Galleri by GRAIL is a blood test that offers cutting edge technology to help detect over 50 different types of cancer before it would otherwise be found.

Dr. Amy Brenner talks with Galleri expert, Dr. Whitney Jones in depth about this new technology and what it could mean for the health of everyone. They discuss what types of cancer Galleri looks for, who should/can do it and why. The world of cancer screening has evolved in many ways and continues to do so, as with other tests for screening cancer there are protocols to follow. Special Guest – Dr. David Toub

Dr. Toub is Senior Vice President, Medical Affairs at Gynesonics, which has developed a transcervical device (the Sonata® system) for radiofrequency ablation of uterine fibroids. Prior to joining Gynesonics in 2008, Dr. Toub received AB and MD degrees from the University of Chicago and earned an MBA with a concentration in management information systems from Drexel University. He is a Fellow of the American College of Obstetricians and Gynecologists and a Diplomate of the American Board of Obstetrics and Gynecology.

After a fellowship in pelvic surgery at Graduate Hospital in Philadelphia, he served as an attending physician at Pennsylvania Hospital, where he held faculty appointments at both Thomas Jefferson University and the University of Pennsylvania.

A laparoscopic and pelvic surgeon with a special interest in providing alternatives to hysterectomy, Dr. Toub has been a faculty member and speaker at several conferences and authored a chapter on radiofrequency energy for a hysteroscopy textbook. He currently serves as an editor for two ob/gyn journals and lives just outside Philadelphia.

In her personal life, Jessica had her own health issues that were due to a gluten sensitivity and recently found out her child had an overgrowth in her gut which led her to want to find a solution rather than symptom management. Dr. Maria Canter is board certified in Female Pelvic Medicine and Reconstructive Surgery as well as Obstetrics and Gynecology. She was one of the first surgeons in the country to be boarded with both specialties. Dr. Canter works with each individual patient and customizes treatment plans for their specific needs. She focuses on improving her patients’ quality of life by using minimally-invasive techniques.

Dr. Canter was born in Washington, D.C. and grew up just outside of the city. She attended Georgetown University School of Medicine and completed a residency in Obstetrics & Gynecology at the Georgetown University Medical Center graduating with the Minimally Invasive Endoscopy Award. A three-year fellowship ensued in Female Pelvic Medicine and Reconstructive Surgery. While in fellowship, she received awards for Academic Achievement, Clinical Investigation and the prestigious American Urogynecology Society June Allyson Award for research in urine proteomics.

Dr. Maria Canter opened Urogynecology Center NoVa in May 2016. However, Dr. Canter has been in practice in Northern Virginia for over 10 years and is proud to say she loves what she does. When asked what attracted her to this field, she replies, “I have the privilege to take care of women. Amy: My special guest is my number one fan in business and in life and that’s my husband Kevin Cox so welcome Kevin.

Kevin: Well, thank you. Excited to be your first guest.

 

A: Yeah I thought it was appropriate to have you and I just wanted to talk today of how I even got started in integrative medicine and I thought you would be the perfect person because you were there from the very beginning.

K: Awesome let’s do it yeah.

A:  So uh for those of you that don’t know, I’ve actually been a board certified OB GYN physician for 20 years and spent the first 11 years of my practice doing traditional Gynecology or OB and Gynecology with a large group here in Cincinnati. Kevin if you could just explain like maybe what life was like for us as a couple and as a family um with a ob gyn taking calls, some from home some from some from being in the hospital

K: Yeah it was, craziness. So like most families we were just starting out. Young kids at home and your schedule for lack of a better term was very demanding and so your call coverage like, lot of traditional Obstetricians was 24 hours spent at the hospital so about every six days you were gone for a day and uh you know there was a lot of uh I guess blessings that it provided for our family but certainly challenges as well just from Logistics. So I had to be at home holding down the fort while you’re gone but then even when you were off call there was usually about a 24-hour recovery period of just you know sleep and uh other things of trying to get your uh your body physically back in shape after probably just going non-stop for 24 hours. So that was our that was our life in the early days if you will.

A: Yeah I mean the good thing about it is uh is we’re relatively young we did that um you know in our 30s uh so basically from 30 to 40 and uh I would say you know in general we were pretty healthy but uh as I look back now uh I really wasn’t as healthy as I thought. Uh kind of a healthy breakfast for me at the time was going up the street and getting a whole wheat bagel with the butter and uh a coffee and I thought that was a a healthy breakfast …

K: Oh exactly let’s put the let’s put the low-fat cream cheese on it and we’re doing well, right? Um and I think we had you know we had energy so maybe we mistook a little bit of you know because we were younger we had a fair amount of energy and you mistake that sometimes for healthiness and that’s not not always the case.

A:Um so when I found myself kind of at my late, late 30s of you know weight wasn’t as easy to keep off like it was up until uh up until that point. Um I felt like I was packing on the pounds despite working out with a trainer and monitoring every morsel that I put into my body. Um you probably remember the days when uh you know intimacy and sexual relations like I wasn’t really that interested because I was tired we had three babies um and just from a health standpoint like I just it really wasn’t that important to me.

K:I think that’s fair, uh but not a lot different from any challenges that uh young families face and you know priority shift and things uh things get out of whack kind of easily and I would argue that it was probably amplified in your case because not a lot of moms are, you know, working 24 hours six days a week without sleep and so that’s that’s a tough thing to recover from on top of just the challenges of of raising raising a young family.

A: Yeah and I know you are in in medi- you’re not in the medical field um but at the time I was you know taking care of myself of how I was taught to take care of my patients and so I was taking birth control pills because we decided that three kids was uh kind of what made our family complete. Certainly having three little babies, somebody was always crying and so I had the home stress the work stress and was taking something to help with mood, sleeping definitely was an issue, even when I what was at home and wasn’t on calls so the combination of those things just led me to a place where you know I was taking some medications and and I just wasn’t that healthy and I don’t know if you remember the the first conference that opened the door of what led me into this practice but um I went to that conference and my mom and my sister actually came with me just as tourists because the conference was located in New York City and I came out of that conference just hearing a little bit about bioidentical hormones and weight loss and it just opened the door for me and uh soon after that I rocked your world and said I’m gonna leave this practice where I’m a a partner in and I’m gonna start my own practice because I learned this aspect of medicine and I can’t do it in the setting that I’m currently doing it. I’m sure you remember that conversation.

 

K: Oh I remember that conversation I thought you were, I thought I was on an episode of “Punked” or something…like are you joking me like we, we’re finally getting over the hump with these kids young and you know you’re you know from my lens you’re a senior partner now and a big Medical Practice like you’ve climbed the ladder you put in the work and you can kind of see the proverbial light at the end of the tunnel so I’m like surely this is just a phase or something it’ll it it too shall pass is what I was thinking at the time. So I fully remember that conversation though.

A: Yeah and you offered to you know I’ll do more at home, I’ll do the shopping, I’ll do the laundry or we’ll get somebody to help and you know just keep doing what you’re doing it’s okay and and you know that lasted for a little while and then uh I think I finally said you know I just don’t want to practice medicine like this anymore. You know there were days in the office at my old practice where I saw 50, 60, almost 70 patients in a day and I just didn’t want to be the doctor of, you tell me a problem and I write your prescription anymore.

K: Yeah I definitely knew that. I think it had been building in you for a while and we I guess unfortunately uh suffered a life-changing event in the in the the passing of of my mother and I think that the timing of that coupled with the just the the the the burning desire that you had been feeling for a while was enough to say it’s time you know life is too short. I want to make a fundamental change in my personal and professional life and it’s time and so um that was that was kind of the impetus that that that that pushed you over the edge there so I remember that time fully well um so you know took a took a life-changing event to to really move in a different direction and then here we are on your first podcast kind of reflecting on on what got it all started.

A:Yeah 10 years later so you know it was kind of multifactorial of uh you’re right uh the the final straw was uh when your mother passed away and I just realized Life’s too short to be unhappy in a job and work-life balances is really important uh now the first couple years in practice in in this current practice is I I kind of was like oh my goodness what have I gotten into because I was still working a lot of hours uh because there was so much to learn I’m sure you remember those days of uh I would work until five but then come home eat dinner with the kids and then still like work until you know twelve one two in the morning.

K: Oh I fully remember those days and I you know truth be told what I think one of the reasons why uh and I’m again I’m looking at this through the lens that I look at things through maybe a little bit different lens than you look through but I uh it started my own business back in the day and then I was in a role with with my company where I was in charge of bringing on new businesses and starting them from the ground and I just knew all the little things that went into launching a business completely irrespective from all the clinical aspects that you’re referring to but just how daunting it can be to to launch a business from the ground up and so I knew that going in and I was certainly uh just just hesitant because I knew that it was hard to really convey that to you um but hats off to you I think you kind of ultimately powered through all those obstacles and you know you’re willing to do whatever it took early on to to make sure that this Vision that you had became a reality.

A: Yeah and luckily I I did have you to help with the the business aspects of it so I could focus on the the clinical or medical aspects because I had a lot tolearn I was totally Shifting the way I practiced medicine now I still continued uh with traditional Gynecology and uh still doing ablations and we trademarked word MyFlow for no more periods, period. And so that was a huge aspect of my practice but uh I decided that after I kind of helped myself of get off birth control pills and and the other prescriptions I was taking is after I did that and tested my hormones and started taking um some supplements myself to get me to help with sleep and to help with stress and adrenal Health um is I look back I’m like I didn’t even think I was you know quote that messed up I end quote and then after I help myself um and was you know feeling better in my mid-40s than I did at 30 I just realized that you know there’s so many other women that don’t know about these things and I have to do this and um so it was great to just you know feel myself again and not be this person that was you know grumpy and heavier than I wanted to be and just really didn’t have any interest in you know in sex quite honestly um so I’m sure you’re glad that we got past that part.

K: Oh there are zero complaints on my end about that and I for uh for most men that might be uh partaking in the in the podcast I I think that’s definite and again it’s it’s stage of Life a lot of its stage of Life related but uh but it doesn’t have to be that way and I think when you finally learned that and uh you know found some some clinical ways to address that and then it just becomes that proverbial you know Snowball running downhill where you’re you have more energy so now you want to take on more things and when you take on more things you feel better which gives you more energy and just a lot of things then just really came into focus and and you started firing on all cylinders as they say.

A: Yeah so because it wasn’t something that you and I were having problems it was more of a a medical thing of you know uh early on in our marriage we I used oral contraceptives to um prevent pregnancy and then we moved to a permanent non-hormonal option and so you know I you know we did have little kids and I was working but there was all also some medical factors that went into that and so you know once we got rid of the the medical factors that were affecting things uh you know it just added that aspect of our relationship that was missing I think because of you know medical factors mainly.

K: Yeah I agree I agree and so that was certainly a great um way for me to kind of understand clinically where you were going with this Venture because you know I’m first of all I’m a man and I you know I was looking at things through a different lens than you so I truth be told I didn’t quite understand the the the the the the realm that you were that we’re focusing on uh because I knew what you did from a traditional standpoint and how you were trained and then my eyes really got opened early onto not only what it did to you but then the the success you were having in treating other women and it really made me it just opened my eyes to understanding an entirely different direction that you were moving and you.

A: I remember you weren’t really on board with bringing in the other services in fact uh from day one we decided in our practice to offer Aesthetics and Botox and fillers and chemical peels and you know things like that and I think you’re exact I don’t know do you remember what you told me when I said I was going to bring Aesthetics into my practice?

K: Um no go ahead and refresh my memory I’m sure I’m sure it was less than kind but uh go ahead.

A: Yeah you said you know why don’t you just stick to what you know and uh why are you gonna complicate things and uh so you were against it and I said you know I just want to create a place of kind of a One-Stop shop where women can come for their gynecological care for hormone therapy for Aesthetics and it can all be Under One Roof um but you didn’t think that was a good idea.

K: No I definitely didn’t. So uh how do I politely say that I certainly resisted some of these efforts um I was I was late to the party most times um but I’m so glad and you know ultimately I tell you how how proud I am of you all the time but I’m so glad that I did listen to you because I realized in in in short order that you certainly have a comprehensive understanding of what you desire which translates into what women desire and um I I learned after a brief period of time that I just need to get out of your way and let you follow up on your vision and I’ll be the person behind the scenes that helps kind of pull the business aspects together but um yes I’m definitely glad that I I finally relented and and as always you were right how about that? I’m publicly stating that you are right!

A: So I think that is a great ending to our first episode of podcasts of women isn’t that isn’t there anything better that you wouldn’t rather hear of you were right and I was wrong? So I don’t think we can have a better ending thanks for joining us.

Episode 8 Female Viagra

 

Narrator: Welcome to the HealthiHer Podcast. Your host, Dr. Amy Brenner is a board certified OB-GYN, with additional certifications in functional and integrative medicine. This podcast is meant to help women find reliable, relevant information to help them feel better, look better and live better. AB: Hey thanks for joining us for another episode of HealthiHer and today I have one of my very good friends and colleagues and mentor. He’s also been my personal physician for endometrial ablation and some other things, we have Dr. David Schwartz today who’s board certified OB GYN who specializes in women’s health in Cincinnati, Ohio. Thank you for joining us today.

 

DS: Thank you for inviting me.

 

AB: Yeah, so today we’re going to talk about something really fun and probably smoothing women don’t even know that out there. Something called the little pink pill or the female “Viagra®” and this new medication called Addyi. I know you’re an expert in this and have been teaching & educating Doctors on this medication. 

 

DS: So Addyi is for women who just aren’t interested in sexual activity with their partners. I think if we go back to the beginning, we should talk about the disease called hypoactive sexual desire disorder “HSDD”. And HSDD is when the average woman is just not interested in having sexual intimacy with her partner or any partner. It is not normal and it is due to a change in certain chemicals in the brain. Fortunately for women nowadays there is medication called Addyi, the generic is called Flibanserin and it is a pill they would take once a day, usually they take it before they go to bed. It’s not like Viagra that you take it and an hour later it relieves the difficulty and you can have sex. It takes about a month to work.

 

AB: Yeah, so even though it’s called the female viagra , it doesn’t work like viagra. 

 

DS: It doesn’t work like viagra, well what it does is enables the pt to want to have intercourse. Where viagra enabled the male to be able to have intercourse due to anatomy. A woman’s always able to have intercourse, but she may not want to or she may not be interested in it or she may not be able to have an orgasm so she may just be doing it for her partner and not enjoying it. It. The beauty of this medication is that it changes the chemicals in the brain such that women are interested in intimacy. She wants to be intimate  with her partner. She wants to have sex. And in doing so it’s a more enjoyable and fulfilling experience.

 

AB: And you & I both know this is a super common problem. I hear this 10-15 times a day from seeing patients  in the office.

 

DS: There’s no question about it. And we as physicians have to remember to ask the pts about it because most of the time a w coming in2 the office is not going to say hey I’m really not interested in sex and she just goes on through the next 10 or 15 years not fulfilling that part of her relationship. Fortunately  w this new medication & its not really that new it’s been around for a couple of years but it’s becoming more prevalent because we are asking pts more about HSDD Hypoactive sexual desire disorder & we’re finding that many women do have it to some degree and when we provide this med  for them they have a much more fulfilling sexual relationship

 

AB: I think a lot of doctors dont ask the question because in the past there was nothing to do about it. We didn’t have anything. So why ask the question, if you don’t have a solution. Kind of like ordering tests, i don’t like to order blood tests unless it’s going to change my managemens.so dont you think that’s a big reason why a lot of drs dont even ask the question?

 

DS: You’re absolutely right, that’s part of the reason, that’s one of the biggest parts of the reason. The other is since the patients are uncomfortable with the question, some of the drs may be uncomfortable asking the same question too. We also are dealing w women who are post menopausal and have vaginal atrophy or the lining of the vagina is dried up and you want to ask them about that because there are medications there are products that we can use to make the vagina healthier so that couples can have a fulfilling sexual intimacy throughout their lives. 

 

AB: That’s a whole other issue, you and I will have to talk about that on another occasion because I know you have a nice device that can help with vaginal atrophy and dryness

 

DS: That is correct and that is vaginal rejuvenation but we will talk about that at a later discussion. But right now going back to hypoactive sexual desire disorder. This medication called Addyi, women should take before they go to bed at night . One of the side effects is it makes you a little tired, so if they take it before they go to bed , they have a great night’s sleep. Wake up feeling refreshed in the morning. There is a little bit of weight loss associated with it. We do not give patients this medication to lose weight but that’s one of the beneficial side effects .If you have low blood pressure there may be some issues with it causing low blood pressure again that’s why we give it to you before you go to bed at night . but the beauty of it is that all of the studies show an increase in satisfying sexual events such as sexual intercourse and the like .

 

AB: I know when it came out, we had to have patients sign a release that they would never drink alcohol and we had to get special training. And then that went away, can you explain the deal with the alcohol issue. That was the big reason when it first came out, is you couldn’t ever drink alcohol, you couldn’t have a glass of wine out to dinner and so it just limited a lot of use. 

 

DS: So this was originally about 3 or 4 years ago when the drug was first released and approved by the fda. And unfortunately the company at the time did the initial studies, having women take the pill w alcohol . Literally with a couple of [unintelligible] of alcohol and the pill at the same time. And it caused them to be dizzy. If I took that much alcohol that they took, whether I took the pill or not , I would have had the same side effects. Well what the FDA required was that they could not take the pill with alcohol . Subsequent to that other studies have come out and the FDA has erased that warning and like any med you’re not going to take it with alcohol. But if you’re having a couple of drinks and then you’re going to take your Addyi 3 hours later, that is perfectly safe and perfectly normal. So no longer do we have any issues with alcohol and taking this pill 

 

AB: Gotcha, so who is not for? I know you mentioned it’s really for anybody that has decreased libido that they are bothered by, which is the definition of HSDD. But are there any patients that you wouldn’t give it to?

 

DS: Well if patients are on other medications that may decrease their desire for intercourse. If they’re on different types of antidepressants and antianxiety pills, they need to discuss those meds with their physician . Now originally this med came out for premenopausal women, women who were under the approximate age of 50 – somewhere between 50- 60 is when they go into menopause -and it was approved by the FDA for premenopausal women. Currently studies are under review for postmenopausal women. It’s important to know that it works for postmenopausal women too. So in my practice I use it off label for postmenopausal women. And though it’s not approved by the FDA, we’re waiting for that approval and it should be coming soon. There are a number of trials out there showing its effectiveness. Just going back for one second, we are scientists, we look at  evidence based medicine and for me there is evidence based medicine there that works for both pre menopausal and postmenopausal women.

 

AB: Yeah I think that’s important, I like to document and tell ppl this is what  it’s approved for but as physicians we use a lot of things “off label”. Like birth control pills,we use birth control pills for heavy bleeding painful periods but they are FDA approved to prevent pregnancy.

 

DS: That is absolutely correct. So physicians do use meds off label, it is perfectly appropriate and because we are scientists and we’re going to look at the evidence based medicine that’s what’s going to make the most improvement and the best process for our patients.

 

AB: So you mentioned that one of the contraindications is using antidepressants like Prozac®, Zoloft®, Pristiq®, that kind of thing, have you ever switched, cause one of the downsides or one of the biggest downsides of those medications is those decrease your libido and interfere with orgasms. One of the things I’ve done is just getting ppl off those drugs, cause what I found is sometimes people are put on those drugs or women are put on those drugs, maybe they went through a divorce or they had a death in the family and then no one ever looks to take them off of those drugs. And that could be, that alone could be the reason why they have decreased libido. But sometimes ppl are really nervous to come off of those drugs, if you’re not replacing it with something else. So have you ever weaned people off one antidepressant  and put them on Addyi?

 

DS: You know Dr. Brenner, you’re brilliant, because you’re absolutely correct.  And yes, I have weaned them off. Yes I have switched them to other ones. Sometimes they’re on certain medications and they can take the Addyi at the same time. My point is they need to let their physicians know what medications they are on and then the physician, like you have already done, can decide to cut the dose in half or wean them off or put them on Addyi instead of that medication . Or there are other medications, for example, Wellbutrin® has an increase in sexual desire, vs Prozac or Celexa®? Or things like that, But if my point is the pt has to let their physician know what medications they are on. And then we as physicians can use the evidence based medicine that’s out there to help them with their problem.

 

AB:Yeah, certainly I feel like there’s a role for antidepressants for somebody who has true anxiety disorder, depressive disorder, maybe they’ve been hospitalized for one of those conditions in the past. But it amazes me how many ppl are just put on those meds for just some mild mood changes that might have been life related, perimenopause related and those drugs definitely come with their own side effects. So, you’re right, I use a lot of wellbutrin or you know other things that help w mood, supplements and testosterone because those SSRI’s, although they might help w mood, they just squash libido and orgasms.

 

DS: And as you’re saying we can lower the dose, because maybe they needed them when they had some lifecycle event, whether it was a divorce or a death in the family and now they don’t need the med and you can wean them off of it or get them on a lower dose for 6 months and then wean them off it. So there’s many ways of helping them with this issue.

 

AB: One of the things that I like about you and why I think you and I are such good friends is we haven’t stopped learning and we’re always looking for the next thing or what else is on the horizon. But it takes a lot of time and effort to learn these new things. Why do you think other drs aren’t using this or switching to wellbutrin or lowering the dose, why haven’t pts even heard of this medication?

 

DS: Well the first part of your question is the doctors have a certain amount of time to do the physical exam. So a patient comes in and the doctor has 15 mins to get through heart and lungs and menstrual periods and contraceptives and everything. And then to start a whole conversation about hypoactive sexual desire disorder is just going to wreck that drs schedule, so a lot of times they’re not going to bring it up . It may also be that the dr either a male or a woman is uncomfortable talking about that and if the pt doesnt bring it up , then the dr isnt going to bring it up . In addition some drs are stuck in their ways and aren’t interested in learning new things. As you and I have talked about in the past, I’ve always described  when I was in medical school, you take a loaf of bread and I learned that that first piece of bread, that first slice in medical school. And I learned that second slice in my residency, the rest of that loaf of bread, as we slice it, I’m learning during my whole career. They didn’t even have hypoactive sexual desire disorder discussed when I was in medical school  and it’s something that is very common now. And so we need to learn new things , all physicians need to learn new things and like you do, I think that’s the way we can provide better care for our patients

 

AB: Yeah, so, one thing that I like is, I think one of their campaigns of this medication is evening the score because up until this point, was this the first FDA approved medication for women? Or was this the second?

 

DS:There’s another one out called vilessi, which is an injection, in which the patient gives herself a shot & an hour later has an increased desire to have intercourse. I’m not comfortable giving myself a shot to increase my desire for activity. I’d rather take a pill, and that’s why i’m more comfortable with Addyi. But as you asked Addyi is the first med that is approved by the fda for hypoactive se 

 

AB:Well thanks for joining us to talk about the first medication to kind of open the gates to talk more about women’s sexuality. Anything else you want to talk about before you get on to seeing patients?

 

DS:Just I would encourage your patients, my patients and all patients out there to ask their physician about these issues. Bring it up. You should be able to talk to your OB-GYN about any issue relating to your sexuality, and if you’re not you need to find a new physician.

 

AB:I think that’s great advice. You really need to have this open relationship. But you’re absolutely right, I think as OB-GYNs we’re taught [to ask] how many partners do you have sex with, are you having sex w males, females or both? What are you using to prevent pregnancy? But at least when I went through medical school and residency we weren’t taught to elaborate on sexuality – are you happy? Are you having pain? Are you satisfied? All of those types of things. You’re absolutely right, if you don’t feel comfortable talking to your dr you probably need to find a new dr. So on that note, because I know you are comfortable talking with your patients -how can ppl find you?
