Dr. Gerig is owner and founder of www.pelvicsolutionscenter.com. She sees men and women who have pain in the pelvic region, which may include the lower abdomen and back, gluteal area, genitals and rectum.
She is an expert in interstitial cystitis (IC). Learn that not everyone that comes into her clinic with interstitial cystitis actually has IC. The classic IC symptom – pain perception in the bladder when it fills up and it is relieved when you urinate. There usually is associated urinary frequency.
Pelvic floor muscle tension (high tone pelvic floor) is more common than IC and is often confused with IC.
IC, pudendal neuralgia and pelvic floor muscle tension are the three main things Dr. Gerig treats.
What a PT and a pelvic floor expert finds on physical exam cannot often be seen on a CT scan.
She integrates the role of physical therapy in her practice. She thinks 95% of her patients need PT. PT is the primary treatment except for ulcerative IC. Ulcerative IC is a condition where there are ulcers in the bladder (Hunner’s ulcers).
Physical therapy is vital in educating women about their bodies and bodily function.
Clean eating is helpful. Your primary fluid intake should be water. Integrate an anti-inflammatory diet. Integrate movement every day into your life. Slowly increase your activity as you are able. Dr. Gerig thinks movement and stress management are very important when you have muscle tension.
Tips for stress management!!!
Healing from pelvic pain takes time. There is no magic wand! It takes time and work.
Medications for IC and pelvic pain. Elmiron side effects. Supplements used – glucosamine and quercetin, aloe vera. Other medications used: histamine blockers, neuromodulators, vaginal muscle relaxants.
Dr. Gerig’s goal is to get her patients OFF medication eventually.
Switching topics, Pudendal neuralgia. This is a pelvic nerve called the pudendal nerve, it feeds the rectum, urethra, genital. Symptoms are burning, throbbing pain, worse with sitting. People often have urinary frequency and a “foreign body” sensation in the pelvis. It can cause pain with urinating, pain with sex, pain with bowel movements.
Post childbirth child pain. Prolonged pressure of the baby’s head on the pudendal nerve can cause urinary issues. Women can have pain with sex, pain in tailbone. Find a PT who is well trained in treating pelvic pain.

What hurts pelvic pain, or what behavior is not helpful? Rumination and catastrophizing. Seeing a therapist can help with this.

Sometimes thoughts are put in your head by other people, things, society. Especially about sex.
I can’t get you to comfortable enjoyable sex if your thought is, “I am not a sexual being”
Your body is listening to your thoughts.
Sex and body image.
Thoughts create our feelings which create our actions.
Some of our thoughts aren’t serving us anymore.
Sometimes we can’t make a thought go away, but we can see it for what it is – not a fact and not helping you. You can work on replacing your thoughts.
Negative thoughts can’t get you to a positive solution.

Wikipedia: Gaslighting is a colloquialism, loosely defined as making someone question their own reality.
The answer provided on social media to this question:
- Regular sexual activity, self stimulation and pelvic floor exercise can help keep your vagina health. - IS THIS ACTUALLY TRUE?
- This doctor said “I tell women to avoid sugar and use probiotics to make sure their entire ecosystem is “humming along” – NO evidence that oral probiotics affect vaginal microbiome – then she plugs her diet and her probiotic products
- She then goes on to say – two additional ways to counteract vaginal dryness is to strengthen the pelvic floor by doing kegel exdrcises and using a “holistic formulation” that helps maintain optimal levels of natural moisture, and “supports a healthy vulva and vagina”. – this is her product – which states it “prevents leaks” and costs 70 dollars. Also claims to “reignight desire” – WTF
I searched in J sex med - does intercourse delay or treat vaginal atrophy? – can’t find a damn thing
I found on an internet site - “Regular sexual activity, with or without a partner, can help maintain healthy vaginal tissues through increased blood flow and avoid or lessen the effects of vaginal atrophy.”
- No citations, so again I went to J Sex med – can’t find a thing
On menopause.org - Regular vaginal sexual activity is important for vaginal health after menopause because it stimulates blood flow, helps keep your vaginal muscles toned, and maintains your vagina’s length and stretchiness.
- No citations, and furthermore, the vaginal muscles are smooth muscles controlled by the autonomic nervous system – you can’t tone them with sexual activity
- I will buy that stretching the vagina frequently probably allows it to be stretched frequently without pain – but is there evidence and why sex, why not just dilation?

Celebrating Five Years of the 'You Are Not Broken' Podcast
Happy New Year! This week, as we step into 2024, I’m reflecting on the incredible journey of the podcast and the amazing guests who’ve joined me along the way. Five years ago, on January 1, 2020, I launched the very first episode of You Are Not Broken. At the time, Instagram wasn’t even part of the equation for me and now I just hit 200K, I hadn’t published a book, hadn’t done a TEDX talk and the term "menopause expert" wasn’t something that I was. Looking back, I can confidently say: that first episode was pretty good!

2. Dr. Mary Claire Haver – April 2023 episode 208
Long before “the new menopause” and the buzz around medications like Ozempic, Dr. Haver was here dropping timeless advice.
3. Dr. Sari Van Anders and the Science of Desire – march 2022 – Epiosde 147
Dr. Van Anders insights reshaped how we think about desire. Her research challenged the heteronormative theory of low desire in women partnered with men, emphasizing that desire is far more complex than we often assume. She explained how sexual thoughts and behaviors can influence our hormones, showing that thinking sexy thoughts can actually increase testosterone levels. This idea—that our minds and bodies are deeply connected—has inspired so many listeners to explore what turns them on and why.
4. Dr. Avrum Bluming and the Hormone Window – Epiosde 222 July 2023
Dr. Bluming joined me to discuss the "10-year window" and the incredible "window of opportunity" when it comes to hormone therapy. We now know that the main risk associated with oral estrogen is clotting in veins, which is why transdermal (TD) estrogen is the preferred option. He reminded us of the critical role hormones play in bone health, as hip fractures kill as many people each year as breast cancer.
5. The Lancet Rebuttal: Are We Overtreating Menopause? – April 2024, #259
This powerful panel discussion featured Dr. Lauren Streicher, Dr. Aoife O’Sullivan, Dr. Heather Quaile, Dr. Heidi Flagg, and Dr. Mary Claire Haver. Together, we explored whether menopause is being overtreated in 2024. Spoiler: the answer is no.
6. Dr. Marty Klein and the Complexities of Fantasy episode 169 – July 2022
Dr. Klein’s book, His Porn, Her Pain, sparked a fascinating conversation about the nature of fantasy. His insights challenged us to think deeply about what fantasy means for us as individuals and how it shapes our relationships.

This is an interview I did with Lesley
McShane for her podcast “Redesigning Midlife” and it was just such a good
interview I asked her if I could share it again.
1.
Basics: a. Menopause means it’s been one year and one day since your last
period. b. The average age of menopause is around 51. and c. for ten years
prior to menopause, women go through peri-menopause, where hormones are doing a
reverse puberty.
2.
A lot of women stop or have pain during penetrative intercourse during this
time due to a decrease in natural lubricant and collagen, but this can be
easily treated.
3.
Even if you don't have menopause symptoms, if you are over the age of
approximately 51, you have gone through it.
4.
The most sexually satisfied people aren’t young people, it is people in their
50s, 60s, 70s.
5.
Great sex is not a passive thing. It’s an active process, just like someone
that is physically fit. It has to be prioritized.
6.
Use of lube increases your success of orgasm by 70-80%. Silicone based lube is
best because you don’t absorb it like you do the water based.
7.
There is no “how long” should I use Vaginal Estrogen cream. The use of it is
like using sunscreen or wearing a seat belt.
8.
You don’t need a “sex drive”. We do things because they are awesome. If
you aren’t having awesome sex, don’t wonder why you don’t desire it.
9.
We feel entitled to a spontaneous sex drive that has been sold to us by media.
And when we don’t achieve that, we end up feeling broken.
10.
Putting something in the vagina only leads to female orgasm 30% of the time so
special focus need to be paid to the female body. Your largest sex organ is
your brain.
11.
Don’t sit passively by waiting for spontaneous desire to happen. Get dressed up
and go to the party even if you don’t think you want to. Chances are you’ll
have a good time once you get there.
12.
Control of stress and cortisol levels in our bodies will help us desire more
intimacy.
13.
If there is a desire mis-match in your relationship, normalize it by talking
about it.
14.
Communication with our partner about menopausal symptoms and what we are going
through is important to our relationship.
15.
Get educated on hormone replacement therapy as an option. When women start
thinking about how they want to live as they get older and get educated, then
they see how beneficial HRT could be for them.
16.
Women between the age of 50-60 that are on hormone therapy live 3-4 years
longer than women in the same age group not on hormone therapy.
17.
For our young adult children that are starting their own sexual lives, the
biggest thing they need to know from the start is about consent.
18.
They need to be aware of is orgasmic equality.
19.We have to educate our girls that pleasure is for everybody.

Genetic testing in 2017 and was positive for Lynch Syndrome. I was just diagnosed with uterine cancer yesterday. 43 years old and total hysterectomy in my very near future. Terrified about trying to get my hormone levels correct post surgery!! Any suggestions or recommendations?
I’m a 38 y/old breast cancer survivor. I’m on estradiol, got all the fancy lubes, and now I’m curious about testosterone… People only seem to talk about estrogen for women, but my libido is nonexistent and penetrative sex is excruciating. Curious if testosterone also takes a hit while on hormone blocking meds post-cancer. I discuss this stuff with the oncologist but it feels trivial compared to the work they did to cure my cancer. However, my PCP won’t touch the hormone topic with me and defers to the oncologist because of my cancer history… thoughts/suggestions?
I’ve gathered so much info from you about the estradiol cream and so much more than I got my gyn to prescribe the cream for me. I’d like to note that I am 59 and I have never had any problems with painful sex or any vaginal issues in the past. I applied the first dose both inside and a bit on the outside areas, it was fine with no noticeable changes. But 4 days later I applied the second dose, this time just inside and within a day or so I felt changes. My clitoral area was sensitive with discomfort and I experienced dryness in the outer labia like never before - not really painful but noticeable discomfort. I have not taken a third dose but I really want to be able to use this so I don’t shrivel up and go away LOL. I also was getting a few hot flashes and they really decreased after the first dose but are back now, although not as much. Is it possible that the discomfort will diminish after a while if I keep taking it? Would it be effective if I only applied it once weekly?
I’m 58 and 2 years in menopause. I use an estrogen patch, progesterone, and a testosterone cream - thanks to you - but it’s not clear to me if my transdermal estrogen will protect the vulva from atrophy. Should I also be using an estrogen cream on my vulva in addition to my HRT regimen? I asked my doctor and she said creams for the vulva are only from lichen sclerosus, but that doesn’t sound right.

2021 update from the authors
- :the WHI has walked back virtually all of their early alarmist findings. In recent publications, they reported that estrogen does not increase “all cause mortality” or deaths from heart disease and cancer. Actually, they said, it increases longevity, most notably when begun within ten years of the last menstrual period. It is the best preventative for osteoporotic hip fracture. It is safe and effective when applied vaginally for local symptoms. And, in the most striking about-face from their 2003 headlines that HRT “did not have a clinically meaningful effect on health-related quality of life” for women in menopause, they stated in 2019 that “Hormone therapy is the most effective treatment for managing menopausal vasomotor symptoms.”
