Today we will be talking about one of my patients' most common challenges, which is overeating at night. Overeating at night is something we have all probably experienced. I know I certainly have. You know how it goes, you've eaten your dinner, in my case getting the kids to bed, and you're sitting down to get some work done or watch some tv, and the next thing you know, you have eaten way too many chips or crackers, too much chocolate, too much ice cream, or even too many fruits and vegetables. Whatever it may be, you've eaten a bunch of things you didn't plan on eating, and you feel like you've undone some of the work you did earlier in the day or earlier in the week. In this episode, we are going to talk all about emotional eating. We will address ways you can recognize and handle it with the "Am I hungry" method. We will also talk about what emotional eating is and what it is not. When I have researched this topic before, studies estimate that about 70 to 80% of people emotionally eat. That means the vast majority of us emotionally eat at least from time to time. 16 Jan 2023How Much Weight Do I Need to Lose? Ep. 01100:31:03

It might surprise you to know that many people who come to me for weight loss don't actually need to lose weight!

So how do you know if you (or your patients) do?

In my newest podcast episode, I run through a few things to consider, because it is possible that stressing about weight is actually causing more harm than good for your health.

I always invite my patients to share with me whether their physical activity is limited due to their weight. I also ask if there is a current health problem or future concern due to family history that losing weight would solve.

Losing a moderate amount of weight can do amazing things for their health, so that's part of why I got into the field I got into. Without adding medicines or treatments, a plan to cut 5-10% of a person's weight can dramatically improve many health issues.

But the key there is "health issues." Sometimes the actual issue is more of a societal pressure or body image concern than an actual health problem or physical limitation.

In today's Dr. Francavilla Show podcast episode, I walk you through the pros and cons of each of the 6 most common diets that are brought up by patients at my clinic. 

Data tells us the biggest predictor of success when picking a nutrition plan is if it's something you can stick with.

As an obesity physician, I typically recommend my patients start with something like this, which I call the Healthy Eating Plan.
 Simply take a plate and fill ...
 - 1/2 of it with veggies 
- 1/4 of it with protein 
- 1/8 of it with fats 
- 1/8 of it with carbs

The veggies provide lots of nutrition and the protein is really good at making people feel full and satisfied, and that is really important for your patients trying to lose weight.

Unless you live a life that always goes as planned, listen up!

Backup plans are a must when it comes to your health because no matter how well-prepped your lunches are for the week, you're bound to forget the lunch bag at home one day or find that your lettuce has wilted (already?!).

So do these things get you off track and change the trajectory of your successful health plan?

Not if you have a backup plan!

Backup plans don't have to mean twice the planning—you won't have to use them much, so don't worry about spending too much time putting them in place. But when you do need to use them, you'll be so glad you thought about them in advance so you don't derail the progress you've made on your health goals!

What, exactly, is appetite?

Our appetite is so much more complex than most people realize. There are actually seven components of appetite, and understanding each of them can give a much clearer idea of what causes us to eat, overeat, or eat the "wrong" foods.

Hunger and cravings are two of the seven—likely the two we think of the most.

Hunger is that physiological desire to eat; you might notice it by a growling stomach, for example.

Craving is a desire to eat something specific; it might be something you often think about (chocolate for me!) or something that comes up sparingly (anyone else just really want a juicy cheeseburger every so often?!).

When it comes to nutrition plans, exercise plans, or really just about anything in life, getting comfortable with the "gray zone" is important. Sure, the black-and-white, all-or-nothing mentality might make for some successes when it comes to diet or exercise (when you're all in, you likely see some great wins!), but inevitably there will be a day when you can't be all in (or in at all) ... and then what? Typically that's when people give up altogether. Any and all wins go by the wayside.

It's simply not sustainable or realistic to approach food, dieting, exercise, etc. with black-and-white thinking. Never going out to eat, for example, might be a good way to control what food goes in your mouth, but is it realistic to think you'll never need or want to eat at a restaurant? Life happens; you start dating and find yourself at restaurants or injury/illness/busy weeks cause you to turn to restaurant meals for convenience. Going to a restaurant doesn't have to mean throwing your nutrition plan out the window! It just means you have to have some intention about ordering something that will be as close to on-plan as possible.

That's the gray zone, and it seems to be where most of life takes place. Weight bias happens on playgrounds, when the child struggling with obesity is bullied. But it happens in subtle ways, too, and it even happens in our medical offices.

Combatting this kind of bias takes education and advocacy, which is why I invited Dr. Leslie Golden to join me on today's Dr. Francavilla Show podcast episode.

Dr. Golden is an obesity medicine physician with graduate-level training in behavior change.

One important type of weight bias she detailed was internalized bias—the negative ideas we have about our own weight. We believe and buy into the idea that our weight issues are our fault, and that can really get in the way of people seeking care, causing long-term harm.

It's critical to recognize obesity as the disease that it is. It's not something everyone can control by moving more, eating less, trying harder.

And, it's important to recognize healthy bodies come in all shapes and sizes. Our patients and our society tend to assume small means healthy and large means unhealthy.

Small does NOT necessarily mean healthy. Large does NOT necessarily mean unhealthy.

These biases can keep people from getting the help they need. Some of these biases even affect insurance coverage of weight management medication.

We as physicians and members of our community can help to mitigate biases by understanding that weight is biological and that medication is a tool necessary for many.

Whenever it is, it's a false projection that things will be better or goals will be easier to achieve at a later time, and that's just not something anyone can predict! Sure, maybe after a certain work project is done there won't be that stressor to worry about, but do you think there won't be another thing that will cause stress or busyness or any type of hardship? This mindset leads to yo-yoing and doesn't make for sustainable change.

Instead, it's best to go ahead and make small changes right away. Up veggie intake by a serving or start lifting weights for 10 minutes a day. Do what's doable now, even with a looming work project or whatever might make next week a "better" time to start! After all, any health plan that's worth starting will need to see you through busy seasons, vacations, stressors, etc.!

If you have been operating under this assumption that carbohydrates are bad, but you find a moderate amount of oatmeal to really satisfy you and keep you full all morning, that doesn't seem bad to me! That's your experience with that food.

Mindful eating lets us experience the food, really enjoy it, get pleasure from it, and it also lets us see how each food actually affects us as individuals.

In this episode, Dr. Matthea Rentea gives incredible perspective on emotional eating. She highlights the phases of emotional eating/overeating, and they are SPOT ON:

  1. Recognizing you've overeaten after the fact.
  2. Noticing you're in the process of overeating and telling yourself you shouldn't be doing that (but you keep doing it).
  3. Before you eat, you tell yourself not to overeat, but you still do.
  4. THIS is the stage where you take a different action due to the progression of awareness you've developed in stages 1–3; you use your tools to keep from overeating.

Do you see patients (or yourself!) going through these phases? Many people think they can jump right to the final step, but that's not how it works. It's important to encourage our patients to keep going through all the phases—they'll get to Step 4 in due time.

What if we changed our mindset so the number on the scale was simply informational and not an emotionally distressing commentary on how poorly we're doing at life?

It's funny because there are lots of numbers in healthcare ... but I don't see people in my clinic feeling ashamed about a low potassium number! No one is beating themselves up when they learn their respiratory rate isn't in the normal range!

We simply examine those numbers and figure out how to get them where they need to be.

The number on the scale is no different.

You don't need to be hard on yourself if it's higher than you want. You just need to take the first step, whether that's talking it over with your physician or going back to what was working—we all stray and lose track sometimes!

It's not a judgment, just like there's no shame in your vision changing from one appointment to the next. That's why you check in! You gather the info and make a plan for managing your _______ (fill in the blank: eyesight/potassium/respiratory rate/WEIGHT!).

In the modern world, though, sugar is no longer rare. There's an abundance of sugar now, and our brain hasn't caught up with that fact.

We no longer need to store as much sugar, so our craving for and enjoyment of sugar can sometimes be a problem for us in the modern world.

Knowing the history can keep us from villainizing sugar, and recognizing things have evolved can help us understand the importance of indulging in moderation.

For nearly a century, the Body Mass Index (BMI) has been a go-to tool for measuring a person's health and medical risks. However, recently the American Medical Association voted to move away from solely relying on BMI. They've recognized its limitations and unreliability in accurately assessing an individual's health status.

In this episode, Dr. Sylvia Gonsahn-Bollie  provides insights into the origin of BMI and highlights why it may not be as helpful as commonly believed. According to her, using BMI as a screening tool for obesity, adjustments need to be made for factors such as age, biological sex, muscle composition, ethnicity, race, and the presence of other metabolic or obesity-related conditions. 

Unfortunately, the original BMI chart from 1832 did not take these factors into account. BMI is just one of several numbers used in medicine as a cutoff point, and its limitations and controversies have raised concerns recently. In certain populations, it can overestimate the risk of obesity and metabolic-related diseases. It's important to remember that the purpose of using BMI is as a screening tool.

The first type of carbohydrate is sugar, and it's considered a simple carbohydrate because it's composed of just one or two molecules, known as monosaccharides and disaccharides. These monosaccharides typically include glucose, fructose, or galactose. Simple carbohydrates like sugar can lead to rapid increases in blood sugar levels when consumed. 

The second type is starch, a complex carbohydrate. It's composed of long chains of glucose molecules, forming a polysaccharide. Examples include foods like pastries, rice, and potatoes.

Lastly, fiber is considered a complex carbohydrate because it's primarily made of cellulose, a polysaccharide composed of glucose molecules linked together. Unlike starch, which we digest for energy, fiber resists digestion and supports digestive health. It's also essential for our gut microbiome, a community of intestinal bacteria that thrive on it. A healthy gut microbiome can influence overall well-being, including potential effects on weight and health.

Oprah discussed how individuals with obesity often blame and shame themselves. She shifted the mindset by highlighting the importance of understanding why people gain weight. It's not solely about eating behavior, lack of activity, and definitely not about intelligence or willpower. She emphasized that knowing the science behind it, or simply put, understanding the biology, plays a major role in how we manage weight. This new perspective might help us stop blaming ourselves and start being kinder when it comes to managing our weight.


Another interesting thing that Oprah shared was her own experience with weight loss medications, saying they not only helped her lose weight but also made it easier to stop thinking about food all the time.

Rubin's framework categorizes people into four personality types:


  • Upholders: They readily meet both inner and outer expectations, making them proficient at sticking to plans and goals, including those related to weight management.


  • Questioners: They meet inner expectations but question outer ones. For weight management, they need reasons and justifications for their health choices.


  • Obligers: They meet outer expectations but struggle with inner ones. They may excel with external accountability, making them responsive to support and encouragement in weight management efforts.


  • Rebels: They resist both inner and outer expectations and thrive on spontaneity and freedom. For weight management, they may benefit from non-traditional, flexible approaches.


By identifying your or someone else's tendency, you can tailor strategies to maximize motivation and overcome challenges in achieving health goals. Understanding these dynamics can make a significant difference in how effectively you approach weight management and overall well-being.
