19 Jul 2022 | The Experiences of LGBTQ Elders From the Stonewall Generation with Dr. Jane Fleishman, Certified Sexuality Educator at Speaking Of, LLC | 00:39:05
In the latest episode of the LTC Heroes podcast, we speak with Jane Fleishman, Ph.D., Certified Sexuality Educator at Speaking Of, LLC. Jane is the author of The Stonewall Generation: LGBTQ Elders on Sex, Activism, and Aging. In the episode, we discuss the book in detail. The book shares the experiences of nine elders in the LGBTQ community who came of age around the time of the Stonewall rebellion. There aren’t many books about the LGBTQ elders' experience, which makes the book stand out. When Jane finished her doctoral research on statistical research on LGBTQ elders and sexual satisfaction, she longed for the stories behind the numbers. Jane heard back from a group of people who had come of age or came out as LGBTQ during the Stonewall rebellion, a very heavy, exciting time of political turmoil. Jane interviewed people and they seemed to like the idea of being called the Stonewall generation. That’s how the name and vision for the book came about. In this episode, we hear all about the book, including common reactions from readers and about Jane’s learning curve. 12 Apr 2021 | Capturing Our Elders’ Stories with Sky Bergman, Director of Lives Well Lived | 00:37:56
Asking the Right QuestionsOn this episode of the LTC Heroes podcast, Sky Bergman, Director of Lives Well Lived sits down with us to discuss her documentary and the fruitful insights that came from it. Sky outlines her inspiration for the film and highlights the importance of having the right questions and framework to start from when capturing the stories of elders. She notes that if your LTC facility wants to start a similar process of gaining insight from your residents, it’s best to take it slow—asking one question per week could be a great place to start. Everyone Has a StoryShe discusses mortality and says it’s important to converse with our elders about it to give them the chance to truly describe their last wishes and desires for the remainder of their lives. Sky describes the positive benefits LTC facilities have gained by screening her film, including the fact that it serves as a concrete reminder for staff that all residents have a story. They wouldn’t want to miss the opportunity to engage with their residents on a deeper level. Sky closes the episode by highlighting the importance of intergenerational connection for breaking down ageism. She also provides nurses and other LTC staff with actionable advice on how to connect with residents about their stories. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/A:What kind of valuable and actionable advice do you expect we’ll cover in today’s episode? I think one of the things that is really important is to collect the stories of our older adults, you know, those will be lost if we don't collect them. And then of course we have to share them, and share them with the people who are working at the long-term care facilities. Because to remind everyone, that these are not just patients, they are people that have amazing lives. And it really helps to validate everyone's life if we collect the stories, and we really listen to our elders. What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? I really love Dr. Roger Landry and Masterpiece Living, and I think that their ideas very much align with the things that I discovered in the film. I would definitely look at his work and his book. Who is one mentor who has influenced the way that you do care in our industry? Of course my grandmother has been my biggest role model because I want to be like her. And then I think secondly, I would say my dad, who is a geriatric physician, and has been since he got out of med school. He is still practicing even now, in the midst of all this craziness. 31 Mar 2021 | Leading by Example in LTC with Erica Knepper, Director of Business Development at Holgate Center | 01:00:52
Empowering Staff and ResidentsOn this episode of the LTC Heroes podcast, Erica Knepper, Director of Business Development/RN Consultant at Holgate Center, sits down with us to talk about her many career transitions and the associated challenges of becoming a young female leader in the LTC industry. Erica shares what it takes to be an effective leader in LTC, noting that being willing to do what you’re asking of others is a key aspect of leadership. In order to empower the people you’re managing or leading, you have to listen to them, find the root causes of problems, and follow through on your promises. Value-Driven LeadershipShe describes the importance of having the courage to speak up for those who need it and going the extra mile to ensure there’s an unwavering focus on high-quality resident care. Instilling these qualities in your staff and organization starts with the example set by leadership. Erica encourages other nurse leaders to leverage any leadership resources they have available to them. Even if you don’t think you have the time, by leveling up your leadership abilities through ongoing training, you’ll end up saving more time and energy for the things that matter most. She closes the episode by encouraging leaders and nurses to stick to their internal morals and values, letting these guide their decision-making and journey in LTC. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/A:What kind of valuable and actionable advice do you expect we’ll cover in today’s episode? I think the biggest thing is to teach nurses how important it is to be allies in many senses of the word, and what they need to do to be successful nurses and leaders. I would also have to say that we’ll address ways to battle burnout. What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? Today’s Geriatric Medicine is a great magazine to be able to learn a lot of what's going on. Another one specifically for long-term care would be Mcknights Long-Term Care News. Who is one mentor who has influenced the way that you do care in our industry? Patti Garibaldi. She functions day-to-day with such a sense of ease, smoothness, and clarity in what she does, and she goes about it in a way that most nurse consultants aren't ever able to successfully do, which is having compassion drive her decisions. Knowing what you know now, what advice would you give your younger self starting in LTC? I think the most important thing is that you really have to stick to your internal morals and values and let them guide you. Don't let others take you down a path that isn't really internally what you believe in. Resources:

09 Aug 2022 | Lessons From a Female Leader and Business Owner with Severine Petras, CEO at Priority Life Care | 00:38:59
In this week’s episode, we chat with Severine Petras, CEO at Priority Life Care, a leader in affordable senior care. In 2009, Severine established Priority Life Care to provide senior living options to older people of all financial backgrounds. Severine and her brother, Bobby Petras, founded and sustained an option for affordable assisted living and memory care. Back then, this was rare in the long-term care industry. In 2022, Priority Life Care sticks quite close to its original mission of providing affordable senior care. However, today, Priority Life Care is a full third-party management company, in contrast to when the business first started. Initially, Priority Life Care had a capital partner and did a lot more back office work. One of the biggest learning curves in Severine’s career has been the pandemic. The company was not fully prepared for dealing with such a crisis. Severine’s role evolved from supporting people at a community level to helping people in the field. At the time, she didn’t have the tools and knowledge to do it, and she had to learn how to navigate the pandemic on the job. We appreciate you listening and we are motivated by your feedback. 14 Jul 2021 | Nursing Documentation with Rosale Lobo, Assistant Professor of Nursing and Health Policy at The University of Bridgeport | 00:58:56
In this episode of the LTC Heroes podcast, Rosale Lobo, Assistant Professor of Nursing and Health Policy, sits down with us to discuss her approach to nurse training and the importance of nursing documentation. Nurse TrainingRosale shares that when training new nurses, she first teaches them about empathizing and understanding people. Without this foundation of interpersonal skills and an ability to see nuances about a patient, other nursing duties such as documentation become much more difficult. Without a big picture view of your patients and knowing what qualitative data to capture, she says, nurses will struggle to understand how their documentation contributes to the overall care of their patients. Nursing DocumentationRosale highlights some key challenges around documentation, including that EHRs are rarely aligned with the workflow of nurses, nurses don’t have enough input in the design of EHRs, and EHRs often don’t reflect the human element of nursing. When it comes to poor documentation habits, Rosale cites a lack of details and vague language as the main offenders. Without precision when nurses are charting, it's difficult to truly gain the big picture around a patient. This can also lead to issues in concerns with litigation and regulation. Glenn outlines the differences between non-profit LTC acquisitions and affiliations, describing why organizations would choose one over the other, or a combination of both. He discusses the main reasons organizations would want to be acquired or become affiliated, which includes needing financial assistance and guidance when there’s a change in management. LTC Legal CounselGlenn shares the importance of mission compatibility when affiliating with an LTC organization, and why proper management is needed at all levels of the affiliation to ensure its success. He also brings up the point that affiliations take time. Both sides need to be comfortable and working from a place of trust in order for the process to run smoothly. Name one mentor in the LTC industry that is active on social media or Linkedin that inspires you and that the rest of us should be following because of their contribution? If you’re talking about individual areas I’d have to recommend the Leading Age again, and for other items like inspiration, people like Simon Sinek and his book, Leaders Eat Last. If you were to start working tomorrow in the LTC industry, day 1, but with the same knowledge and wisdom you have today, what advice would you give to your younger self? Be sure to use common sense and look at the big picture, it is so important. Also, I tell young attorneys that they will never know all the answers. 06 Jul 2021 | Successfully Implementing EHRs with Charles Oliver and Ayisha Bradley | 00:45:20
Benefits of Implementing EHRsIn the episode of the LTC Heroes podcast, Charles Oliver, Director of Customer Success at Experience Care, and Ayisha Bradley, QA Informatics Technician at Care Centers Centers Management, sit down with us to discuss the benefits of electronic health records and how to implement them properly. With Ayisha and Charles both having experience in nursing, they talk about what life was like before switching to paperless operations, noting that once you get over the learning curve of a new EHR, daily workflows become much more efficient. Challenges When Implementing EHRsAyisha and Charles both say that one of the biggest challenges when implementing an EHR is getting the nursing staff on board with the solution. Part of the challenge is getting nurses off of the floor to learn how to operate the system. The other part is properly educating the nurses about how the EHR will positively impact their patients, because that’s what truly matters at the end of the day. To find success with EHRs, Charles describes the importance of gaining input from all users in order to use the EHR to its full capacity. Without this big picture consideration, organizations often create more work for themselves instead of experiencing the benefits of EHRs. 20 Apr 2022 | The Middle Market Playbook in Long-Term Care with LeadingAge Virginia President and CEO, Melissa Andrews | 00:35:18
In this week’s episode of the LTC Heroes podcast, we speak with Melissa Andrews, President and CEO at LeadingAge Virginia. Melissa is responsible for the overall management of the company and leading the membership. LeadingAge represents more than 5,000 nonprofit aging services providers and other organizations that improve millions of lives daily. LeadingAge uses applied research, advocacy, education, and community-building to make the United States a better place to grow old. The episode takes an in-depth look at Melissa’s book, The Middle Market Playbook. This was organized by Melissa back in 2019 and was published by LeadingAge. The idea for the book started when Melissa began traveling around Virginia when she became CEO almost six years ago. She would ask members what kept them up at night. There were two main issues: the workforce; and that many faith-based members felt like they weren’t being served well. The book was created to help solve a problem that existed for that group. Other topics discussed in the episode include the challenges of diversity in the long-term care workforce and strategic planning. 22 Feb 2021 | Regulatory and Care Delivery Challenges at Nursing Homes with David Hoffman, JD, FCPP | 01:10:23
Nursing Home Regulatory ChallengesIn this episode of the LTC Heroes Podcast, David Hoffman, President and Founder of David Hoffman & Associates, sits down with us to share his insight on overcoming the regulatory and care delivery challenges associated with nursing homes and other LTC facilities. With many years as an attorney under his belt, David shares his experience working on negligence cases, leading regulatory efforts, and discusses why aggressive oversight of this industry is a must to ensure proper care for residents. Nursing Home Staffing and ResidentsDavid describes why once money becomes the main motivator for these facilities, they look at areas to cut costs, and the first place they look is staffing. As a result, these facilities are understaffed and the residents are the ones that pay the price. David outlines why care facilities need to focus more on outcomes from the residents’ point of view instead of simply looking at the care they’re providing. With the right teamwork, this can happen while still balancing documentation requirements. In order for real change to occur, David says we need to reduce the tension between the regulator and the regulated. He suggests better communication is the best place to start. He closes the show by challenging us to consider the issue of ageism in our society and what we can all do to show more compassion to our elders. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/AIn today’s chat, what kind of valuable and actionable advice can LTC lovers and listeners expect to learn from you today? David: I’ll be providing your listeners with a better understanding of the nursing home industry in terms of a regulatory construct, why it's so highly regulated, and what the impact of that regulation is currently and going forward. What is one lesser-known book, newsletter, or industry resource you go to for up-to-date LTC info? David: The Office of Inspector General’s Work Plan.Name one mentor in the LTC industry that is active on social media or LinkedIn that inspires you and that the rest of us should be following because of their contribution?David: I clerked for a judge both in state court and in federal court named Anthony J Scirica. He taught me an important lesson, which is to be a good listener and that there are two sides to every story. If you were to start working tomorrow in the LTC industry, but with the same knowledge and wisdom you have today, what advice would you give to your younger self? David: I think that the focus on compliance with regulatory requirements is a two-way street. I would focus on how to have a better approach in the relationship between the regulated and the regulators. Resources:Office of Inspector General: Work Plan

At only 28 years of age, Joe already has many years under his belt as a successful LTC administrator. He shares that building confidence as a young leader is a process, and that there’s always a little bit of imposter syndrome you have to work through. These insecurities, however, can help you stay on your toes and strive to improve everyday. LTC CultureJoe discusses the cultural changes that he’s seen over his years as an administrator, including the push to make LTC facilities feel more like home for the residents. Part of accomplishing this, according to Joe, is to give residents as much autonomy as possible so that they feel like they are playing a bigger part in their plan of care. He says that some unique traits he brings to the table as a young administrator is having a deep understanding of technology and systems, which allows him to support other staff with new technologies. He also notes that his ability to be decisive has played a big role in his success. Learn more and sign up to be notified about future episodes at LTCHeroes.com. Rapid Fire Q/AWhat valuable or actionable advice will we chat about that might be helpful for someone else in our industry? I think regulation is always a hot topic about long-term care. We get regulation from so many different areas, whether that is CMS, CDC, Department of Health, Department of Human Services, and regulation is always a topic. What is one lesser-known book, newsletter, or industry resource that you would send me to understand long-term care? Leading Age, for our area especially, is a huge resource for me. On a not so long-term care-related topic, Ted Talks. I love TED Talks. I always listen to TED Talks and they help me become a better leader. Name one mentor that has influenced the way that you deliver care in our industry? Chad Stroschein, the President of Caring Professionals. He's the one who gave me my first job at 21 years old. He had that faith and confidence in me. And if you are in long-term care in South Dakota, you probably know the name Chad Stroschein or Caring Professionals. He’s a wonderful guy so big shout out to him. He also helped me with confidence. He was a man of faith, and I am as well. He just taught me to embrace that faith in my professional life, as well as my personal life. What advice would you give to your younger self? Putting out those fires, having those tough talks, and being more present is the most important thing for me personally, especially given my personality. And I would tell myself to have that confidence and not to be afraid to pick up that phone. Don't be afraid to ask questions. People are generally nice, and they don't want to upset you or to hold you back. So if you just talk to them and ask questions, they'll help you. Resources:

19 May 2021 | Predictable Quality Outcomes with Mark Klyczek, President and CEO of Virginia Health Services | 00:38:02
Quality Outcomes in LTCOn this episode of the LTC Heroes podcast, Mark Klyczek, President and CEO of Virginia Health Services, sits down with us to share his insight on creating predictable quality outcomes in LTC. When it comes to improving quality, Mark highlights the importance of focussing on small incremental improvements that lead to big lasting change. He notes that quality measures are much like an open-book test, meaning that there really shouldn’t be any surprises if you’re gathering and analyzing quality data correctly. Quality Improvement ResourcesMark shares the important role that your administration plays in finding quality trends and organizing data for providers. This enables providers to stay focused on their patients while also gaining insight on where they can improve quality. He suggests that if your organization doesn’t have data analyst resources, it can be helpful to partner with larger organizations for assistance—or you can reallocate the resources you do have to support this important role. Mark also talks about how team engagement is a must for successful quality improvement initiatives. He says boosting engagement is a product of truly helping your staff grow in their work and personal life. Learn more and sign up to be notified about future episodes at LTCHeroes.com What kind of valuable and actionable advice do you expect we’ll cover in today’s episode? What I'm really interested in getting across to the group is how predictable quality can be in the nursing homes as we're looking to be ever better in quality in long-term care, and how important it is to engage your team and the different strategies that people can use to engage their team. What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? Sometimes looking outside of long-term care is great, to some things that are more universally applicable in long-term care, hospitals, or physician practices. One of those would be the “Checklist Manifesto” book. That is a really great resource, especially for LTC, because we're used to doing audits, not checklists, in long-term care. The other one would be a book called “Good to Great”, which I believe a lot of people have read. Any business, including long-term care, is not excluded from any of the normal principles of running a good business. The last one is a little obscure, and it's called “Extraordinary Relationships.” And that's a book about self and how to be a good leader, both in your family and in work. Who is one mentor who has influenced the way that you do care in our industry? A person that helped me early in my career is Aimee Gomlak. She taught me how to be fearless in a leadership role. And then I've been very fortunate to have other leadership coaches. John Engels and Eric Thompson are a couple of other ones that have helped me. I've been surrounded by a lot of good people that have helped me get to where I am today. What advice would you give your younger self starting in LTC? I'd say one thing that's gotten me into trouble, but I think I'll accept the trouble, is to think big and always try to push yourself beyond what a nursing home should be able to do or what you should be able to do with your team inside of the nursing home environment. So whether it be improving quality, improving people for their careers, I think that's important. Resources
31 May 2021 | Crisis Management in LTC with Melissa Orth, President and CEO of The Legacy Senior Communities | 00:49:44
The Learning Curve as a New LTC LeaderOn this episode of the LTC Heroes podcast, Melissa Orth, President and CEO of The Legacy Senior Communities, sits down with us to discuss her journey into LTC and how her organization took the recent Texas power outage in stride. Melissa talks about her experience of transitioning from education to LTC, noting the skills she was able to reapply and the new knowledge she had to learn around regulations and requirements. She discusses the learning curve around marketing in LTC, highlighting all the different parties and constituents that need to be targeted in this industry. Melissa also describes the challenges of having to go from high-touch marketing tactics to virtual marketing amidst the pandemic. Managing the Texas Power Outage Crisis Melissa opens up about her experience of leading The Legacy Senior Communities through the Texas power outage, noting the crucial role that clear communication and problem solving played in their success. She says one of the keys to handling a crisis like this one is decisiveness from leadership. When there’s a clear plan of action, it’s much easier to collaborate with other teams and keep everyone working toward the same goal. She also points out that when there isn’t a formal handbook around handling these types of issues, leaders need to be able to follow their intuition and make sense of whatever data is available. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/AWho is one mentor who has influenced the way that you do care in our industry? What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? There's a book that goes along with it that I've yet to read called “Traction”. There's also some reference to “Good to Great” and having the right people in the right seats. And also I know that's not necessarily unique, but I think it is really important that you don't work in silos, and that the organizational values, mission and vision are consistent across all of your operating entities. What advice would you give to your younger self, as you started to move into leadership positions, knowing what you know, today?

17 May 2021 | Organizational Leadership in LTC with Reed VanderSlik, President and CEO of Baptist Retirement Homes | 00:42:26
Communication as an LTC LeaderOn this episode of the LTC Heroes podcast, Reed VanderSlik, President and CEO of Baptist Retirement Homes, sits down with us to discuss what effective organizational leadership looks like in our industry. Reed talks about his unique transition from working in the manufacturing industry to entering LTC, and touches on the challenges of understanding regulations and reimbursement as a newcomer to the industry. He shares the details of a tragic story involving coworkers, and what he learned by navigating that experience. He notes that the frequency and clarity of communication make all the difference when working through a crisis situation. Seeking out New LTC LeadershipReed provides some great insight on how to seek out emerging leaders in your organization. He highlights the importance of looking at where results are manifesting within your organization, and then finding the face behind those results. To close the episode, Reed gives us some tips for how to look outside your organization for support, and shares examples of the partnerships Baptist Retirement Homes have been a part of. Learn more and sign up to be notified about future episodes at LTCHeroes.com. Rapid Fire Q/AWhat valuable or actionable advice will we chat about that might be helpful for someone else in our industry? I think a big part of our conversation will be about the dos and don'ts of crisis management. What is one lesser-known book, newsletter, or industry resource that you would send me to understand long-term care? It may seem kind of strange, but it's a children's book called The Giving Tree. I've used it in leadership development with my team. But, I reflect on it a lot too, because I think oftentimes in our industry we're the tree. And you know, we keep giving and we keep giving and we keep giving, and that's part of what makes us wonderful in the industry. But, we also have to realize the effect that has on us, and we have to be sensitive to that and support each other so that there are still limbs left for the future. Name one mentor that has influenced the way that you deliver care in our industry? I think of Dr. Thomas, and I'm sure you're familiar with him. I was at an organization that developed Greenhouse Homes fairly early on. I really listened to what he had to say about the approach, about his belief in Greenhouse Homes, and the changes that really benefit the residents that we served. What advice would you give to your younger self? I certainly wish I had started on my journey of developing future leaders because the industry needs it, and it brings such joy to see individuals grow. Resources:

He describes the current approaches to interoperability, what needs to change, and the ongoing challenges the healthcare system faces in this regard. Scott talks about the work at DirectTrust and how they’re able to create a trusted information exchange ecosystem. He closes with how DirectTrust plans to create differentiation in the messages their network handles to help information receivers behave differently for the purpose of more LTC interoperability success. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/AWhat valuable or actionable advice do you expect we'll cover in today's chat? Scott: I'm really hoping people understand that there are really inexpensive, quick, and easy ways to achieve interoperability for long-term care organizations. What is one lesser-known book, newsletter, or industry resource that you recommend I go to for a better understanding of the long-term care industry? Scott: Jerry Seelig’s Newsletter called ‘Revitalize.’ Name one mentor or influencer in your life that has inspired you for the work that you do in the long-term care industry? Scott: I had the pleasure of working for the founder of Cerner Corporation, Neil Patterson. 23 May 2021 | Getting to Yes with Todd Andrews, President of the CCRC Division at Asbury Communities Inc. | 00:41:15
LTC Leadership and Problem SolvingOn this episode of the LTC Heroes podcast, Todd Andrews, President of the CCRC Division at Asbury Communities Inc., sits down with us to share his experience of developing the people and LTC communities he has served. Todd says his guiding principle throughout his leadership journey has been the concept of “getting to yes.” In other words, how do we get to a point where we’re listening to the needs of our employees and giving them the proper resources in order for them to say yes to the challenges they face? Also, how do we get people to say yes to visiting campuses and other activities amidst the challenges presented by the pandemic? Todd explains that listening to the people closest to a given problem is the best way to “get to yes” and find a solution that aligns with their goals. He discusses why we should focus on getting back to now instead of normal, noting that we can only move forward by directing our efforts in the present moment. LTC ImprovementsTodd also discusses some of the positives that came from the isolation of the pandemic, including seniors embracing technology and the emphasis put on clear communication. Todd closes the show by encouraging us to find a balance between being an operator and a dreamer, and also reinforces the idea of putting employees and residents at the center of everything we do in the industry. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/AWhat kind of valuable and actionable advice do you expect we’ll cover in today’s episode? How we “get to yes.” We're going to talk a lot about how we can come out of Covid, what things we should be thinking about, and how we can better serve the elders that we care for. We’ll talk about how to engage families differently and also some of the learnings I’ve had over the years. What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? For me, the best resource is the people that I know that work in the industry, that I've gotten to be friends with over time, that I've been in competition with, and that I've met at conferences. I think it's easy to forget sometimes the wealth of knowledge you have in your network. And I think using that network is very wise when it comes to uncharted times. We're all learning this as we go. I think you could go to a book, and we all read plenty of management books, but I get a lot more from friends and colleagues in the industry. What advice would you give your younger self starting in LTC? Listen, listen, listen, not talk. Listen to what people tell you. Pay attention to what's around you and learn from your mistakes. And then I would say the biggest one is to be reflective. Realize your part in whatever you’re engaged in. Resources:
28 Jun 2021 | Innovation in LTC with Steve Lindsey, CEO of Garden Spot Village | 00:50:38
In this episode of the LTC Heroes podcast, Steve Lindsey, CEO of Garden Spot Village, joins us to discuss how to approach innovation in the LTC industry. Building a Culture of InnovationTo start the show, Steve describes innovation simply as “Applying resources and finding new ways to solve real problems.” He says people often overcomplicate innovation by confusing it with creativity, leading them to develop solutions for things that aren’t truly problems. Steve highlights the importance of developing a culture of innovation where everyone at your facility thinks through challenges and focuses on addressing them in a new way. Innovation often comes about by looking forward, anticipating changes, and implementing novel solutions that can handle the change. Self-Actualization for SeniorsOne of the key LTC innovations Steve is an advocate of is encouraging more self-actualization for our seniors. He says we need to look beyond the “institutional” qualities of LTC in order to bring fulfillment to all areas of our seniors' daily lives. Steve talks about the importance of being comfortable with failing in concerns with innovation. At some point, our innovative ideas will fall short, but the key is to fail forward and learn from the experience. He also addresses the need to embrace certain constraints while innovating, so as to produce something that is realistic and actionable. 08 Jun 2021 | Elder Abuse Awareness and Prevention with Carol Silver-Elliott, President and CEO at Jewish Home Family | 00:55:47
On this episode of the LTC Heroes podcast, Carol Silver-Elliott, President and CEO at Jewish Home Family, sits down with us to discuss the ever-important issue of elder abuse and how we can increase awareness of this problem. Carol starts the episode by defining elder abuse as an intentional act by a trusted caregiver that does harm to an elderly person. This can include physical abuse, verbal abuse, emotional abuse, and also acts of neglect. She points out the sad reality that between 3.5 and 5 million seniors suffer from elder abuse every year. She says many people witness or experience elder abuse, but don’t have a label, context, or understanding of the issue at hand. A great portion of elder abuse victims suffer in silence and only 20% of those that are able to reach out to an elder abuse hotline actually end up going to a shelter for assistance. Carol highlights the characteristics of elder abuse victims, noting that most are over the age of 85 and have cognitive impairments or other ailments that make them vulnerable. In most cases, the abuser is an adult child of the victim, which makes it difficult for the victim to accept that abuse is actually taking place. These abusers slowly isolate and take control away from their victims, making it very hard for our elders to seek any sort of outside help. The prevalence of ageism in our country also enhances this problem because it influences how we perceive the credibility of our elders. In some cases where elders do speak up, they’re often ignored or written off as “delusional.” That’s why Carol says the elder abuse conversation needs to be elevated to a higher level to increase awareness about this issue. Carol discusses the importance of widespread education about elder abuse for first responders, police officers, and any other community members that want to prevent this problem. She says that we shouldn’t be afraid to speak up if we suspect elder abuse. It’s better to be wrong than it is to stay silent about such a serious offense to our elders. Carol shares the important role that elder abuse shelters play in helping victims create separation from their abusers and gain a new perspective on their situation. A shelter acts as a waypoint on the journey from abuse to safety while also being a source of any necessary care. Carol closes the show by explaining how existing LTC facilities can also act as shelters for these victims. With less than 20 elder abuse shelters across the country, she encourages other LTC facilities that have the resources to offer this support. She points out that many facilities can partake in this life-changing work without overwhelming their facilities. At the end of the day, the incremental cost of providing care for these abuse victims is far less important than reducing elder abuse in our country. 25 May 2021 | The PACE Philosophy of Care with Stephanie Button, VP at PACE CNY | 00:33:35
How PACE Organizations Deliver CareOn this episode of the LTC Heroes podcast, Stephanie Button, VP at PACE CNY, sits down with us to discuss what distinguishes a PACE organization from other LTC organizations. She also discusses how they navigated the challenges associated with the pandemic. Stephanie describes PACE as a philosophy of care that combines home-based care delivery with access to a day center for their patients. Acting as both a provider and payor source for long-term care, this all-inclusive model gives patients an alternative to traditional LTC communities. Stephanie points out that anyone who is eligible to stay at an SNF would also be eligible to receive care through a PACE program. Challenges for PACE OrganizationsShe says one of the major challenges for PACE organizations is educating patients and their families about how a PACE organization operates. She also notes that there can be a learning curve for new LTC workers transitioning into a PACE organization. Stephanie dives into how PACE worked through the pandemic, describing their approach to expanding telehealth offerings and organizing mobile care teams. She says PACE plans on leveraging what they learned throughout the pandemic in order to grow as an organization. An article I read in grad school called Successful Aging. I really gravitated towards the idea of successful aging and how to define successful aging for yourself through health, wellness, lifestyle, and creating a vision of how you see yourself aging. Name one mentor that has influenced the way that you deliver care in our industry. My grandma was very influential in my life when I was young and we had a really close relationship. I just loved listening to her stories about her life and how she aged and she was quite proud of her age. She lived to 92 and was very healthy. That had a big impact on me and was really why I went into geriatrics. What advice would you give to your younger self?I would advise myself to be a better educator for people going into health care and social work to help them identify issues and be problem solvers. I think that helps you prepare for the future, especially as we look at aging. Resources:

17 Mar 2021 | Problem Solving through Change Management with Annette Greely, President and CEO at Jones-Harrison Residence | 00:52:12
Change Management in LTCIn this episode of the LTC Heroes podcast, Annette Greely, President and CEO at Jones-Harrison Residence, shares her experience of improving LTC facilities through effective change management. She says that although some LTC facilities look good on paper, you need to enter those facilities with “eyes wide open,” taking an honest look at what needs to be improved. She shares how she addresses change with leaders that are stuck in their ways by finding a balance between pushing them in the right direction and respecting where they’re at. Improving Culture at LTC FacilitiesAnnette describes the importance of involving all parties affected by the change from the beginning and supporting them to stay accountable so that the changes can last. She notes that cultural change happens when there’s buy-in from management, and even more importantly, when lower-level staff truly understand the “why” of making certain culture changes. Annette also shares why change management leaders need to be transparent with their decisions so that all organization members understand why the changes are necessary. She closes the episode by discussing how having flexibility as a leader leads to more effective outcomes for those you are leading. Learn more and sign up to be notified about future episodes at LTCHeroes.com Rapid Fire Q/A:What kind of valuable and actionable advice do you expect we’ll cover in today’s episode? Annette: I think what we're going to talk about is change management. I don't think we ever really use the language in our industry, but that is an essence of what we are doing when we're always managing and looking at system changes. What is one lesser-known resource, book, or newsletter that you go to when you want to be up to date on LTC info? Annette: I really read everything. And I think that's the way to do it. I like reading things, even outside of our industry, that might be talking about our industry to give us a different perspective. Who is one mentor who has influenced the way that you do care in our industry? Annette: I don't really pick just one person. I feel like people come and go through our careers and I pick the best parts of multiple people. If you were going to start over in LTC with the knowledge you have today, what advice would you give to your younger self? Annette: Don't be afraid to make decisions that people might not like. Because if you're going to be a leader, you're going to be in that position, you're going to make decisions people don't like and families don't like and as long as you're doing it for the right reason, hold your ground. 21 Jun 2021 | Geriatrics and Person Centered Care with Mike Wasserman, Past President of the CALTCM | 00:58:58
On this episode of the LTC Heroes podcast, Mike Wasserman, Past President of the California Association of LTC Medicine, joins us to share his insights on leadership and person centered care in the field of geriatrics. The Field of GeriatricsMike explains that the difference between the geriatric approach to care and traditional primary care is that geriatrics focuses on the function and quality of life for patients rather than diagnosis, treatment, and cures. Taking a person centered approach to care, geriatrics is about truly getting to know a person in order to figure out what matters most to them with the time they have left. Mike touches on the unfortunate fact that the number of geriatricians has dropped about 50% over the past 20-30 years. The reasons for this decline include lower pay for geriatricians, poor reimbursement rates for geriatric care, and medical students not being given quality exposure to the field while in medical school. Leadership TrainingRegarding leadership in LTC, Mike says oftentimes when physicians are put in leadership positions, they haven’t had the training or don’t understand how to be effective leaders. This is the product of health systems being stuck in dated approaches to how leaders are developed. He also discusses the issue of non-clinically trained administrators not understanding the consequences of their choices and how it relates to the overall health of residents. Mike says clinical leaders and financial leaders need to have an equal voice within their organizations to truly improve outcomes.
