Metformin is very commonly used as a standard prescription in PCOS, it’s also fairly well known that it can come with some *inconvenient* symptoms like loose bowel movements, nausea and fatigue. A seemingly small price to pay when you’re desperate to lose weight and control your hangries. What I want you to know is that Metformin does come with the potential of other side effects which are more serious and less commonly talked about. The longer you use it, the higher the dose, the more likely you will be feeling the effects of some of these side effects. Mitochondrial damage B12 deficiency Microbiome changes No one seems to be talking about these? Using Metformin as a standalone intervention for PCOS is doing you a disservice. Not only due to the side effects, but due to the fact that it completely ignores the effects of not changing your diet and lifestyle to reduce your insulin. 29 Apr 20234 Things To Consider If You Have PCOS and Are Considering Stopping Birth Control00:22:23

If you went on birth control to help manage your PCOS symptoms it can be SUPER daunting to consider coming off it.

Many of the clients I work with feel anxious about their symptoms returning. Especially the ones they can't hide, like acne and unwanted hair growth.

I get it! The thought of dealing with those symptoms again can be so overwhelming. But if you've been thinking about coming off birth control because:

- You want to have a baby.
- Want to manage your PCOS without synthetic hormones.
- You've realised birth control isn't the best for your long term health.

There are a few things you should consider to ensure a smooth transition. Here's what I recommend:

1. Find Out Your PCOS Root Cause: PCOS can have a few root causes, including insulin resistance, inflammation, and hormone imbalances. It's essential to identify your specific root cause so that you can develop a targeted approach to managing your symptoms. You can start doing this BEFORE you come off birth control, which is exactly what I recommend.

2. Implement Your Unique PCOS Strategy Based On Your Root Cause: Once you've identified your root cause, it's time to develop a strategy to manage your symptoms. This may include dietary changes, lifestyle modifications, supplements, and other therapies. Your strategy should be tailored to your unique needs and goals.

3. Address Any Micro-Nutrient Deficiencies: The birth control pill can deplete several vitamins and minerals, including B vitamins, magnesium, and zinc. It's essential to address any deficiencies you may have before coming off the pill. All of the deficiencies can be identified through comprehensive blood testing, from there a food and if needed supplement plan can be implemented to start correcting things. This is SUPER helpful for supporting your PCOS symptoms as nutrients like B-vitamins enable your insulin receptors to work better and nutrients like zinc can help to reduce the effects of high androgens.

4. Support Your Detoxification Pathways - Liver & Gut: The liver and gut play a crucial role in detoxifying hormones and other toxins from the body. You want to imagine these like a drain through which various hormones are detoxified. Supporting these pathways can help reduce symptoms of PCOS and improve overall health. Eating a nutrient-dense diet, staying hydrated, and incorporating liver-supportive herbs and supplements can all help support your body's natural detoxification processes. A little note here that if you suffer with constipation then you absolutely have a major issue with your detox pathways - this needs to be addressed!

In summary, if you have PCOS and are thinking of coming off birth control, it's essential to take a targeted approach to managing your symptoms. By identifying your root cause, developing a personalized strategy, addressing any nutrient deficiencies, and supporting your body's natural detoxification pathways, you can minimize symptoms and optimize your health.

This week on the podcast I’m sharing the 5 key reasons why I see clients, not losing weight with PCOS. 

 

Here’s what we cover:

  • High insulin levels: Insulin is a fat storage hormone, high levels make it difficult for your body to burn fat. Not sure if you've got an issue with insulin? It's easy to find out, you just need a fasting insulin test! Note: A fasting blood glucose or HbA1c% will not tell you if you have an issue with insulin! ⁠

 

  • Thyroid issues: The thyroid plays a critical role in regulating metabolism, it's estimated 25% of women with PCOS have a thyroid issue. If you suspect your thyroid function is low, talk to your healthcare provider and consider getting tested. Note: TSH is not a thyroid function test. You need TSH, T4, T3, RT3, TPO Ab, Tg Ab. ⁠

 

  • No dietary changes with Metformin: Metformin is a medication that can help improve insulin sensitivity and regulate blood sugar levels. However, if you're taking Metformin without making dietary changes, you may not see the weight loss results you're hoping for. If it sounds too good to be true, it probably is right?⁠

 

  • High stress hormones: PCOS can cause an increase in DHEA-s and or cortisol, the hormone associated with stress. High cortisol levels can cause your body to hold onto fat, especially in the midsection. Note: Metformin, the Oral Contraceptive Pill, Ozempic won't fix your high stress hormones. If you want to get tested you can ask for an AM-Cortisol and DHEA-S blood test, noting these are best tested within the hour of waking. ⁠

 

  • High inflammatory markers: PCOS is linked to chronic inflammation, which can make weight loss more challenging. Inflammation can also contribute to other health issues like insulin resistance, heart disease, and autoimmune conditions. The anti-dote? Your nutrition & lifestyle. Inflammation can be complex to 'test' but you can request a test called high-sensitivity CRP to help you identify if this could be an issue.⁠

Stress. Burnout. Two very common things we hear a lot about, and know negatively affect our health and hormones. In this episode I’m sharing my personal experience with burnout this year, after having my son in September 2022 and jumping back into a business that was not a match for my new phase of life. This year I’ve struggled with at times crippling anxiety, aura migraines with vomiting, crying at nothing, until I made the counterintuitive decision to work even harder but this time, in service of reducing the stress and burnout long-term. My hope and goal in sharing this is that this might inspire someone to radically redirect something in their life in service of a calm nervous system! 

There’s a lot of talk online about stress and burnout management strategies. But, what if the real answer for stress and burnout is acknowledging that we have one short sweet life and sometimes we need to make radical redirections to remove the very thing causing the stress and burnout.

How long will it take for your PCOS symptoms to improve?

Of course, the answer to this is very individual but in the episode I’ll explain how long most symptoms take and how to know if you’re on the right track. 

In general you’ll see improvements in symptoms like energy levels, weight loss and digestive comfort in the first 3-6 weeks, followed by cycle changes like ovulation and reduced cycle length in the following 6-12 weeks. 

One of the hardest things when it comes to staying committed to your health habits with PCOS is that you don’t necessarily see your main PCOS “irk” like say facial hair growth improve for quite some time, so it’s important to know what other changes to look out for to know that you are on the right track.

Have you considered taking Ozempic? I am genuinely concerned about this drug. It’s taken the internet by storm. Ozempic is being prescribed more and more for PCOS...sometimes, off label as a weight loss drug.

Zoe came off the pill in late 2022 after 10 years taking it, and didn’t get a period. In early 2023 she suspected that something wasn’t right and went to see her GP to seek a diagnosis, she was later diagnosed with PCOS.

High insulin is without a doubt the number 1 cause of why you aren’t losing weight with PCOS. There are other possibilities including low thyroid function, but high insulin and blood sugar dysregulation is the most common. 

 

Let’s unpack what’s happening in the body when you have high insulin:

 

Insulin and Blood Sugar Regulation:

  • Insulin is a hormone produced by the pancreas in response to an increase in blood sugar (glucose) levels after you eat.
  • Its primary role is to help regulate blood sugar by facilitating the uptake of glucose into cells, where it can be used for energy or stored for later use.
  • When blood sugar levels are high, such as after consuming a meal rich in carbohydrates, the body releases more insulin to lower blood sugar by storing excess glucose as glycogen in the liver and muscles or as fat.

 

Role of Insulin in Fat Storage:

  • When insulin levels are high, it signals the body to store more fat.
  • High insulin levels can also inhibit the breakdown of stored fat (lipolysis). This means that even if you’re consuming fewer calories than you burn, your body may struggle to use stored fat for energy if insulin levels remain elevated.

 

What this all means is that it’s not enough to simply move more or and eat less to lose weight with PCOS. Instead, you need to eat in a way that prioritises blood sugar control. Rather than eating less calories, you need to eat macro-nutrients that stabilise your sugars and allow your insulin to reduce.

Rather than a single disease, PCOS encompasses various underlying biological mechanisms, leading to a range of symptoms such as androgen excess and anovulatory cycles. Identifying the specific functional type of PCOS is crucial for effective treatment.

  • PCOS is primarily defined by androgen excess, often characterized by elevated male hormones, after ruling out other causes.
  • Contrary to its name, PCOS does not necessarily involve cysts on the ovaries. The so-called "cysts" are typically normal follicles or eggs.

Insulin Resistance:

  • Insulin resistance, associated with high insulin levels, is a common feature of PCOS.
  • Symptoms of insulin resistance may include cravings, weight gain, and dark patches on the skin.

Post-Pill PCOS:

  • Coming off certain contraceptive pills can lead to a temporary surge in androgens, qualifying as post-pill PCOS under current diagnostic guidelines.

Chronic Inflammation (Inc Thyroid Issues):

  • Chronic inflammation plays a significant role in stimulating excess testosterone production in the ovaries, contributing to PCOS.
  • Identifying and addressing underlying sources of inflammation are crucial for managing inflammatory PCOS.

Adrenal PCOS:

  • Adrenal PCOS, accounting for 10 percent of cases, involves elevated DHEAS levels without insulin resistance or inflammation.
  • Treatments for adrenal PCOS include various supplements and herbs to modulate adrenal androgens.

    • Estrogen and progesterone, crucial for hair health, are produced through ovulation and a natural menstrual cycle, underscoring the importance of ceasing hormonal birth control.
  • PCOS Consideration:
    • PCOS, characterized by high androgens, can contribute to hair loss, acne, and hirsutism; blood tests can aid in diagnosis.
  • Thyroid and Hair Loss:
    • Thyroid disease is another potential cause of hair loss, necessitating blood tests, especially for those with symptoms or family history.
  • Importance of Nutrients (Iron, Zinc, Iodine):
    • Adequate iron levels are essential for hair recovery; there is a lot more to iron sttus than ferritin. Rarely is an iron infusion appropriate. 
    • Zinc promotes ovulation, reduces inflammation, and inhibits androgens, making it beneficial for hair health, especially for those with deficiencies or on hormonal birth control.
  • Addressing Inflammation:
    • Chronic inflammation can sensitize hair follicles to androgens, exacerbating hair loss; resolving gut issues or food sensitivities may help alleviate inflammation.
  • Patience in Treatment:
    • Results from hair loss treatment may take at least two months due to the lag time in the hair's telogen phase; consistency and patience are crucial for seeing improvements.
