25 Oct 2019Kate Scarlata, MPH, RDN on The Low Fodmap Diet, IBS, Bloating, Constipation, and More00:51:06

The Low Fodmap Diet is the most researched diet for Irritable Bowel Syndrome (IBS) and is estimated to help alleviate symptoms in upwards of 40-70% of IBS patients.  Not many people know more about the low fodmap diet then our guest Kate Scarlata, MPH, RDN.

FODMAP is an acronym that defines foods that are composed of Fermentable Oligosaccharides (fructans and galacto oligosaccharides), Disaccharide (lactose), Monosaccharides (Fructose) AND Polyols (sugar alcohols). These carbohydrates are small chained carbohydrates that many people with IBS and related digestive disorders have a difficult time tolerating these carbohdrates and may develop symptoms of bloating, pain, constipation, or diarrhea, or a mixture of these symptoms.

We dive deeply into the Low Fodmap Diet and also into other advanced conditions that mimic IBS and must be considered if you are not responding to the Low Fodmap Diet. I have show notes on my blog at this link.  Now more about our guest.

Kate Scarlata, MPH, RDN, LDN is a registered, licensed dietitian with a nutrition consultation business in Medway, Massachusetts. 11 Dec 2019The Gut-Skin Axis with Lucy Mailing, PhD (MD candidate)00:51:05

One of my favorite sayings in life is " As above , so below" . This can certainly apply to the skin disorders.  As we learn more about skin conditions like eczema, psoriasis, rosacea, and acne we are seeing that that deeper in the body at the gut/immune interface there is likely a driver of inflammation. I speak about this on my most recent episode of my podcast with Lucy Mailing PhD.

Lucy Mailing is an MD/PhD student at the University of Illinois. She recently completed her PhD in Nutritional Sciences but continues to perform research on how diet and exercise impact the gut microbiome in states of health and disease. She has authored several peer-reviewed journal articles related to the microbiome and health and was recently named an Emerging Leader in Nutritional Sciences by the American Society for Nutrition.

Lucy has also been as staff research associate for Kresser Institute for four years and writes about evidence-based gut health on her blog, www.NGmedicine.com. She currently plans to continue her training and begin her MD at the University of Illinois in 2020 after a year dedicated to her blog and the launch of a gut-related startup.

31 Jul 2019Autophagy and Molecular Nutrition with Jeffrey Novack, PhD00:42:43

Autophagy is  a term that refers to cellular self-digestion.  We have heard the term self-attack-self in situations like autoimmunity; Well this a different type of process that is a normal every day process in the cell of cellular self-digestion. Technically autophagy is a catabolic metabolic program/process that is activated when intracellular nutrient uptake or availability is diminished.  It was coined in the 1960’s by the late Christian de Nuve. It has resurfaced quite predominately since  Yoshinori Ohsumi was awarded the 2016 Nobel Prize in Medicine for his discoveries of mechanisms for autophagy – a study into how cells detoxify and repair themselves.  Autophagy has a number of pathways that are studied that are associated with it such as the Mammalian Target of Rapamycin (MTOR) and  Nuclear factor erythroid 2-related factor 2 (NRF2). This is relavant because we know inflammation causes disease and many protective pathways of inflammation are regulated by NRF2 . Likewise we know that protein aggregating in cells is one of the key findings in disease.  Autophagy is one of the pathways that clears up protein debris from harming cells.   So understanding inflammation and autophagy is kind of important.  Inflammation can knock down autophagy so these topics must be studied together. Our guest Dr. Jeffrey Novack understand these pathways quite well and most interesting to us is that he understands how nutrition and chemicals from plants can influence these pathways.   More about our guest.

Dr. Jeff Novack earned his Ph.D. in pharmacology from the University of Washington School of Medicine (Department of Pharmacology) in signal transduction (lab of Dr. Joe Beavo). He did post-doctoral research in immunology and tyrosine kinase signaling at the Fred Hutchinson Cancer Center (lab of Dr. John Cooper). He taught pharmacology, immunology, infectious disease, and pathology at the medical school level for 13 years. He most recently was a tenured faculty at Bellevue College, where he taught anatomy and physiology, microbiology, cell biology, and nutrition. Dr. Novack joined the faculty at PNWU in September of 2012 and has taught immunology, portions of pathology, and pharmacology. He has done research on the effects of PSK on immune function in mice and humans and is interested in the effect of natural products on the innate and adaptive immunity and on inflammation. He likes to hike (has climbed all of the volcanoes in Washington state), bike, fish, cook and do polar bear swims in the winter.

28 Dec 2022Episode 82 Reversing Interstitial Cystitis /Painful Bladder Syndrome with Dr. Mandy01:00:24

Current studies estimate that 2.7% to 6.5% of women in the United States have symptoms consistent with a diagnosis of interstitial cystitis/bladder pain syndrome (IC/BPS). The broad range in prevalence depends on whether a high-sensitivity definition (6.53%) is used versus a highly specific definition (2.7%)
And that is really an underestimate, because most people complaining of painful, urinary discomfort,  urgency, and frequent urination; in the absence of an infection are just brushed off by their health care provider and sent home with no solutions. Meanwhile they suffer, with constant urging to urinate and pain sometimes to the point that urinating feels like peeing chards of glass.  It is not something that is over in a few days. This is a chronic and intermittent condition  In previous episodes of the one thing podcast we spoke about IC and got a nice view of the microbial  and also biofilm component; however there was more to the story that I wanted to explore. So I reached out to Dr. Mandy LaGrega who is known as the IC healer.  I was intrigued by her work in this area especially when it comes to her knowledge of the Oxalate component of IC/PBS and her understanding of the gut, fungal, mycotoxin, and psychoemotional role of these disorders.  We had a great conversation on this episode about how IC/PBS develops, how oxalates play a role,  the risk factors for IC/PBS, and overall approaches to healing this disorder from a nutritional approach.  Enjoy!

Androgen excess in women has been poorly understood from a diagnosis standpoint and treatment standpoint . Many women have been told they only fit a Polycystic Ovarian Syndrome (PCOS) diagnosis if they fit a finely defined criteria and many are offered the same treatment regardless of their unique disfunction.  In this episode we discuss a new definition of androgen excess disorders and understanding subtypes of PCOS. Our expert Dr. Lara Briden, author of The Period Repair Manual will walk us through the following:

  1. A need for a new definition of PCOS and androgen dominance. Hint : you don’t need cysts on your ovaries to be diagnosed.
  2. Some of the root causes
  3. Look alike conditions like hypothalamic amenorrhea , congenital adrenal hyperplasia , medication causes, cushing syndrome.
  4. 4 functional subtypes of androgen dominance.
  5. A functional approach to balancing androgen dominance. Your subtype matters!

27 Sep 2019Psychobiotics and the Gut-Brain Axis with Scott Anderson01:12:22

Do the bugs in your gut influence whether you are anxious, depressed, clear-headed, or distracted?  Well, it turns out quite possibly. Future depression prescriptions may be custom probiotics rather then Prozac.  I invited Scott Anderson the author of the The Psychobiotic Revolution to speak with us today about this most interesting topic. We dive into this and the gut-brain axis, how diet influences the microbiota,  fecal transplants, measuring the microbiota,  anxiety, depression, and more.

About our guest:

Scott is a scientist and a science writer who was drawn from the wine country in California to work with horses in Ohio. There, Scott set up a laboratory to help develop equine prebiotics. He found that a less diverse microbiota was associated with colonic ulcers and poor performance. That ultimately led him to John Cryan and Ted Dinan, two pioneers of the gut-brain axis who found that gut microbes can affect the behavior of mice. Scott ended up writing a book with Cryan and Dinan called The Psychobiotic Revolution, from National Geographic. It is now going into the second printing.

14 Oct 2020Joint Pain with Dr. Angela Cortal00:43:14
Chronic Joint Pain often leads to a diagnosis of degenerative joint disease (also known as osteoarthritis) and patients are given the array of physical therapy and anti-inflammatories with the hope of delaying and preventing joint replacement surgery.  This is a standard approach and joint replacement procedures often need to be redone 10-20 years late. . A multi-disciplinary approach to the same problem might look at diet, lifestyle ,exercise, nutrition, hormonal health, and other factors to prevent or delay surgery.   In this episode I welcome Dr. Angela Cortal who is an expert in Joint health and regenerative therapies and the author of Younger Joints Today. This episode covers: 1.  How hormone health matters in joint pain.  2. Nutritional and lifestyle influences 3. Regenerative therapies 4.  Dr. Cortal's  7 step plan for Joint pain . Short BIO: Dr. Angela Cortal is a Naturopathic Physician based in Oregon, and the best-selling author of Younger Joints Today. She is passionate about reversing degenerative joint disease by addressing the root causes. She loves partnering with her patients to optimize their health, happiness, strength, and well-being. 03 Feb 2021WIALD : A focus on Polyphenols00:15:36

Polyphenols fall under the category of phytonutrients.  Which are plant nutrients that confer health benefits.  Polyphenols specifically are food for our microbiota and are involved in housecleaning of potential pathogenic pathobionts.  We should really strive for about 1000mg-1500mg of polyphenols in our diet daily as the evidence is stacking up for their  health benefits for colon health, brain health, cardiovascular health, and overall inflammation support.  In this episode of WIALD (what I am learning daily) I go into the best sources and further unpack polyphenol benefits. The WIALD episodes are snack-size podcast recordings that complement the longer episodes that feature guests from various backgrounds. You are invited to come into my personal learning lab on WIALD. 09 Apr 2019Dementia and Alzheimer's Disease with Dr. Kabran Chapek of the Amen Clinics.00:34:14

Dr.   Kabran Chapek is on the cutting edge of early detection and early treatment of Dementia and Alzheimer's disease.  As a  clinician at the Amen Clinics in Bellevue, Washington he specializes in integrative approaches to neuropsychiatry . In this episode he talks about early warning signs of memory loss compared to attention issues.  In addition, he dives into the some key early markers of the potential of Alzheimer's Disease such as Apolipoprotein E4 and Amyloid Precursor Protein (APP). He talks about ways to address neuroinflammation to help slow progression of disease.  It was really great  speaking with this old colleague of mine who I  first met over  15 years or so ago when we were fellow student clinicians at Bastyr University.  Alzheimer's disease currently has no known cure.  Changes in the brain indicating dementia and Alzheimer's Disease potential can start as early as 20 years before the onset of the disease.  You don't want to miss this episode as it gives some really important information as to how to get ahead in detection and possibly prevention of dementia and Alzheimer's Disease. 18 Sep 2019Microbes as a driver of Autoimmunity with Dr. Amy Proal00:54:23
What is driving Autoimmunity? Is it time to turn our beliefs about autoimmune conditions around and look at what is the underlying cause?  According many researchers, including our guest Dr. Amy Proal , a key driver of autoimmunity might be microbe based.  As researchers are finding out specific microbes that turn pathogenic called "pathobionts" may be leading to a chronic inflammatory process that goes unchecked.  In this episode we dive into pathobionts, intestinal permeability, the problem with standard immunosupressive therapy, biophages, and so much more. Get ready to learn as Dr. Proal is a force . Now more about Dr. Proal. Dr. Proal is a microbiologist who studies the molecular mechanisms by which bacterial, fungal and viral pathogens dysregulate human gene expression, immunity and metabolism. Her work further examines how dysbiosis of the human microbiome and/or the human virome can contribute to chronic inflammatory disease processes. As a member of the research team at Autoimmunity Research Foundation, she has authored papers and written book chapters for organizations like the J.Craig Venter Institute and The Autoimmunity Network, and lectured at the NIH and numerous USA/international conferences. Proal graduated from Georgetown University in 2005 with a degree in biology. In 2012, she obtained a PhD in microbiology from Murdoch University in Australia. 02 Dec 2020Biofilm in human health . Dr. Deanna Berman00:42:02

Biofilm are communities of bacteria that form in many places in the body including the lumen of the gut where bacteria transition from a free forming and moving organism into joining a community of bacteria living in a unit called biofilm.

Biofilms are most acknowledged in dentistry as the purpose of flossing is largely to disrupt biofilm. Also, they are of grave concern in medicine as biofilms often form to the detriment of the host on implants, tubes, and catheters.

The significance of biofilm is generally around the concept that bacteria and other organisms (i.e. Yeast) in a biofilm generally evade the immune system and can resist antibiotics.

In this episodes we speak about how biofilm appears in the body, how it is tested,  what conditions are most effected by it, and what to do to treat it.

15 Jul 2020The Estrobolome with Dr. Carrie Jones00:33:13

Estrogen metabolism dysregulation may be linked to symptoms of bloating, constipation, menstrual problems, weight gain, low libido, and also conditions like pms, acne, endometriosis, polycystic ovarian syndrome, and breast cancer.

The link could be explained by the Estrobolome. Which was first mentioned in 2011 by Plottel and Blaser in 2011. as “the aggregate of enteric bacterial genes whose products are capable of metabolizing estrogen." Simply put some bacteria in the gut may be causing increased estrogens to recirculate into our blood instead of being eliminated.  When this happens, it can be not so good for us. In this episode, I welcome Dr. Carrie Jones an international expert in Women's Health and Hormone health to help us unpack this topic.

In this episode we cover:

  • What is the Estrobolome.
  • Symptoms of estrobolome      dysfunction
  • Phase 1 detoxification
  • Phase 2 detoxification
  • Phase 2.5 detoxification
  • Phase 3 detoxification
  • How is bile involved with      estrogen detoxification?
  • What lab tests would identify      detoxification issues
  • Beta-glucuronidase
  • Other hormones affected by gut      health
  • How      to correct imbalances.

About Dr. Carrie Jones:  Dr. Carrie Jones is an internationally recognized speaker, consultant, and educator on the topic of women's health and hormones. She graduated from the National University of Natural Medicine (NUNM), School of Naturopathic Medicine in Portland, Oregon where she also completed her 2- year residency in women's health, hormones and endocrinology. Later she graduated from Grand Canyon University's Master of Public Health program with a goal of doing more international education. 08 Apr 2021Chronic Infections and Myalgic Encephalomyelitis/Chronic Fatigue Syndrome with Dr. Amy Proal00:51:37

Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a debilitating condition with symptoms of persistent fatigue, neuroinflammation,  post-exertional fatigue, joint pain, nerve pain, muscle pain, sensory hypersensitivity, disturbed sleep, headaches, and bowel changes.

It often takes years to get a proper diagnosis and most patients are brushed off as having a psychosomatic illness.  '

Thanks to researchers like our guest Dr. Amy Proal; we know so much more ME/CFS. It turns out there chronic viral and other microbe components that drive a significant part of ME/CFS. These pathogens/pathobionts can set up shop and change DNA, mitochondria, and immune function that contributes to the manifestation of ME/CFS.

In this episode , Dr. Proal dives into how viruses find tissue to hide out in, disrupt mitochondria, alter the vagus nerve and how this understanding guides us how in treating ME/CFS

Dr. Proal shows us that it is not one infectious organism but multiple co-infections overwhelming the immune system pushing into low function.. If we can help the immune system by reducing the overwhelm ; then we can see good progress.

About our guest:

Dr. Proal is a microbiologist who studies the molecular mechanisms by which bacterial, fungal and viral pathogens dysregulate human gene expression, immunity and metabolism. 05 May 2023Episode 87 Anxiety , Trauma, and Addiction Relief with Eva Angert Harren01:05:41

When people are anxious, they often get sweaty hands, increased chest pressure, shortness of breath, feel lightheaded, and they may feel tingling sensations in different parts of their body.

By paying attention and having awareness to these symptoms and , they can start to calm down a little bit and self-regulate.

Yet when stuck in this loop of anxiety , people often turn to learned coping behaviors or addictions to deal with these unwanted sensations.

Learning to deal with this pressure cooker , before it turns into dysfunctional behavior, is something that is the work my guest Eva Angert Harren.

And according the Eva, dealing with the pressure cooker does not require changing your thoughts or trying to control things. Actually its quite different; it's more about the being with the body/somatic experience and learning to release it.

In this episode we go into this and much more.

Eva talked about her unique work helping people with anxiety , ptsd, and also spectrum related disorders; connect with their body and create awareness. This allows for more comfort with sensations and less anxiety and dysregulation.

Eva and I share a common bond as bereaved parents. I was surprised that we jumped into this topic in our conversation. Yet it was relevant. If you have difficulty falling asleep, difficulty staying asleep, early morning awakening, restless or non-restorative sleep, it may be time start addressing this concern.

There are three types of insomnia. 1) Transient Insomnia: which lasts up to 1 week and is usually due to a short-term stressor 2) Short-term Insomnia: which lasts 1-6 months and is usually associated with a more persistent stressor (ie. death or illness of a loved one or environmental factor), and 3) Chronic Insomnia: which lasts more then 6 months and is associated with a wide variety of disorders.

Potential causes and risk factors for insomnia include transient concerns such as stress, illness, travel, environmental disruption. Or medical concerns, such as depression, anxiety, sleep apnea, orthopnea, circadian rhythm disorders, restless leg syndrome, chronic pain, and others.

In addition, insomnia can be caused by medications, drug, alcohol, or stimulant use.

13 May 2021The Science of Psychedelic Medicine with Dr. Erica Zelfand 01:03:00

- The 5 big psychiatric epidemics are: Depression, Pain, Anxiety, Addiction, and Post Traumatic Stress Disorder (PTSD). And, depression is the leading cause of disability worldwide. -

Traditional models are Psychotropic medications and/or counseling. While widely successful this model may fail in certain populations.  t

Treatment failures might be due to not accessing certain neural networks and possibly psychospiritual levels that might be needed for healing.

Because of this Psychedelic Medicine and Psychedelic-Assisted Psychotherapy has emerged.

This traditional medicine now has the science and trials to help understand how to apply in a clinical setting

In this episode , I am joined by Dr. Erica Zelfand who will guide us through  Psychedelic-assisted psychotherapy.  She is a perfect guide as she is the instructor of the course called The Science of Psychedelics

Dr. Zelfand gives us some information and highlights on  MDMA, Psilocybin, and Ketamine.

We learn about which psychedelics impact Brain Derived Neurotrophic Factor, (BDNF), the Default Mode Network (DMN) which involves the medial prefrontal cortex, posterior cingulate cortex/precuneus and angular gyrus. Also those that act on 5HT-2A receptors, and NMDA-receptors.

The effect of these therapies appear to allow the brain to change, access new resources, become more neuroplastic, and gain psycho spiritual insight. Before long psychedelics, will soon be a legalized part of mainstream psychiatry.

About Dr. Zelfand

Erica Zelfand, ND is a licensed family physician specializing in functional medicine and integrative mental health. Blending conventional pharmaceutical medicine with nutrition, herbal medicine, counseling, psycho-spiritual work, and other therapies gleaned from her international travels, Dr. Z’s philosophy of healing aims to nourish the whole person. 28 Oct 2020Disorders of Gut Brain Interactions with Dr. Douglas Drossman and Johannah Ruddy M.Ed.00:48:40

Receiving diagnosis like Irritable Bowel Syndrome (IBS), often comes with a stigma.

Some believe IBS is a diagnosis a doctor uses to describe digestive symptoms when no serious pathology has been found.

Thankfully we are moving a way from this definition.

And now we recognize that conditions like IBS involved scientifically proven imbalances in both the gut and brain and must bet treated as such.

The progress made can largely be attributed to the work of The Rome Foundation and the guests of this episode Dr. Douglas Drossman and Johannah Ruddy, M.Ed.

Dr. Drossman and Johannah are the authors of the new book, Gut Feelings, Disorders of The Gut-Brain Interaction and are pioneering the efforts for patients and doctors to unite to have better approaches to treatment and care of these digestive disorders.

Disorders like IBS have now become part of a larger group of digestive disorders now called Disorders of Gut Brain Interactions.”.

In this episode we discuss:

● From a “diagnosis of exclusion” to where we are today.

● How the Gut and Brain Interact. What science tells us.

● Doctor-Patient teamwork

● Modalities of treatment for Brain-Gut Interactions

26 Apr 2024Episode 97: IBD Nutrition with Kim Beall, Pharm.D01:11:36

Nutritional therapy plays a significant role in managing inflammatory bowel disease (IBD), but it is often overlooked in traditional medical care.

Exclusive enteral nutrition is effective in inducing remission, while therapeutic diets like the IBD-Anti-Inflammatory Diet and the Specific Carbohydrate Diet have shown positive results.

Monitoring nutrient deficiencies and biometric measurements like grip strength are essential for optimal management of IBD.

To help develop a framework for this topic I welcomed on the Kim Beall, Pharm.D., who is the Cofounder and Managing Director of Nutritional Therapy for IBD, http://www.nutritionaltherapyforibd.org a non-profit dedicated to establishing evidence-based nutritional therapy as a foundational part of IBD care to improve the well-being and health outcomes of those living with Crohn’s disease and ulcerative colitis. Kim and I met related to a mutual colleague the late Trina Seligman, ND who had a courageous life long battle with Crohns/Colitis . We share a strong bond with Trina.

Here are some of the timestamps of the conversations. 00:00 - 🎙️ Introduction to the One Thing Podcast and the guest, Kim Beall, Pharm.D, who is a pharmacist and the co-founder of Nutritional Therapy for IBD, a nonprofit dedicated to incorporating evidence-based nutrition into IBD care. 02:45 - 💔 Discussion about the passing of Dr. Trina Seligman, a mentor and advocate for IBD patients, and her impact on the IBD community. 05:23 - 🏥 The importance of nutrition in managing IBD and the role of exclusive enteral nutrition (EEN) in inducing remission and mucosal healing. 08:32 - 💡 Dr. Beall's personal journey with IBD and her son's diagnosis with Crohn's disease, which led her to advocate for nutrition as a foundational part of IBD care. 12:15 - 🤝 The need for earlier intervention and education on nutrition for IBD patients and the importance of collaboration between patients and healthcare providers. 16:10 - 🤔 The confusion surrounding dietary recommendations for IBD and the lack of official guidelines, leading to the need for ongoing research and evidence-based approaches. 19:05 - ⚖️ The importance of avoiding blame and shame when discussing the role of diet in IBD and the complexity of factors contributing to the development of the disease. 21:20 - 📊 The significance of biometric evaluations, such as grip strength and body mass, in assessing nutritional status and overall health in IBD patients. 24:20 - 🌱 The current evidence for routine nutrient monitoring in IBD patients, including the importance of micronutrients like zinc, magnesium, and vitamins. 27:10 - 🍽️ The different dietary strategies and therapeutic diets for IBD, including exclusive enteral nutrition, the anti-inflammatory diet, autoimmune protocol, and specific carbohydrate diet. 32:00 - 🔍 Ongoing research on the microbiome and its potential role in IBD management, along with the need for personalized nutrition approaches based on an individual's microbiome and metabolic profile. 35:05 - 💪 The potential benefits of partial enteral nutrition and its underutilization in IBD care, including its role in maintaining remission and reducing surgical recurrence. 38:40 - 🌟 The importance of raising awareness about the efficacy of nutritional therapy in IBD and advocating for insurance coverage of nutrition-related treatments. 20 Feb 2022Biological Factors In Long-Covid with Dr. Amy Proal01:15:04

According to the CDC, Post-acute sequelae of COVID-19 (PASC), also known as “Long COVID,” are long-term symptoms that might be experienced weeks to months after primary infection with SARS-CoV-2, the virus that causes COVID-19.  Post-viral syndromes have been around for ages but nothing to the scale we are seeing now.

The symptoms of PASC are extensive with over 200 symptoms reported ranging from general systematic fatigue , sensorimotor, neurologic , digestive, and cardiovascular.

Like any new  problem it helps to have a framework to think through problems and its especially helpful if other models are are available from other conditions that have been treated. We desperately need a framework to help with Long-Covid.

Our Guest, Dr. Amy Proal,   as been looking at post-viral syndromes for many years especially in her work with Myalgic Encephalitis and Chronic Fatigue Syndrome

Dr. Amy Proal, Phd is Microbiologist with expertise on the molecular mechanisms by which pathogens modulate human gene expression, metabolism and immunity.

She recently used this knowledge and experience to author a highly cited paper with her co-researcher Dr. Michael VanElzakker called Long COVID or Post-acute Sequelae of COVID-19 (PASC): An Overview of Biological Factors That May Contribute to Persistent Symptoms

This outstanding paper gives a framework to think through the Long Covid puzzle and start treating it.  In this episode we dive into all aspects of the Biological Factors of Long Covid PASC and discuss how this research will give us a roadmap to help people feel better.

07 Dec 2022Episode 80. Heart Rate Variability with Marco Altini, PhD 00:43:51

Heart Rate Variability (HRV)  is a key metric in the overall homeostatic balance of the body.
HRV is simply a measure of the variation in time between each heartbeat. This variation is controlled by a primitive part of the nervous system called the autonomic nervous system (ANS).  It is largely an indicator of how well the body is handling the Allostatic Load (stress load).

I didn't truly grasp HRV until I was exposed to the work of Dr. Marco Altini; so I decided to ask him to come on and help educate my listeners.

In this episode we talk about:

  • Overview of Heart Rate Variability (HRV
  • The Autonomic Nervous Systems as an informant of health and homeostasis.
  • Daily HRV fluctuation vs. Baseline changes over time. Factors, value of data, and implications
  • Different tools to measure HRV
  • Can HRV fluctuations predict chronic or acute flareups (ie  Autoimmune Disease Flare?)
  • The impact of  breathing practices  and lifestyle changes on HRV?

About Dr. Altini

Marco holds a PhD cum laude in applied machine learning, a M.Sc. cum laude in computer science engineering, and a M.Sc. cum laude in human movement sciences and high-performance coaching. He has published more than 50 papers and patents at the intersection between physiology, health, technology and human performance. He is the co-founder of HRV4Training, advisor at Oura, guest lecturer at VU Amsterdam, and editor of the Wearables department of IEEE Pervasive Computing. He loves running.

17 Jun 2020Plasmalogen Deficiency And Dementia with Dr. Dayan Goodenowe00:54:00

Cognitive decline is one of the most concerning aspects of aging and is a major cause of morbidity and mortality in the US.  As of 2014 the CDC stated that of those older than 65 years of age, there was an estimated 5.0 million people suffering with dementia. This number is projected to be nearly 14 million by 2060.There are laundry lists of ideas to prevent cognitive decline, however, few really address the root cause.  My guest today Dr. Dayan Goodenowe a neuroscientist and inventor with Prodrome Sciences. I was introduced to Dr. Goodenowe by a mutual colleague and he really struck a chord with me when he told me about plasmalogens. As we discussed dementia and its root causes, he shared how these unique chemicals of the membrane of our brain called plasmalogens can become deficient and that they may be one of the key links to understanding neurodegeneration and cognitive decline. In todays episodes we discuss plasmalogens, what they are, how they are made, how they help with our heart, brain, immune system, and nervous tissue, and how they can be measured and potentially restored.

We also explore

· How Plasmalogens help people with high risk for Alzheimer's Disease who have the APOe4 gene

· How Plasmalogens inter-relate with other aspects of Dementia such as Amyloid Beta

· Specific plasmalogens that might help with multiple sclerosis and autism

About our guest:

Dr. Goodenowe’s research into the biochemical mechanisms of disease started in 1990. His curiosity about the biochemistry of life is as insatiable today as it was 30 years ago. In those 30 years, Dr. Goodenowe invented and developed advanced diagnostic and bioinformatic technologies, designed and manufactured novel and natural biochemical precursors, and identified biochemical prodromes of numerous diseases including Alzheimer’s disease and dementia, Parkinson’s disease, multiple sclerosis, stroke, autism, amyotrophic lateral sclerosis (ALS), multiple system atrophy, schizophrenia, bipolar disorder, depression, and cancers of the colon, pancreas, ovary, breast, lung, kidney, esophagus, liver, stomach, and endometria. At Prodrome Sciences, Dr. Goodenowe is going beyond the diagnosis of disease and has set his sights on figuring out how long the human body can maintain the physical and biological functions of life.

03 May 2022Obesity and the Hypothalamus with Stephan Guyenet, PhD01:03:52

If weight loss were as easy as calories in vs. calories out then, it wouldn't even be something we talk about. It turns out the brain regulatory pathways are a core reason why weight loss is so difficult.

Sitting in the brain is a key regulatory region called the Hypothalamus. About the size of an almond, is located below the thalamus and above your pituitary gland., the hypothalamus is involved with many endocrine functions; including fat loss or fat gain.  If we were understand the function of the hypothalamus in fat gain and loss, we can better understand how to direct approaches to obesity and weight loss resistance.

There is no better person to help us understand this then Stephan Guyenet ,PhD the author of  The Hungry Brain: Outsmarting The Instincts That Make Us Overeat.

Dr. Guyenet is a Neuroscientist and obesity researcher who has become a leader in understanding the brains role in obesity.  He also is the founder of Red Pen Reviews a website that distills and grades health related books based on number of factors.

Listen in as we take the journey into the world of obesity including starvation responses, leptin resistance, hypothalamic inflammation, high fat diets, and the promise of glp-1 agonists.

30 May 2019Mental Health and Neurodiversity with Dr. Teray Garchitorena Kunishi00:49:03

If you are alive, it is safe to say you know someone diagnosed with a mental health disorder.  You also likely know how important it is to receive an accurate diagnosis and limit the stigma attached to a mental health diagnosis.  In this episode Dr. Teray Garchitorena Kunishi an experienced expert in mental health and a Naturopathic Physician in San Francisco speaks about mental health and the concept of neurodiversity ; where mental health issues are seen as a variant of what is considered as a norm rather than as as firmly abnormal.  This has become a particularly important concept in Autism Spectrum Disorders,  Bipolar Disease, and Schizophrenia.  It is not without controversy and we look at both aspects of neurodiversity approaches in this episode.  We also speak about societies where conditions like Schizophrenia are treated like a gift and people who deal with this gift are supported and nurtured rather then ostracized.   This is a very deep and meaningful episode and one that I am sure will be thought provoking. Please follow Dr. Teray on her website where you also find her course on the Art, Science, and practice of Happiness. It was an honor to interview Dr. Teray and I hope you enjoy  the episode as much as I did. Subscribe to our episode in your favorite podcast player including itunes and stitcher

29 Jan 2020Happiness and Your Inner Mammal with Dr. Loretta Breuning00:48:23

In this episode we learn about happiness from an inner mammal perspective. Including questions like;  how do we generate  dopamine, serotonin, oxytoxin, and cortisol? and how do we work with our mammalian brain (the limbic system) and balance it with our developing/intellectual brain (the cortex)? It is quite fascinating to learn that we can be happier if we don't entirely squash the part of our brain that is hardwired for survival moves .  I invite esteemed author Loretta Breuning, PhD on this episode to take us through her teachings on this subject.  I first learned about Dr. Breuning after I experienced a tragic loss in my life and my friend and colleague and previous podcast guest,  Dr. John Neustadt, told me to check out one of her books; Habits of a Happy Brain and after reading it  I was enriched with additional tools to navigate my loss.   More about Dr. Breuning....

Dr. Loretta Breuning is Founder of the Inner Mammal Institute and author of Habits of a Happy Brain: Retrain your brain to boost your serotonin, dopamine, oxytocin and endorphin levels. As Professor of Management at California State University, and as a mom, she was disappointed by prevailing views of human motivation. She searched for better answers and found studies of the brain chemistry we share with earlier mammals. Suddenly, everything made sense! She began creating resources to help people manage their inner mammal. The Inner Mammal Institute offers videos, books, podcasts, multimedia, and a training program, to help you make peace with your inner mammal. InnerMammalInstitute.org Her latest book is Tame Your Anxiety: Rewiring Your Brain for Happiness.

26 Jan 2022NAD with Dr. Chris Meletis00:55:30

Slowing aging , preserving memory, and preserving energy are big topics of concern in medicine and health care. Over the past 10 years research and development has show a special interest in how to modify Sirtuin-1 as a protein for longevity and slowing the disease process.  A cofactor of Sirtuin-1 named Nicotinamide Adenine dinucleotide  (NAD) is of particular interest.    NAD seems to be depleted as we age and thus turning down Sirtuin-1 activity.  This means accelerated aging.   The study of how to modify NAD is blossoming much thanks to the work of David Sinclair, Phd. And, supplements, dietary modifications , and lifestyle moves are being studies and marketed to modify this pathway. I have been hesitant to jump into this world due the hype; so I brought back an expert in this space Dr. Chris Meletis to help translate the science for practical application. We discuss the different forms of NAD precursors such as Nicotinamide Riboside (NR) and Nicotinamide mononucleotide (NMN) as well discuss things like fasting, ketogenic diets, and exercise.

About Dr. Meletis:

Dr. Chris D. Meletis is an educator, international author and lecturer. His personal mission is “Changing World’s Health One Person at a Time.” He believes that when people become educated about their body, that is the moment when change begins. He has authored over a dozen books and in excess of 200 national scientific articles in such journals and magazines as Natural Health, Alternative and Complementary Therapies, Townsend Letter for Doctors and Patients, Life Extension, Natural Pharmacy and the Journal of Restorative Medicine

Dr. Meletis served as Dean of Naturopathic Medicine and Chief Medical Officer for 7 years at NUNM, the oldest naturopathic medical school in North America. He has received numerous awards including, Physician of the Year by the American Association of Naturopathic Physicians; Excellence Award for his  work in treating and advocating for the medically underserved; and most recently a NUNM Hall of Fall award at the State Association’s annual banquet.

His passion for helping the underprivileged drove him to spearhead the creation of 16 free natural medicine healthcare clinics in the Portland metropolitan area of Oregon.

He is a medical consultant for Tru Niagen among other companies and is considered an expert in mitochondrial health and organic acid testing.

22 Mar 2019Post Meal Glucose Response With Richard Sprague episode #200:29:33

What is the best diet? This question remains the million (probably billion) dollar question in health, nutrition and medicine. The winner will likely be the healthy diet is one that prevents the most disease and weight gain. Now there seems to be an approach that has caught the attention of patients and providers internationally and that looking at individualized post-meal glucose response as a guide to one’s individualized diet. And guess what? The researchers say that the post meal glucose response is largely dependent as to what kinds of bugs are growing in our gut. Listen in on today’s episode as we explore the individualized diet and post meal glucose response.

Some back ground information.

Post meal glucose response is defined classically as the rises in blood glucose measured in ng/dl. Why does it matter? As a biomarker it predicts one’s ability to regulate glucose. Glucose regulated means less inflammation in the body, less risk for heart disease, less risk for diabetes, less risk for kidney disease, less risk for eye disease, and less risk for obesity…you get the picture.

Typically in a non-diabetic a fasting blood glucose will range from 70 ng/dl to 99 ng/dl and after eating a carbohydrate containing meal the blood glucose will rise between 100 ng/dl-140ng/dl. This can be influenced by protein and fat content in the food source however it is largely driven by carbohydrate intake. Until researchers started looking to the gut microbiome, the large dietary agencies that be would look at glucose content of foods and make predictions as to how these foods would spike blood sugar. This list is called the glycemic index of foods. Then later they looked at how combinations of foods might spike one’s glucose and this is called glycemic load. Now a group out of the Weizmann Institute developed a test and algorithm of post meal glucose response based on DNA sequencing of the bugs that are growing in human guts. By monitoring glucose response in human subjects, they were able to predict that one’s own signature gut microbiome, can largely influence ones glucose response to food.

26 Jun 2020WIALD: Stress and the Intestinal Barrier00:17:49

The brain and gut are intrinsically linked . The nervous system has been explored as being the link specifically the via the vagus nerve. Yet, more information is being revealed about how neurotransmitters and stress hormones may interface with the gut microbiome and effect gut function such as intestinal barrier function.

In this episode I review a presentation I attended called Gates of Wrath: Role of intestinal epithelial cells in Stress response by Dr. Jacob Allen as part of the May, 2020 Microbiome Summit.

We dig into the research he is doing about how stress effects the microbiome and how it effects the intestinal lining ultimately.  Fascinating how the brain and gut interface to act favorably or unfavorably.

20 Nov 2022Episode 79. Gut Directed Hypnotherapy Linda Stelluti00:43:14

A game changing moment in my career happened when I learned that mind/body therapies  in Irritable Bowel Syndrome have a number needed to treat (NNT) of 4.  Meaning 1 out of 4 people, experience statistically significant  benefit when using mind body therapies to address their IBS.

1 out of 4 might not sound much to you but realize that even the best drugs earn a Number Needed To Treat of about 13.

Plus mind/body therapies are often empowering, have no side-effects, and are very affordable.

So I started learning more about Gut-directed clinical hypnotherapy and then encouraged my patients to get on board.

Many patients continue to benefit.

In this episode, I reached out to an expert in the field of Gut-directed hypnotherapy , Linda Stelluti , CHt to help demystify hypnotherapy .

You'll learn that it is not some circus act and it taps into the deep underlying blocks to healing and the subconscious mind . It helps heals limiting thought patterns, anxiety, and stress patterns that perpetuate chronic illness.

We had a delightful conversation about what its like to have hypnotherapy performed, how it helps the brain/gut axis, and much more.

Enjoy and please make sure to see Linda's programs she is running for Gut-Directed hypnotherapy. See below.

21 Oct 2020A Fat Loss Lifestyle with Alex Leaf, MS, CISSN00:45:35

There is an obsession in this world with weight loss.

The search term “weight loss” outpaces the search term “fat loss” by leaps and bounds.

In fact, on May 3, 2020, “weight loss” reached the most popular search term status on google in The United States. Reaching a popularity score of 100/100. On the same day the term “fat loss” only had a popularity score of 4 out of 100.

It may seem trivial and semantic to some; but to me that says a lot about the mentality and understanding of metabolism, body composition, and largely why the dieting culture frequently fails.

On the subject of metabolism, fat percentage and fat quality are what matters in the grand scheme of health, fitness, and longevity. Not weight.   However, most “diets” are designed to set you on the path of reaching a specific weight that somehow reflects health.  These weight focused diets often overlook the impact of their weight loss program on your emotions, muscle mass, bone health, brain health, and hormone health.

The topic of weight and fat can be very triggering to discuss. Especially for those of us who have had ongoing struggles with metabolism and have reached a point of frustration and even mental health concerns around weight.

When approaching this topic, I could think of no one better to bring on Alex Leaf, MS, CISSN one of the up and coming stars in Nutrition Research and one of the pioneers of the Fat Loss Blueprint.  After learning about his approach on this topic, I felt that it was one of the most responsible and integrative approaches to fat loss I have come across.

In this episode we speak about:

1. Fat loss factors such as ghrelin, orexin, AMPK, protein muscle synthesis, cortisol, and satiety

2. Resistance training and its effect on metabolism

3. Discuss the impact of sleep and circadian rhythm disruption

4. His take on macronutrients such as protein

5. His take on calories

And much more.

One of the things about Alex is that he is not hype. He’s grounded, full of science, and his approach to fat loss is very thoughtful and careful.

13 Nov 2024Episode 109. Type 2 Diabetes Reversal with Sami Inkinen00:49:12

In this Episode I welcome on Sami Inkinen CEO of Virta Health to discuss Virta Health's approach to Type 2 Diabetes reversal'

Virta Health utilizes a continuous remote care intervention (CCI) that incorporates individualized carbohydrate restriction, health coaching, and telemedicine to manage and reverse Type 2 Diabetes

We cover topics such as:

Impact of individualized carbohydrate restriction:

The role of the patient-provider relationship:

Addressing quality of food

The Virta change model:

GLP-1 Receptor agonists as a bridge:

Sustainable solutions require addressing nutrition:

Research Findings of Virta's approach.

Long-Term Effects of CCI: A 2-year non-randomized clinical trial published in Frontiers in Endocrinology showed significant and sustained improvements in multiple clinical markers of diabetes and cardiometabolic health in the CCI group compared to the UC (usual care) group . These included reductions in HbA1c, fasting glucose and insulin, weight, blood pressure, triglycerides, and liver alanine transaminase . The CCI group also saw higher rates of diabetes reversal and remission .

GLP-1 Deprescription: A recent study published in Diabetes Therapy explored the impact of discontinuing GLP-1 agonists in patients who had successfully integrated Carb-Restriction Nutrient Therapy (CRNT) supported by Virta's telemedicine model . The study found that patients did not experience weight regain after stopping GLP-1 medications, and while HbA1c levels increased slightly, they generally remained below the diagnostic threshold for diabetes

04 Apr 2024Episode 96: Managing Hot Flashes with Hypnotherapy with Claire Hall of Mindset Health00:50:33

Claire Hall, a researcher at Mindset Health, joins us on episode 96 discusses menopause and the effectiveness of hypnotherapy in treating hot flashes.

    Summary of episode:

    • Hypnotherapy is an effective non-drug alternative for managing hot flashes, as it has been shown to reduce their frequency and severity. It has shown in trials that hypnotherapy can reduce Hot flash severity by up to 70% . It also seems to be helpful in reducing anxiety and depression and sleep disturbances related to menopause.
    • Mindset Health develops hypnotherapy-based apps to deliver evidence-based protocols for various conditions, including irritable bowel syndrome (Nerva app), menopausal hot flashes (Evia app), and smoking cessation (Finito app).
    • Menopause is often viewed negatively in society, but it is a natural life transition that involves hormonal changes leading to symptoms such as hot flashes.
    • Hypnotherapy works by guiding patients into a deeply relaxed state and then providing therapeutic suggestions that reinforce coolness and control over body temperature.
    • A study involving 10,000 Evia users found that 70% experienced a clinically significant improvement in vasomotor symptoms.
    • Mindset Health prioritizes publishing both clinical trial data and real-world user data to provide a comprehensive understanding of the effectiveness of their digital therapeutics.
    • Hypnotherapy may be more effective than traditional treatments for hot flashes. A study published in the journal Menopause found that women who used hypnotherapy to treat their hot flashes had a 70% reduction in symptoms, compared to a 20% reduction in women who used hormone therapy.
    • Hypnotherapy may help to reduce anxiety and depression during menopause. A study published in the journal Maturitas found that women who used hypnotherapy to treat their hot flashes experienced a significant reduction in anxiety and depression symptoms.
    • Hypnotherapy may help to improve sleep quality during menopause. A study published in the journal Sleep found that women who used hypnotherapy to treat their hot flashes experienced a significant improvement in their sleep quality.
    • Hypnotherapy is a safe and effective treatment for hot flashes that has no side effects. Unlike hormone therapy, hypnotherapy has no known side effects, making it a safe and effective option for women who are looking to relieve their hot flashes.

08 May 2019Postpartum depression episode with Sunita Iyer, ND, LM00:29:06

Having a child is supposed to be great joy for a parent. However  up to 11% of women and 2% of men, experience postpartum depression; a unique mental health condition that is often downplayed, overlooked, and under-treated. It has significant implications on child and parent alike.  My colleague Dr. Iyer of Eastside Natural Medicine of Kenmore, Washington joined me in this most thoughtful discussion of this topic. Dr. Iyer has been someone I have respected for many years. She is compassionate, clear thinking, brilliant, and keeps it real. Through her work as a  licensed midwife, naturopathic physician and University of Washington school of nursing instructor,   she has developed a special interest in grief.  She feels  the perinatal period is a type of grief process that needs proactive support. It's quite fascinating to listen to her. It’s no doubt why students, patients, and colleagues love Dr. Iyer as she is someone you can really trust. I hope you enjoy it and please share it with new parents or new parents to be as this episode is important to hear  .Find Dr. Iyer and her bio at https://eastsidenaturalmedicine.com/

13 Jul 2022Follicle Chronicles; A deep dive into Alopecia with Jill Grunewald00:59:32

    Alopecia

The human scalp contains approximately 100,000 to 150,000 hair follicles; its easy to take hair for granted until you start to lose it. Hair loss is a complex area of health care as there are many different types including Lymphocytic driven hair loss, Neutrophilic driven hair loss, mixed forms, non-scarring hair-loss, scarring hair loss, patterned hair loss, focal hair loss, and the most common diffuse hair loss. You can see this isn't so straightforward.

According to our guest Jill Grunewald, the creator of The Reversing Alopecia Roadmap ; they all share similar underpinnings and imbalances . Jill believes in addressing the gut, immune system, and hormones to reverse alopecia and she shares some of these concepts with us in our discussions.

She also shares some success stories that involved the immune system, her favorite hair loss supplement, and some comments on when to use some latest hair loss technologies like microderm rollers and red light therapy helmets.

    About Jill:

Jill McLaughlin Grunewald is a holistic nutrition and hormone coach, Functional Medicine Certified Health Coach, and founder of Healthful Elements LLC. She specializes in alopecia, Hashimoto’s, adrenal dysfunction, women’s health, and blood sugar imbalances and is the co-author of the #1 bestselling Essential Thyroid Cookbook: Over 100 Nourishing Recipes for Thriving with Hypothyroidism and Hashimoto’s.

For nearly 17 years, Jill has successfully guided her clients and students to health and vitality—she has helped numerous with Hashimoto’s successfully manage their condition and has helped many with alopecia reverse their devastating hair loss with her first-of-its kind, signature program for the hair loss community called The Reversing Alopecia Roadmap.

Jill’s own Hashimoto's has been successfully managed since late 2008 without the use of thyroid drugs. She had suffered from mild alopecia, off and on, starting at the age of 13 and then became devastatingly half bald with the alopecia ophiasis pattern in her early 40s. She refused to accept her fate and after digging into all the root causes of alopecia, grew all of her hair back. She’s suffered some relatively mild setbacks in the last two years, but all of those spots have filled in or are currently in a growth phase.

Jill has written for various publications, blogs, and online magazines, including Huffington Post, MindBodyGreen, Dr. Frank Lipman’s BeWell blog, Dr. Susan Blum’s blog, and Experience Life magazine, has contributed to articles in Self and Shape magazines, and was quoted in

Oprah’s O Magazine about thyroid nutrition.

18 Mar 2020Mast Cell Activation Syndrome with Dr. Jessica Pizano00:41:14

Mast Cell Activation Syndrome encompasses a consortium of imbalances in the body that results in suffering from sensitivities to foods, chemicals, and environmental triggers. These sensitivities seem to be mediated by over release of histamine and other inflammatory mediators by mast cells. The consequences are vast including digestive, neurologic, immune, dermatologic, endocrine , musculoskelatal, and psychological impact. This episode brings in an expert in this area Dr. Jessica Pizano to help us understand this complex scenario that often gets missed clinically or misdiagnosed leaving people suffering with out answers.

    About our guest.

Dr. Jessica Pizano graduated with honors from the University of Bridgeport's functional nutrition master's program. She continued her education at Maryland University of Integrative Health (MUIH) where she graduated with a Doctor of Clinical Nutrition. During her years in academia, she became very interested in working with those with rare diseases. The mantra for rare disease sufferers around the world is "When you hear hoof beats, think zebra's, not horses. This is, of course, the opposite of what has been taught in medical schools.

Dr. Pizano is also the owner of Mast Cell Advanced Diagnostics where she sees patients with mast cell activation disorders and other rare diseases. The practice provides patients with a comprehensive approach to rare disorders incorporating nutrition, nutritional genomics, metabolomics, neural retraining, and medical management. Her goal is to provide a comprehensive care solution for patients helping bridge the gap in care that these patients typically face. Dr. Pizano works very closely with some of the top doctors treating mast cell activation syndrome, Ehlers-Danlos Syndrome, immunodeficiency disorders, familial alpha hypertryptasemia, and autoimmune small fiber polyneuropathy. In addition to seeing patients from around the United States, Canada and the United Kingdom, she also frequently provides support to physicians working with these rare diseases via practitioner education and consults. In addition to the conditions listed above, Dr. Pizano has worked with multiple chemical sensitivities, Lyme disease, biotoxin illness, mood disorders, autism, autoimmune diseases, rare genetic deletions, mitochondrial disorders and other diseases.

Along with her business partner Dr. Chrissie Williamson, Dr. Pizano is the co-owner of SNPed, a webinar-based series to educate practitioners in nutritional genomics and systems medicine. She has developed course work at the master's level at MUIH in nutritional genomic counseling, has published a series of peer-reviewed journal articles on the microbiome and contributed two chapters on nutritional genomics in a graduate level nutrition textbook.
