The first step is to determine why you are leaking.  There are different reasons that women leak.  They are:
~stress urinary incontinence
~urgency urinary incontinence
~mixed urinary incontinence
~overflow urinary incontinence

Bladder leaking is a huge quality of life issue that is usually a benign condition.  But there are lots of medical issues that can cause leaking that need to be evaluated.  In this episode, I discuss the medical conditions that can cause symptoms of bladder leaking.  These medical conditions are rare but always need to be considered.  Most of them can be evaluated with a simple history and physical exam.

This is why it is important that you are up-to-date on your health care before you consider treating your bladder leaking on your own.

25 Apr 2023How to strengthen your pelvic floor with the Leva Pelvic Health System and Dr. Mandy Pullium00:28:05

In this episode, I interview Dr. Mandy Pulliam, a board-certified urogynecologist who is currently the CMO of Axena Health.  This company offers the Leva Pelvic health System which is a prescription digital therapeutic device to improve pelvic floor strength.  (find out more here:  https://www.levatherapy.com ).

This system is unique because it measures the movement of the muscle and can tell if you are doing a contraction correctly.  This is different than other devices.  It also provides an app to follow your progress, human coaching and reports back to your prescribing physician.

There are currently studies to see how long the effect from the leva lasts.

This is one way to strengthen your pelvic floor but a pretty smart way!

16 May 2023How hormonal changes might be impacting you with Dr. Karen Adams00:21:54

This episode is about perimenopausal symptoms and specifically how they can impact the vagina and bladder.  I’m talking to Karen Adams, a board certified gynecologist who is a professor emeritus.  Dr. Adams is an expert in menopause and is currently creating a Menopause and Healthy Aging clinic at Stanford University.

We discuss the perimenopausal period which is the time before your period stops but when your hormones are erratic.  This period can last between 1 and 10 years but can be packed with symptoms like:

Irregular periods

Hot flushes

Night sweats

Mood changes

Depression

Anxiety

Sleep disturbances

Vaginal symptoms like dryness and pain with sex

Bladder symptoms like frequency and recurrent infections.

 

We also discuss the impact of estrogen on vaginal tissues and using local or vaginal estrogen to treat these symptoms.

There are a lot of products out there!  This episode is all about incontinence pads and underwear.  When you are thinking about pads, it is important to use pads designed for bladder leaking- they work better.  The most common pads in stored are Poise, Depends, Always discrete, Tena, and Kroger generic.  I think it is always a good idea to look at the websites before you go to the store, so you have an idea of what would work for you.  The different types of pads describe their absorbency in different ways- there isn't one system- so it is hard to compare them.  If you try a few that don't work for you, there are a few websites that offer recommendations or have experts you can talk to.  These include:

09 May 2023The who, why, when and how of hormone replacement therapy for vaginal and bladder symptoms00:19:59

In this episode, I get into the details of hormone replacement therapy for vaginal and bladder symptoms.
 
 I start with talking about how we only have a finite number of eggs and when they are gone, we stop making estrogen.  This leads to menopause.  In the period before they are gone, we make estrogen in an irregular fashion- this is the perimenopause.
 
 I discuss who should use estrogen (women with symptoms), why they should use it (to help with symptoms like vaginal dryness and recurrent bladder infections), when they should start it (before age 60 or within 10 years of the start of menopause if using oral estrogen), and how they should use it (if the symptoms are vagina or bladder related, you should use vaginal estrogen).  I talk about the "rules" around estrogen therapy- like if you have a uterus, you need a progesterone.  And then I talk about common symptoms, how we describe them medically and what I hear patients saying.

As a urogynecologist, my work frequently overlaps with that of a colorectal surgeon.  Bowel and rectal issues can have a huge impact on bladder and pelvic floor symptoms. Join my for my conversation with Dr. Bethany Malone, a board certified colon and rectal surgeon in private practice in Fort Worth, TX.  Dr. Malone uses TikTok and social media to provide patient education and disrupt typical referral streams.  She has contributed to Forbes, Insider, and Healthline to increase physician led health information distribution.  Her (amazing!) goal is to increase patient access to quality healthcare and to rebuild trust in the medical system.

I find that most of my patients are more worried about anesthesia than they are about the surgery (this may be my surgeon's view of the world).  Anesthesia is actually very safe but it is important to have a team that you trust.

In this episode, I talk with Dr. Danielle Doyle, a double board-certified anesthesiologist and pain doctor with experience as an award-winning professor and Washingtonian top doctor. She has spent her career administering and teaching anesthesia at tertiary care centers and level one trauma centers, providing diligent and compassionate anesthesia care for critically ill adult patients.  She is a trailblazer in the field of ketamine care and chronic pain, having directed the George Washington University Pain Center and ketamine infusion program, providing care to thousands of patients. She is currently the CEO of Cultured Therapeutics.  She has been widely published.

We discuss the safety of anesthesia and how common memory loss is after anesthesia (spoiler: it is not).  We talk about how to manage the anxiety around anesthesia with medication but also by trust-building, expectation setting and education.  We talk about managing the nausea that women commonly get after surgery with hydration before surgery, medications, and avoiding certain types of anesthetics.  We talk about managing postoperative pain with preemptively treating pain, proper positioning, educating, and medications.

In this episode, I talk with Kim Pierpoint, a retired healthcare administrator and sprint triathelete.  She'll tell us her personal story of leaking while training and not liking what was on the market.  She developed a prototype and created her own brand of leggings and shorts designed for leaky runners.

21 Aug 2023All about Innovo with Ruth Maher PT PhD DPT00:28:28

This episode is all about the Innovo.  The Innovo is a non-invasive way to strengthen the pelvic floor to help improve stress urinary incontinence.  It is a pair of shorts that uses an electrical strimulation to contract the pelvic floor.  The sorts are designed to be more for 30 minutes 5 times per week and should improve incontinence in 12 weeks.  No prescription is needed to obtain this device.

In this episode, I talk to Lauren Barker about uresta, an incontinence pessary.  Uresta is available in the US with a prescription but it is designed to be fitted and managed at home (they're working on making it OTC in the US like it is in Canada).  While it may look uncomfortable, most women do not find it to be so and find it easy to use.  Studies have shown that around 90% of women have improvement in their leaks and 90% were still using it after 12 months.  
Uresta can be worn for up to 24 hours.  It is recommended to get a new one annually.
The initial starter kit is 139$ and a replacement pessary is 89$.  They don't have discounts but often have sales on Black Friday, Mother's Day and International Women's Day.

09 Oct 2023what is Axonics for overactive bladder?00:32:36

In this episode I talk to Amy Anderson who provides support for the sacral nerve stimulator (SNS) developed by Axonics.  SNS is a therapy that was to developed to help treat urgency urinary incontinence, urinary retention, and fecal incontinence.  It stimulates the third sacral nerve via an implant – think of it like a pacemaker for the bladder.

Amy and I discuss how the therapy works and how successful it is.  And we talk about the practical details of having this type of implant.  There is a 1-2 week long trial period where we determine if the implant is successfully addressing your symptoms.  During the trial, there is a wire implanted that stimulates the nerve (there are 2 different ways this can be done) and an external battery.  You have to keep voiding diaries before and during the trial.  

We talk about how the device can be programmed if it becomes less effective over time (this is done via Bluetooth and without undressing just using an i-pad like device).  We also talk about what the stimulation feels like, what different types of batteries can be used, the possible complications,  the ability to get MRI’s after the procedure and the current TSA regulations that impact implants.

What is mindset- it is your attitude to the world.  There are different types of mindset that can impact how you interact in the world.  Dr. Hidalgo talks about visualization exercises that she has her clients do.  She focuses on how the situation or action makes them feel.  She talks about different types of mindset and how different situations call for different mindsets and how there are times when a fixed mindset can be helpful.

What are the ways to develop a better mindset?

  • Visualize how an activity or situation will make you feel
  • Be ok with not making everyone happy
  • Surround yourself with people who have a similar mindset

What are the myths about mindset?

  • Mindset does not make everything possible
  • Growth mindset is not the same as a positive mindset- it is looking for the change that might be needed even if hard
  • You are never too old to change
  • You can have more than one type of mindset

You can change your mindset on your own but many women benefit from help.  

 And remember to be kind to yourself.  Visualize how you want to feel and then do what it takes to create that feeling,  And remember to separate your thoughts and feelings from your worth.

Our systematic exploration of treatments begins with containment products, including specialized incontinence pads and eco-friendly options like Attention Grace.


Specialized incontinence underwear designed for reuse also plays a role in this journey, effectively concealing mild to moderate leakage.

Pelvic floor strengthening is a crucial avenue to explore. Pelvic floor physical therapy provides personalized exercises and valuable in-person or virtual guidance.


Various tools like vaginal weights, muscle measurement devices, and specialized exercise wear are available for independent pelvic floor strengthening. Weight loss, backed by studies showing a 70% improvement in SUI after losing 8% of body weight, is a noteworthy consideration.


Moving forward, we discover pessaries. Impressa, an over-the-counter option with multiple sizes, offers a disposable solution. Healthcare providers can also provide silicone or reusable pessaries, effectively improving SUI in 50-80% of women.
