In this episode, we dive into Seasonal Affective Disorder (SAD), a form of depression that many experience, particularly during the winter months. This condition can bring on low moods, irritability, fatigue, and changes in sleep or appetite due to decreased sunlight and shorter days. For those affected, SAD often leads to a feeling of "hibernation," where energy levels drop, and motivation wanes.

 

The consequences of SAD are not limited to mood; they can also extend to sexual health. Depression and fatigue can significantly impact libido, lowering sexual interest and activity. It’s common for men to experience a decline in libido or even erectile dysfunction as energy and mood diminish. Furthermore, SAD doesn’t just affect individuals—it can impact relationships too. Partners may notice changes in sexual routine or emotional responsiveness, and this shift can strain intimacy.

Dr. Noble is a pelvic floor therapist who works with all types of people to resolve sexual function challenges. She has specific expertise in helping people address erection and ejaculatory issues. Pelvic floor is a group of muscles and tissues that support the pelvic organs, including the bladder, uterus, and bowel.

One of the important roles of pelvic floor is to contribute to the process of achieving and maintaining strong erections. During sexual arousal, the pelvic floor muscles contract, which increases blood flow to the penis and helps trap blood within the erectile tissue, resulting in a firm and sustained erection. However, weak or dysfunctional pelvic floor muscles may not contract effectively during sexual arousal, leading to difficulties in achieving or maintaining strong erections. The pelvic floor also plays a role in ejaculation. As the muscles contract during orgasm, they help propel semen through the urethra and out of the body.

Good pelvic floor health is essential for sexual function and overall quality of life. Regular exercise and pelvic floor muscle training can help maintain strong and healthy muscles, which can improve sexual function and prevent sexual dysfunction. Pelvic floor exercises, also known as Kegels, are a simple and effective way to strengthen the pelvic floor muscles.

Today we’re joined by cardiologist, Dr. Jeffrey M. Banker, for part three in our four-part series covering a case study that illustrates the complex nature of erectile dysfunction. This case study required a multidisciplinary approach to treating the patient’s erectile dysfunction.

Dr. Banker is a board-certified cardiac electrophysiologist and general cardiologist with LifeBridge Health in Baltimore, Md. His input to this conversation is vital because the patient featured in this case study reported having shortness of breath and fainting spells after sexual activity.

As a quick recap, our podcast host, certified sex therapist Mark Goldberg, has found that too many men assume their erectile dysfunction is caused by either a physical or mental factor. 03 Jun 2024Rogaine and Propecia: Impact on Erections and Sexual Function00:24:18

Today, we have the pleasure of speaking with Dr. Marah Hehemann, an assistant professor of urology and the Associate Program Director of Andrology at the University of Washington in Seattle. Dr. Hehemann specializes in male sexual function and fertility and also practices clinically at the university.

 

Hair loss can be a distressing experience for many men, leading them to seek out treatments like minoxidil (Rogaine) and finasteride (Propecia). However, these popular medications may come with potential side effects that impact sexual function. Minoxidil is unlikely to significantly affect erectile function when applied topically, as only about 1% of the medication is absorbed into the bloodstream. On the other hand, finasteride, which blocks the conversion of testosterone into dihydrotestosterone (DHT), has been associated with a 5-10% chance of negatively impacting sexual function, including reduced libido, erectile dysfunction, ejaculatory problems, and decreased sexual pleasure. These effects have been reported with both the higher 5mg dose used for benign prostatic hyperplasia (BPH) and the lower 1mg dose for male pattern baldness.

Today we are joined by Dr. Ariel Moradzadeh, he is a board certified urologist practicing in Beverly Hills, California. He specializes in men's sexual health and sexual issues. He will enlighten us on the intricacies of vasectomy, vasectomy reversal, and their potential impact on sexual function.

 

A vasectomy, a brief office-based procedure taking 15 to 20 minutes, involves cutting, clipping, and cauterizing the vas deferens, the tube carrying sperm from the testicle. Contrary to common misconceptions, a vasectomy does not impact sexual function, erection issues, testosterone levels, or cause long-term testicular pain. Some individuals, after having a vasectomy, decide to undergo a vasectomy reversal. This is often due to remarriage or a desire to have more children with a new partner. The reversal process is a delicate four-hour operation conducted under general anesthesia and requires precise microscopic techniques. The success rate varies, diminishing over time from the original vasectomy.
