A 43 year old male with a past medical history of hypertension, diabetes, and hyperlipidemia presents to the emergency department with dizziness that started 14 hours ago. He complains of nausea and vomiting. He denies recent trauma, fever, headache, visual disturbance, or weakness. He says he requires the assistance of his wife to walk because he feels unsteady. His symptoms are relieved when he sits down at rest, but they never fully resolve. When you speak to him, his symptoms looks a lot like vertigo. He tells you that his dad had vertigo and he would lay down and turn his head in different directions. But, you know this could be more serious than a simple case of BPPV. With the holiday season in full swing, it also comes with more people getting sick and getting sinus infections. The specific sinus infection that we will discuss is rhinosinusitis, which is an inflammation of the lining of the sinuses and the nasal cavity. It can be caused by a variety of factors, including allergies, infections and structural abnormalities. We will go over the four different types of rhinosinusitis: acute, subacute, chronic and recurrent acute. Next, is the symptoms your patient will present with such as facial pain, nasal congestion and loss of smell. Today, we're going to discuss anticholinergic toxicity. This is an important one as there are a ton of medications out there that have anticholinergic properties. We have a 75 year old female with a past medical history of Parkinsons, COPD, hypertension, and hyperlipidemia. She presents from a skilled nursing facility with altered mental status. Per the SNF, the patients baseline is alert and oriented x4 but does need assistance with ADLs. This morning the patient had intermittent agitation and was disoriented to self, place, and time. Patient had a temp of 102.3, HR 120, BP 130/89, RR 16, and 98% O@ RA. Of note, the patient has not urinated in over 12 hours. Per staff, she was seen yesterday for a low back musculoskeletal strain and was given flexeril and ibuprofen. Your next patient is a 21 year old female who was brought in to urgent care by her husband with the chief complaint of fatigue and vomiting. Before you go in to see the patient, you notice she has no past medical history and her vital signs are normal, aside from a heart rate of 122 BPM. The patient thought she was coming down with something, because she was feeling very tired and lost her appetite. She developed vomiting this am. They blame the sushi they ate. During the physical, you notice she looks very ill. She's laying on her side and holding her belly uncomfortably. Her skin looks pale and appears to be dehydrated. He has a two week history of a viral upper respiratory symptoms, including a low grade fever, cough, nasal congestion, rhinorrhea, sore throat, decreased oral intake - followed by a persistent nose bleed. Today, we're going to discuss the last topic of the toxicology series and that's going to be serotonin syndrome. We have a 32 year old male with a past medical history of depression who presents with an intentional overdose of Zoloft. Per the family, the patient was found in his bedroom acting abnormal, and an empty bottle of Zoloft was found on the floor. His last known normal was when he got home at 3pm, which was about 4 hours prior to arriving to the ER. Vitals: 103.9 F, 130 HR, 110/78, and 96% O2 RA. On exam he was agitated and irritable. He was flushed and sweaty with dilated pupils. He also had this inducible ankle clonus along with hyperreflexia of the lower extremities. Serotonin Syndrome can be fatal, therefore you never want to miss this, and this should always be on the differential when a patient presents with altered mental status, agitation, and/or with some neuromuscular abnormality. Knees causing trouble again? This time, it is not gout, but an imposter known as pseudogout. Now what is pseudogout, you ask? Think of it as gout’s mischievous twin. Like gout, it triggers sudden knee pain and inflammation, but instead of uric acid crystals, it is calcium pyrophosphate dihydrate (CPPD) crystals in your joints. The symptoms are similar to gout, swelling, redness and pain, often in the knees, ankles or wrists. To diagnose pseudogout, you’ll do some blood work to check for uric acid and X-rays and joint fluid analysis will help identify the culprit. A 43 year old male presents to the emergency department with right wrist pain after falling on his outstretched right hand. You obtain an x-ray of the wrist, which demonstrates no acute fracture or dislocation. The patient is neurovascularly intact, you decided to discharge the patient home with instructions to treat with: ice, rest, and some naproxen. 14 months later, you're being sued by this very same patient, apparently, after you saw the patient their wrist pain never got better and in fact it persisted for over 8 months before they were finally seen again at another facility. They were then referred on to hand surgery where the diagnosis of a Scaphoid Fracture with Osteonecrosis due to non-union was made. The patient is suing you because they say you missed the fracture and now they have chronic wrist pain and limited functionality of the hand, so they're unable to make a living as a plumber, and their quality of life has dropped significantly. This is a fictional story, but very very common. Today, we'd like to introduce Dr. Niket Sonpal as the new host of the Medgeeks podcast! Who is Dr. Sonpal? Well, besides being an internist and gastroenterologist, he's an assistant professor at Touro College of Medicine in Harlem New York. In addition to that, he's had the fortunate opportunity to be the co-author of "Master the Boards USMLE Step 2 and Step 3" . He's also faculty at Medquest Board review and was faculty at Kaplan for their medical test prep. He's also done a variety of lectures with Medgeeks as well. All these things combined has led to a 10 year history of teaching students medicine, the boards, and of course the grand round circuit. In this latest podcast episode, we will talk about radiation. No, not the radiation that gives you superpowers like in the movies. But, radiation proctitis, which is inflammation of the rectum caused by exposure to radiation therapy. Typically for pelvic cancers like prostate, cervical and rectal cancers. There are two types of radiation proctitis, acute and chronic. Acute develops up to 3 months after treatment and chronic develops more than 3 months after. To treat acute radiation proctitis, it is often managed with dietary changes and medications like stool softeners and antidiarrheal drugs. Treatment for chronic proctitis will vary depending on severity and may involve medications, lifestyle modifications, procedures like balloon dilation or surgery. In this episode of the Liver Series, Dr. Niket Sonpal discusses drug-induced liver injury (DILI), a common and potentially serious side effect of many medications. DILI can occur in people of all ages, but is more common in older adults and people with pre-existing liver disease. DILI can be caused by a wide variety of medications, including over-the-counter medications, prescription drugs, and herbal supplements. Some of the most common medications that cause DILI include Tylenol, NSAIDS such as Advil and Aleve. DILI is usually treated by stopping the medication that is causing the injury. About one year ago I had an 80 year old patient who kept complaining of nausea and abdominal pain after having a cholecystectomy. The surgeons kept telling her this was normal following this type of procedure and prescribed Zofran. Well, she eventually came into my ER and she didn't look to good. She became septic. In this week’s episode of Bugs and Drugs, we will dive into two worlds of food borne bacteria. One can be found in reheated rice, known as Bacillus cereus. The other is found in raw or undercooked pork, known as Yersinia enterocolitica. B. cereus can present with vomiting syndrome or diarrheal syndrome. You won’t need to do any testing unless there is an outbreak and antibiotics are not needed. Your patient will need some IV fluids and letting the bacteria go through their system. Patients that come in with Yersinia enterocolitica, will present with diarrhea and abdominal pain. In some cases, your patient can experience reactive arthritis and erythema nodosum, affecting joints and skin.
