How can Tom get a constant, sustainable stream of new patients? When he finds that Tom is so willing to accept his guidance, George lays down even more golden nuggets of information in this week's episode. Some helpful golden nugget quotes below: "The reappointment rate in the hygiene chair is immensely higher than the reappointment rate at the front desk." - George 2:37 "...and if you see her making that next appointment, do not interrupt her. Wait for her to finish making that appointment." - George 9:27 "If you're gonna remember anything from this interview it's this: start with why...always begin with the reason before the request." - George 6:28 "If they don't make their next appointment, there's a 10% chance that they're gonna come back on time." - George (regarding patients who leave without making an appointment) 8:09

Practice Under Water brings together dentist and dental practices into our interview space, with a twist! For our new Practice Under Water episodes, George and Matt will be diving (pun intended!) to the deep end of the swimming pool with mystery guests. They will ask them driving questions to find out how they are growing their business, mistakes they've made and how our guests have made their comeback through trial and error. Practice Under Water is designed to enhance the dentist's lives by moving past difficulties to experience joy.
Today, Matt and I are going to interview a new mystery guest, who we call “Liam.” Liam wanted to give back to the podcast world, which is the main reason he decided to come onto the show today. Liam is 40 and has just purchased his first practice. He waited a long time as he went through several residencies and fellowships. In GPR, he found that he liked surgery and implants. He worked in perio, inside a fellowship, until he was $400,000 in debt.
Our mystery guest Liam is a specialist and realized how isolated he would be in periodontics and didn’t want to be dependent on others for his income. He did enjoy pairing up and learning specialty skills from other professionals and students. Liam and I compare the traditional educational route and getting a dental degree with CE’s.
Liam tells Matt his journey to buying a general practice office. He started in 2011 and was newly married. His wife was a medical student and ranked in a different state for her medical school fellowship. Liam decided to take a position in practice with a path to ownership, but he decided to leave to be with his wife in medical school.
Liam and his wife eventually found a place to call home where he found a position in practice doing implants. The owner had medical issues, and unannounced to Liam was trying to sell the practice while he was working there. This practice was losing $10,000 a month at this point, and the owner offered to sell the business to Liam. Liam called George (from our podcast) and was told not to buy this practice.
Digging in, Liam found that the area around the practice was a high-income area and he was the only periodontist in town. The practice was making $550,000 yearly, and previously was making over $1 million. The owner talked to Liam and offered him his practice for $325,000; however, the bank wouldn’t give him the loan. Liam’s accountant said the practice was worth $250,000. After 6 months, the owner sold the practice for $225,000.
This year, Liam is on track to do $1million! Since the practice is a GP, he uses referrals from orthodontics and other general dentists for specialty cases.
Listen in to find out how Matt and Liam figure out what metrics he needs to improve or correct to make his practice more efficient, where he wants his practice to go and how he wants to grow his practice.

Rohit's background is in engineering; and, while he was working as an engineer, his wife opened her own dental practice. Rohit supported his wife while she was building her practice, assisting her with the numbers side of the business. He would spend many hours figuring out the financials for her practice. Rohit realized there was a need for software to automate the business side of dentistry and developed Practice By Numbers in 2016.
Are you ready? We are taking you on a Practice By Numbers dashboard journey, where we will delve into why Rohit chose the metrics for his software, how the dashboard is set up and how valuable Practice By Numbers is for your dental practice.
We converse about collections, scheduling, and how to look at the numbers. Learn how you can be proactive and measure metrics in your business. Three positions we look at with this software are: Where you are, where you want to be, and how can you take action to get there. With these metrics, you can now project your future earnings for your practice. We walk you through real examples and how you can build your practice with this fantastic software. After using Practice By Numbers, you’ll wonder how you ever practiced without it!

Today we are picking up from PUW Episode 04 with Jane. She is a female entrepreneur who asked to be on the show for a deep dive into her new business. She was interested in finding out what pieces of her practice weren’t performing as well as she would like them to perform. Jane currently doesn’t track her numbers like she should and wanted to know what she is missing in her practice.
Digging in, I find that Jane is treating very well and she is in the top 10% for several factors, but she doesn’t believe she is doing well. She talks about getting past “being productive equals being extremely busy.” She does minimal marketing as her practice is booked out well in advance. Jane admits to being a phenomenal owner, but a terrible associate. She is banking over $145,000 a month in production with her overhead being $35,000 a month (see bonus PDF for this episode). Jane chats about building outsmart and having low overhead to increase revenue.
Listen in to find out what Jane can do to bring her practice closer to $1.5 million in revenue.

25 Mar 2022 | POUW: Debt and Net Worth: You Need Both | 01:04:08
Our guest’s journey is similar to that of many of our listeners. We’re reminded that, for so many of us, EQUITY in a dental practice—even if it substantially increases your debt—is the best way to pay off student loan debt, and reliably build net worth. Reality is more forgiving than a negative mindset. “I work to provide for my family, then I look at my paycheck as an associate, and half of it is gone because I’m not the owner. That really lit a fire under me to work toward ownership”. After several iterations of his vision, “Dr. Thomas” now brings his prospective acquisition to Dr. Matt Ford.
