21 Feb 2022Pelvic Floor Assessment - Are We Choosing the Wrong Anchor?00:23:47

How do we avoid going down the wrong road with a patient after their evaluation and assessment?  

This was prompted by a question in the Huddle regarding how we evaluate pelvic floor strength in patients.  Many of you said you only do it because it helps with getting insurance reimbursement.  But it's really, really hard not to allow that information to cloud your plan of care!

In this 'sode we talk about the 'anchor bias' - the tendency of the mind to over-focus and rely too heavily on a single piece of information.  There are several ways this can affect pelvic floor therapists, including:

  • An arbitrary 'strength' measurement taken during the evaluation
  • Relying on a medical diagnosis
  • Focusing more on their initial primary symptom rather than looking at the whole picture

The information we gather is 'sticky' in our minds, and it's hard - even if we know the information is extraneous - to not allow it to affect our actions.

We all inherently feel evaluating a patient's max contraction in a supine position isn't an accurate picture of their functional pelvic floor strength.  But we're afraid to challenge what we've been taught, or unsure about what to replace it with.

If you knew you only had one more session with your patient, what would you choose to do in your session?  What would you focus on?  What would you send them home with?

One of the biggest challenges with research is we're usually basically asking 'is this intervention better than nothing?'

  • Is this pain medication better than a sugar pill?
  • Is guided biofeedback better than just getting a paper handout?
  • Is an isolated Kegel strengthening program better than general exercise?

But in reality, those aren't the only options patients have.

In fact, they have hundreds of options and combinations available.

Is it hard for you to be proud of yourself?  Or at least to say that you're proud of yourself?  All too often we struggle to articulate what we're proud of!

Looking back at how far we've come is so important.  We can take lessons and motivation from where we've been.  One of the most impactful exercises we had with our Pelvic PT Rising Business Mentorship Retreat was asking that very question - "what are you most proud of?"

Despite being kick-ass business owners and awesome clinicians, many of us struggle to be able to answer such a simple question.  While we could have gone around and said 5 different things we were proud of for them, it was hard for many to say it about themselves.

05 May 2022"Convincing" Patients to Come to a Cash-Based Clinic00:30:01

Why would a patient ever want to spend money at a cash practice when they can use in-network insurance benefits?  This question is the cornerstone of a successful cash practice.

Fantastic question from a Rising podcast listener.  First thing is replacing the word 'convince' with 'provide information about their options.'

At the end up the day, getting a patient to come and see you in a cash practice depends on only two things.  They are simple, but extremely hard to implement:

  1. Provide better (or at least different) care
  2. Communicate how your care is different

That's it!  People will pay more for a Mercedes than a Kia even though both are cars.  They'll pay more for a Louis Vutton bag than one they can pick up at Target.  If they know there is a difference in quality people will pay for the better option when it comes to things they value.

16 May 2022New IC Guidelines: A Unique Opportunity for Our Field!00:27:43

The American Urological Association just announced new Guidelines for the treatment of interstitial cystitis.  It's the first update in 7 years, and it's big for pelvic rehab professionals!

We basically have an entire category of treatment ("behavioral and non-pharmacological") that we are unique qualified to administer.

Among the highlights (and things we've been advocating for...

  1. The AUA specifically calls out that Kegel exercises should not be done for patients with IC
  2. Additional studies showing Elmiron has limited efficacy (or, in several trials, no efficacy at all) in treating IC over a placebo, as well as the chance for long-term eye damage
  3. Acknowledging an elimination diet (and not the 'IC Diet') is the gold standard for nutritional care

It may seem like a simple question, but it's something we often overlook.  Does our patient really know that we can get them better?

Have we said it explicitly?  "I can get you better....this is not your life....we've seen this before....we can help you...."

We often think we're saying this by telling them how everything is interconnected.  That we've been able to reproduce their symptoms.  That we have found things on the initial evaluation related to their complaints.

But does that actually translate to the patient understanding we can help?

And if we aren't doing this, why?  Is it because we just forget when we get excited in their evaluation and are diving into the weeds?  Is it because we aren't really confident we can help?  Is it because of our philosophy (I'm just their coach, they will heal themselves...)?

If you are feeling conflicted about actually telling your patients "you will get better here," it's something we need to sit with, understand why, and take action on.

As both an employee and employer, it's important to realize there's no such thing as a perfect job.  We want everyone working in pelvic health to be happy and content in their job!

A good match between a position and employee requires 1) self-awareness and 2) open communication.  Usually this breaks down when either party doesn't truly understand what they value in a position.  These can include:

  • Compensation
  • Management
  • Work Environment
  • Patient Population
  • Treatment Times
  • Documentation Time
  • Growth and Mentorship
  • Medical Benefits
  • 401(k) or Retirement Plans

It's important to understand which of these are most important to you when selecting a job, and realizing a current position may not be 'perfect' on all these different factors!

For an employer, we need to recognize we can't be all things to all people.  We need to be confident in what we offer (and what we don't).  And when we aren't, we start making concessions and changes that don't benefit either us or our new employees!

09 Jun 2022Do I Need a Longer Eval Time?00:19:11

What length of time is appropriate for an initial evaluation? 

We really feel the minimum length for an eval is one hour.  If you're in a setting where that isn't being provided it's something to fight for.  Patients deserve to have the time to tell their story and still get a full evaluation on the first day.

When you have your own practice it can be tempting to further extend the evaluation to 90 minutes (or longer).  We go through the business implications of a longer eval and ask the question "who are you doing that for?"

In this case report of a patient with chronic prostatitis, we go through his case history and how he saw two pelvic PTs without relief.

The biggest common denominator?  Not frequently conducting an internal pelvic floor assessment to actually assess what was going on.  In fact, over the course of 20 visits this patient had an internal assessment twice.  Twice!

So how often should we be performing an internal assessment?  Is twice in a given session 'too much'?  And why are we uncomfortable with the assess -> intervene -> reassess model we were taught in PT school when it comes to pelvic health?

How do we make decisions consistent with our values, and not out of fear?

In creating a new resource for you pelvic health 'wannabes' (we were all there once!), we realized one of the biggest reasons we make mistakes is because we give in to our fears.

Starting a mobile practice when we want to have a brick & mortar.  Taking some insurance because we're afraid we can't get patients in our community.  Setting our prices too low.  Doing a side hustle when you don't really need to.  Hiring independent contractors instead of employees - or not hiring at all.

The first step is identifying when we are fearful (fear can masquerade as wisdom or prudence).

And then we need to move beyond it towards our vision for our future.  One of the best questions to ask..."What decision would I make if success were guaranteed?"

Are we still using the biceps analogy for the pelvic floor?  Trying to explain the length-tension relationship?  There's a better way!

In the Essential Pelvic Strengthening: Not Your Mama's Kegels course I teach we should be explaining the pelvic floor more as a trampoline.

It's a more nuanced analogy that helps explain resting tone, how important the surrounding structure is, and how the pelvic floor behaves in a more involuntary fashion.

What do you do when you have a patient who doesn't believe you can help?

Many times these are the patients we end up thinking about when we go home at night.  Or over our lunch break.  Or even during our next patient.

They can make us feel inadequate, sap our energy or cause us to replay conversations in our heads.

"What do I say in order to convince them I can help?"

Many of these patients have very valid reasons to be skeptical.  Their emotions can range from anger to depression to "yeah, but...ing"  So what do we do?

  1. Take our ego out of it.  It's not about us, and we don't have to be right and win.
  2. Stop talking.  There's nothing you can say to change someone's mind.
  3. Show them a change.  Help them to see a change.  Focus on 1-2 things.  Demonstrate a change in the session and help them to see it.

In this case report from a male patient who scheduled a remote consultation, my biggest takeaway was the lack of functional assessment he had received.

He was dealing with pelvic pain, urinary urgency/frequency, post-void dribbling, incontinence during exercise and pain with sitting.

While he had been to two different pelvic PTs, and did have minor improvements, his life was still very different.  Normally an avid runner, he had cut running out completely (after usually doing 70-80 miles a week).  Sitting caused such pain he switched to a standing desk, without actually addressing his underlying issues.

In talking with him, together we saw five different areas where a more functional assessment and treatment could (and I would argue, should) have been done.  Hope you enjoy this case report!

It's a question we're inevitably asked.  One we're not trained in how to answer.  And one that - most times - I think most pelvic rehab practitioners get wrong.

"How long until I get better?"

First we think about what they are really asking.  It's more about "are you the right person to help me" or "what is working with you going to look like."

And we often do our patients a disservice when we try and guess.  Because the truth is - there's no way to know after just the first visit!

So how do we answer it instead?

  1. Ask them back (flip it and reverse it)
  2. "I'm actually not sure, BUT..."
  3. Map out their plan of care and phases of treatment so they know where they are going

If you can do this well, and with confidence, 99% of patients will be satisfied and understand where they are going with treatment.

In retrospect, some don't look controversial anymore (which is awesome!).  And you don't have to agree with each of these, but we do hope they make you think and drive conversations in our field forward.  Do you have a favorite of these?  Let us k now!

  1. Biofeedback is not the best use of your time
  2. We are (or should be) primary care providers
  3. Don't be the evidence police
  4. Voluntary strength testing in your evaluation sends you down the wrong road
  5. If we're not routinely doing rectal, we aren't thoroughly assessing the pelvic floor
  6. You don't need more clinical skills, you need to better utilize the ones you have
  7. The pelvic floor is more like a trampoline than a biceps
  8. You can't always trust research
  9. The isolationist strength approach (Kegels) is outdated

Keeping our professional boundaries can be a real challenge in our profession.  We spend a lot of time with our patients.  They are often really cool people.  And we're discussing intimate topics about their lives they may only talk about with really close friends.

And we should certainly still have fun.  Laugh.  Make a connection with our patients.

But when they cross over into the 'friend zone' we often struggle to provide the same level of care.  It's a challenge for all of us, but we have to recognize when we're acting more like a friend than a clinician.

We can shy away from challenging patients we get too close too.  Break boundaries by communicating outside of our sessions.  Worry more about being liked than about providing the highest level of care.  Or even just allow the conversation to stray from clinical topics to more personal ones.

Is there a divide between physical therapy and wellness?  Prevention?  Maintenance?

If so, where is it?  And if not, why is it something that has invaded our profession?

You might be surprised to know this isn't something that comes from our law or practice act.  Legally there is no difference between physical therapy and wellness.

And you might also be surprised our governing body (the APTA) specifically encourages wellness, prevention, fitness, and health promotion.

Common sense and ethics also don't draw a distinction.  After all, why would we not want to promote wellness ("Active pursuit of optimal health")?

So where does this distinction come from?  Insurance companies.  Specifically, the desire of insurance companies to pay as little as possible as they maximize their own profits.

We discuss what this means for our field and our individual practice in this 'sode.

Growth in our business or career isn't ever linear.  It's often punctuated bursts.  Sometimes we're standing still, and others we're rushing forward.

It's unreasonable to expect our business to continue to grow as rapidly as we want (whatever that means).  We can't put in maximum effort all the time.

Seasonality is a rule of nature and it applies to business as well.  So how do we manage the different seasons and still enjoy the process throughout?

When it's slow, build your foundations.  Mend the nets.  Get your business ready for the next burst.  Put in the consistent work to build momentum.  Build relationships.  Take vacations.  Enjoy the slower pace.

When it's fast, lean in.  Embrace the hectic.  Spend more time.  Get excited.  Dive deep into your passion projects.  Take advantage of the tailwind.

What do we do when a patient just isn't getting better?  When they feel frustrated?  Or when we don't understand why they aren't making more progress?

These plateaus can be a major challenge in our clinical practice.  They make us feel insecure, defensive, or like we're just guessing.  

In this 'sode we go through how to examine a plateau and how to respond (not react!) for the best patient outcomes.  

Three tips for dealing with plateaus: 

  1. Plateaus aren't necessarily bad!  They can be expected and also give us more time to implement long-term strategies to benefit the patient's overall health.
  2. Don't knee-jerk into a change.  If you are going to change strategy, make sure you've given your original plan a chance to work!
  3. Pre-frame with your patient.  Progress isn't linear in anything to do with our health, but we need to let the patient know what to expect.

We all know we need "boundaries"....but what does that even mean?  It's often throw out as a panacea for any situation.  "You just need boundaries."  

New definition of boundaries: "Decisions you've had time to consider in advance"

If you're an employee and unhappy at work, it likely has to do with boundaries.  Either your employer has boundaries (rules/decisions) you don't agree with OR they don't have boundaries at all and you're left trying to create them for yourself.

This is a huge struggle for new business owners.  We don't realize how many of our past boundaries were actually created by our prior employers.  We existed within their ecosystem and rules, not realizing we were depending on that structure.  Until it's gone.

Here are some areas where we can establish our boundaries...

  • Schedule
  • Finances
  • Fun
  • People
  • Situational
  • Clinical
  • Productivity

So your homework is to identify three areas of your life causing stress, discomfort or overthinking.  And make a decision in advance (boundary) around them.  Big or small - make a pact with yourself and stick with it!

10 Nov 2022Changing Your Money Mindset: Return on Investment00:23:07

What if we though about spending money not as an expense, but as an investment?  It’s one of the most important mindsets from business (and you can borrow it even if you aren’t a business owner).

We’ve been conditioned to think of certain things as ‘expensive.’  And this is different for all of us.  It depends on how we were raised, our money mindset, our current financial situation, and much more.  Maybe that $5 latte is expensive, or maybe it’s the $50,000 car.

But that’s only one way to look at money.

What if we used the lens of Return on Investment?  This is how business owners (should) think about money.

  • Is $1,500 for a hi-lo table expensive?  Maybe.  But the Return on Investment is years of treating without injury, aches and pains, or poor body mechanics.  It’s an investment that prolongs your healthy career, and is absolutely worth more than $1,500.
  • Is $500 for a con-ed course expensive?  Perhaps.  But if it makes you a more confident clinician, helps you get patients better, and ultimately lets you enjoy your job more, maybe it’s a no-brainer investment.
  • Mentorship or coaching can objectively be expensive.  But the entire point is it propels your business forward and makes you far more money than you spend.  In the Rising Mentorship program, we aim to have at least a 10x return on anything we do for business owners (in 2021, it was actually 20x!).
  • An event like PelviCon has cost associated with it – travel, hotel, cost of the ticket.  But if even one of the 16 talks resonates…if you make a new friend or connection….if you get the confidence to ask for one raise…if you leave feeling inspired and loving your career…what’s the return on investment?

What do we do when our plans for a session get derailed?  When - for whatever reason - what we wanted to do is "too much"?

This can happen for a variety of reasons.  Some examples include:

  • A patient who is contrarian - there's always a reason you can't do something
  • When a patient is in too much pain - they are flinching, can't be touched, or petrified they are going to flare
  • When a patient is emotional or distraught

We have to be able to recognize this change and still provide value for our patients!

It helps to have a specific toolbox in mind when things are "too much."  This way we can still provide a feeling of safety for our patients and a valuable session (even if it's not what we planned).

14 Nov 2022Every Pregnant Person Can Benefit from Pelvic Therapy00:27:58

We didn’t think this was a controversial statement, but it seems some explanation is in order.  Yes, we can help anyone who is pregnant.  Even if they don’t have ‘symptoms.’  Or a complaint.  Or something wrong.

There are changes in the body happening daily.  We are the biomechanical experts.  And we actually have the time to work with our patients.  Alleviate fears.  Screen for issues.  Support their pregnancy journey.  Here’s four ways we can help our pregnant PEEPs (yep, it’s an acronym!).

  • Educate on body changes and what is normal
  •  Exercise safely and confidently throughout pregnancy.  Some are scared by what they hear from their OB and think all exercise is dangerous.  Others completely ignore medical advice and continue with their pre-pregnancy exercise plan without any changes.  We can be the bridge to a safe, confident exercise program in pregnancy.
  • Prevention of low back pain, hip pain, pubic symphysis pain and other pregnancy issues.  No, there isn’t a ‘study’ on this.  But common sense – are we able to help optimize the body?  Can that prevent or mitigate pain and symptoms?  We can help outside the pelvic floor as well, in the mid-back, feet, hands or other areas where pain can develop during pregnancy.
  • Prepare for birth.  We can teach relaxation techniques, labor positions, pushing mechanics, perineal massage, breathing.  Many pregnant people don’t get much (if any) birth prep.

28 Nov 2022Ticklish Patients Are No Laughing Matter00:30:06

We've all (likely) had a patient who is extremely sensitive or ticklish.  The laughter is often contagious, and we find ourselves cracking up as well.  But there's a lot to consider about the ticklish response - this 'sode is all about dealing with these patients.

When we really think about it, if we subtract the laughter the tickle response looks an awful lot like a pain response.  Contracting, pulling away, getting into a fetal or defensive position.  

The tickle response is processed in the hypothalamus of the brain - the same place where the emotional threat response is processed.  Here are some techniques that can be extremely helpful with this patient population!

  1. Use the anticipatory system to make sure the patient isn't surprised by the touch
  2. Calm the nervous system using a variety of techniques
  3. Start away from the area that is ticklish

One of the big fears - we talk about 10 'Mindset Anchors' - that can hold us back is fear of criticism.

This can be from bosses, from colleagues, from patients, or online comments.  

Anytime we're branching outside of our comfort zone we can be afraid of getting criticized.  If we're not careful this can hold us back in our business or career.

So how do we get past it?  Here's a few ideas: 

  • If it's valid criticism, accept the kernel of truth, grow from it and move on
  • Judge the judger - is this someone whose opinion you truly value?
  • Recognize no one is judging or criticizing you who is doing MORE than you're doing
  • You're always being judged anyway, so you might as well be judged for doing what you believe in and feels authentic
Ultimately, the answer is action.  Keep your mind on your goal and the reason you're doing the new thing.  Remind yourself of the higher purpose and the bigger reason for what you're doing, and the fear of criticism will start to fade in the rearview mirror.

How do we get unstuck - with a patient or project or business?  Often it's because we're trying to fix something that wasn't great in the first place.  

Often the answer seems counterintuitive - what if we just threw away the old 'prior' or previous model and started over?  It often will free your creativity and inspire you to create something better than you eve would have.

Need examples?

  • Getting stuck with a patient when you're going down the wrong road and just need a new set of eyes
  • Making incremental changes to your business website
  • Working with a logo or color scheme you don't really love
  • Charging less than you're worth because you started your business too low

15 Dec 2022Maternity Leave: An Opportunity for Your Business00:33:21

How can we view maternity leave as an opportunity in our business (or career)?

Many of us first start our own business after a maternity leave.  Our priorities have shifted.  We want more time freedom.  Or we have a new passion for pelvic health after having experienced life as a patient.

For those of us who get pregnant after starting our business, we have a few different options.

We can be ok with just shutting down during that time.  It takes careful planning, financial discipline and a plan for both patient care and marketing, but you can return to a thriving practice.

You can find someone to temporarily cover your patient load.  You'll have to bring them in, get them up to speed, and have all the timing work out in order to make this work.

Or you can put your foot on the gas to hire, onboard and train a team before you leave.  But make sure it's not just a knee-jerk reaction to being pregnant!  If you didn't dream about growing beyond yourself, this is not the time or reason to make that decision.

No matter what you choose, your business is resilient.  You'll be able to return to a thriving business and continue to grow!

How do we want our lives to be better at the end of next year?  What would make 2023 great?

We often work with our mentorship group on setting better, more meaningful, and more manageable goals for the coming year.  We usually struggle to do this well, which means a lot of frustration, guilt or given-up goals.

Here are a few ways we can make 2023 great!

1.  Broaden Our Goals - Instead of hyperfocusing on easy-to-measure areas (our raise at work, our weight, our business revenue), we should be broadening our goals.  We suggest making goals in the following 6 categories: Career/Biz, Financial, Wellness, Relationships, Personal Growth and Fun.

2.  Create Virtuous Circles - Identify goals that will make everything else in your life easier.  Is that getting to bed earlier, doing meal prep at the beginning of the week, or doing yoga 3 times a week?

3.  Make Goals More Realistic - What if we cut our goals in half?  Make them more realistic, build momentum and not let ourselves given in to disappointment when we aren't perfect.

4.  Get Visible - Where are you putting these goals?  Is it in a place you'll see them every day, or week?  Or somewhere in a pile of papers you discover at the end of the year?

5.  Remove to Improve - We're not usually swimming in free time.  So if we want to add something to our busy lives, we're going to have to remove something else.  What are you looking to remove?

09 Jan 2023No Such Thing as a 'Simple' Patient00:22:56

You’ve heard us say it before, but here we dive into why we believe there isn’t a ‘simple’ pelvic health patient.

Simple (n): easily understood or done; presenting no difficulty.  plain, basic, or uncomplicated in form, nature, or design

Our patients are not easily understood.  They do present difficulty.  And the pelvic floor is anything but uncomplicated!

1.       The pelvic floor is utilized constantly.  We can’t tell the patient to take a break.  They’re using it and having to think about an issue all the time.  As we said in an earlier podcast, you can’t RICE the pelvic floor.

2.       The nervous system and complexity of the anatomy makes the pelvic floor unique.  We have to be integrating the nervous system component if we want lasting change for our patients, and the nervous system is inherently complex.  The pelvic floor is also influenced by so many other areas of the body.

3.       The length of time and/or multiple issues.  We’re not generally getting a patient immediately after symptoms start.  Even our post-partum patients have just been through 9 months of crazy body changes!  This is more complex than treating a sprained ankle.

4.       Better life.  If we embrace our role as helping the patient live a better life – and not just resolve their symptoms – we can never work with someone simple.  Can they leave a better person?  Can we positively impact their nutrition, their sleep, their exercise?  Can we make changes that will have a positive impact on their overall health and life for years to come?

5.       Top down and bottom up.  The complexity of the pelvic floor means we have to be working both top-down and bottom-up for our patients, no matter their diagnosis.

So let’s not dismiss any of our cases as ‘simple’ in our minds!

Let’s start with common ground – telehealth is not the gold standard of pelvic health.  In an ideal world we would be able to examine the patient, work with them, look into their eyes, see them move and interact in person.

We need to resist the urge to make a shift to telehealth just because it feels ‘easier’ to us, or we can do it from the comfort of our home.

23 Jan 2023What To Do With a Long Wait List00:35:49

One of the major career concerns we heard about in our end-of-year survey was working in a facility that is understaffed with a long wait list.  We dive into what this means, how you can keep your sanity and still provide the best possible care.

First of all, we have to realize we are going to have a major choice to make.  And each option will have major limitations and be a compromise.

We can provide the best possible care to a very limited number of patients.  We limit the number of evaluations, and only get someone new onto the schedule when you have room to provide the follow-up care you believe they need.  

Or.

We can provide care to as many patients as possible, realizing we’re providing less-than-ideal clinical care.  When this happens we tend to start modifying our recommendations to fit the schedule, seeing patients less often, and patients are getting squeezed off the schedule before they’ve met their goals.

We talk about how to deal with both situations, but clinical honesty is paramount.  Regardless of the situation where you’re working, we believe you should never compromise your clinical honesty.  Tell patients what you think would be best, and then they can work within the system.

14 Feb 2023Our Journey of Grief and Hope: Supporting Patients Through Pregnancy Loss01:03:20

In another personal 'sode we share our own challenges with pregnancy loss and how it's changed Nicole's clinical practice and how we interact with those experiencing loss.

(We do discuss our pregnancy loss in detail in this 'sode - if that may be triggering for you, please skip it or ignore it entirely)

Our journey has included a second-trimester loss, a first-trimester loss and a failed IVF transfer.  It's led to several medical procedures, two ER visits and a bout with sepsis.

We hope our story can provide more clarity on how we interact with our patients dealing with loss and set boundaries for ourselves.

One of the biggest takeaways of this 'sode is there is such variability and complexity with how everyone deals with grief and loss.  It really is n = 1, and we have to treat it that way.

Another lesson is the importance of drawing our own boundaries.  If you're not ready to see a certain patient, or a certain type of patient, it's ok to step back from providing their care.  One of the biggest blessings and challenges of what we do is we're dealing with important, life-changing topics for our patients, but we need to have space for ourselves as well.

If you're dealing with loss or know someone who has, we just want to you to know you're seen.  We hope you have the support of your family, friends and (if necessary) a mental health counselor.

16 Feb 2023Breaking News - Changes to the APTA Chaperone Statement00:33:58

There's been a lot of confusion and concern over a recent release from the APTA on the use of chaperones in pelvic health.

This new document surprised a lot of us when it came out in January.  Among some important changes in the document were:

  • Redefining chaperone as 'a member of the health care team' and specifically excludes family members or friends from acting as chaperones
  • Recommending asking the patient if they would like a medical chaperone at each visit and documenting their answer
  • The inability for a pelvic physical therapist to provide the first vaginal or rectal examination for a minor patient, regardless of consent or situation.

These are obviously - if they were implemented and followed - monumental changes in our field.  These recommendations are impossible for small businesses or solopreneurs to comply with.  They would eliminate pelvic physical therapy in in-home settings (obviously you aren't having a chaperone 'ride-along' to each patient's house!).  And they could make it financially impossible for even large hospitals to pay for enough medical chaperones, causing programs to close (or never open) and reducing access to care.

In response, we gathered some comments from the Pelvic PT/OT Entrepreneurs group - thanks to all that contributed - and added our own concerns.  We then sent these on to the APTA.

To their credit, they responded quickly - and it looks like in a very positive way.  They have promised changes to this document and the entire process by which these position statements are created, including:

  • Removing the definition of 'chaperone' as only a member of the health care team and allowing the patient's family and friends to serve as a chaperone if desired.
  • Adding legal language meant to future guidelines to make it clear they are only suggestions and providing additional legal support for clinicians
  • Adding a 30-day comment period to future publications to avoid similar issues in the future.

We'll have to see when the new guidelines come out, but wanted to pass on the update and changes!
