Some links and formulas!

How does it measure cardiac output?

Using thermodilution and utilisation of Stewart Hamilton’s equation of:

(Tb-Ti)*Volume*k/intergration of change in T

What is venous admixture?

Venous admixture is the amount of mixed venous blood that needs to be added to end-capillary blood to account for the difference seen in pO2 between arterial blood and end-capillary blood.

It is calculated using the equation:

Qs/Qt = Cc’O2 – CaO2 / Cc’O2 – CvO2

Alveolar gas equation.

PAO2 = FiO2 (Patm – PH2O) – pCO2/R

Is there anything else at the end?

Yes there is a correction factor (+F) which is FiO2 * pCO2 * (1-R)/R and this is normally less than 2mmHg.

O2 content = SaO2 * Hb * 1.34 (Huffner’s constant which ideally is 1.39) + pO2 * 0.003

the isoshunt diagram is figure 8 here:

15 Dec 2020Control of breathing made a bit more practical00:19:33

Control of breathing is one of the most practically useful subjects you will learn in respiratory physiology!

In this video we go through the most interesting aspects, exam tips, structures and how it relates to our own practice.

The particular SAQ we address is from the ANZCA 2006 February exam paper, question 10:
List the physiological factors which increase respiratory rate and include a brief explanation of the mechanism by which each achieves this increase.

20 Dec 2020Functional Residual Capacity (FRC) and how anaesthesia makes a mess of it!00:15:31

In this episode we tackle the SAQ, explain the changes on FRC that take place with the administration of anaesthesia.

July SAQ paper 2005 question 16.

https://networks.anzca.edu.au/d2l/le/content/7669/viewContent/86778/View

This is an important core topic that requires a broad understanding of the
1) A definition and normal values
2) How to measure FRC
3) and how it changes with anaesthesia
4) functions of FRC

I use a categorised answer of anaesthetic, surgical and patient factors to get a broad bite into this answer.

and don't forget to classify the functions as well!
Oxygen store, buffer,
minimise work of breathing -  airways resistance and  compliance,
minimise pulmonary vascular resistance,
decrease shunt/VQ mismatch and atelectasis,

24 Jan 2021How do you PREVENT HYPOXAEMIA when performing a rapid sequence induction (RSI)?00:38:11

You've probably performed a few RSIs in your anaesthesia practice. But have you though about all the techniques you do to make this inherently risky technique safer?

In this episode we chat about the approach to the question from the April 2014 and September 2013 SAQ paper

Describe the physiological basis of methods used to prevent hypoxaemia prior to intubation in a rapid sequence induction. Include any adverse effects of these methods.

01 Feb 2021How does morbid obesity affect washout of volatile anaesthetic?00:46:35

This is one of the more complex questions in anaesthesia!

Here's the actual WHO definition that doesn't use the term morbid obesity but rather obesity class 1, 2 and 3.

Obesity is frequently subdivided into categories:

  • Class 1: BMI of 30 to < 35
  • Class 2: BMI of 35 to < 40
  • Class 3: BMI of 40 or higher. Class 3 obesity is sometimes categorized as “extreme” or “severe” obesity.

19 Feb 2021Miller's Anesthesia proven WRONG by Dr Stan! What REALLY happens to volatile washout with changes in cardiac output00:37:49

Here is our deep dive into what exactly occurs to volatile washout in an anaesthetised person if cardiac output changes. Out of the textbooks we reviewed, only MILLER'S ANESTHESIA (thought by many to be the bible of anaesthesia) comments on exactly what occurs!

In this episode we reveal what Dr Stan discovered by running his very own transparent simulations using Gasman.

https://www.gasmanweb.com

The cardiac output/washout analysis begins at approx 9mins 30seconds.

Our parameters for wash-in and washout analysis

Time for round 2 with Pat are very generous trainee who's offered to be quizzed on our podcast.

Here are the questions from this round:

Stem 5

1.     How does heparin work?

2.     How about low molecular weight heparin?

3.     Why do we monitor unfractionated heparin but not low molecular weight heparin?
Due to lower binding to plasma proteins and to cell surfaces, the plasma anti Xa activity generated by a given dose of LMWH is more predictable than for UH.

4.     How do we monitor unfractionated heparin’s effect?

5.     How is the clearance of unfractionated heparin different to that of low molecular weight heparin?

Stem 6

1.     What are the main differences between intracellular and extracellular ion concentrations?

2.     What is the function of potassium?

3.     What is the NERST potential?

4.     What are the ECG changes in hyperkalaemia?

5.     What is the treatment for hyperkalaemia?

Stem 7

1.     What are the functions of the placenta?

2.     How is the placenta different to the lung?

3.     How are substances transferred across the placenta?

4.     What are the factors that affect transfer of O2 across the placenta?

5.     What are the factors that affect placental blood flow?

Stem 8

1.     During anaesthesia, you notice a difference between the End Tidal CO2 and Arterial CO2. What can you do to minimise this difference?

2.     How do you measure physiological dead space?

3.     How do you measure anatomical dead space?

4.     What increases anatomical dead space?

5.     What is closing capacity?

And best thing is that all proceeds will go to a reputable charity!

In this episode we cover categorisation of core cardiovascular topics
Here is a brief summary:

Hypotension

Preload
Rate
Rhythm
Contractility
Afterload

Or Shock
Hypovolaemia
Cardiogenic
Distributive
Obstructive

O2 supply demand
Supply
= o2 flux or delivery to heart
CBF x o2 content (Hb, sats, PO2)

CVR and CPP
CVResistance = metabolic, myogenic, neural, physical compression, viscosity, pathology
CPP = Cor Pressure – RAP or LVP.

O2 extraction.
High basal extraction of 55-65%
other tissues only 25%
so O2 demand is met by increasing CBF

Demand
Depends on Metabolic rate and myocardial work
Wall tension – PL and AL
Contractility
HR
Rhythms

Wall thickness
Basal rate
Type of work (pressure > volume work)
Electrical activation

When this is unbalanced = ischaemia

Determinants of coronary vasc resistance

Resistance = pressure difference / flow = corPP/ CBF

= 8nl/Pi R4. (n = eta)

Radius
Neural
Physical – compression, HR,
Pathology
Autoregulation
            Metabolic
            Myogenic
Viscosity

Coronary blood flow

= CPP/CVR

Neural
Physical
-       Compression
HR
Viscosity
Autoreg
-       Metabolic
-       Myogenic: A myogenic response refers to the intrinsic tendency of vascular smooth muscle to contract in response to increased distending pressure and to relax in response to decreased distending pressure.

And best thing is that all proceeds will go to a reputable charity!

Effect of standing due to hydrostatic pressure

 Compensation is Global

-       Baroreceptor

o   A reduction in mean arterial pressure is sensed by the baroreceptors. They decrease their rate of firing which results in an increase in SNS discharge and decreased PNS discharge causing:

o   increased HR

o   increased myocardial contractility ® increased CO ® increased MAP

o   peripheral vasoconstriction ® increased TPR ® increased MAP (greater effect on resistance vessels than on capacitance vessels)

o   venoconstriction ® increased venous return ® increased CO

-       Atrial stretch

o   Over an extended period of time, decreased blood volume will be sensed by the cardiopulmonary receptors as decreased stretch and decreased firing of the A and B receptors will stimulate:

o   release of angiotensin, aldosterone and ADH ( Na+/H2O reabsorption, restoration of blood volume)

o   increased SNS activity in the kidney (vasoconstriction, ¯ GFR and ¯ urine flow)

-       Venous flow

o   Skeletal muscle pump

o   Valves

o   Thoracic pump

-       Microcirculation changes

Local

-       Cerebral autoregulaton

-       Myogenic mechanism

-       Metabolic

-       Increase O2 extraction

24 Oct 2021Why is there a difference between you measured ETCO2 and PaCO2?00:23:57

In this episode we talk about this very common phenomenon in anaesthesia, the difference between ETCO2 and PaCO2.

Essentially this all comes down to patient and equipment factors.
BUT the essential element that makes the largest difference is DEAD SPACE.

we discuss the factors effecting it, and the Bohr equation that measures dead space.
