtual medical advocacy arena. While the 2020 pandemic raged, her innate creativity blossomed with the launching of MenopauseMenu, a free resource and blog website providing nourishing support for menopause and beyond.

Always a keen observer of the human condition and a lover of nature, Susan recognized her strengths as a storyteller and applied them to a variety of nonfiction writing venues. ABOUT DR. ISABELLE AMIGUES
Isabelle Amigues, MD, is a physician scientist with a speciality in rheumatology. She has trained both in Paris, France as well as Columbia University, in New York City and was an Associated Professor at National Jewish Health. She is based in Denver, CO where she sees her patients. She is the author of multiple book chapters and scientific articles in rheumatology. She was on the path to becoming a full time researcher when she was diagnosed with stage IV metastatic breast cancer. A timely meeting with a non-traditionally trained practitioner taught her a different approach to disease. She experienced the power of meditation, visualization, energy healing and love.
Her journey through cancer inspired her to learn more about these alternative approaches, and now that she has studied many of them she has integrated them into her own practice of medicine. She is the co-creator of a community meeting called coExist where inspiring speakers share their wisdom to improve people’s lives. She has her own podcast called UnabridgedMD and a youtube channel called Rheumatology 101.
