Four Things to Know about Carbon Monoxide Poisoning

1. Know the symptoms, which include headache, dizziness, upset stomach, vomiting, weakness, confusion, and chest pain. Skin is pink or pale and the lips bright red.
2. Most CO in planes is from a faulty heating system. So shut off the cabin heat, open windows, and land as soon as possible.
3. 43 percent of poisoned patients still have cognitive problems a year after their encounter.

Max talks about two critical factors, sleep inertia and false glide slopes, which played significant roles in an Air India Express accident. Despite being unrelated phenomena, they converged to contribute to the crash of Flight 812 in 2010. Air India Express, a low-cost airline operating in India, experienced its first fatal crash during this flight.

The aircraft, a Boeing 737, was returning from Dubai to Mangalore, India. However, the cockpit voice recorder revealed that the captain had been asleep for a significant portion of the flight, awakening only shortly before the crash. Compounded by issues such as inadequate descent planning and reliance on visual cues due to radar unavailability, the crew found themselves on an unstabilized approach, ignoring multiple warnings and calls for a go-around from the first officer. The aircraft ultimately overshot the runway.

Air Canada flight 759 had a near miss last week, when it inadvertently lined up to land on a taxiway instead of on the runway. The FlightAware online tracking service showed the Air Canada Airbus 320 dropping to as low as 175 feet before increasing altitude above Taxiway C, flying over three fully loaded United Airlines and one Philippine Airlines airliners. At 11:55 p.m., the time of the incident, Runway 28L was closed with its lights dark, according to the FAA. It’s possible that shifted the Air Canada pilot’s orientation to the right, leading him to think that Taxiway C was actually runway 28R. We talk about the key reason that a disaster was narrowly averted, and how that applies to pilots flying general aviation aircraft. We also talk about the poorly understood squelch controls on radios and intercoms and how to set them properly. There are a couple of ways to load an instrument approach when flying an approach with vectors from ATC. In this show we talk about the tradeoffs involved. The easiest way is to load the approach with vectors, but then you won’t see all of the approach waypoints. A better alternative is loading the approach with an IAF and then activating the leg to which ATC is vectoring you. Trump’s personal pilot being considered for FAA Administrator.

Mike talked about that job, about his activity as a Private pilot and his thoughts on NexGen and the future of General Aviation. Mike talked about factors that led to the decline of GA after the 1970s, and his optimism for further growth due to demand for more pilots, the Part 23 rewrite, and new technologies like electric aircraft. He also talked about NextGen, ADS-B, and his prop strike. This is a replay of that 2019 interview.

03 Jan 2025362 Jeju flight 2216 Mystery: Why Aircraft Didn’t Slow During Slide on the Runway00:27:19

In this Aviation News Talk episode, Max Trescott analyzes the recent crash of Jeju Air Flight 2216, a Boeing 737-800, in South Korea. The aircraft landed gear-up on runway 19 at Muan International Airport, slid down the runway, hit a concrete berm, and burst into flames, leaving only two survivors among the crew. He performed a video analysis of the aircraft’s speed along the runway and concluded that the aircraft didn’t slow until after it left the runway, suggesting the engines were still at high power during the slide.


The incident unfolded rapidly, with the pilots issuing a Mayday after a possible bird strike on the right engine. The timeline from the distress call to the crash was just five minutes, during which the crew attempted a go-around, circled back, and crash-landed. Analysis of video footage showed the engines were likely producing high power during the landing, preventing proper deceleration.

This episode underscored the importance of minimizing speed during crashes to reduce impact energy and highlighted how modern accidents often involve complex scenarios requiring deliberate and thoughtful responses. The crash investigation is ongoing, with preliminary findings expected in the coming months.

Max talks about the details of the crash of a TBM 960, N960LP, at Truckee, California last weekend. He also talks in detail about his factory tour last week of Robinson Helicopter.

Weather at the time the pilot flew the approach was ½ mile visibility and snow with clouds scattered at 300 feet and overcast at 700 feet. However the approach minimums were 1 mile visibility and the MDA is 582 feet AGL. The pilot flew the instrument approach more or less successfully, though he descended more than 200 feet below the minimums, and he started his turn for the missed approach 1.3 miles beyond the missed approach point. While he flew the approach with the autopilot, he flew the missed approach by hand, and lost control.

Not only are there too many of them, but they also have the highest lethality rate of any accident type, as 90% of these accidents are fatal. They are unique to general aviation, as they are almost non-existent in airline flying. He also talks about how to calculate the bank angle required to get a standard rate turn.

Max talks with Nicholas about his recent experience buying a Cirrus SR22 and flying it from California to Missouri. After Nicholas arrived in San Jose, they drove to Santa Maria, where the plane had been relocated for a pre-purchase inspection. Nicholas discussed how he arranged for a local pilot to ferry the plane to the inspection site and his use of the Cirrus Owners Pilots Association (COPA) to find an independent shop.

The journey itself was full of challenges and learning opportunities. Departing under overcast skies, they flew through mountainous terrain in California, dealt with downdrafts and updrafts, and navigated around weather in Arizona and New Mexico. Their first stop was at the Grand Canyon Airport, where they met a friendly Twin Otter pilot who lent them his truck for lunch.

Fatigue and weather influenced their decision to stop in Albuquerque for the night. The next morning, they flew to Wichita, where they made a fast refueling stop before heading to Missouri. Along the way, they had to deviate around weather and faced an unexpected issue when their engine instrumentation failed, but they completed the journey successfully.

08 Jun 2017Low Cost, Garmin G5 Electronic Flight Instrument for certificated, experimental and LSA Aircraft – Garmin Interview - EP1400:33:53

Garmin recently announced that their low-cost, G5 Electronic Flight Instrument will now be available for use in certificated aircraft. It fits directly into a standard 3 1/8 inch hole used for instruments in most aircraft, which reduces installation time and cost. In this Newsmakers episode of the Aviation News Talk podcast, we interview Garmin software engineer Joe Gepner, who helped design the G5. The Garmin G5 is a direct replacement for a round-gauge attitude indicator, and it was originally introduced for experimental and LSA aircraft in July 2016. Pricing of that version starts at $1199. Later, Garmin introduced a second page in the G5 that displays a HSI, so that it can also be used as a direct replacement for a DG, heading indicator, or HSI. If a pilot has two of these instruments in an aircraft and the one displaying the attitude indicator fails, the G5 displaying an HSI can be switched in flight to display the attitude indicator. In May 2017, Garmin announced a version of the G5 Electronic Flight Instrument that can be used to retrofit attitude indicators and DG/heading indicators in over 600 models of the most common certificated aircraft. That makes it easy for Cessna, Piper, Beechcraft and other aircraft owners to easily retrofit their aircraft with a low cost glass panel display. A single Garmin G5 for certificated aircraft sells for about $2500, and includes an installation kit, magnetometer and 4-hour backup battery. When configured as a attitude indicator replacement, the Garmin G5 looks like a miniature version of a Garmin G1000 PFD (primary flight display). Like the G1000, it displays the six primary flight instruments: airspeed, altitude, vertical speed, an attitude indicator, HSI, and turn coordinator with slip/skid indicator. It also displays ground speed and ground track from a built-in GPS. The experimental/LSA version of the Garmin G5 also has a built-in autopilot. Max discusses a tragic accident involved a 55-year-old physician who crashed his Beech A36 aircraft, N84R, at Tucker Guthrie Memorial Airport in Harlan, Kentucky, on November 3, 2022. The pilot departed from Knoxville at around 9:32 a.m. with the purpose of attending scheduled appointments at a medical office near the destination airport.

Despite low instrument flight conditions at the time of arrival, the pilot did not file a flight plan, communicate with ATC, or receive a weather briefing before departure. Upon arrival in the airport area, he announced over the CTAF his intention to circle for landing. However, subsequent flight track data revealed that the airplane completed a total of three approaches to the runway, none of which were consistent with the published instrument approach procedure, and all were conducted in low instrument flight conditions.

Max talks with Eric Peterson, Director of Santa Clara County Airports, discussed the landmark arrival of G100UL, an unleaded 100-octane aviation gasoline, at Reid-Hillview Airport in San Jose, making it the first airport in the U.S. to offer GAMI’s G100UL fuel. Santa Clara County, motivated by studies linking lead exposure to nearby airports, decided to phase out leaded fuels in 2022, initially adopting Swift’s UL94 fuel. However, the new G100UL caters to aircraft needing higher-octane fuel, expanding lead-free options for pilots.

To encourage adoption, GAMI is offering incentives: aircraft based at Reid-Hillview and San Martin Airports can receive the required Supplemental Type Certificate (STC) for free by following certain steps, while Bay Area pilots who buy an STC by November 1 can get 25 gallons of free G100UL. Peterson highlighted challenges in sourcing G100UL due to limited distribution channels, which required direct contracting with the manufacturer Vitol Aviation. Currently priced at $6.99 per gallon, the fuel may decrease in cost once rail delivery becomes available.

With EPA regulations pushing for lead-free aviation fuel, Peterson emphasized that the shift is imminent, positioning Reid-Hillview as a national leader in environmentally sustainable aviation practices.

Max talks with Jason Blair, a DPE, about multi-engine ratings and training challenges. Jason explains that most pilots pursue multi-engine ratings as add-ons to their commercial certificates. He advises following an efficient training path: private single-engine, instrument rating, commercial single-engine, and finally, the multi-engine add-on.

 

Challenges with multi-engine training include mechanical delays, as many trainers are older aircraft, and higher accident rates during instruction. Jason highlights the VMC demonstration as a critical yet risky maneuver that simulates engine failure. Pilots must maintain control and recover methodically by managing power and airspeed, avoiding loss of directional control.

Performance considerations include accelerate-stop distance, single-engine ceilings, and reduced performance in high-density altitude environments. Underpowered twins, like the Twin Comanche, demand careful handling during single-engine operations.

Jason also stresses the importance of multi-engine instructors (MEIs) understanding systems thoroughly, teaching methodically, and prioritizing safety. He advises MEIs to practice emergency procedures during maintenance opportunities and conduct training at safe altitudes.

Twelve days ago, Operation Airdrop didn’t exist. What did exist was dozens of towns in Texas that were cut off from the outside world by Hurricane Harvey's flood waters, downed trees, and power lines. After driving a trailer load of supplies to Rockport, TX, Doug Jackson wondered if general aviation airplanes could be used to fly in relief supplies to isolated communities. Twelve days later, over 200 Aircraft from Mexico and all over U.S. have flown over 500 flights and delivered over 250,000 pounds of supplies. All with small, general aviation aircraft. Now the focus of the operation shifts to Florida, where Hurricane Irma is still raging as this show is being released. In our interview with Doug Jackson, he describes the catalyst for the organization--a moving encounter with a down-on-his-luck man with a sick dog--how the operation was set up in such a short period of time, and how pilots can donate money or use their airplane to deliver badly needed hurricane relief supplies. Summary
284 The final NTSB report is out for Transair flight 810, which crashed at night in the ocean shortly after takeoff from Hawaii. The pilots thought that they had a rare double engine failure. In fact, they had just a single engine failure, but they left both throttles at idle, dooming the plane. The probable cause said “contributing to the accident were the flight crew’s ineffective crew resource management, high workload, and stress.” Max talks about the huge distraction that consumed both pilots for a half hour, just moments before they took off.

More than 75% of CFIT accidents occur in daylight, and more than half of these are in visual conditions. 25% of accidents were VFR into IMC. CFIT accidents often occur while using the autopilot. For example, many people don’t know the exact meaning of “Make Right Traffic.” It means, fly to a position where you can enter the traffic pattern on the 45, then turn to downwind and fly the traffic pattern. It doesn’t mean to enter on the downwind. And of course you should be at pattern altitude when you’re on the 45. We also talk about doing a forward slip in a Cessna 172 and use of flaps during crosswind landings. Air Canada flight 759 had a near miss last week, when it inadvertently lined up to land on a taxiway instead of on the runway. We talk about how confirmation bias may have contributed to the lack of awareness that the Air Canada pilots had about their situation. We also talk about a recent fatal Cirrus SR22 crash in Sonoma, California, and the importance of never pulling the parachute below 400 feet, as it will most likely make things worse! I give details about a recent flight I took from El Paso, Texas to Concord, CA with the new owner of a Cessna 206. We postponed the trip because of a heat wave 3 weeks ago. Even if you aren’t planning to take a checkride, you’ll learn lots of tips from Seth about how to use the Documents tab of ForeFlight to organize information that you may want to have at your fingertips when you fly. He recommends you create a Binder for your checkride and then add documents to that binder. For example, for an instrument checkride, you may want to include some of the FAA Handbooks, including the Instrument Procedures Handbook and the Instrument Flying Handbook. Other things you might want to include are: Part 61, Part 91, Part 135, ACS Instrument Rating Airplane, Aeronautical Information Manual, Digital Terminal Procedures Supplemental and FAA International Flight Plan. He also mentions a glitch with the Bookmarks function, which is keyed to a particular page in a document, rather than a particular topic. This can become an issue as documents get updated with new pages, and the topics shift to different pages.

101 On Sept 30, 2018, a student pilot with 4 hours solo time departed Potomac Airfield on a cross country flight. Within minutes after takeoff, she noticed oil starting to cover the windshield. She immediately requested a vector from Potomac Approach controller Casey Whittaker to return to the airport. Her first attempt to land had her skimming the treetops of the neighborhood next to the airfield. Her next plan was to ditch the aircraft in the Potomac River, to avoid any loss of life on the ground.

231 Max talks about the crash of a Pilatus PC-12 that occurred on takeoff in ½ mile visibility in snow in SD. The aircraft, which has 10 seats, was carrying 12 people on a hunting trip. It was overweight and operating outside its C.G. limits. The pilot spent three hours removing snow and ice, but couldn’t reach the tail. A simulation showed the plane could have flown except for one additional mistake.

Surprisingly, you can tow gliders with just a Private certificate, though most operators will probably want their prospective tow pilots to have a commercial rating. Most new tow pilots will  probably be required to have at least a couple hundred hours of total experience. And since most tow planes are taildraggers, they’ll also need a taildragger endorsement.

On Super Bowl Sunday, Dr. Peter Edenhoffer was flying IFR at night in a Cessna Cardinal when he lost his electrical system. He’d already texted his son to say goodbye. Then he received a text message from Fort Worth Center which gave him hope for surviving when he realized he wasn’t alone. Max interviews NATCA’s Archie League Medal of Safety President’s Award winner controller Phil Enis about this save. Watch the NATCA video to hear more of the ATC radio transmissions and see more of the text messages from this save. It also includes a link where to where you can watch Dr. Peter Edenhoffer talk about what was going through his mind as he was alone in the dark.

Here are some ideas you should consider on future IFR flights including:
1. Include a cell phone or satellite phone number in the Remarks section of your IFR flight, so that controllers can attempt to reach you by phone or text, if they lose radio communications with you.
2. Bring a handheld radio when you fly. Set it up and test it ahead of time so that you know that it works. Handheld radios in metal airplanes receive OK, but often don't transmit well because they're essentially inside a shielded cage (the airplane!). Installing an external antenna on your airplane will greatly improve the transmit capability of a handheld radio.
3. If you have an electrical failure, use an EFB app like ForeFlight to navigate to an airport with better weather.
4. If you have cell phone service, consider calling 911 and have them pass along  your cellphone number to an air traffic controller.

Thanks for listening! A Crash that sent pilot to closed runway prompts FAA changes. When pilot Joseph Milo reported engine trouble, an air traffic controller directed him to a nearby airfield. But the airfield had actually closed 25 years earlier, and industrial buildings occupied its former runway. Milo crashed a quarter-mile away and died. His death prompted the Federal Aviation Administration to update its procedures to include weekly accuracy checks of its radar video maps The FAA has adopted a new airworthiness directive, effective Wednesday, in response to a report of an event aboard an American Champion Aircraft Corp. airplane. Specifically, a pilot reported dealing with a stuck aileron during a flight in his Super Decathlon (model no. 8KCAB), and while he was able to “un-stick” the aileron and land successfully, the subsequent examination of the aircraft uncovered a “cracked structure around several of the aileron hinges,” according to the AD. Area Forecasts (FA) to be discontinued in October 2017. AOPA Regional Fly-in at Camarillo, CA on April 28-29 EAA officials announced this week that the Founder’s Innovation Prize competition will continue through 2020, with five years’ worth of finalists eligible for a major grand prize. Each year’s competition welcomes ideas to counter loss of control accidents in amateur-built aircraft. Five aircraft violated air restrictions during President Trump’s 4-day Easter weekend stay at the Mar-a-Lago club in Palm Beach, the Federal Aviation Administration said Monday. If you’re keeping track there have been 13 weekends since the President was inaugurated on Friday, January 20. Seven of those weekends have been spent in Palm Beach. So pilots in Florida going to have to get used to the fact that it appears that on any given weekend, there’s a greater than 50% chance there will be a 30 NM TFR around Palm Beach. So far, those visits have averaged over 3 days per visit. A Southwest Airlines pilot has been arrested on a weapons charge in New York after airport security officers found a loaded handgun in his carry-on bag. Albany County Sheriff Craig Apple says 55-year-old Erik Gibson was slated to pilot a flight to Tampa, Florida, on Monday morning when TSA officers found the gun during routine screening. Over the weekend a Cessna 208 lost power and landed in a field safely in Texas. And there’s a winner…for the Red Bull races in San Diego last weekend. Japanese pilot Yoshi Muroya placed first. Stemme AG, best known for its high-end motor gliders, the S10 and S12, has announced its merger with Remos AG, the German maker of the GX light sport aircraft. Rachael Webster recounts the surreal experience of having her airplane stolen from Palo Alto Airport. Initially mistaking the tampered aircraft for a kind gesture, she later received a call from the club informing her that her Citabria had been stolen and flown toward San Carlos with its transponder off. The aircraft was eventually discovered, nose-down but largely intact, on a beach south of Half Moon Bay. The thief, a man calling himself “Sun Rock,” had allegedly sought to expose airport security flaws. After landing, he went for tacos and was promptly arrested.

The recovery process was complex, requiring approvals from multiple agencies, delaying removal and worsening damage due to saltwater exposure. Although insurance nearly totaled the plane, it was instead repaired after months of delays due to maintenance backlogs.

Despite the ordeal, Rachael remains positive, humorously considering marketing her aircraft as the only tailwheel trainer with "real beach landing experience." She has since launched a specialized tailwheel flight school, Next Level Tailwheel, incorporating Citabrias and Kitfox aircraft. Reflecting on the experience, she emphasizes patience in aircraft repairs, valuing skilled trades, and accepting that determined thieves will find a way—bike locks and all.

Podcast Show Notes - Ep 2 After the News, I give my review of my flight in a 2017 Cirrus SR22 G6, which uses the Perspective+ glass cockpit. Here's a list of some features from that review. For full details, or if you're trying to decide between buying a new or used Cirrus, contact me and I can give you some guidance on the tradeoffs. For anyone interested in eventually buying the SF50 Cirrus jet, you may want to start your training in a 2017 Cirrus SR22 G6, since it has virtually the same cockpit! Most of the features differences I discuss about the 2017 Cirrus SR22 G6 are related to differences between the Perspective and Perspective+ avionics. But there are some external differences. For example, the Cirrus has keyless entry, so you can unlock it with a key fob. When you do, the new light tubes which wrap around the length of the wing tips illuminate. The lights stay on until you climb above 300 feet, when they switch to a pulsating "wig-wag" functionality, which greatly enhances the plane's visibility to other pilots. Some of the 2017 Cirrus SR22 G6 features you'll find inside include:

  • Cell phone storage pocket on front of pilot seat.
  • Optional automatic yaw damper turns on at 200 feet and off at 300 feet
  • Weight & Balance page lets you enter weights, fuel, TKS, baggage, & plots position on graph.
  • QWERTY keyboard makes it easier to enter flight planes
  • At shutdown, flight plan saved and transponder set to 1200
  • Can load a Visual Approach to any runway using the PROC key
    • Choice of Straight in, which takes you to a 5.1 mile final
    • Or Vectors, which provides a curved path to a 1.4 mile final
    • Visual approach also calculates a descent profile to fly
  • Option to display sectionals, IFR high, or IFR low en route charts on MFD
  • You can transfer flight plans between the airplane and an iPad or smartphone
  • On Traffic Page, turn knob to sequentially view info for each aircraft

Some of the new 2017 Cirrus SR22 G6 features on the PFD include:

  • Can displays maps in HSI on PFD
  • Coms – displays name of facility you’re talking to
  • Aircraft Callsign displayed on PFD near Coms
  • SurfaceWatch displays description of where you are located on ground
  • Groundspeed displayed next to TAS at bottom of Airspeed tapeCirrus says the Perspective+ has ten (10x) times faster processing speed than prior Cirrus Perspective® avionics. I found no delays in using the displays.

If you're interested in the new 2017 SR20, it has a power upgrade to a Lycoming IO-390 engine with 215hp! And it comes with a useful load increase up to 150lbs more. It also has the same wingtip lights and Perspective+ found in the 2017 Cirrus SR22 G6. General Aviation News

  1. ForeFlight Glide Advisor™️ helps you to quickly assess your landing options in case you ever lose engine power in flight. Using terrain, GPS data, and your aircraft’s best glide speed and ratio, ForeFlight shapes a glide range ring around your own ship icon on the moving map display.
  2. When Garmin released its NXi upgrade of the G1000 integrated flight deck in January, it also announced the new visual approach feature, and that has now been added to the GTN 650/750 touchscreen GPS/com/navigators. The visual approach guidance feature adds a new visual approach in the procedure menu, and it provides advisory vertical guidance “based on a published glide path angle or a three-degree glideslope from the threshold of the runway, while considering terrain and obstacle clearance,” according to Garmin. The procedure is designed to help pilots fly a stabilized approach. If the pilot hasn’t already selected the visual approach when nearing a destination airport with a flight plan loaded, the GTN automatically provides a short cut to load and activate the visual approach when the aircraft is within five miles of the airport.
  3. The Garmin G5 is a low cost, drop in replacement for attitude indicator and/or a directional gyro. It will be soon be available for certificated aircraft. The TruTrak autopilot will also soon be available for certificated aircraft. Aspen Avionics is offering a $1000 discount in April only on the VFR version of their PFD.
  4. UAvionics, introduced four new ADS-B products. And there's a new dual band ADS-B receiver from Dynon for LSA and experimental aircraft.
  5. In LSA news, the Viper SD-4 light sport aircraft was introduced at Sun n Fun. Belite introduces the low cost Chipper kit aircraft.
  6. In Privatization news, American Airlines CEO Doug Baker argues that airline trips have increased a half hour since 1979 and it attributes that to ATC delays. Max Trescott puts that myth to rest; in 1979, the airlines were flying 727s and 747s that were faster than any of today airliners. Also, airlines weren't padding their schedules so that they could improve their on-time performance reports now compiled by the Commerce Department. Also, members of the Trump administration are visiting Canada this week to see their privatized ATC system.
  7. In International news, the new electric Volta helicopter will give a demonstration flight at AERO 2017 in Friendrichshafhaven, Germany. It recently hovered for 15 minutes. And REMOS AG, has delivered the first production line copy of a GXiS that conforms to German Ultralight standards. The aircraft, registered as D-MIDA, expands the fleet of a flight school operation known as UTC, based in Schoenberg (EDPK), Bavaria.
  8. Once again, a California flight school, this time in Fresno, is closing its doors, and foreign students from Taiwan and other countries, who've paid as much as $58,000 to attend, may be out their money. NEVER pay a flight school more than 10-20% in advance, even if they offer you a discount.

Listener Question An air traffic controller asks "Is LPV the most accurate and is it considered a precision approach. Do you prefer ILS or RNAV?" Max tells him the LPV is the low minimums to which you can fly an RNAV (GPS) approach, and while it's almost exactly like a precision approach, it doesn't meet the international definition for a precision approach. Max prefers the to fly an LPV approach, though flying an ILS is easier for pilots who aren't expert at using their GPSs!

Noise at the ground also decreases by the square of the height above ground. Max talks with CFI Jim Simon, about an incident involving a Cirrus SR22T and a parachute deployment. During a routine flight, the engine of their aircraft suddenly seized, resulting in a complete loss of power. Jim, who was flying with a learner, took control and performed emergency procedures. He opted not to land on a congested highway or in water, instead aiming for a greenbelt area.

At 1,500 feet MSL, he pulled the aircraft's parachute, experiencing a violent but controlled descent. They narrowly avoided power lines and landed softly in a marshy area. Both occupants exited the aircraft unharmed.

Jim emphasized the importance of training and preparation for such emergencies. He credited their survival to the Cirrus Aircraft Parachute System (CAPS) and noted that without it, a safe landing in such a dense urban area would have been nearly impossible. The incident underscored the value of safety features like CAPS and the need for pilots to be thoroughly trained in emergency procedures.

Summary
Max talks about Buzzing and Low Altitude Flight, its dangers, and some of the things you can do to mitigate some of the risk if you have to fly low. Even high time pilots have accidents when they fly close to the ground. You can mitigate some of the risk by careful planning ahead of time to identify towers, wires, and other obstacles. Still, unless you have to fly low to the ground, you can avoid a lot of risk by flying at normal altitudes.

Risk Management: The goal of risk management is to proactively identify safety-related hazards and mitigate the associated risks. Max talks about four fundamental principles of risk management, and then talks about how the word "probably" should be a trigger word that gets you to consider other options when you're flying.

He also talks about Pilot in Command and what it means. He suggests pilots think of it as a verb, meaning the pilot takes action when needed to prevent an incident or accident, and takes responsibility for whatever occurs.

Listeners have questions on starting instrument training, how the difference between magnetic north and true north comes into play when flying a VOR approach with a GPS receiver, and how to safely fuel an aircraft when it's raining.

Max discusses the tragic midair collision at Reagan National Airport between a PSA Airlines CRJ700 regional jet and a U.S. Army Black Hawk helicopter. The midair crash over the Potomac River was captured by a Kennedy Center webcam.

ATC instructed the Army helicopter to follow the airliner to Runway 33, but it did not comply. The helicopter was not transmitting ADS-B Out, though its position was tracked using Mode S transponders. TCAS alerts are disabled below 400 feet, meaning the pilots had no automated warning. Nighttime perception challenges likely contributed to the crew’s failure to see and avoid each other.

Trescott reviews ATC audio and past midair collisions, emphasizing the Swiss Cheese Model of accident causation—multiple failures aligned to cause the crash. A reported FAA radar screen showed collision alerts 30 seconds prior, but no action was taken. He urges pilots to use traffic displays with relative vectors for better situational awareness, particularly at night.

The episode ends with a call to fly defensively and use all available tools to prevent midair collisions.

187 Max talks with Dr. Greg Van about how hearing loss affects people and the many ways that pilots and others can permanently damage their hearing. Hearing loss can also lead to depression, insomnia and high blood pressure. Exposure to noise above 85 dBA causes damage. Even driving with the window down exceeds that level. Pilots and ramp workers need to protect their hearing even when walking around.
