A 43 year old male with a past medical history of hypertension, diabetes, and hyperlipidemia presents to the emergency department with dizziness that started 14 hours ago.

He complains of nausea and vomiting.

He denies recent trauma, fever, headache, visual disturbance, or weakness. He says he requires the assistance of his wife to walk because he feels unsteady. 

His symptoms are relieved when he sits down at rest, but they never fully resolve. When you speak to him, his symptoms looks a lot like vertigo. 

He tells you that his dad had vertigo and he would lay down and turn his head in different directions. But, you know this could be more serious than a simple case of BPPV.

So, how are you going to approach this patient, to differentiate what's causing his symptoms?

With the holiday season in full swing, it also comes with more people getting sick and getting sinus infections. The specific sinus infection that we will discuss is rhinosinusitis, which is an inflammation of the lining of the sinuses and the nasal cavity. It can be caused by a variety of factors, including allergies, infections and structural abnormalities.

We will go over the four different types of rhinosinusitis: acute, subacute, chronic and recurrent acute. Next, is the symptoms your patient will present with such as facial pain, nasal congestion and loss of smell. The different treatments that you can offer are over-the-counter medications, antibiotics and nasal corticosteroids.

Today, we're going to discuss anticholinergic toxicity.

This is an important one as there are a ton of medications out there that have anticholinergic properties. 

We have a 75 year old female with a past medical history of Parkinsons, COPD, hypertension, and hyperlipidemia. 

She presents from a skilled nursing facility with altered mental status. Per the SNF, the patients baseline is alert and oriented x4 but does need assistance with ADLs. 

This morning the patient had intermittent agitation and was disoriented to self, place, and time. 

Patient had a temp of 102.3, HR 120, BP 130/89, RR 16, and 98% O@ RA. 

Of note, the patient has not urinated in over 12 hours. Per staff, she was seen yesterday for a low back musculoskeletal strain and was given flexeril and ibuprofen. 

Let's finish walking through this case together and discuss anticholinergic toxicity.

Foreign body or large food bolus ingestion occurs primarily in children and in the mentally impaired older adult. Food (meat) bolus impaction above a pre-existing esophageal stricture is the most common cause in adults. The majority of foreign bodies in children are coins. Today I want to discuss the emotional burden of caring too much for your patients. You can't let your mistakes get to you - we all make them! You have to find the balance where you aren't completely emotionally invested while making sure your patients aren't just another number. 

I also want to talk about caring for patients who don't seem to care about themselves. Going into medicine is tough - but if you really want to do it, you will. If you don't, you wont. It's all about your state of mind.

Your next patient is a 21 year old female who was brought in to urgent care by her husband with the chief complaint of fatigue and vomiting.

Before you go in to see the patient, you notice she has no past medical history and her vital signs are normal, aside from a heart rate of 122 BPM.

The patient thought she was coming down with something, because she was feeling very tired and lost her appetite. She developed vomiting this am. They blame the sushi they ate.

During the physical, you notice she looks very ill. She's laying on her side and holding her belly uncomfortably. Her skin looks pale and appears to be dehydrated.

So, you ask for a glucose, and come to find, it's extremely high and reads ">500".

Today, we have a 35 year old African American female with a past medical history of hypertension, who presents to the emergency department.

He has a two week history of a viral upper respiratory symptoms, including a low grade fever, cough, nasal congestion, rhinorrhea, sore throat, decreased oral intake - followed by a persistent nose bleed.

The nose bleed began over the last couple days and isn't stopping despite persistent pressure. And over the last 24 hours, she had a change in her mental status per her family.

Her exam was unremarkable.

Her vitals: 100.2 F, HR 90s, BP 145/82, and 98% O2 RA. 

Initial labs revealed:

• WBC: 18,000
• H/H: 9/27
• Platelets: 7,000
• Creatinine: 1.4
• Total bilirubin: 3
• LDH: 1,500
• Normal fibrinogen, D-dimer, PT, INR
• Peripheral smear: moderate schistocytes

What's your diagnosis?

You have a 54 year old male patient with a past medical history of schizophrenia, ETOH abuse, drug abuse (crack cocaine), hypertension, hyperlipidemia, and diabetes.

He was brought in by police for aggressive behavior in public. The patient was roaming the street, yelling and banging on restaurant windows. 

Upon police arrival, the patient was awake and alert. However, he wasn't calm or cooperative; ultimately he required restraints by EMS.

You're unable to get his vital signs due to his agitation.

Today, we're going to discuss the last topic of the toxicology series and that's going to be serotonin syndrome.

We have a 32 year old male with a past medical history of depression who presents with an intentional overdose of Zoloft. 

Per the family, the patient was found in his bedroom acting abnormal, and an empty bottle of Zoloft was found on the floor. His last known normal was when he got home at 3pm, which was about 4 hours prior to arriving to the ER.

Vitals: 103.9 F, 130 HR, 110/78, and 96% O2 RA. 

On exam he was agitated and irritable. He was flushed and sweaty with dilated pupils. He also had this inducible ankle clonus along with hyperreflexia of the lower extremities.

Serotonin Syndrome can be fatal, therefore you never want to miss this, and this should always be on the differential when a patient presents with altered mental status, agitation, and/or with some neuromuscular abnormality.

Something clinicians struggle with is knowing when to call for help. They feel pressured to handle everything which comes there way - for fear of looking "stupid". But, as you'll see in today's video, that shouldn't be the case.

Instead, I want to review anticoagulation pharmacology...any medication that inhibits one or more steps of that coagulation cascade.

Imagine you're in your office, you have your cup of coffee, and you're about to start reviewing some of the results that came back for your patients.

You look at one of the right upper quadrant sonograms you ordered...

Giant Cell Arteritis is a chronic vasculitis affecting medium and large sized vessels. Always look for a patient over 50 with an elevated ESR.  This has never been documented in a patient under 50.  Let's go over the presentation, workup, and treatment.

Knees causing trouble again? This time, it is not gout, but an imposter known as pseudogout. Now what is pseudogout, you ask? Think of it as gout’s mischievous twin. Like gout, it triggers sudden knee pain and inflammation, but instead of uric acid crystals, it is calcium pyrophosphate dihydrate (CPPD) crystals in your joints.

The symptoms are similar to gout, swelling, redness and pain, often in the knees, ankles or wrists. To diagnose pseudogout, you’ll do some blood work to check for uric acid and X-rays and joint fluid analysis will help identify the culprit. Some treatments are rest, ice and anti-inflammatory medications.

Today we're going to be speaking with Amy. She's a recent graduate from the University of South Alabama PA program. She'll give her advice on what to do and what not to do for those going through didactic and clinical year. She also touches on how she personally overcame a tragic event that happened 3 months prior to graduating - and the mindset she adopted to over come it.

A 43 year old male presents to the emergency department with right wrist pain after falling on his outstretched right hand. You obtain an  x-ray of the wrist, which demonstrates no acute fracture or dislocation. The patient is neurovascularly intact, you decided to discharge the patient home with instructions to treat with: ice, rest, and some naproxen.

14 months later, you're being sued by this very same patient, apparently, after you saw the patient their wrist pain never got better and in fact it persisted for over 8 months before they were finally seen again at another facility. They were then referred on to hand surgery where the diagnosis of a Scaphoid Fracture with Osteonecrosis due to non-union was made. 

The patient is suing you because they say you missed the fracture and now they have chronic wrist pain and limited functionality of the hand, so they're unable to make a living as a plumber, and their quality of life has dropped significantly. This is a fictional story, but very very common. Today we're going to talk about how to diagnose and how to not get burned by Scaphoid Fractures.

We have an 18 year old male with a past medical history of depression and anxiety who presents with remorse following an intentional ingestion of anything he could find in his parents medicine cabinet.

He believes he took about a handful of tylenol tablets and thinks they were about 10 pills of 500 mg extended release tablets.

It's the most commonly used fever reducing and pain control agent on the market. 

The most common cause of liver injury is drug induced, and half the cases, are due to Tylenol. 

This is why it's so important to know what to how to identify and manage these patients. 

So, let's dive in on learning about Tylenol overdose.

The next patient on your list is a 35 year old male with a past medical history of IV drug and alcohol abuse. He's been sober for the last 7 years. 

He presents to the ER after a mechanical fall at home. The nursing triage notes states the patient was in his normal state of health, until about two days prior.

He may have hit his head, but is unsure. The patient had a loss of consciousness. No neurological deficits. The day after his fall, he's experienced decreased urine output, despite increasing his water intake.

The urine that was seen was noted to be brown.

Labs were notable for

• BUN of 63 mg/dL
• Creatinine of 8 mg/dl. His last creatinine from a wellness visit was .8 mg/dl.
• Potassium 6.8 mEq/L. EKG is normal sinus rhythm.
• CK: 40,000
• AST: 1066
• ALT: 450
• Urine analysis was positive for blood, but negative for RBCs.

Today, we'd like to introduce Dr. Niket Sonpal as the new host of the Medgeeks podcast! 

Who is Dr. Sonpal? Well, besides being an internist and gastroenterologist, he's an assistant professor at Touro College of Medicine in Harlem New York. In addition to that, he's had the fortunate opportunity to be the co-author of "Master the Boards USMLE Step 2 and Step 3" . He's also faculty at Medquest Board review and was faculty at Kaplan for their medical test prep. He's also done a variety of lectures with Medgeeks as well. 

All these things combined has led to a 10 year history of teaching students medicine, the boards, and of course the grand round circuit. 

Today, we'll be going over some background data and the management of upper GI bleed. The reason for making this the very first episode is based on the fact that the guidelines have just been updated as of May 2021. So, the data presented today and the summation of it, will help you get through taking care of your patients who have upper GI bleeds.

​Today, I want to share why I love being a PA. Specifically, an emergency medicine physician assistant. First off, I love the fact you can practice in a specific field for a given duration, and later transition to another down the road.

As nice as this is for most, I don't see myself doing this. Reason being is I have such a love for emergency medicine! With that said, I do see myself working in trauma to hone my schools further.
I recently switched hospitals, where I've been allowed to handle the more complicated patients (codes, intubations, etc). And let me tell you - I have learned a ton!

Hyperparathyroidism is one of those topics that is confusing, as it has primary, secondary, or tertiary causes. 

There are PTH, phosphate, calcium, and vitamin D levels.

During school, I considered doing an ER residency. I even did an elective rotation at arrowhead regional medical center, as they had a paid fellowship.

There was always something about the ED that caught my eye. But, instead of pursuing emergency medicine, I went down another path.  

That wasn't the case for my buddy Gray; he did go down that path. 

He's been practicing as an ED physician assistant for two years and is loving every minute. Two days out of the week, he also precepts students.

Since starting, he's learned, and done a ton - this means he has a lot of knowledge to share 😎

In this latest podcast episode, we will talk about radiation. No, not the radiation that gives you superpowers like in the movies. But, radiation proctitis, which is inflammation of the rectum caused by exposure to radiation therapy. Typically for pelvic cancers like prostate, cervical and rectal cancers.

There are two types of radiation proctitis, acute and chronic. Acute develops up to 3 months after treatment and chronic develops more than 3 months after. To treat acute radiation proctitis, it is often managed with dietary changes and medications like stool softeners and antidiarrheal drugs. Treatment for chronic proctitis will vary depending on severity and may involve medications, lifestyle modifications, procedures like balloon dilation or surgery.

This is a significant health issue characterized by a reduction in the blood flow to the intestines. As a result of this diminished blood supply, individuals suffering from this condition often experience a range of symptoms, most notably abdominal pain and a loss of weight.

In this episode of the Liver Series, Dr. Niket Sonpal discusses drug-induced liver injury (DILI), a common and potentially serious side effect of many medications. DILI can occur in people of all ages, but is more common in older adults and people with pre-existing liver disease.

DILI can be caused by a wide variety of medications, including over-the-counter medications, prescription drugs, and herbal supplements. Some of the most common medications that cause DILI include Tylenol, NSAIDS such as Advil and Aleve.

DILI is usually treated by stopping the medication that is causing the injury. But, in severe cases, the patient may need to be hospitalized.

About one year ago I had an 80 year old patient who kept complaining of nausea and abdominal pain after having a cholecystectomy. The surgeons kept telling her this was normal following this type of procedure and prescribed Zofran.

Well, she eventually came into my ER and she didn't look to good. She became septic.

We followed the septic protocol - but she ended up dying.

In this week’s episode of Bugs and Drugs, we will dive into two worlds of food borne bacteria. One can be found in reheated rice, known as Bacillus cereus. The other is found in raw or undercooked pork, known as Yersinia enterocolitica.

B. cereus can present with vomiting syndrome or diarrheal syndrome. You won’t need to do any testing unless there is an outbreak and antibiotics are not needed. Your patient will need some IV fluids and letting the bacteria go through their system.

Patients that come in with Yersinia enterocolitica, will present with diarrhea and abdominal pain. 

In some cases, your patient can experience reactive arthritis and erythema nodosum, affecting joints and skin. Once you confirm with a stool and blood test, antibiotics are recommended along with IV fluids.

PhysicianAssistantBoards.com - Todays topic will be community acquired pneumonia.  Lets go over the definitions, sign/symptoms, testing, and treatment plans.  When do we hospitalize?  Be aware streptococcus pneumoniae is the most common pathogen, regardless of the patient population.  Also, chest x-ray lag! Do not use this test to base treatment failure or success!
