Okay guys, here's a hard truth.


All of your procrastination is ultimately due to you trying to avoid emotions. Your want to avoid the unpalatable or even downright repelling emotion of what every task or activity you planned to be doing may be conscious or, more commonly, subconscious.


When not conscious (aware) of our reasoning, our minds will default to avoiding discomfort. Whenever we're not conscious about our choices during our day, we are likely to subconsciously choose to avoid the discomfort.


When we appreciate that about our human minds, we can start to appreciate how important it is that we prepare ourselves with strategies for following through with things we associate with discomfort - even the SLIGHTEST tinge of discomfort.


In this episode, we zoom in on what things we typically find ourselves procrastinating on and dissect out what discomfort is driving that procrastination.


If you can see yourselves in these examples, you will have a first key piece of awareness that can allow you to break your procrastination Cognitive Programs in the future. Getting conscious of something that is currently subconscious to you is an enormous step towards being able to change it.

Sticks and stones may break my bones but SILENCE… Well, it makes me a wee little bit crazy…

 

Negative feedback is not the only place that our minds can spin out on us. For many of us, we have a painful reaction to receiving no feedback at all. In that space, our mind fills in the gaps between people's (in)action and their implications.

 

Specifically, the situation we're talking about is when we're consciously or subconsciously expecting recognition or positive feedback ad we don't get it. We make a simple calculation: if positive feedback means I'm on track, I'm not failing, I'm good enough. 

 

… Well then, a lack of positive feedback means I'm not on track 

… I'm a failure 

… I'm not enough 

 

You personally may not recognise such blatant statements filling this space in your mind, but nonetheless, this episode is here to encourage you to notice where you have unmet expectations that a reaping havoc on your happiness and confidence.

 

This is particularly important for those of us working as doctors or in the healthcare in general as a culture that hasn't got a lot of positive feedback (or feedback of any flavour) built in. This isn't something that is going to rapidly and dramatically change, so let's do what we can to best support ourselves to show up and have the experience we want.

You're a human too. You Don't Get to Be Perfect as a Doctor


On one hand, we can all nod our heads at the idea that perfectionism is unrealistic, unfair to expect of ourselves and even detrimental to try to aim for...


... But what about when you're a doctors? What about when your performace effects other people in their most vulnerable states?


... Isn't this a time where we really should be perfect?


In this episode, we discuss the unrealistic expectation of perfection in medicine. We bring to light a difficult fact, which is that doctors are humans and humans are fallible. At times, this means patients will, to our devastation, not be perfectly cared for.


We discuss the importance and our reliance on the "Swiss Cheese Model" of caring for patients and how teamwork is essential to minimising and preventing medical errors.

  • Doctors are expected to be perfect, but this is an unrealistic expectation.
  • Ironically, perfectionism can lead to burnout and medical errors.
  • The "Swiss Cheese Model" shows how multiple layers of defense can prevent errors.

27 Feb 202425: Are you AFRAID to be POSITIVE? The PRODUCTIVITY of positive feelings00:42:58

For a long time, Becc was afraid that being positive would mean she'd drop the ball. She thought her self-doubt and self-criticism gave her an edge. 


She couldn't imagine success that didn't come with anxiety and stress. 


In this episode, we take a page out of Kristeen's core and characteristic outlook on life: using positive energy. We've both struggled with negativity in the past, and let's be honest, who hasn't? But guess what? We've also discovered the amazing power of positivity and how it can completely transform your life, both inside and outside the hospital.


So, how do we avoid getting stuck in the "worst-case scenario" loop? Well, we've come up with a handy framework to help you flip the negativity switch:

  1. Become Aware: Pay attention to your thoughts. Are they making you feel bored, overwhelmed, or stressed?
  2. Ask Why: Pinpoint the specific thoughts causing those feelings. What stories are you telling yourself?
  3. Set an Intention: Decide how you WANT to feel. Maybe you crave calmness, focus, or confidence.
  4. Find the Thought: Identify thoughts that can evoke those desired feelings.

Here are some practical tips Kristeen uses to stay positive:

  • The Joy List: Create a list of activities that bring you immediate joy (dancing, singing, listening to a favorite song) and do one when you need a quick mood shift.
  • Clear Your Head: Meditate, take a walk, or do anything that gives you mental space.
  • Set the Intention: Declare how you want to feel and who you want to be in the present moment. This helps you embody your best self.

Remember, everyone, shifting towards a more positive mindset is a journey, not a destination. Don't expect instant change! Experiment, observe the results, and find what works best for YOU on your path to a brighter outlook. As always, you're not doing it wrong!

Many of us hold an idea about what a 'typical' doctor is... Or even a 'good' doctor. Consequently, it's common to feel 'othered' in medical culture.


It's never *easy* to feel othered. But, together, we can make this not only a better experience for you but also even a wildly enhancing one.

What's a common feature of a doctor's life? The DEADLINE AVALANCHE


One deadline... Piece of cake

Two deadlines... No problem 

Three deadlines... Okay we can do this 

Four deadlines... IT'S ONLY TUESDAY 

 

Time management 101 isn't enough to save us in this situation of competing demands and conflicting priorities 

 

The deadline avalanche can be managed by SURVIVING. This looks like staying up late to get everything done, forgoing things you'd rather keep, and even sacrificing things you said you wouldn't. 

 

Becc can't forget missing a wedding of a loved one when she was in med school. At the time, it seemed necessary to meet the demands of these deadlines. 

 

On the other hand, the option is not always obvious as a possibility... is THRIVING in the deadline avalanche. What this requires is DIVERTING resources away 

 

It's true, we can't do it all. 


But it's okay, because we CAN do what's important. 

 

So here's the catch - and the strategy. To free up our resources for what's important, we need TAKE THEM from somewhere else. 

 

And we can hear some of you… PERMISSION TO DO LESS? I'm IN. 

 

And then some of you like… PERMISSION TO DO LESS? … But … It won't be good enough 

 

THOUGHT: It won't meet the mark unless I give all of my effort

 

Becc used to have this same thought process. And it meant that she could never give herself permission to give less than 100% effort to any of these competing deadlines. It was too much of a risk. 

 

The problem with this is, she also never tried less than 100% to see if it would suffice. 

 

Evidence base for success giving LESS THAN 100% effort: nil 

Evidence base for success giving 100% effort: robust 

 

When we say 'There's no evidence for that' in medicine, we want to be clear if we're trying to communciate: 

 

"There's experiments that don't show evidence for that to be true"

OR

"There's no experiments that investigate that question"

 

And here's what we have to offer on this: 

 

The belief that is POSSIBLE that less than 100% effort needs to be strong enough for you to choose to do the experiment. If you don't believe it's possible, why try, right?

 

So, here's an invitation from our shared experience: 

 

It's possible that you can GIVE LESS EFFORT to some of these deadlines WITHOUT LOSING ANYTHING. 

 

With that extra effort diverted to the deadlines that are IMPORTANT, it's possible you can GAIN A LOT.

 

The key here is first getting clear on what's important before your mind gets carried away with what's urgent. 

 

When you are clear about what's IMPORTANT, you'll have the fuel for the bravery required to divert resources away from its competition. 

 

Sometimes good enough is good enough.

In this episode of Am I Doing It Wrong, we dive into a common self-care misstep: thinking that JUST ANY time away from work will be rejuvenating.

Nope.

Just because you're not at work or working doesn't mean you're putting deposits in your energy bank.


In particularly in this episode, we dive into how we've fallen into the trap of mistaking *socialising* for a method or rest and rejuvenation. Maybe you can hear this in your thoughts too as you say, "I've had the weekend off and spent time hanging out and having fun with friends. So I should be refreshed for work on Monday".

In fact, having MORE social time can actually create the need for MORE rejuvenation time. It's all about making sure you can listen to what your body & mind is asking for in the moment.

 

There are 2 main ideas we want to prompt you with

1️⃣ We want you to stop thinking something is wrong with you if you DON'T feel refreshed after a weekend socialising. For may of us, we have to recognise this as something that we want to invest energy in but that we don't *always* get energy back from.

2️⃣ We want you to be so clear about HOW YOU REJUVENATE so that when you want do, you can hit the right buttons to get that result.

🧠 Kristeen and Dr. Becc share personal stories of how, even with a break from work, genuine rest and rejuvenation often require solitude.

🧠 Reflecting on how grief, loss, and intense social commitments can amplify the need for personal time and self-connection.

2️⃣ Evaluating the close-circle energy vampires in your life: Not everyone is good for your well-being. It’s okay to distance yourself from people who drain your energy. If you're someone who values making an impact in the world, it's all about looking at if net-good is created from your choices.

  • Becc explains the difference between a shame spiral and processing a difficult experience. Shame spirals involve stacking negative stories on top of the initial experience, whereas processing an experience involves understanding the emotions and learning from them.
  • Kristeen used a mindfulness technic called grounding to help her connect with the world around her in the present moment. This was a great example of how using mindfulness doesn't have to look like sitting cross-legged in silence but she engaged in sensory experiences like smelling candles to make her mindfulness more enjoyable yet effective.
  • Becc explains the concept of mood-congruent memory bias, which is the tendency to recall memories that reinforce the emotions we are currently feeling. Shame spirals can worsen this bias.
  • Shame can be a signal that something needs to be addressed, but it's important to go beyond the surface-level thoughts and identify the deeper beliefs that are causing the shame.
  • By meeting the reality of the shame experience and challenging the negative thoughts associated with it, we can break free from the shame spiral and move forward.

Why are doctors burned out?

THIS EPISODE is an important episode for anyone out there who is not feeling like they're living a life that they enjoy living in. This episode is not only for doctors who are at that awful, end stage of burnout but it's for anyone who feels like 

… your hand work isn't 'working' - it's not giving you back the results you want 

… you're willing to work hard but it's not feeling worth it 

… no matter how hard you work, you're not getting the results you want 

… no matter how hard you work, it's not fixing the problems

… no matter how hard you work, it's not filling you up 

 

The way that the burnout conversation is oversimplified at the moment has a lot of highly intelligent people feeling stuck.

 

The problem is, if burnout is a problem of working too hard… Of workloads being too high… Of trying to do too much with too little. 

 

We're stuck. 

 

Our only option is to work less or to work less hard. In our current system, that brings up unacceptable subsequent problems for doctors: 

 

"BUT my patients will be the ones who miss out and suffer if I do less"

"BUT my colleagues will just have to do more if I do less"

 

Enter: MORAL INJURY. 

 

Here, the only option is to 

… allow our patients to have a lesser standard of care 

… put our equally stretched, distressed and struggling colleagues under more pressure

 

Those solutions are simply not acceptable to us. 

 

Notice, we can either work less hard to solve our burnout… But then we face moral injury. 

 

Or 

 

We can lessen the moral injury at the cost of working more and becoming burnt out. 

 

We think the problem is working too much. 

So we need to work less.

But when we work less, the consequences are unacceptable. 

 

It's no surprise that we feel STUCK. 

 

… trapped 

… defeated 

… hopeless

 

The current burnout conversation largely centres around what is DRAINING & DEPLETING us. And that's an incredibly important aspect of burnout. 


But if we don’t look at the other side of the equation, we miss out on the other half of the solutions available to us: what's ENERGISING & FULFILLING to us. 

 

In this episode, we talk about common misunderstandings of burnout that keep us reliant on solutions that don't address the problem. We uproot the thought errors that perpetuate these cycles working hard to over-compensate for an inadequate healthcare system. 

 

We talk about THE OTHER SIDE of the burnout equation: doing meaningful, fulfilling work. And how we can realistically do that in the setting of an under-resourced healthcare system.

Last week we talked about the PROBLEM. This week, let's talk about the SOLUTION. 

 

Last week's episode, episode 20, was an episode that resonated with a lot of you - we started to dissect why clinical work seems to be especially exhausting cognitively and emotionally… And because of that, also physiologically and physically.

 

Paradoxically, an important part of finding solutions for ourselves is getting closer to the reality of the problem. When we can see the problem clearly, we are better positioned to choose impactful solutions. So if you haven't listened to our last episode, we'd highly encourage you to start there in favour of 'shortcutting to a solution'. 

 

In this episode, we introduce the concept of DEPOSITS and WITHDRAWALS on your energy. NOTE: thinking about energy in terms of deposits and withdrawals is not an original concept but it is widely spread and we are unaware of the original creator to give credit to.  

 

In this episode, we first talk about what it really means when we say we want to rest or we need rest. For most of us, what most of us mean is that we want to feel less tired and we think that being idle and not exerting ourselves is the way to achieve that. Like us, many of you may have found the 'just rest' and 'just do less' advice… essentially useless. It seems that with this model we can never seem to rest enough. 

 

Luckily, there's a more sophisticated way to think about rest. In this episode, we detangle the result of feeling rested or more energise with being idle or creating ease in the body. 

 

And, as always, we share our own personal examples of unexpected ways that we create more energy for ourselves so that you can try it on and see if it fits in your life.

We all leave behind a trail of unanswered questions in our day-to-day lives. 

 

As a doctor working in the boundless universe of things we can know to make us better doctors, asking questions at opportune moments is an advantage that can't be compensated for with publications and uptodate articles. 

 

In this podcast, we talk about Becc's accidental step out of her integrity when she let herself silence a clinical question that arose in her mind. 

 

There are many reasons any of us may not ask a question when it would otherwise be in our integrity to do so. 

 

It reveals our knowledge gaps 

We think we "should know this by now"

We think it's not acceptable not to know the answer 

We think we should learn it ourselves, on our own time

We don't want our colleagues to draw false assumptions about us 

We don't want our colleagues to draw true assumptions about us 

We assume people don't want to take time to answer questions 

We think asking the question is annoying 

We don't want others to feel frustrated with us 

We don't want to make others uncomfortable if they don't know the answer 

We would rather avoid any negative reaction

We decide it's not worth it if there's a possibility of a negative reaction 

We are uncomfortable taking up space

We are uncomfortable asking for others' time 

We don't think we have 'earned' the question 

 

And this is just a small sample of thoughts that can accidentally get in our way. 

 

There's another component too. 

 

Simply not being present and intentional. 

 

Being present and intentional are critical ingredients for creating change in our lives. This requires core skills of mindfulness and directing attention (focus). It also often requires some time spent in advance to decide what helpful rules you create for yourself in advance of opportunities. 

 

For example, you might decide in advance, like Becc, that you will err of the side of asking questions. In this case, the decision in the moment has been made simpler - I will ask my questions UNLESS there is an outstanding exception (say, time-critical situations). 

 

There's a lot to be said for setting intentions, especially when you consider biases of minds such as CONFIRMATION BIAS. 


If we set an intention of asking questions - and taking opportunities to ask more questions, we will register more opportunities in my day. Opportunities that may have been missed if we were prioritising other things.

We start this episode on dealing with devastating situations as a doctor with Kristeen's memory of needing to lift her hands from the chest of her patient after the decision had been made to stop CPR. Any person would agree this is a discomposing situation to face for the first time - or even the fiftieth. However, it's not an unfamiliar situation for any doctor.   

 

Today's podcast holds an important conversation about how we respond both to ourselves and in our medical culture when bad things happen, which they frequently do in our work.

 

Doctors find themselves somewhat relentlessly in situations that aren't most humans would find challenging. Our work selects in for a high level of exposure to tragic human situations. 

 

When it comes to maintaining our integrity and health as doctors, while most of us would agree that emotional suppression is not a highly functional or adaptive long-term strategy, it's a strategy that many of us employ subconsciously. 

 

Is it because we don't know how to do anything else?

 

Is it because our fast-paced, busy clinical environments don't create space for an alternative?

 

Is it because we actually think it's the most productive and adaptive response for a doctor?


Is 'normalising' to suffering and loss a good thing as a doctor?

 

By asking and answering these questions, we hope to create some more clarity around what's going wrong, what's going right, and what we might be able to do better for ourselves and for our medical culture. 

 

We are looking forward to you chiming in on this one. Please share your experiences and wisdom so that we can collectively learn how we can show up for ourselves, each other and our patients when bad things happen. 

02 Nov 20231: Pretending as a Doctor. Can doctors really have balance? 00:41:16

Everyone deserves a space in their human lives to be their real human self - even doctor humans. 

 

It's less common today in a medical culture of strict expectations of this controversial thing we call 'professionalism', which can be unforgiving to the human condition. 

 

Life can be simply described a series of joys and dilemmas and there's no human who's exempt. To pretend that our reality is not real makes a challenging job and life even more so.

 

It's time for doctors to have permission to be more human. You don't need to pretend.

 

… It's exhausting.

 

And, there's something better on offer: authenticity. 

 

In this episode, we start with a health-related guilt that plays on the minds of many doctors - that we should be balanced. 

 

The nature of medical training programs in Australia is such that trainees are often left with little time in terms of absolute hours and also little control over their hours. 

 

The irony is, the popular ideas of what it is to be 'healthy' and 'balanced' create a demanding list of wellness activities that we should be doing. 

 

It's easy to look at your life as a doctor and think, "I'm doing it wrong" 

 

And this can lead us to override our actual needs in the name of ticking the 'balanced' box. 

 

And we haven't even started to talk about the effect of the guilt that this 'balance' prescription creates. 

 

We want to relieve you of the idea of 'balance'. Instead of fitting into this idea of what a balanced life looks like, a better question is, 

 

"What do I need right now?"

 

Leave your expectations at the door. Take your finger off the override button. And trust that you know what is best for you. 

 

Sometimes, we require something that looks less like classic 'balance' and more like 'survival mode'. At those times, the best thing we can do for ourselves is not add a layer of guilt and inadequacy around what we 'should' be doing.

 

This will likely be a new process for your mind to go through as many of us hold these firm ideas about what's 'balanced' and 'healthy'.

This is a MAJOR part of being a doctor that doesn't get enough attention. It needs to be done well to create good outcomes - for our patients and for ourselves as doctors and teammates. 

 

As the doctor coming on, you have a short period of time to extract all of this important information from your colleague. Information that isn't always documented or possible to find elsewhere. You're receiving a lot of important information at once. You're also often handed a lot of uncertainty. 

 

As the doctor giving the handover, you're synthesising and summarising to save your colleague's precious brain space. And you're not only communicating what you do know but, importantly, what you don't know. 

 

… That's a lot.

 

And it's the perfect storm for these mindsets of perfectionism, imposter syndrome and people-pleasing to come along and make your life harder.

In this episode, Kristeen & Doctor Becc will talk about the most empowered way to use boundaries with both a short-term and long-term game plan in mind. Here, boundaries can be used as a temporary measure to buy us time to address the underlying issues that are causing us to need them in the first place. That's how we use boundaries to make our lives BIGGER - not smaller.

Here's some cliff notes:

❤️ Boundaries can be implemented to protect & maintain our standards and safety.

❤️ Setting boundaries is about taking control of your own experience - not about controlling, manipulating or sending a message to other people.

❤️ We may want to use boundaries as a temporary measure when we find ourselves feeling affected by another and we are not able to attend to our thoughts or nervous system response in the moment.

❤️ Ultimately, we want to aim to be so secure in ourselves that boundaries become unnecessary - others don't affect us.

❤️ As humans - relational beings - we want to acknowledge that this is an *ideal* to work to rather than something that you 'should' have mastered in every scenario.

❤️ Non-judgmentally expecting that being unintentionally affected by others will always be a part of our experience, this is where we can use boundaries to take great care of ourselves.


🚀 The next episode will focus on the "difficult second step" of boundaries, which is how to deal with the potential negative reaction from the other person when you set a boundary.

Comparison Almost Stole Our Friendship: How to Respond to Comparison, Not Retreat From It

If your current strategy to deal with your concerns, fears and anxieties are to dismiss them, this episode is for you.


Doctors aren't generally used to being BAD at things. But Becc has recognised something that we're pretty bad at across the board. 

 

Self-validation

 

She's very passionate about bringing this skill to doctors for the benefit of both our professional and personal lives. 

 

Why? Because it's extremely productive. 


And because invalidating ourselves can be extremely destructive. 

 

What is self-validation? What does it mean to validate yourself? The simplest way to think about self-validation is that it's the opposite of self-denial, dismissal and resisting our own experience. 


The objection to self-validating that Becc has found to be ubiquitous in her high-achieving, successful clients is that looking by giving more attention to the 'negative' aspects of our experience, we'll only magnify them. That they'll then be more likely to play out in our behaviours or take up more space in our minds.

 

Paradoxically, it's *usually* the opposite. 

 

Think… Can you can recall many experiences where resisting and denying yourself has cost an enormous amount of attention and energy? That's attention and energy that can't go to your productive outputs. 

 

In this episode, Kristeen and Becc talk about Becc's BOARDROOM MODEL OF THE MIND which helps us relate to our minds in a different way. This analogy helps us to make decisions about how to treat the competing, conflicting and concerning thoughts that are normal and expected in the mind of someone taking on challenges and risks on their missions in life.

23 Jan 202420: Doctors, let's talk about WHY clinical work is SO EXHAUSTING00:43:43

Are you EXHAUSTED or is it just me? 

 

Clinical work is pretty magical. There is so much to love and enjoy about it. Something that comes with such highs apparently fairly comes with some lows too. 

 

One of them seems to be that most of us will agree it's EXHASTING. 

 

You can seemingly come home after a day of simply circling the wards at a pace that would make weekend golfers look athletic… Or sitting in a comfortable chair, chatting away with your patient and tapping away at the computer… YET, come home and be magnetised to the couch. 

 

All you can think of is being still and doing nothing… Maybe you can tolerate a bit of TV or a book. Your body is heavy, your mind craves to be quiet. You WANT to spend time with your kids and have a laugh with your partner. You WANT to be the kind of person who goes straight on to a nice dinner… Or even just the gym. 

 

But you hear your body's exhausted desperate whisper… "I can't". 

 

Okay, that may be a little dramatic. But it feels not too far from reality. Many of you likely notice your non-clinical partners can come home with notably more pep in their step. So it's natural to ask… Is it YOU? Are YOU the problem here?

 

We've cherry-picked this topic today because clinical work seems to tire the most resilient and enthusiastic of us. In this episode, we talk about what makes clinical work special including what makes it ESPECIALLY tiring.

It is extremely inconvenient to be tired right now. Relatable?


We're talking about that kind of tiredness that isn't neutralised by a sleep-in, where you don't get re-booted from a workout or revived when you just work out you were missing some Vitamin Social Life. 

 

We're talking about the kind of tiredness that won't allow itself to be ignored. The kind of tired where it seems that your body shuts down all non-essential systems to the point that you are able to be still and blank.

 

Tiredness is not a problem in itself. When paired with being committed to your commitments, however, it does create significant challenges.

 

On one hand, it's great that you're committed and that you have the discipline to show up regardless of how you feel.

 

On the other hand, to disregard how you feel is a dangerous line to walk or anything but a short-term emergency. 

 

Being able to recognise the signals that our bodies send to us is critical for long-term success. Our human bodies have needs that must be met to facilitate health and performance. And how we feel is the mechanism by which our body can communicate this to get our attention. 

 

Doctors have, unfortunately, been encouraged to practice emotional suppression making it easier to miss or dismiss the messages. This may appear adaptive in the short term but emotional suppression is, however, a blunt tool by which I mean, we cannot suppress selectively just one feeling but instead, it's all or nothing. When we choose to suppress one emotion, we take the others down with it. 

 

And we lose important information about our life direction and our health in the process. 

 

In this episode, we are talking through our current real-time experience of managing this tiredness without avoidance, overriding, or denial. And we're meeting our limitations and revealing areas for further work here. We are walking the talk from Episode 1 of Am I Doing it Wrong the podcast!

 

Episode 1 brought forward the question of whether 'balance' was achievable or even the goal for doctors. Instead of balance, we have landed on contrast - allowing alternating times of being over-subscribed and then more free and light.

This week on the podcast, we're inviting you to a challenge for December: THE "ASK SANTA" CHALLENGE. 

 

This is a challenge for you to take on the "Ask Santa" mindset. 

 

Ask Santa is a permission-giving mindset. 

 

It's a mindset that, at its foundation, asks you to stop auto-rejecting your wants and wishes and start honouring and acting on them. 

 

Years ago, Becc noticed she'd developed a pattern of reflexively silencing the sentences that started as 

 

"I wish…"

"It would be great if…"

"Wouldn't it be nice if.."

 

With her mind dismissively responding

 

"That's unrealistic"

"You shouldn't ask"

"You should just be easy"

"You should just accept"

"Don't be too much"

"Don't inconvenience them"

"They'll feel like they have to say yes"

"They wouldn't say no even if they wanted to"

"Don't risk putting pressure on them"

 

So, before she'd even considered what she wanted, she'd counted herself out. 

 

Here's the thing about Santa. 

 

If you don't write the letter, how can he fulfil your wish?


And, if you don't believe in Santa, you won't ask for what you want. And, so, you won't get it. 

 

If you don't believe, you won't receive. 


Or, more accurately, if you don't believe, YOU WON'T ASK, and so you won't receive. 

 

When we allow these responses that arise in our minds to make what we want seem unreasonable or unrealistic, we don't act like it's possible - we don't ask for it. 

 

We need to allow ourselves to believe in order to get in the arena. 

 

And, it's time to get in the arena. 

 

In this podcast, we talk about the what and why of the ASK SANTA mindset. It's about expanding your life, taking up space and advocating for yourself, and assuming the best. 

 

It’s not about whether you got a 'yes' or 'no' from Santa but about making yourself heard - first and foremost, by yourself.

Firstly, we're so glad you're here. As you'll hear on this episode, one of the most important and adaptive steps you can take if you're struggling with this failure is facing your reality instead of - what we all want to do - hide from it.


This episode is not your typical pep talk post-failure where we tell you to c'est-la-vie-it or don't-worry-about-it. We're not going to ask you to look on the bright side, just think positive or distract yourself until one day you wake up and it's not the first thing you think of.

This episode is also for you if you have 'failures' from the past that still haunt you. And we know that's not uncommon. That's because it's very common for doctors to have developed perfectionism and imposter syndrome mindsets. These mindsets allow their past 'failures' to carry into our present and future because of what we make the failure mean.


We're going to walk through the first steps of how to deal with this failure that doesn't involve a unhealthy avoidance nor a toxic positivity. You'll learn:

21 Nov 202312: When you DON'T do not harm00:39:01

The first rule: do not harm. Okay but what about when you do? It sounds like the simple, obvious and easy minimum - to not harm our patients as we care for them. But we all have a library of experiences where our treatment decisions did do harm. And that's because medicine is not just hard from a sense of needing to build and accumulate knowledge and skills to care for patients. It's not just hard because we are working to integrate multiple data points and data sources from each individual patient and synthesise that into a congruent impression. It's not just hard because executing one treatment plan can exacerbate or create another issue or reveal another vulnerability in the patient. It's not just hard because of the limitations and demands of the healthcare system that mean that we cannot access ideal clinical care conditions or even conditions that we feed are the minimum standard. But it's also hard because we are constantly working with uncertainty. And the truth is, when you're working with such limitations, complexity and uncertainty, you don't get to simply do no harm. In a doctor's professional life, there will be many instances where we cause more harm than good in the noble pursuit of doing the best thing for our patients. That reality is extremely difficult for a lot of us to face. It's important that we find a clear balance between what is in our control and what is not in our control. Both situations are equally difficult. It can be extremely difficult to know that you have made a mistake, or that your skill level contributed to the harm done to a patient. Taking responsibility is a cornerstone of growth. For doctors, however, it's easy for us to not recognise the contextual influences of the 'mistake', for example … that we work as a team in healthcare, not as an individual … that we are in training … that our skills are never perfected even when we are experienced … that support, supervision and training provision should appropriately mitigate deficits and risk … that the healthcare system 'Swiss cheese' model should usually function to prevent any domino effects of a mistake. It's common for us to over-estimate what is in our control It's common for us to take over-responsibility … And make erroneous and hindering conclusions in retrospect about how we single-handedly could have prevented something or how we have really just proven we're not the 'right kind of person' to do medicine. This is over-responsibility. To be someone who is conscientious to provide great care for our patients and, with that, to do no harm wherever humanly possible, is an advantageous trait of a doctor. Let's not use that against ourselves. There's nothing useful or empowered about trying to control something that you can't. There's nothing that will make you feel more out of control than trying to control something that you can't. 02 Nov 20236: Doctors, are you held back by a Fixed Mindset? 00:48:59

Most of us would say we are the type of person with a growth mindset. We're doctors, right? We signed up for a career of ongong learning skill development.


But, if you know the growth mindset well, you know that it's not something that you 'are' or 'aren't'. It's not a personality trait. It is, by definition, a mindset. A mindset that we can shift in and out of.


And it's not uncommon for ourselves and our colleagues to be in a fixed mindset rather than a growth mindset when it comes to medicine.


When we're in a fixed mindset, it means that we interpret our performance or current capabilities as measures that reveal our intrinsic capacity. Our capabilities are 'fixed' if you like, and you can see where they're fixed reflected in exactly where you are right now.


In a growth mindset, on the other hand, we understand that our current performance or level of achievement isn't telling of we're capable of.


In this episode, we share examples of places in our lives in the past and present where we're in a fixed mindset as opposed to a growth mindset. We reflect and recognise the consequences we face of being in the fixed mindset including, importantly, the opportunities we miss out on.


We discuss how it can be challenging to be in a growth mindset in our work as clinicians where the stakes are high when it comes to getting things wrong and making mistakes. We also discuss how we see the fixed mindset widely in medcial culture from medical students to consults with our unwillingness to openly express our knowledge and skill gaps.


Being in a fix mindset creates self-imposed and self-fulfilling limitations in our performance and our opportunities. A growth mindset is also a self-fulfiling prophecy - it promotes learning and creates opportunites.


To stop and reflect on where we're in a fixed mindset gives us an opportunity to authentically switch to a growth mindset - or to engage in the work we need to do to make that available to us.

TALL POPPY SYNDOME. Be excellent. But be quiet about it.

 

Australia has a strong preference for or even culture of humility. We don't like it when people think a lot of themselves … Even if they've got a lot of good stuff to think about. Being humble is the expectation. And bragging is perhaps the worst offence. 

 

We even have a name for the cultural quirk: THE TALL POPPY SYNDROME.

 

The Tall Poppy Syndrome refers to our tendency to criticise successful people. Knowing this tendency, we form fears of loudly successful or standing out for any good reason because we have the fear that we will be criticised - we'll be cut down. It's not necessarily something we do consciously, but often something we have learned implicitly and we see play out in our decisions and behaviour.  

 

For some of us, even just THINKING a good thought about yourself can evoke feelings of bragging. Let alone SPEAKING it.  And 'bragging' is certainly a dangerous thing if it makes you stand out as a Tall Poppy.

 

This actually presents a challenge for people in medicine. Many of us have learned these sentiments on our way to medicine: 

 

  1. Meritocracy: if you just work hard, you will be rewarded 
  2. Tall Poppy: if you stand out as successful, you will be criticised

 

Together: work hard and be quiet about it. 

Don't think positively about yourself.

… And, eventually, if you just work hard enough, you'll be rewarded or anointed. 

 

Recipe for prosperity and a happy life? Not quite. 

 

In this episode, we talk about our experiences with Tall Poppy Syndrome, the way that it disadvantages us and how we choose to think about success and humility instead.

The Nature of Self-Sabotage

  • Difficulty Identifying in the Moment: Self-sabotage is often hard to recognize as it happens, requiring hindsight or the help of others to see the patterns.
  • Case Study: The 'Good' Doctor - our example of prioritizing patient care over studying was a form of self-avoidance. She created excuses to protect her sense of self-worth and avoid the risk of failure in her exams.
  • Emotional Drivers: Self-sabotage isn't about deliberately harming yourself. It's about feeling good in the moment, even at the expense of long-term goals. It's choosing the emotionally comfortable option.

How Self-Sabotage Manifests

  • Motivational Triad: Humans are wired to seek what feels good, avoid pain, and conserve energy. This drives our choices.
  • Study Avoidance: Procrastination cleaning, feeling incapable when studying, etc., are ways to avoid the discomfort of studying and potential negative thoughts about oneself.
  • Self-Doubt Fuels the Behavior: Thoughts like "I'm not getting this" or "I should be further along" trigger challenging emotions (frustration, disappointment) that hinder learning and focus.

Counteracting Self-Sabotage

  • Self-Awareness is Key: Monitor your mind and recognize your emotional states. How do your thoughts make you feel as you approach the task you're trying to avoid?
  • Choose Your Emotional State: Recognize that suboptimal emotional states can interfere with learning and performance. You can choose to study from a calmer, more focused perspective.
  • Compassion vs. Condemnation: Your brain's self-critical thoughts may stem from a desire to protect you from potential failure and a blow to your identity. Understanding this allows for a more productive negotiation with your mind.

  • Transform Failure: If you understand your self-sabotaging tendencies, failure becomes less destructive to your goals and identity. Even if you don't succeed initially, you did work hard, giving less power to self-doubt.
  • Improve Your Experience: Studying and pursuing goals can be more pleasant and effective once you break self-sabotaging patterns. You can feel better while striving for what you want.

  • Don't Confuse Tiredness with Self-Sabotage: Learn to distinguish between needing rest and self-sabotaging avoidance.
