Saccharomyces boulardii is a probiotic fungus, a relative of Saccharomyces cerevisiae used to ferment wine and beer, but better adapted to living in the human gastrointestinal (GI)  tract. Although it does not take up long-term residence, its passage through the GI tract has been associated with substantial health benefits.

Here I read an excerpt from my new book, Super Gut: A Four-Week Program to Reprogram Your Microbiome, Restore Health, and Lose Weight, specifically chapter 5, Mind Your Mucus.

Before we get to the business of restoring lost microbes and pushing back unwanted species from the upper GI tract, we can also take action to strengthen the mucus barrier. Some strategies I advocate can bring relief from issues like abdominal discomfort and bloating within minutes to hours of adopting. I discuss:

  • What role mucus plays in gastrointestinal health

Over the past 30 years, I have made a number of blunders in health. But I always made it a point to learn from my mistakes. Doing so taught me a number of important lessons in health and nutrition, lessons that revealed just how little conventional ideas on health yielded and how doing things very differently yields enormous health advantages. In this episode of Defiant Health, I therefore recount:

  • How cutting dietary fat and eating plenty of "healthy whole grains" leads to metabolic disasters
  • How the death of my mother from sudden cardiac death showed me how inadequate heart procedures are for dealing with coronary heart disease
  • How rejecting cholesterol and resorting to more advanced lipoprotein testing illustrated how not cutting fat and banishing all grains and sugars yielded huge advantages and led to methods to reduce coronary calcium scores
  • Despite the enormous advantages of the Wheat Belly grain-free lifestyle, there were people who had residual health issues such as stalled weight loss, food intolerances, or autoimmune conditions that finally responded to more vigorous efforts to rebuild a healthy microbiome
  • How identifying, then replacing, microbes lost from the modern human microbiome yields extraordinary benefits such as deeper sleep, restoration of youthful muscle, smoother skin with fewer wrinkles, and preservation of bone density.

Conventional "solutions" for depression are largely inadequate: often ineffective, with side-effects that include weight gain, sexual dysfunction, and suicide.

Evidence is increasingly pointing towards the intestinal microbiome as a source for depressive symptoms. This tells us that prescription drugs such as the selective serotonin reuptake inhibitors, or SSRIs, that only address a step in the pathway of serotonin metabolism, do not address the underlying cause of many, if not most, cases of depression: disruption of the intestinal microbiome. Species such as Turicibacter sanguinis are proving to be crucial players in mood, while bacterial and fungal overgrowth, SIBO and SIFO, can also be common contributors to depression. The bad news: these disruptions of the intestinal microbiome are common and widespread. The good news: there are steps you can take to remedy the situation and thereby potentially have marked effects on mood and depression.

Small intestinal bacterial overgrowth, or SIBO,  in which unhealthy bacterial species have proliferated and ascended up the 24-feet of small intestine, is now epidemic, an epidemic that underlies common health struggles that include weight gain, obesity, pre-diabetes and type 2 diabetes, autoimmune diseases, irritable bowel syndrome, fibromyalgia, and numerous other health conditions.

Solutions for SIBO typically involve antibiotics, despite being a condition that was likely caused, or at least worsened, by antibiotics in the first place. But I believe that we may have a solution that involves cultivating specific microbial species as a yogurt, something I call "SIBO Yogurt" that provides microbes with specific characteristics that may reduce or eradicate SIBO.

Lactobacillus reuteri is one of the most important bacterial species in the human microbiome. Yet it has been lost by the majority of Americans due to this microbe's susceptibility to common antibiotics like amoxicillin and ampicillin. One of the terrific benefits of L. reuteri  is its ability to provoke release of the hormone, oxytocin, from the brain.

Until recently, there was no user-friendly way to boost oxytocin. It cannot be ingested orally because the digestive process breaks it down. It can be injected but lasts only about 3 minutes. It can be administered via nasal inhaler but requires 3 squirts four times per day in each nostril, with each dose lasting only 60-90 minutes.

We now know that restoring this lost microbe, L reuteri, is key: restore it and you create a source of endogenous oxytocin.

Oxytocin is the hormone of love and empathy. It causes you to engage with other people, take an interest in them, understand their points of view better--can you appreciate how important this hormone can be for human social interaction?

Spread the word: you can restore L reuteri and thereby oxytocin and perhaps make the world a little better place.

In this episode of Defiant Health, let’s talk about some new insights into a very serious issue that afflicts many females: repeated urinary tract infections, or UTIs. This can happen to a female at any age, but becomes an especially frequent problem as women get older. Of course, the conventional “solution” is to prescribe course after course of antibiotics that disrupt the entire body’s microbiome and not uncommonly leads to dysbiosis in the colon and other organs, SIBO, vaginal candida infections and other problems. 

A number of new observations have emerged that are completely changing the understanding of this common problem. One new and exciting insight is that the urinary system has its own unique microbiome, i.e., a collection of microbes that normally colonize the bladder and other portions of the urinary system, despite having been regarded as sterile for many years. It is disruption of this urinary microbiome that is a major contributor to repeated UTIs. It’s also become clear that the vaginal microbiome is a major determinant of urinary health. Let’s talk about why this happens and what steps you can take to stack the odds in favor of putting a stop to this annoying and health-impairing issue.

While the official definition of constipation is having 3 or fewer bowel movements per week, I would say that the official definition is way off. 3 or fewer bowel movements per week is an awful standard, one that is likely to impact health in other ways, as well as being uncomfortable.  And the standard remedies for constipation have problems, real problems. So let’s cut through the nonsense and talk about how and why constipation develops and what you can do about it without resorting to absurd and harmful conventional solutions.

In this episode of Defiant Health, let’s discuss how the U.S. Dietary Guidelines for Americans got a lot of things wrong. Of course, I have previously discussed on many occasions the hazards of a diet reduced in fat and rich in “healthy whole grains,” a style of eating that increases the likelihood of weight gain, fatty liver, type 2 diabetes, even increases cardiovascular risk, largely due to amylopectin A carbohydrate of wheat and grains, as well as gliadin protein-derived opioid peptides that are appetite stimulants. This was the focus of my Wheat Belly series of books. 

But the dietary guidelines that encouraged Americans to reduce fat intake also led to some inadvertent blunders that pose real health consequences. Perhaps we would never have learned about these issues if we had not made the national mistake of reducing fat. So let’s talk about these inadvertent blunders and what you can do to compensate for them.

In this episode of Defiant Health, I’d like to talk about how the intensity of love, affection, and other emotions and social interactions have lost intensity. The love of today, I’m going to argue, is less involved, less intense, less soul-wrenching, involving more superficial commitment than it used. If we could compare the love and affection of, say, 50, 100, 1000 years ago, I am going to illustrate how our current version of these emotions is an anemic, watered-down version. And, along with this change, I’m going to propose that many of the social phenomena that we are all familiar with—-divorce, social isolation, loss of community, increasing suicide—-are all related.

The good news: I believe that you can fix this broken part of the human experience, both for yourself and for the people around you.

I met Dr. Ozment during one of my two-way weekly video meetups on my DrDavisInfiniteHealth.com where she provided a presentation on the oral microbiome. I was impressed with her tremendous command of this area that drives home the message that oral health and the oral microbiome are major determinants of overall health. She discusses, for instance, how the oral microbe Porphyromonas gingivalis plays a role in Alzheimer’s dementia, and Fusobacterium from the gums is proving to be a major cause for colon cancer---all factors you are able to address. I believe you will find Dr. Ozment’s interview informative and enlightening.

A graduate of Oklahoma University College of Dentistry, she served as adjunct faculty there for seven years. Although I know that my programs, whether you call it Wheat Belly, Wheat Belly 10-Day Grain Detox, Undoctored, or my most recent umbrella program, DrDavisInfiniteHealth.com and the Super Gut book, although I know that my programs work, they don’t always work for everybody. We cannot, of course, influence genetically determined factors. But we have enormous control over the other factors. 

The sad truth is that a conventional diet and modern healthcare advice accelerate aging via these mechanisms. Just look around you the next time you’re in a supermarket or mall—-do you see slender, vibrant, vigorous people who look younger than their years? Or do you see overweight, tired, people who look beaten down by life, older than their chronological age? If you want to impact the rate at which you age you therefore have to reject much conventional wisdom and adopt strategies that are somewhat unconventional. So let’s discuss the factors that you have control over that influence the rate at which you age. I believe that some pretty spectacular results are achievable that will cause the people around you to ask “What are you doing to look so good?

I’m always impressed that, because TV ads, misinformed dietitians, and other dietary conversations all urge you to consume grains, it’s easy to become complacent, often forgetting how your health improved by banishing all wheat and grains. If this is new to you, be prepared to be surprised, even angered, by coming to understand what a toxic, chronic poison modern wheat is, this thing created by crude genetic manipulations, and its destructive effects on your health. 

In the early days of my Wheat Belly message, some people dismissed it as nothing more than promoting a gluten-free diet, or another version of a low-carb diet. But I believe that, once you hear the full arguments based on scientific evidence, you will come to understand the magnitude of mistake made when humans first began to incorporate the seeds of grasses—that’s what wheat is—-into the human diet and the enormous impact it has on health, despite being advised by virtually all agencies in the business of offering dietary advice to include it in every meal, every day.

You have likely observed that humans, especially overweight humans, come in a variety of shapes and sizes. We all know that excess body fat is not a good thing for health, nor for self esteem. But where fat is distributed on the body plays a big role in whether excess fat increases risk for conditions such as type 2 diabetes, coronary disease, stroke, cancer, cognitive decline, dementia, and premature death—or whether it is only a cosmetic issue and a challenge to weight-bearing joints but not for all the common chronic conditions mentioned. 

Doctors often rely on a measure called the body-mass index, or BMI. BMI is obtained through a simple calculation (or referring to a table that lists BMIs) of height divided by weight. A BMI of 18.5 to 24.9 is considered normal, overweight is 25 to 29.9, over 30 is obese. But BMI suffers from a fundamental flaw: It assumes that body fat is distributed uniformly throughout the body which, of course, it is not. They also rely on total weight which also does not necessarily give you any real health insights. So let’s talk about how you can do better than the doctor to make determinations about whether the fat on your body is interfering with your health and longevity or whether it is not.

In this episode of Defiant Health, I ask the question: Is your skin aging faster than it should? If you are engaging in conventional ideas about diet, it probably is. But there are indeed strategies you can adopt that slow or even reverse signs of skin aging.

Those of you who are concerned about skin health and appearance likely engage in applying products externally, products such as retinol or hyaluronic acid serums for smoother, moister skin. But wouldn’t it be better to view skin as the outward reflection of internal health? Because it is. If you are inflamed, it will show on the skin. If you have liver or kidney problems, it will show on the skin. Wouldn’t it therefore be better to address internal health first? Or do both: obtain healthier skin that reflects better internal health complemented by your topical efforts. Because, after all, applying products topically does not make you or your skin healthier; it just generates an outward change in appearance.

Many of you following my conversations are making yogurt (not really yogurt, of course, nothing like the products sold in stores) making yogurt with Lactobacillus reuteri, a microbe lost by nearly all modern people. Many of you have reported wonderful skin effects, as well as other health benefits such as increased strength, deeper sleep, increased libido, increased sense of well-being. Many report increased skin moisture sufficient to reduce or do away with moisturizing creams, and reduction of fine wrinkles. 

So this got me thinking about the power of replacing other factors lacking or absent from modern life that might influence skin health and appearance. So, in this episode of Defiant Health, let’s talk about other factors lacking in modern life that you can replace for improved skin health. This was also my motivation for creating the Gut to Glow product that I personally formulated that I will tell you more about.

In this episode of Defiant Health, let’s talk about why cutting calories in your diet to lose weight is a really bad idea, a practice that yields substantial health problems and even reduces, even eliminates, your ability to lose weight in the future. All the forms that this can take—-reduced portion size, using smaller plates or bowls, pushing the plate away, even pharmaceuticals and bariatric procedures, i.e., all forms of reducing calorie intake, result in significant health problems. It can result in long-term increased risk for frailty, osteoporosis, falls and fractures, and impairs your ability to lose weight again in future, not to mention people who cut calories almost always regain the weight they initially lost. 

So let’s discuss why cutting calories is not just an ineffective strategy for weight loss, but can actually be dangerous, and how you can lose weight, if that is among your goals, but not experience any of these adverse health problems.

In this episode of Defiant Health, let’s discuss Why cholesterol testing, despite being promoted by nearly all practicing physicians, achieves almost nothing in reducing risk for heart disease. Early research, much of it conducted at the National Institutes of Health, NIH, in the 1950s and 1960s intended the measurement of cholesterol in the bloodstream to be a crude and indirect marker for the real causes of heart disease, i.e., the lipoproteins, lipid- or fat-carrying proteins, in the bloodstream that do indeed cause coronary atherosclerosis and thereby dangerous events such as heart attacks. Cholesterol in its various fractions such as total cholesterol and LDL, “bad”,” cholesterol were therefore intended to be indirect markers. But they got construed as causes. The the pharmaceutical industry got into the game and saw an enormous profit opportunity that eventually led to statin cholesterol drugs, one of the biggest financial success stories in pharmaceutical history. Yet more than 80 million Americans now reduce their cholesterol by taking statin drugs and there has been almost no reductions in cardiovascular events like heart attacks and hospitals continue to add multi-million dollar heart centers, since heart disease continues to be the number one money maker for healthcare systems. A recent clinical trial that we shall discuss that reducing cholesterol to extremely low levels may even increase the risk of cardiovascular events and death.

So let’s talk a bit about the history of cholesterol, why it’s not what it’s cracked up to be, and why the real tragedy of cholesterol is that it took eveyone’s attentions away from the real causes of heart disease that, in truth, are easy to identify, easy to manage, and  require zero prescription drugs.

For over 50 years, people in conventional healthcare have urged Americans to reduce their intake of saturated fat, eat more “healthy whole “grains,” and take statin cholesterol drugs to further reduce cholesterol. Has it worked? No, it absolutely has not worked. As I’ve discussed previously, both in my Defiant Health podcast as well as my DrDavisInfiniteHealth.com blog, and of course my books, conventional strategies have little, if any, impact on the incidence of heart disease. And, in some instances, conventional strategies increase risk, especially their dietary advice. 

Witness, for example, that more than 80 million Americans now take a statin cholesterol drug, yet the incidence of heart disease has not gone down and hospitals do a very brisk business in heart procedures while the pharmaceutical industry continues to profit by dispensing drugs that hardly achieve anything. 

The recent Fourier trial has brought the cholesterol paradigm to its knees by taking it to its extreme. In this very large clinical trial involving over 20,000 participants, every participant was on a statin drug but half were also given the injectable drug Repatha to further reduce LDL cholesterol. So half of participants were on both a statin drug and Repatha injections. Participants receiving statin + Repatha had LDL cholesterol drop from 92 mg/dl to 30 mg/dl, an extremely low value. If the “LDL cholesterol is bad” argument is valid, then reducing it to this extremely low value should yield a significant reduction in heart disease—and it did not. There was a barely measurable 1.5% reduction in cardiovascular events over 2 years and, while the numbers have been a topic of debate, there was a likely increase in overall death in people receiving Repatha—not to mention that the drug costs around $600 per month per person. And, of the 13,000 people on Repatha plus statin with LDL cholesterols of 30, 1300 of them died, had heart attack, stroke, or required a heart procedure over the 2 years—in short, reducing cholesterol, even to extremely low values, does not work. 

As I have often said, the real tragedy of focusing on cholesterol is that it takes everyone’s attentions away from the REAL causes of heart disease. Among them: VLDL, or very low-density lipoproteins. 

So, in this episode of Defiant Health, let’s talk about VLDL particles and why, if your interest is in minimizing or eradicating risk for heart disease that remains the number 1 killer of men and women in the U.S., you need to understand VLDL.

The science supporting our ability to shape our bodies, to control how much muscle we have, where fat is deposited, which fat stores are mobilized when you lose weight, in other words what we call “body composition” has recently made some important leaps forward. This becomes especially important during any effort to lose weight. If you have been following my Defiant Health conversations, you know that reducing calories virtually guarantees disaster, as it causes loss of lean muscle mass, the primary determinant of metabolic rate. You lose, say, 30 pounds, by reducing calories, about 10 pounds of the lost weight is muscle. This causes your metabolic rate to drop typically 10-25%. It means that, even if you maintain a low calorie intake, you will regain the weight. And the weight you regain is mostly fat, meaning you are worse off than when you began the diet. 

So, in this episode of Defiant Health, let’s take a more scientific approach to shifting body composition, including losing weight, to gain advantages for better appearance, as well as health, with none of the booby-traps set by conventional weight loss methods.

Do you have the sense that people are acting more selfishly, less considerately, less empathetically, less affectionately as time passes? How often can we blame incidents such as someone cutting you off in traffic, or the break up of a relationship, or the abandonment or neglect of children on narcissistic tendencies? 

Narcissistic behavior has been around for as long as humans have walked this planet. But, remarkably, psychologists have been charting the prevalence of narcissistic behavior since the 1960s using instruments or questionnaires such as The Narcissistic Personality Inventory, or NPI, or the Pathological Narcissism Inventory, or PNI. They have documented an astounding rise in narcissistic behavioral traits over the past half-century. The chart chronicling NPI scores, for instance, shows a steady upward climb at a 45-degree angle over the last 40 years. People responding positively when asked whether they felt that “I am an important person” increased from 12% in 1960 to over 80% in our time.

Ask most doctors, “Can arthritis be prevented or reversed?” Most will say no, or say that it’s genetic and therefore unstoppable. The better informed will say “Lose 10 pounds.” But that’s about as far as it goes and they tell you that, when the pain starts, take ibuprofen or naproxen, submit to steroid or hyaluronic acid injections, then undergo joint replacement surgery. Prevent the disease in the first place? No way. 

But there are indeed strategies that are natural, available, and effective that can prevent or reverse arthritis, provided you have not progressed to the most advanced “bone on bone” stage in which all the cartilage has been eroded and gone. 

When I say “arthritis,” I am referring to the most common and prevalent form, osteoarthritis. This discussion does not therefore include forms of arthritis such as rheumatoid arthritis or psoriatic arthritis. Those less common forms are indeed important, but require a separate and distinct conversation and are, by the way, also reversible. Let’s now however focus on this most common form, osteoarthritis. 

For years, osteoarthritis, especially of the knees and hips, was viewed as nothing more than wear-and-tear, the end-result of years of use. But that is not true. There are numerous metabolic and physiologic phenomena that may be as important, likely MORE important than simple wear and tear. So let’s explore these questions to show you that, yes, osteoarthritis can be prevented and can be reversed.

It’s happening with alarming frequency: Someone has routine blood work done and an increase in liver markers, AST, ALT, and sometimes GGT, are noted. The doctor sometimes then orders a liver ultrasound, CT, or MRI and infiltration of the liver with fat is identified, the situation called fatty liver. If caught in later stages, fibrous tissue can be seen replacing normal liver tissue, meaning that someone is progressing to cirrhosis, a very bad condition. 

Add fatty liver to the widespread epidemics of overweight, obesity, and type 2 diabetes. People with these conditions are also the ones who typically also have fatty liver. In other words, fatty liver is, in the majority of cases just another facet of obesity and type 2 diabetes. The risk is that fat infiltration of the liver can progress to more serious liver diseases such as cirrhosis and liver cancer. Fatty liver is a situation that applies to about 30-50% of the U.S. population, even higher in some groups of people. 90% of people undergoing bariatric procedures to lose weight, for example, have fatty liver.  

Most doctors will then tell you that there is nothing you can do about this situation. Perhaps lose a few pounds, that might help. But he/she tells you that they will monitor liver tests as you progress to cirrhosis and liver failure, then prepare you for a liver transplant. These is awful advice. Cirrhosis is a very serious and often fatal condition with extremely messy complications such as esophageal varices, i.e., varicose veins in the esophagus that rupture, causing profound hemorrhage. It causes jaundice and accumulation of ascites, or fluid in the abdomen, causing grotesque enlargment of the abdomen. And liver transplant is a terrible procedure that only adds a few years to your life if it is successful.

 The truth, however, is that most people can reverse fatty liver on their own within a few weeks without drugs, without procedures, without surgery. So let’s talk about that.

Dietary guidelines advise us that a diet low in fat and rich in “healthy whole grains” helps maintain healthy weight. Is this true? It absolutely is NOT true. This belief is based on flawed evidence, questionnaires, for instance, that asked people questions about their diet. What did you eat for breakfast on Monday? You scratch your head and search your memory. You say something like “I had a bowl of stone-ground oatmeal in skim milk with some sliced strawberries.” What did you have for lunch? You say “I think that was the day i ordered out for lunch and got a submarine sandwich with cold cuts and cheese.” What did you eat for dinner, and so on. They query you about the foods you consumed over several days. Several years pass and they re-contact you and ask whether anything has changed in your health diagnosed, perhaps, with hypertension, or coronary heart disease, or colon cancer. They then try to correlate the development of these conditions with your diet from 5 or 10 years earlier. 

As you can imagine, this is a deeply flawed way to study the health effects of diet. In one real study, for instance, females who chose more whole grains gained 10 pounds over several years, while females who chose more white flour products gained 12 pounds. Interpretation: whole grains help control weight. I hope you see the fallacy in logic here. A better interpretation would be that white flour products are associated with weight gain, while whole grain products are associated with weight gain, also, but just a little less than white flour products. Yet evidence like this is held up as proof that whole grains help control weight. This is called observational evidence that is weak and commonly yields false associations. After all, your recall for the contents of your meals is sketchy and often incomplete, diet changes over the years, often substantially. Trying to associate the development of disease with the diet followed several years ago is, consequently, nearly useless. 

So let’s not talk about how whole grains help manage weight, but how ALL foods containing wheat products make you fat.

It’s happening over and over again: Participants in my program look, act, and feel younger and younger, the longer they engage in all the strategies of my programs. Just the other day, for instance, I had a video conference with a woman who was in good health, but had added the Lactobacillus reuteri yogurt one year ago, the yogurt that we make that restores this important keystone species to your gut microbiome, triggering release of the hormone oxytocin from the brain. The last time I saw her one year ago at age 62, she looked pretty good: vigorous, happy, and appearing several years younger than her chronological age. One year later, she looks like someone in her 30s: smooth moist skin without wrinkles, speaking and moving with the vigor and rapidity of someone much younger. She also reported that she was happier, experiencing better feelings of well-being, and was stronger.

Monitoring blood glucose, or blood sugar, can be a useful tool in achieving a number of important health effects. You can apply it to accelerate weight loss, for instance, or accelerate the reversal of type 2 diabetes, high blood pressure, triglyceride blood levels, fatty liver, slow many of the phenomena of aging, and other effects. And you can achieve this by following what I call the blood glucose “No change rule,” meaning allowing no change in blood sugar from just prior to a meal to 30-60 minutes after the start of the meal. 

This is, of course, completely contrary to conventional thinking on how to use blood glucose measurement, nearly always used to assess your response to drugs that reduce blood sugar. In other words, people are told by doctors that the usefulness of blood glucose monitoring is to assess whether the drugs you’ve been given to reduce blood glucose are doing the job and controlling blood sugar. But that is NOT what we are talking about here. We are talking about how to put blood glucose monitoring to use to achieve significant health benefits.

Cholesterol testing is among the most common blood tests run in conventional healthcare, yet it is a perennial source of confusion and, to be honest, abuse by doctors who don’t appreciate the full implications of the values. It may surprise you to know that doctors focus on the least helpful values such as total or LDL cholesterol while often ignoring the two genuinely helpful values, HDL cholesterol and triglycerides. HDL for instance is packed with useful insights into your health, as it is a reliable index of metabolic health. If you have been following my discussions in this Defiant Health podcast or in my blog, DrDavisInfiniteHealth.com, or my Super Gut book, you know that I have been arguing that there is an epidemic of SIBO going on in the U.S., an epidemic involving every 1 in 2 people. It is an epidemic responsible for an astounding amount of health struggles. 

The more we all come to understand small intestinal bacterial overgrowth, or SIBO, the more you have to appreciate all the varied ways this situation can show itself. First of all, for those of you unfamiliar, what is SIBO? SIBO refers to the situation in which modern people, due to exposure to multiple courses of antibiotics and other disruptive factors, have lost hundreds of beneficial bacterial species in our colons. These beneficial species were responsible for suppressing the proliferation of unhealthy microbial species, mostly fecal microbial species such as E. coli, Salmonella, and Citrobacter. The loss of beneficial microbes allows unhealthy fecal species to proliferate, then ascend up from the colon where they originated and into the 24-feet of small intestine. The small intestine is poorly-equipped to deal with this invasion of fecal microbes. The small intestine is, by design, permeable, since that is where the bulk of nutrients such as vitamins, minerals, and amino acids are absorbed. But trillions of fecal microbes in the small intestine, microbes that live for only a few hours, live and die and release their toxic components that can gain access to the bloodstream. One of the primary toxic components is called endotoxin. When endotoxin enters the bloodstream, that’s called “endotoxemia” and endotoxemia is now recognized to be the primary way in which gastrointestinal microbes are able to exert effects tin every organ of the body. It means that SIBO and thereby endotoxemia can be experienced as a wide variety of health problems and diseases. 

So, in this episode of Defiant Health, let’s discuss whether this is a situation that applies to you or to someone close to you and what steps you can take to undo these harmful effects.

Personally, I take no prescription medications. I do not take a statin drug to reduce cholesterol, nor baby aspirin. I take nothing for high blood pressure, acid reflux, or irritable bowel syndrome. I take nothing for headaches, pain, or sleep—not a single prescription medication.

Yet my blood pressure is typically around 108/72. My triglycerides are 40 mg/dl, HDL cholesterol is 94 mg/dl, and I have zero small LDL particles. My fasting glucoses are in the 80s mg/dl, last HbA1c was 4.8%, my liver markers are all at the low normal range with no fatty liver suggested. In other words, I have perfect metabolic health without reliance on any medication. 

The majority of the followers of my programs likewise enjoy perfect metabolic health without reliance on any prescription medications. Of course, there are people who adopt the strategies of my program after incurring some form of irreversible damage such as permanent loss of pancreatic beta cells that result in type 1 diabetes, Hashimoto’s thyroiditis that results in impaired ability to produce thyroid hormones, or loss of stomach parietal cells that results in absence of stomach acid, hypochlorhydria. So key is to adopt these strategies before such permanent damage is done. People who have incurred such damage will therefore need to take insulin, thyroid hormones, or engage in efforts to acidify the stomach. But barring the development of such irreversible changes, the majority of us on my programs have no need for pharmaceutical medications. 

So, in this episode of Defiant Health let’s discuss how and why, by adopting just a handful of strategies, how you can enjoy perfect metabolic health with no reliance on prescription medications.

21 Feb 2024How to Stop "Treating" Health Conditions00:31:24

It’s a concept that many people struggle to embrace or understand. But once you ‘get” it, a lot of issues in health will become clearer and you can save yourself a considerable amount of time, effort, and money. The idea is to get away from the idea of “treating” health conditions. This is the approach used in pharmaceuticals and employed by most doctors. If, for instance, blood glucose is high, drugs like insulin, metformin, or a GLP-1 agonist like Wegovy or Ozempic is introduced to force blood glucose down. If blood pressure is high, a drug such as a thiazide diuretic, beta blocker, or ACE inhibitor is prescribed to for blood pressure down. If there is an inflammatory or autoimmune condition, they pick a single inflammatory mediator and block it. But there are major drawbacks to approach health in this way that I will discuss in this episode of the Defiant Health podcast. 

Instead, I would like you to consider a completely different approach. Rather than try to improve some health measure with a pharmaceutical or even a medical procedure, I would suggest that we instead address and correct the common factors that allow such health conditions to emerge in the first place. I’ll explain what that means. But it means that this approach can not just reduce blood glucose or blood pressure or reverse many inflammatory or autoimmune conditions, but also result in weight loss, loss of abdominal fat, reduce inflammation, improve insulin resistance, restore youthful characteristics, smooth your skin, restore healthy musculature and provide other effects that yield benefits that go beyond the original issue. It is not uncommon, for example, to take this approach for, say, weight loss but find yourself no longer a type 2 diabetic or pre-diabetic, or no longer hypertensive, while enjoying relief from multiple forms of skin rashes, irritable bowel syndrome, fatty liver, anxiety or depression and hundreds of other conditions. 

So, in this episode of Defiant Health, let’s discuss this radically different approach to health.

I recently had an experience in which I had to take an antibiotic. Those of you who have been following my Defiant Health podcast discussions, or have been reading my thousands of blog posts on DrDavisInfiniteHealth.com, or my books, especially my latest Super Gut book, know that I advocate for absolutely minimizing our reliance on antibiotics, as they are extremely destructive to the microbiome. Antibiotics disrupt microbial composition of the gastrointestinal tract, the airway, the sinuses, reproductive organs, urinary tract, and other body parts with serious potential long-term consequences. 

Well, I had a failed root canal and crown put in more than 30 years ago. It recently failed and I had the tooth extracted, following by insertion of an implant. Because the periodontist had to drill into the jawbone and insert an anchor for the implant, he prescribed an antibiotic to prevent osteomyelitis, or infection of the jawbone that can be catastrophic. I therefore spent weeks dreading this process as, in past, I’ve had some nasty experiences with antibiotics: GI distress, abdominal discomfort, skin rashes. But I was especially dreading the awful insomnia that I feared I’d experience with loss of the L. reuteri that I had been supplementing for several years that converted me from a severe chronic insomniac to a deep sleeper. 

To my great surprise, I experienced none of these effects and I wanted to share with you why and how I believe I, and my listeners who have engaged similar strategies, avoided these effects from antibiotics. It’s my n of one experience, along with some of my listeners, so it’s proof of nothing. But, because these concepts are evolving rapidly with very little formal evidence, it’s worth at least being aware and even trying some of these strategies.

But the microbiome is proving to be an exceptionally powerful factor in causing or preventing cancer, in some unexpected ways. For instance, it has become clear that many instances of colon cancer get their start in the mouth. It is also becoming clear that numerous forms of cancer, from breast cancer to prostate cancer, originate with the composition of microbes in the gastrointestinal tract. It is the one common thread among many forms of cancer. This single insight has the potential to give you enormous control over your lifetime risk for these diseases.

You have likely heard the headlines: cognitive impairment and Alzheimer’s and other forms of dementia are on the rise. Part of the increase is due to an aging population. But there are reasons that are adding to the increase beyond this. The epidemics of insulin resistance (recall that dementia is often labeled “type 3 diabetes,” meaning that the brain becomes insulin resistant), SIBO endotoxemia, the ubiquity of heavy metal toxicity, and deficiencies of important nutrients such as omega-3 fatty acids and vitamin D are likely making their own contributions.

There are a number of strategies that you can adopt, however, that hold potential for reducing your risk for developing cognitive impairment, or at least delay its onset. There are also various factors that you can supplement, some of which hold potential for improving cognition and, in some cases, slow or help prevent cognitive impairment and dementia. But this area is filled with imprecise thinking and, sadly, misleading marketing. Despite having written 7 books in the Wheat Belly series pointing out the harms that this “food” has inflicted on people, harms that doctors nearly always misdiagnose as an opportunity to “treat” with pharmaceuticals and procedures, questions and ideas to the contrary continue to crop up. Some of these ideas are simply due to the barrage of advertising that comes from Big Food and Big Agribusiness, industries that continue to make a lot of money by propagating fictitious claims that grains are healthy. Some of it is due to the continued repeated urgings by the U.S. government, insisting that grains should remain a staple of the American diet. Dietitians stick to their argument that all foods fit into a healthy diet, including sugary soft drinks, candy bars, and grains, provided it’s all consumed “in moderation” and in the context  of a “balanced diet.” And sometimes it’s just that some people are looking for novelty, the newest and shiniest thing to titillate their dietary habits. And make no mistake: the grain industry is extremely well funded, keeping many dietitians, nutritionists, publicists, etc. on the payroll to repeat the same misconceptions over and over again: “There is no evidence that wheat is bad for you, with the caveat that you eat the right amounts as recommended.”

So let me dispel common myths and misconceptions that continue to swirl and threaten to derail you off the path of health and slenderness. (As in my books, my use of the term “wheat” encapsulates all forms of grains, as they share numerous characteristics.)

The oral cavity is therefore teeming with hundreds, perhaps thousands, of species of bacteria, fungi, Archaea, and viruses. It is a virtual tropical jungle of life. 

But what is especially remarkable is that the oral microbiome is an ecosystem of life that plays a role not just in dental health (cavities, tooth loss, gingivitis, periodontitis, breath odor) but also in contributing to causing diseases in other parts of the body such as cardiovascular disease, colon cancer, reproductive and cognitive health.

So, in this episode of the Defiant Health podcast, let’s consider how the oral microbiome contributes to all sorts of health issues. You’ve likely heard the headlines gushing about the weight loss effects of the GLP-1 agonist class of drugs, agents that go by names like Wegovy, Mounjaro, Rebelsus and others. Doctors are declaring these drugs as breakthroughs or even magical, witnessing the rapid and dramatic weight loss they can achieve. 

People appear willing to accept the side-effects of nausea, vomiting, even bowel obstruction, a catastrophic complication, or thyroid cancer, or pancreatitis that can irreversibly damage your pancreas and make you a type 1 diabetic. They also accept the costs that vary widely, depending on who dispenses it, but not uncommonly hundreds of dollars, even over a thousand dollars a month. And doctors jump at the chance to prescribe these drugs, as they often have arrangements with the dispensing pharmacy for kickbacks in one form or another. Prescribe the drug, make a lot of money, for many doctors an irresistible temptation. 

People lose weight and even can become non-type 2 diabetic and modestly reduce risk for heart disease—so what’s not to like? 

Plenty. That is the topic of discussion today on the Defiant Health podcast, a place where you hear ideas you cannot hear anywhere else because we dig deeper, are not swayed by big payoffs, and thumb our nose at conventional wisdom. Sleep issues represent a major struggle for millions of people: too little sleep, trouble falling asleep, trouble staying asleep, early morning awakenings followed by difficulty falling back asleep. Some people complain that, despite having a full night of sleep, they awake feeling like they didn’t sleep at all, struggling with sleepiness, fog, and fatigue all through waking hours. There are also known associations of health conditions such as irritable bowel syndrome and Parkinson’s disease with disruptions of sleep. 

The science that explores the role of gastrointestinal microbes in sleep are yielding new and exciting lessons. Recently, for instance, lack of sleep has been shown to adversely impact the composition of the GI microbiome, i.e., there is a bidirectional cross-talk: the brain communicates with the gut, the gut communicates with the brain in ways that influence sleep. But new observations are also emerging from our fermentation projects, especially our yogurts in which we ferment unique microbial species to very high counts, increasing the number of microbes a hundred, or even a thousand-fold, efforts that are yielding new lessons that have not yet been formally described in human scientific studies. So let’s discuss some of these interesting lessons that are emerging involving sleep.

Atrial fibrillation, or A fib for short, is the most common heart rhythm issue that plagues people. About 30% of people will experience this rhythm disorder at some time in their lives, an issue that becomes more and more likely as we age. 

The rhythm is typically experienced as the sudden onset of breathlessness and lightheadedness, and sometimes chest pain, even while you are sitting or relaxing. This is because, with A fib, the heart rate is typically in the range of 150-220 beats per minute, way above the normal resting heart rate of 60-70 beats per minute. It’s the result of chaotic electrical signals originating from the atria of the heart, the normally thin-walled sacks at the top of the heart on the right and left side, i.e., the right and left atria. Beyond the symptoms of breathlessness and lightheadedness, A fib puts you at risk for stroke—not mini-strokes, but large catastrophic strokes. This is because there is a passive sack or appendage attached to the side of the left atrium called the left atrial appendage. Because during a bout of A fib, the atria stop their normal contractions in synchrony with the powerful ventricles, the main pumping chambers of the heart, blood stasis or standstill occurs, allowing the formation of blood clots in the left atrial appendage. Should the blood clot fragment or dislodge, it can go to your brain, resulting in a catastrophic stroke. Over 30 years ago, I help set up the first CT heart scan device in Wisconsin, one of the first in the entire midwest. This was so long ago that it was really an electron beam tomogrraphy, or EBT, device that predated the more recently developed CT devices. The point is that these devices acquired images quickly, about 1/10th of a second. This is important because the heart is perpetually in motion through various cycles of its beating rhythm, with motion also provided by breathing. But these devices allowed us to precisely quantify calcium in the coronary arteries of the heart, the arteries that close and cause heart attacks. My friend, Dr. John Rumberger while at the Mayo Clinic, performed studies demonstrating that calcium consistently occupies 20% of total atherosclerotic plaque volume in the coronary arteries. In other words, quantifying calcium in the coronary arteries served as a gauge or dipstick for total atherosclerotic plaque in the heart’s arteries.

Some years later, cardiologist Dr. Arthur Agatston, whose name you may recognize from his popular South Beach Diet books, developed a scoring system for coronary calcium, yielding something that came to be called an “Agatston score”: the higher the Agatston or calcium score, the more atherosclerotic plaque was present in the coronary arteries. Subsequent research has shown that the Agatston or calcium score is, by a long stretch, the best predictor of future cardiovascular events, far better than crude measures like cholesterol and these scores, in the 30+ years since my team and I started doing these scans, have become well-established as predictors of cardiovascular events like heart attack.

But what to do with the score—can it be stopped? Can it be reduced? That is the topic for this episode of the Defiant Health podcast, highlighting what my colleagues call, even to this day, “optimal medical therapy” that has repeatedly been shown to NOT work and the answers lie elsewhere, answers that I shall discuss.

Those of you who follow my discussions that started with the Wheat Belly series of books already know that there is a long list of reasons why humans made an enormous blunder when we first resorted to the consumption of the seeds of grasses, meaning wheat and related grains. We know this from the anthropological record of human disease, we know this from modern science that has exposed the many problems that arise with their consumption from tooth decay to arthritis.

But in my many conversations with people, I find that, because we are all bombarded with the opposite message from TV commercials, media reports, even doctors and dietitians, much of the rationale and science that tells us that wheat consumption is awful for health becomes diluted, or discounted, or ignored. My Wheat Belly arguments, for instance, have been dismissed as just another “gluten-free” message that has no bearing outside of celiac disease. Or that there are bad health consequences of banishing an entire category of food. Or that, if the U.S. Dietary Guidelines for Americans tell us that grains must be a part of the human diet, it must therefore be true. Of course, this is all not true: misinterpretations, misrepresentations, sometimes outright lies, all to prop up continued consumption of a class of “foods” that accelerate disease, aging, and should have no place in the diet of humans. 

So for this reason, I am going to be guilty of repeating myself and once again reiterating why the arguments against the inclusion of wheat and grains are overwhelming, the health impact of their consumption shockingly awful, and the health benefits of their abandonment huge and life-changing.

I’ve spent some time reflecting back on all my years practicing in hospitals, often not sleeping for extended periods, sometimes days, covering hospitals floors and ICUs, resuscitating people who experienced cardiac arrests, taking them to the cath lab to open arteries, but also witnessing people with all sorts of other health conditions: cancers, wound infections, sepsis, delirium, the various stages of dementia, and hundreds of other debilitating disease. 

I now recognize that, looking back, so many health conditions can be better understood in light of the contribution of the microbiome, i.e., the trillions of microbes occupying the gastrointestinal tract from mouth to anus, the nasal sinuses, the airways, the brain, the prostate, the vagina and uterus, the skin—virtually every part of the human body. We all know about acute bacterial infections. But I’m talking about something different. While microbes exert effects via a number of different routes—think the gut-brain axis, the gut-skin axis, the gut-muscle axis, effects exerted via hormones, inflammatory mediating proteins, and bacterial breakdown products. But beyond this, there is a curious phenomenon in which bacteria and fungi themselves, by a number of means, travel through the body to take up residence and cause trouble. But the “trouble” is not an infection in the conventional sense, but something different, less acute, less urgent, less dramatic, but nonetheless playing a big role in your health. This is the fascinating and scary phenomenon called “translocation,” the topic for this episode of the Defiant Health podcast.

I’ve been showing thousands of people how to restore this microbe lost by the majority of modern people, Lactobacillus reuteri. But, of all the spectacular improvements in health we can gain by managing our microbiomes, such as an increase in empathy and generosity, reduction in anxiety and depression, deeper sleep, increased libido, restored youthful musculature and strength, the females in my audience have repeatedly said something like, “Well, that’s all well and good, but I’m mostly interested in the skin benefits.” So I’ve spent a good deal of time, energy, and resources in exploring this question, including performing a small human clinical trial to better understand how and why restoration of L. reuteri, especially when combined with other non-microbial synergistic factors, can restore youthful healthy skin. 

One issue that has emerged to the forefront in our quest to better under skin health and appearance is the issue of skin pH. We may not think about skin this way, but healthy skin is acidic. The pH of healthy skin in someone not plagued by eczema, acne, rosacea, psoriasis, or other skin conditions is typically around 4.5. Recall that neutral is 7.0 and the blood is mildly alkaline at a pH of 7.4. Unhealthy skin, in contrast, skin with various skin rashes, has a pH of around 5.5—given the logarithmic nature of the pH scale, unhealthy skin is therefore 10-fold less acidic than healthy skin, a huge difference. This difference in pH is a crucial determinant of what you skin looks and feels like, whether you will be prone to these sorts of skin rashes or whether you can enjoy smooth youthful skin. So let’s dive into that topic, the importance of managing skin pH.

I’ve touched on the topic of endotoxemia in past episodes of the Defiant Health podcast. In this episode, let’s dive deeper into this topic that is absolutely crucial to understanding and managing SO many aspects of health, from subduing anxiety and panic, to depressive, to joint pain and skin rashes, to gastrointestinal conditions, even weight management. 

What is endotoxemia? Fecal microbes, so-called Gram-negative species because of the way these microbes take up stain for examination under a microscope, species such as E. coli and Salmonella, have something called lipopolysaccharide endotoxin in their cells walls. Other species, so-called Gram positive species such as Enterococcus, Staphylococcus, and Streptococcus, that stain in a different manner, have something called lipoteichoic acid in their cell walls. When these microbes die, both Gram-negative and Gram-positive, these toxic factors are released into the intestines. If these fecal microbes are confined to the colon, where they belong, a section of GI tract adapted to their presence, the entry of these toxic components are limited and current evidence is unclear in how importat this process is. The real trouble occurs, however, when fecal microbes have invaded the 24-feet of small intestine, a process we label small intetinal bacterial overgrowth, or SIBO, because the small intestine—the stomach, duodenum, jejunum, and ileum are not well-adapted to the invasion of fecal species. Here, they die, release their toxic components, which then enter the bloodstream, the process labeled endotoxemia. People with SIBO therefore have high blood levels of endotoxin. This is how microbes in the GI Tract export their effects to other parts of the body. Over the past decade, most of us have converted our home lighting to light-emitting diode (LED) bulbs, as they save a significant amount of energy, requiring 75-90% less energy than older incandescent lights. Personally, I converted the 50 or lightbulbs in my house to nearly all LEDs.

Most current LEDs emit, of course, visible light. But they also emit a significant amount of blue light and very little red and infrared (IR), in contrast to old-fashioned incandescent bulbs that emit some blue and a lot of red and IR. Remember when you had to change an incandescent bulb when it died? If you tried to unscrew it, it could burn your hand because the bulb was extremely hot. That’s the effect of red and IR that generates heat. Change a LED bulb and you can do so immediately, as the bulb is cool, even after dying just moments earlier. This is due to the absence of significant amounts of red and IR.

While well-intended to save energy, it is now becoming clear that the wholesale conversion to blue-emitting LED lighting has been exerting adverse health effects, especially visual. We all recognize that American healthcare is a badly broken system. Wildly overpriced, exploitative, with a focus on pharmaceuticals and medical procedures to the exclusion of basic issues such as nutrition, the role of nutrients, and the microbiome. Can we expect doctors, healthcare executives, pharma or medical device executives to find better solutions? 

I highly doubt it. One of the problems plaguing conventional delivery of healthcare is that doctors lack the time, knowledge, and frankly the interest in spending more time with their patients, hearing about their issues, then creating solutions that address their unique concerns. 

Enter the age of health coaches. A number of years ago, when I first heard about this phenomenon, of heath coaches playing a role in the delivery of health, I was skeptical. But over the past decade, as I’ve worked with health coaches, witnessed what they could accomplish, and heard plenty of positive feedback from the public engaging their services, my opinions about health coaching has completely changed. I now see them as part of the engine that will drive improvements in healthcare. 

I therefore tracked down Sandra Scheinbaum, a Phd clinical psychologist who counseled patients for many years but then diverted her career into functional medicine and health coaching, bringing the unique insights into human behavior and emotions that came from decades of practicing clinical psychology, now applied to health coaching. Dr. Scheinbaum founded the Functional Medicine Coaching Academy that educates and trains people in acquiring the skills and knowledge to practice as a health coach. She is also author of several books including Functional Medicine Coaching: Stories from the Movement That's Transforming Healthcare, and her newest book, Your Health Coach Will See You Now available in April 2025 on Amazon and selected bookstores. In this episode of Defiant Health, Dr. Scheinbaum recounts how she made this change in direction and what she foresees for the future of this important phenomenon of health coaching.

I first met Dr. Eric Westman at a low-carbohydrate meeting a number of years ago where he discussed an extremely important and insightful human clinical trial he had conducted in which he counseled participants with type 2 diabetes to follow a very low-carbohydrate, essentially ketogenic, diet. He was almost prohibited from conducted the study because some colleagues felt it was too dangerous. Nonetheless, the study was completed and demonstrated that type 2 diabetic participants no longer needed insulin and most diabetes drugs while achieving improved blood glucose measures by engaging in a lifestyle that was the direct opposite of conventional dietary advice. As groundbreaking as this study was for its time, it is simply not talked about enough, as it is one of the most important pieces of evidence that validates the idea that limiting carbohydrates and sugars, not fats or saturated fats, is key to, in this case, improving diabetes control, even helping make many people non-diabetic. I therefore thought it would be a good idea to ask Dr. Westman to describe his rationale for the study, discuss the results, and share the lessons he has learned since then.
