"My sister called me. I answered. And, then, I lost it: 'He died. He died,' I repeated. 'Our patient died. I literally did CPR on this guy. We tried to save his life, and he died,' I repeated over and over. The tears streaming down my cheeks were cathartic. More than signifying a loss, the tears signified that I was still human, that I haven’t lost touch with reality. Residency hasn’t changed me. Death still affects me. I’m not numb. I’m human.

My tears were confirmation that I haven’t changed in that way. I was not calloused. He died. And I still felt it. I had a heart."

This will allow you to start the journey of healing.

Instead of just pushing yourself deeper until the bottle of unprocessed emotions causes an explosion, notice each moment. The more that we are aware of what we are experiencing, the easier it is each time to ground yourself and process before moving on. This is not easy and requires practice, but we can do hard things. We graduated from medical school and residency! We spend so much time healing other people. It is time we heal ourselves too."

We just visited the hospital days before to refill his drugs. He would be better if he used the drugs. Magic drugs. That is what he called them. I stood up from the mat where I slept beside them to find him not moving.

But why?

When he got sick, he moved. He moved a lot. Then, he got better and stopped moving. But he always moved first. My mother noticed me. Her eyes reminded me of a movie. The warrior dropped her sword in the middle of a battle. Her comrades were all dead. But she was not. I never understood why she dropped her sword. She could have fought and lived. But she died. My mother held the same look as the warrior."

Inevitably, this brings up the greater concern of an unintentional exacerbation of disparities because low-income groups face limited access to both computers, mobile services, and the internet. Hence, investing in access to the internet and smartphones should be prioritized for underserved populations. As telehealth expands, broadband providers and municipal broadbands will cater to the demands in unserved areas by expanding their internet services."

And what I have come to realize is that the nexus of all of these extraordinary stories of tragic afflictions being overcome was the individual’s remarkable propensity for gratitude and their mental aptitude to reorient, reframe, and reappraise all that was bad … into all that is good.

Unlike biological antibodies that fight microbial marauders just by our very nature of existing until the illness passes, the acquisition of emotional antibodies is a far less passive process. We have to be proactive in our efforts to construct and build meaning around the negative impact of the trials around us, and integrate all that we ascertain into the person that we are … and the person that we become—our identity."

"We eventually got the news that Dr. Rathburn had hung himself from one of those beautiful trees that the wind made dance. How sad. It adds to my own grief. Who had found him? How did he look? Babies born with a nuchal cord have the cord wrapped around their neck. That’s the closest I have seen to a hanging. I once delivered a baby who had it wrapped around three times, and by the time he was born, his little face looked bloated and bruised, tongue swollen and sticking out, and he had petechiae (small dots of microscopic veins that burst) on his face. I wonder if he looked similar. Did he have petechiae? How would even do that? I wish I knew. I wish I had said goodbye. I wish the image of him shuffling by me would leave my head. Would every warm and beautiful summer afternoon with the tree limbs dancing remind me of him; of this place; of this horrible feeling? That was something to look forward to. At least I was thinking of a future. That had not been possible just a few short weeks ago. When it’s dark, it is bleak, like black silence and extraordinary effort to move and operate the body. The pain of my empty feeling heart and just holding my head on my neck seemed unbearable at times. Right, I am here, and I am feeling better. At least the empty blackness is gone most of the time. I light another cigarette. It’s better than crying.

"Misinformation about COVID-19, if propagated, can be deadly. The stakes are high. Physicians need to have a voice in this discussion and work towards impacting policy in a positive way. Physician-led groups such as IMPACT aim to advocate for evidence-based solutions for the protection of communities and serve an advisory role to those in charge of public policies. As physicians, our primary purpose is the well-being of our patients. We owe it to them to share not only information about sinus infections and sore throat, but how to protect themselves from COVID-19. We must share what we know.

We can and should begin to reintegrate into society, but as physicians, our responsibility to the community is to educate and protect. We should not shy away from sharing our expertise with others. This may include uncomfortable conversations with friends and family whose views differ from our own, but if we come from a place of respect and understanding, we can and will get through this together."

To interact with other writers, all you need is a champion―someone to organize a time and space for you to get together and share what you’ve written. It’s great to exchange ideas and give feedback to one another, either online or off. Participants in these events can feel energized, enlightened and creative. You can even tweet haikus or flash fiction to one another. The work doesn’t have to be perfect; it just needs to be thoughtful and come from the heart.

Not everyone wants to write, but there’s room for those who have not yet tried. Who knows? You might wind up writing a book or two yourself."

"Life can feel full of uncertainty when battling cancer, with few guarantees. 'Is there still cancer in my body?' and 'Will it come back?' are common questions that have long been difficult to answer confidently until recently. Advances in next-generation sequencing — and a growing understanding of how our bodies respond to illness and treatment — have brought us closer than ever to predicting our individual futures, at least when it comes to cancer.

In recent years, cancer researchers have embraced new noninvasive technologies that can detect small amounts of DNA shed by tumors into the blood, also known as circulating tumor DNA (ctDNA). This is no easy feat, as ctDNA is extremely scarce after surgery and treatment — as low as a single molecule in a tube of blood. But by broadly sequencing one’s tumor, we can identify its unique genetic signature, allowing for the creation of a highly sensitive and personalized assay that can track cancer in the body over time."

"It is still extremely early in the Bitcoin story. This is due to the same network effect that Facebook, Amazon, and Apple have had as adoption of new technologies rapidly expand and are adopted by society. Bitcoin has passed its 'tipping point.' Converting some of your money to Bitcoin now is like taking an ownership stake in the Internet itself back in 1995.  Ultimately, Bitcoin will either have a massive price appreciation or go to zero - meaning it is a hugely asymmetric investment.  A mere 1% allocation could potentially 2x the value of your portfolio; a 10% investment could 11x the value of your portfolio. Losing 1% of your portfolio is an extremely small risk given the upside to bitcoin. Finding the right allocation is up to you, but having 0% allocated to Bitcoin is a massive risk since risk in bitcoin is asymmetric to the upside."

"The victims of this now distorted concept of liberty are ones that we physicians encounter every day. The one that inspired this essay for me is an 11-year-old boy that I saw three weeks ago. He is a patient of mine in my pediatric practice who came to see me with typical respiratory symptoms that led to a diagnosis of COVID-19. While he recovered uneventfully, his father got sick the next day and died from the same illness five days later. Like the vast majority of people who die from COVID-19 now, he was unvaccinated, believing that getting vaccinated was unnecessary and part of a greater effort to undermine his personal liberty. His son is now dealing with the unimaginable grief of losing a parent at such a tender age and asking his mother if he killed his father by getting sick and causing his death. This happens every day now in our communities across our nation. These are wounds that will never heal for this generation of kids."

These are highly complex and time and resource-intensive endeavors. "To help the reader understand the issues better, I would like to relate some stories. A friend of mine was dying of pancreatic cancer. He had an implantable morphine pump and was on both hospice and palliative care. Still, he found that his suffering was unbearable and wanted to die sooner. Doctors told him that his only option to end his suffering was to voluntarily stop eating and drinking, which he did. As a result, he developed psychosis, and his loving family has been scarred by the experience. He died after 10 agonizing days. I heard about an individual with ALS (Lou Gehrig’s disease), which is incurable and causes the death of neurons that control muscles and eventually leads to the inability to move and then the ability to breathe. This person killed himself with a gun before he lost the use of his arms. I had a number of patients with cancer that we were not able to control their pain, and I gave them palliative sedation (a morphine drip that caused them to become unconscious) to control their suffering. Had it been available, medical aid in dying could have helped all these people.

When my cancer gets worse, I expect to have a great deal of pain. I would like the option of having medication to put me to sleep, end my suffering, and to die, at home, with my family and friends around me."

"I think doctors are just wired that way. We are productive. We get things done. It is expected. We are supposed to do more, do it all, and be all the things to all the people. We come to a point where we try to satisfy this, and then we cannot do enough for ourselves. We cannot rest. We must keep trying to check things off the list to get things done. And it is overwhelming. It does lead to burnout.

I am still a work in progress. I am writing this on a work night because it is burning in my brain, wanting to come out. Perhaps I should be doing nothing, resting for a full day. But if I do that, I will feel guilty that I have wasted this time when I could have been putting my thoughts down on paper. The mind is always going.

This is me, but I think it applies to many other physicians. Look at your habits and see if you have some of my same tendencies. If you do, let’s start talking about how to reverse this tide of overwhelm to create some time for yourself when you are doing nothing and not regretting it."

"I’m a small-time doc in a small town, and I work at a small hospital in the Midwest, and I’m proud of it. The huge university hospitals (like Cleveland Clinic) get all the glory (especially in society and in the press), but small hospitals like mine drive this country.

It has been really weird working at a small hospital, where it seems like the emotions are more magnified than in the big city. A year ago, it seems like there was a local community group every day bringing food or treats for the hospital workers. Nationally, there was this outcry to recognize those in the medical field.

And, then, something changed. Now, for some reason, both nationally and locally, medical workers are seen as “the problem,” and there is a lot of negative emotion directed at us. And, it seems locally, like that negative emotion is magnified. Why did that happen, and how did we get here?"

"The rapid change-over to telemedicine in March of 2020 brought predictable challenges to health care at large and substance use disorder treatment in particular: patients without the skill set to navigate HIPAA-compliant apps, phones with too little smarts to handle video conferencing, lack of connectivity in rural and economically depressed areas. But telemedicine brought unexpected benefits: the no-show rate plummeted. Patients were more empowered by their own motivation to engage in care than limited by bus schedules’ vagaries. Suboptimal face-to-face interactions among individuals in the throes of addiction (otherwise known as “mayhem in the parking lot”) ceased immediately.

Most of all, telemedicine mitigates the stigma associated with substance use treatment."

I didn’t think I could stop myself—transplant was my duty, my responsibility, and what I was trained and programmed to do. It was hard to imagine leaving my post, but I needed to find a graceful exit, an elegant off-ramp that would satisfy my need for a tidy conclusion. One way or another, I wanted off the roller coaster, so I could never again be crushed by a patient like Tina. That had been my life for nearly twenty years. That was enough.

I felt yanked around by the ups and downs of the job, my emotions seesawed, and all the while I had to make rational decisions for my patients. But for me, the job was no longer a pure and simple exercise in rationality. I had mastered the mechanics of being a transplant doctor— that was the easy part. But now, from an emotional standpoint, every wife was my wife, every child was my child, and every father was my father. And I needed to save them all."

"What exactly is my obligation to medicine? Am I supposed to practice medicine forever? Is it my duty? Do I have to continue serving my patients, the hospital, and society because of these expectations?

The answer is simple: You get to decide. Most physicians I know love practicing medicine. It is and has always been a calling. The problem lies only in the thought that you should practice medicine. When used in this way, the word should is a cognitive distortion. A cognitive distortion is a pattern of thinking that is generally false or inaccurate and carries the potential to cause psychological harm. When you use the word should, there is often an accompanying shift in focus from what you want to what others might think."

"These encounters made my evening shift much more emotional than usual. I am still not sure what it was exactly that evoked such strong feelings of sadness. Was it having a patient who was a health care worker? Was it the rapidity in which all three patients’ conditions deteriorated? Was it realizing that without timely, expert care, all three would die very quickly?

Or, was it hearing people wanting to relax the measures and not wear masks, without insight into how quickly things can go wrong and end up deadly?

Or, was it the cost of empathy, and just feeling the pains of my patients way too deeply?

Or, was it talking to the family members of patients and trying to answer their many questions: How will it be in the ICU? Will it be lonely? Can you please let us visit? Just one time? What can we expect? When will they recover?

Or, was it my inability to answer some of these questions? My lack of a crystal ball? Or, was it being already emotionally exhausted from managing everyday challenges of keeping life as normal as possible, with remote learning, running a household, and helping elderly in-laws with lots of medical, emotional, and personal needs?"

"Instead of trading up for a faster horse by drawing in the exam room, it felt like I’d just built a motor car.

I created a new workflow around my digital assets. When patients checked in for a clinic visit, I scanned the chart for their visit diagnosis and fired off a content link with the relevant videos. My MA roomed the patient and they watched content while I completed other tasks. When I enter the room, their questions are more targeted, insightful and our visits are faster and more satisfying.

Doctors are unique in that our knowledge and experience are incredibly valuable, but distributed scarcely via single episode live events. When we convert our knowledge into digital assets that can be consumed at scale, we earn time, efficiency and carve space to do only the things we can do."

"Guess what? The mouth is attached to the rest of the body. And though dentistry and medicine are typically separated from the very beginning of professional training, they are irrevocably linked for patients and have an impact on each other. Here’s some foundational knowledge that will help set the stage for discussing the connection between overall health and oral health: Our mouths are teeming with bacteria – mostly the good, harmless kind – and when an appropriate hygiene routine is followed, that bacteria is kept in perfect balance to keep our teeth strong and gums healthy. When that bacterial balance is thrown off track, the results can be disastrous. Yes, cavities are one consequence, but so are yeast infections and periodontal disease—i.e., inflammation of the gums and loss of the bony levels around teeth. A quick scan through any wellness magazine and medical journal will tell you that inflammation is bad news. Researchers continue to connect it to some of the worst chronic diseases physicians see in their offices today. Our mouths are a gateway into not only our respiratory system and digestive tract, but also our bloodstreams (there are a lot of surface-level blood vessels right there), so patients’ periodontal inflammation can be a harbinger of other health issues in their bodies."

"The lack of diversity in academic medicine is a significant issue that can compromise our patients’ health and the education of our training clinicians. There is a vicious cycle in which there are few black academic physicians, leading to seemingly fewer available mentors for black students, which in turn leads to even less of them pursuing careers in academic medicine. As a medical community at large, it is imperative that we understand the implications of this problem, not just on the black patient population, but on the nation as a whole. Its effects ripple through our economy and finances, public health/disease transmission, and educational infrastructure. Knowing this, it is more important now than ever that we promote diversity in academic medicine and to be more specific, that we encourage students, residents, and fellows to pursue such careers."
