With the subject being largely ignored in medical school and residency, it can be a struggle to find a practitioner who is both knowledgeable and willing to dig deeper and provide the care that’s needed.

Today I’m joined by my friend, Integrative Dietitian and Menopause Expert Esther Blum, to debunk menopause myths and discuss how you can go through menopause like a rockstar. Welcome back to the final installment of my special series of interviews with my kids. So far, you’ve heard from my firstborn, Hannah, my eldest son, Hunter, and my youngest, Anthony. Today, I’m chatting with my thirdborn, Angelo.

Angelo and I share very similar birthdays - only four days apart - and he’s the one who’s most like me emotionally. He was born about five weeks early, right before 9/11, and was the only one of my children who was breech. Unlike his older siblings, I never know what to expect from Angelo in terms of his school and career journey, and it’s always interesting to see where he ends up.

Many patients are choosing providers outside of the traditional medical system because of negative experiences with MDs.

In a time where there are so many doctors, experts, and gurus to choose from, there’s no need to stay with a provider that is failing you. At the same time, figuring out who is well-positioned to help you has become quite confusing.

From doctors who aren’t medical doctors to specialists advising outside of their specialty, how are you supposed to know what to look for?

With this episode, I want to get you thinking about your providers and to be empowered in choosing whose advice you follow. I’m sharing what the letters behind providers’ names mean, the differences between similar-sounding specialties, and the red flags that you should be aware of when selecting a provider.

The confusion caused by non-medical practitioners with the title “Dr.”

Core tenet for credential clarity: Always clarify what type of Dr. you are

Getting clear on the training and experience behind:

MD: Medical Doctor; 4 years of undergrad, 4 years of medical school, residency and possibly fellowship

DO: Doctor of Osteopathic Medicine; similar training path to MDs but with an additional study on manipulative therapy

Naturopath/Naturopathic Doctor: operate on the six principles of naturopathic medicine; usually a 4-year program

Functional Diagnostic Nutrition (FDN)/Integrative Functional Nutritionist: receive nutrition training but don’t necessarily have a medical background

Traditional Chinese Medicine (TCM) practitioners: lengthy degree requirement; aim to find the energetic body aspect of your illness and correct it from that angle

Acupuncturist: degree program differentiated them from someone who does needling

Progesterone is the great calmer. It’s the hormone responsible for pregnancy and is probably my second favorite (right after testosterone). With its central role in the menstrual cycle, I’ve seen progesterone help many women in my practice.

I can't bring myself emotionally, spiritually, psychologically, or physically to use hormone pellets with my patients.

Hormone pellets are tiny pellets containing hormones and medications inserted under your skin and replaced every three to six months.

In my clinic, many unhappy patients come to me with complications from pellets. Outside of my clinical experience, I also tried pellets myself a few years ago, which was the worst experience of my life.

All too often, it’s women who bear the brunt of medical device failures.

A major example is Essure, which came on the market in 2002 as a female sterilization device. It was sold to physicians and patients as a non-surgical, FDA-approved option that could be inserted without much downtime. Sounds good, right?

That’s what many of us thought at first, but by 2016, many women reported issues, and I started removing them. At the end of 2018, Essure production was stopped, and the distributors took it off the market.

Today Women’s Fitness and Nutrition Coach Bree Argetsinger, also known as The Betty Rocker, joins me to talk about her experience with Essure and ultimately having it removed. 13 Oct 202212: Does Estrogen Cause Breast Cancer? No - Well, Maybe00:39:35

How are we at a point where women are terrified of a hormone they've had in their bodies most of their lives?

Many of us have heard horrible things about estrogen and its connection to breast cancer. A lot of this public aversion stems from a study that came out in the early 2000s called the Women's Health Initiative (WHI).

The WHI was a landmark study designed to define the risks and benefits of interventions, notably hormonal therapy. Unfortunately, when journals and the media reported on the findings, they didn’t put them into the appropriate clinical context and did not mention the study’s limitations. Ultimately, this led to women ditching their hormones en masse, suffering in silence without an alternative.

Today I’m sharing what you need to know about the WHI, what the data shows about the connection between hormone therapy and the risk of disease, why we need to shift our perspective on hormone therapy, and alternatives for women who can’t benefit from estrogen therapy.

The bottom line is hormone replacement therapy is a personal choice. We have to look at the risks versus benefits, not expecting it to be a panacea for everything that ails you, but as something that could significantly improve your quality of life when done with caution.

Let’s try not to be afraid of estrogen - it’s not a bad thing.

Endometriosis is a long-term disease, but it’s not a terminal illness.

While endo symptoms usually present at a young age, it takes approximately seven to ten years for most women to get diagnosed. It’s not uncommon to hear of women being bounced around the medical system and silenced with birth control pills to mask their symptoms, with no one taking a closer look. Plus, a doctor needs to confirm the presence of endometrial implants through surgery before you can get a definitive diagnosis.

23 Nov 202218: The Pitfalls of Hormone Pellets: The Bad, The Worst, and The Ugly00:38:25

I can't bring myself emotionally, spiritually, psychologically, or physically to use hormone pellets with my patients.

Hormone pellets are tiny pellets containing hormones and medications inserted under your skin and replaced every three to six months.

In my clinic, many unhappy patients come to me with complications from pellets. Outside of my clinical experience, I also tried pellets myself a few years ago, which was the worst experience of my life.

Looking at the science, there are much better methods of getting bioidentical hormones, such as topical creams and sublingual troches, and they don’t come with the downsides of hormone pellets.

Today I’m sharing the top ten reasons why I’m a pellet anarchist, misleading ways that pellets are marketed, the things women need to know before using pellets, and what to do if you’ve had pellets and want to make a change.

How hormone pellets work

Pitfall #1: They’re expensive

  • On average, pellets are around three times more expensive than other delivery options for bioidentical hormones
  • Pellets average around $100-125/month versus around $45/month for sublingual troches, tablets, or topicals from a compounding pharmacy

Pitfall #2: Hormone levels are too high

  • The sawtooth pattern that occurs with pellets: initially dramatically increased hormone levels followed by a steep decline

Pitfall #3: It’s a surgical procedure

  • Adverse side effects from the procedure
  • Although it’s a minor procedure, it’s one you’ll have to have repeated every couple of months

Pitfall #4: Once they’re in, they don’t come out

  • If you have side effects or complications, you’re stuck with the pellets for 3-6 months

Pitfall #5: You’re subjected to minor surgery every 3-6 months

  • Two irreversible side effects of pellet testosterone therapy if the levels go too high

Pitfall #6: Irregular absorption rates

  • Pellets are sold as being the ultimate in stable absorption rates, but research shows that’s not true

Pitfall #7: It’s a cash cow for providers

  • The financial upside incentivizes providers to push pellets as the top hormone replacement option

Pitfall #8: Who’s inserting them?

  • Lots of times, the providers putting in pellets are hormone dabblers, not experts

Pitfall #9: It’s a club

  • Hormone pellet companies charge providers a monthly fee to use the company name, logo, and product

Pitfall #10: You’re a number

  • Hormone treatment plans should be personalized
  • Why I don’t like protocol-driven hormones

Today, I’m breaking down a new study that came out earlier this year, reported in the New York Times. It was about menopausal hormone therapy and dementia, and it’s been kicking up a stir. I’m talking about this today as an example of how to look at these types of studies and determine what’s legit in a world where seemingly contradictory research seems to be constantly popping up.

Birth control pills have been around for decades, and most of that time, they were viewed as a net positive. Nowadays, you can’t say “birth control pills” without being bombarded with their evils.

The truth is, they’re not evil. If you need the pill for birth control, they're empowering and allow you to be in control of your reproductive health.

But physicians prescribe birth control pills for every problem under the sun, and I believe we are to blame for the stigma around the pill. The pill has become the band-aid solution for lazy medicine, whether it’s acne, painful cramps, heavy periods, or many more symptoms. Add to that the lack of education these doctors give to their patients, and you find that many women choosing the pill did not do so from an informed place.

As you probably know, I’m a firm advocate for the removal of foreign bodies and foreign materials, particularly within my practice with the Essure coils. Many women go through a long journey to have the problem acknowledged and often spend years dismissed by doctors. A big part of this podcast is about patient advocacy and empowering you to stand up and speak your mind regarding your health.

Today I’m joined by actress Candice Barley, who has become a patient advocate after her harrowing experience with breast implant illness. Within a few months of getting breast implants at age 24, Candice started experiencing extreme chronic fatigue, which her doctor dismissed as a side effect of having two young children. From then on, Candice experienced one issue after the next, with increasingly debilitating symptoms, until she connected her implants and her illness. Almost immediately after her explant surgery, everything began to improve.

In this episode, Candice shares her journey with breast implant illness and explant surgery, the symptoms many women experience but never connect to their implants, the dismissive response most women receive from their doctors, what to do if you think you might be experiencing breast implant illness, and more.

While there are risks and potentially serious side effects, there’s a lot of misinformation out there on these drugs. In this episode, I’m sharing how they work, the truth about the problems, what you need to know before taking them, and why I believe they are an excellent option for many women struggling to lose weight.

We’re back for part two of this special series of interviews with my kids. Last week, you heard from my oldest-  and only daughter - Hannah, and today, I’m talking to my oldest son, Hunter.

Hunter is 28, and the only one of my children who decided to go into medicine, and he’s currently in medical school. Hunter was born in Oklahoma City during the third year of my residency in 1996. Probably the most adaptable of the four, his mother and I divorced when he was young, and later, in the sixth grade, he came to live with me and his stepmom, Katie.

How are we at a point where women are terrified of a hormone they've had in their bodies most of their lives?

Many of us have heard horrible things about estrogen and its connection to breast cancer. A lot of this public aversion stems from a study that came out in the early 2000s called the Women's Health Initiative (WHI).

The WHI was a landmark study designed to define the risks and benefits of interventions, notably hormonal therapy. Unfortunately, when journals and the media reported on the findings, they didn’t put them into the appropriate clinical context and did not mention the study’s limitations. Ultimately, this led to women ditching their hormones en masse, suffering in silence without an alternative.

Today I’m sharing what you need to know about the WHI, what the data shows about the connection between hormone therapy and the risk of disease, why we need to shift our perspective on hormone therapy, and alternatives for women who can’t benefit from estrogen therapy.

Let’s try not to be afraid of estrogen - it’s not a bad thing.

In this episode, Dr. Shawn Tassone discusses a recent study published in the Journal of the North American Menopause Society that challenges the previous negative views on hormone replacement therapy (HRT) for women over the age of 65.

The study found that HRT, specifically estrogen therapy, beyond the age of 65 is associated with significant risk reductions in mortality, breast cancer, lung cancer, colorectal cancer, congestive heart failure, venous thromboembolism, atrial fibrillation, heart attacks, and dementia. Unfortunately, thyroid disease is very common, and women are around five to eight times more likely to be diagnosed with a thyroid condition than a man.

Estrogen has many protective effects, from bone and heart health to thyroid function and sexual motivation.

I can't tell you how often I’ve put a woman on estrogen only to return, saying she felt better for a couple of weeks but decided to stop the medication. Why? Because her friends told her that estrogen causes breast cancer.

This narrative is spreading like wildfire, and it’s frankly misogynistic. Why should women have to suffer the life-altering adverse effects of low estrogen when they have an alternative? How can you say across the board that no one should use this treatment when it has improved the lives of so many people?

We’ve already done a couple of episodes on estrogen, and today I’m diving in again to get the message across that estrogen should not be feared. While it may not be for everyone, every woman suffering the effects of low estrogen deserves to make a well-informed decision about their treatment.

There’s room in the world for all the different ideas. Unfortunately, many of my colleagues would disagree.

I’ve been on Facebook a lot lately, and I’ve noticed that many doctors who don’t share my opinion on alternative medicine jump straight to name-calling. Instead of respectful disagreement or sharing of ideas, culturally, we’ve gotten to a place where everything is so polarized that it’s them and us, and there is no room for a middle ground.

The sad part is that the patients will suffer the most as a result.

Endometriosis is a debilitating condition affecting millions of women worldwide, yet many go undiagnosed or misdiagnosed for years.

Early diagnosis leads to better outcomes and improved quality of life. As part of our Endometriosis Awareness Month series, I’m joined by holistic health coach Khush Sra today. In addition to being an Endometriosis Nutritionist, Khush has personally suffered with endometriosis and was diagnosed around 20 years ago. She knows the ins and outs of endo, not just from a practitioner’s perspective and as a woman and a patient.

In my practice, I’ve found the best approach to treating endo is multifaceted and covers multiple angles. Many surgeons who are excellent at removing endometriosis tissue don’t have a plan for preventing recurrence beyond prescribing the pill.
